1,276 young people, between 6-20 years old, representing the urban school population from Oviedo ... more 1,276 young people, between 6-20 years old, representing the urban school population from Oviedo (Spain) were evaluated by the Community Periodontal Index of Treatment Needs (C.P.I.T.N.). From this population: 1. 66.1% were found to have bleeding on probing, without evidence of attachment loss (gingivitis), or presence of local irritants. This population represented a treatment need of improving oral hygiene. 2. 29.6% had moderate attachment loss (probing pocket depths between 4-5 mm.) and/or presence of local irritants. They were determined to need scaling and root planing. 3. 0.7% had probing pocket depths higher than 6 mm., which requires a more complex specialized type of therapy. Based on these treatment needs, the number of treatment hours was determined, estimating that it would be necessary to spend 1649 hrs. to diagnose and treat this detected periodontal conditions. 93% of this time could be provided by dental auxiliary personnel. The aim of this study was to examine the relationship between differences in dental attendance an... more The aim of this study was to examine the relationship between differences in dental attendance and oral hygiene patterns and dental caries and periodontal treatment needs. 1469 young people, aged 7, 12 and 15-19 years, and representing the urban (60%) and rural (40%) population from Spain were evaluated. Dental caries and periodontal treatment needs were registered according to the index D.M.F.T. and C.P.I.T.N., following the criteria of W.H.O. Regular dental attendance was observed in 16.6% of subjects examined, and only 9.4 saw a dentist regularly for dental prophylaxis. Statistical analyses showed that while the more frequent the dental visits, the lower the rate of caries, and periodontal treatment needs, the higher, however, the average number of fillings and the D.M.F.T. scores. The goal of antibiotic prophylaxis in Odontology is to prevent the onset of infections through th... more The goal of antibiotic prophylaxis in Odontology is to prevent the onset of infections through the entranceway provided by the therapeutic action, therefore it is indicated providing there is a considerable risk of infection, either because of the characteristics of the operation itself or the patient s local or general condition. Nonetheless, clinical trials with antibiotics in dental pathologies have had scant regard for the required methodological criteria and, in addition, are not sufficiently numerous. This text presents the results of an expert conference comprising the Presidents of the most representative Scientific Societies in Spain who have analyzed the existing literature and have drawn on their valuable professional experience. It describes the technical circumstances, analyzes the biological and pharmacological foundations and their application to the most representative medical situations. The objectives of this study were to determine the accuracy of Periotest to monitor primary impla... more The objectives of this study were to determine the accuracy of Periotest to monitor primary implant stability at first-stage surgery, to identify by multivariate analysis the variables associated with early implant failure and to compare Periotest with radiographic study in the diagnosis of implant stability at second-stage surgery (during osseointegration period). A 10-year retrospective study was conducted on 1084 Brånemark implants placed in 316 patients. Clinical variables, implant diameter and length, Periotest values (PTVs) and radiological variables were analyzed in bivariate and multivariate studies in order to determine their influence on early implant failure. After examination of the sensitivity and specificity values obtained for different PTV cutoff points, a cutoff PTV of -2 was selected (84% sensitivity and 39% specificity). In the bivariate analysis, early failure was significantly related to smoking habits, implant location, bone type, implant features and PTVs (-2 and >or=-2). In the final multiple logistic model, only age (odds ratio (OR)=4.53; 95% confidence interval (CI), 1.34-15.27), smoking habits (OR=2.5; 95% CI, 1.3-4.79), bone type (OR=1.93; 95% CI, 1.01-3.7) and PTV at first surgery (OR=3.01; 95% CI, 1.5-6.02) were independently related to early failure. The Periotest (with -2 cutoff) at first surgery offers high sensitivity in the prognosis of early implant loss and shows a greater capacity to evaluate stability during the osseointegration period compared with radiographic study.

La profilaxis antibiótica en Odontología tiene como objetivo prevenir la aparición de infección a... more La profilaxis antibiótica en Odontología tiene como objetivo prevenir la aparición de infección a partir de la puerta de entrada que produce la actuación terapéutica, por lo que se encuentra indicada siempre que exista un riesgo importante de infección, ya sea por las características mismas de la operación o por las condiciones locales o generales del paciente. Sin embargo, los ensayos clínicos con antibióticos en patologías dentarias responden poco a los criterios metodológicos requeridos, y además no son lo suficientemente numerosos. Se presentan los resultados de una conferencia de expertos integrada por los Presidentes de Sociedades científicas españolas más representativas que han analizado la bibliografía existente y han aportado sus valiosas experiencias profesionales. Se describen las circunstancias técnicas, se analizan los fundamentos biológicos y farmacológicos y se aplican a las situaciones médicas más representativas. Se concluye que la profilaxis antibiótica en Odontología cuenta con indicaciones bien fundamentadas y precisas, ofreciendo a la comunidad científica internacional un protocolo práctico de actuación.
