Acute cholecystitis is a rare condition in pregnant women, potentially affecting the maternal and... more Acute cholecystitis is a rare condition in pregnant women, potentially affecting the maternal and fetal prognosis. Our aim was to report the main clinical and paraclinical features of acute cholecystitis during pregnancy and therapeutic modalities. Methods: We conducted a case series analysis recording pregnant patients with acute cholecystitis admitted to our surgery department over a period of 11 years. We collected clinical data, paraclinical features, and management modalities related to cholecystitis. Results: There were 47 patients. Twenty-eight percent was in the first trimester of pregnancy, 40% in the second, and 32% in the third trimester. Abdominal pain was located in the right hypochondrium in 75% of cases. Fever was noted in 21% of cases. C-reactive protein was elevated in 39% of patients. Cholestasis markers were high in four patients. Abdominal ultrasound showed a distended gallbladder in 39 patients, with thickened wall in 34 patients, and gallbladder lithiasis in all cases. No patient had a dilated main bile duct. All patients received intravenous antibiotic therapy. Tocolysis was indicated in 32 patients. Laparoscopic cholecystectomy was performed in 32 cases (68%), and open cholecystectomy in 15 cases (32%). Postoperative course was uneventful in 42 patients, and complicated in 5 patients. Rate of complications was statistically higher after open cholecystectomy (p = 0.003). Morbidity rate was higher in the third trimester (p = 0.003). Conclusions: Delay in the diagnosis of acute cholecystitis during pregnancy can lead to serious complications. Management is based on antibiotic therapy and cholecystectomy. Laparoscopic cholecystectomy appears to be less morbid than open cholecystectomy.

The retroperitoneal localization constitutes less than 4%. Treatment consists of surgical resection. Histological examination is essential for definitive diagnosis. We reported an unusual case of mature retroperitoneal teratoma discovered in a young man with abdominal pain.

Its management may have many particularities. The aim of our study was to describe clinical, morphological features, and therapeutic modalities of complicated hydatid cysts in pregnant women. We conducted a case series analysis and a monocentric study, including patients who presented a complicated hydatid cyst during pregnancy, admitted in our surgical department, over a period of 11 years. A total of 8 patients were included. Mean age was 26.1 SD 4 years. Mean term was 16.7 SD 5 weeks. All patients had abdominal pain; one patient had fever; and no patient was icteric. Hyperleukocytosis was present in 2 patients; C-reactive protein was elevated in 2 patients; and liver function tests were disturbed in 3 patients. In abdominal ultrasound, hydatid cyst was located in the right liver in 5 cases and in the left liver in 3 cases. Average size of cysts was 7.6 SD 2.1 cm. All the patients underwent surgery. The approach was a right subcostal incision in 6 cases and a midline incision in 2 cases. “Lagrot” procedure was performed in 7 cases and bipolar drainage in one case. Postoperative course was simple in 6 cases. One patient presented a miscarriage at 10 weeks. Another patient presented an abdominal wall abscess. Complicated hydatid cysts in pregnant women have a noiseless clinical presentation. Abdominal ultrasound is the morphological examination of choice. Conservative treatment is preferred. Fetal prognosis is better if operation is performed during the second trimester.

We aimed to report clinical, paraclinical, and therapeutic features, and to identify mortality risk factors of gas gangrene of the lower limb. Patients and Methods We conducted a one-center case series retrospective study over a period of 13 years from January 2006 to December 2018. Results A total of 19 patients were included: 12 men, 7 women. Mean age was 59.3 ± 11.3 years. There were 17 diabetic patients and one patient was on radio-chemotherapy for lower rectal cancer. Septic shock was initially present in 4 cases. Hyperleukocytosis was noted in 17 patients. C-reactive protein was elevated in 11 patients. All patients were put on antibiotics then operated on. Different techniques proposed can be conservative or radical. AIMS: The aim of our study was to compare postoperative outcomes of both methods in one-stage treatment strategy. METHODS: This is a comparative study including patients operated on for pilonidal abscess, with a satisfactory postoperative follow-up, over a period of 4 years. We looked for the occurrence of postoperative recurrence in the medical records or by interviewing reachable patients. RESULTS: We analyzed 57 patients: 33 males and 24 females. The mean age was 26.9±10 years. The type of operation was excision in 46 (81%) cases and incision in 11 (19%) cases associated with curettage in three cases and drainage in 1 case.
