In this study, it has been examined whether the bottom ash (BA) released to the nature as waste m... more In this study, it has been examined whether the bottom ash (BA) released to the nature as waste material from the thermal power plant can be used in reinforced concrete structures (RCS) and how much cost reduction will be caused in construction technology if BA is used in RCS. For this purpose, coal BA produced by Turkey-Zonguldak Cates power plant is mixed in different ratios into the concrete mixture of the beams. The different coal bottom ash ratio (BAR) is used instead of aggregate in the mixture. The beams created for 5 different BARs are subjected to tests and it is observed that the beam with the most critical bending is the beam with 75% coal BA. This BAR is applied to bearing elements of finite element model (FEM) and then current cost of the building and the construction cost used 75% coal BAR are compared with each other. İnterstisyel akciğer hastalığı (İAH), kanser ve enfeksiyon gibi nedenlerle oluşanlar hariç, hücre... more İnterstisyel akciğer hastalığı (İAH), kanser ve enfeksiyon gibi nedenlerle oluşanlar hariç, hücresel proliferasyon, interstisyel enflamasyon ve fibroz veya bu üç bulgunun kombinasyonundan oluşan akciğer parankim hastalığıdır. [1] İdiyopatik pulmoner fibroz (İPF), bağ dokusu hastalıklarına (BDH) bağlı İAH, kronik hipersensitive pnömonisi, sarkoidoz ve pnömokonyozlar İAH örnekleridir (Şekil 1). [1,2] Romatoid artrit (RA), Sjögren sendromu (SjS), sistemik skleroz (SSk), sistemik lupus eritematoz (SLE) ve idiyopatik enflamatuvar miyopatiler [dermatomiyozit (DM), polimiyozit (PM) ve anti-sentetaz antikor sendromu (AAS)] BDH olarak kabul edilen enflamatuvar romatizmal hastalıklardır. BDH Öz Bağ dokusu hastalıkları (BDH) sıklıkla solunum sistemi tutulumlarına neden olabilmektedir. Rutin pratikte hasta ve hekimini en çok zorlayan, en sık mortalite ve morbidite nedeni olan solunum sistemi tutulum örneği interstisyel akciğer hastalığıdır (İAH). İdiyopatik pulmoner fibroz (İPF) sık karşılaşılan İAH nedenlerinden bir diğeridir. BDH-İAH ile İPF arasındaki patojenik, klinik ve radyolojik bulgular bakımından önemli oranlarda benzerlikler iki hastalığın ayrımını güçleştirmektedir. Ancak, BDH-İAH ile İPF'nin tedavisi çok farklıdır. Bu nedenle, her ikisinin ayrımını yapmak oldukça önemlidir. İAH, histopatolojik ve radyolojik olarak, usual interstisyel pnömoni (UİP), non-spesifik interstisyel pnömoni (NSİP), lenfositik interstisyel pnömoni (LİP) ve organize pnömoni olarak gruplandırılır. NSİP paterninde BDH-İAH olan hastalarda immünosüpresif tedaviye yanıt UİP paterni olan hastalardan daha iyidir. UİP paterni olan hastalarda antifibrotik tedavinin etkinliği daha belirgindir. İPF UİP paterninde iken, BDH-İAH sıklıkla NSİP paternindedir. Ancak, romatoid artrit UİP, Sjögren sendromu ise LİP paterninde olabilir.

OBJECTIVE The knowledge of physicians about complementary and alternative medicine (CAM) applicat... more OBJECTIVE The knowledge of physicians about complementary and alternative medicine (CAM) applications is limited. However, especially in chronic diseases, patients and their relatives can often refer to CAM applications. Rheumatic diseases are chronic in nature presenting with a wide clinical spectrum. Despite developing treatment options, achieving treatment goals may not always be possible. For this reason, patients seek different treatment and use traditional and complementary medicine. The aim of this study was to investigate causes, consequences, and the frequency of applying to CAM in rheumatic diseases. METHODS Ninety-five patients admitted to the rheumatology outpatient clinic were enrolled in the study. Health assessment questionnaire and short-form-36 were used to determine the quality of life of patients. Anxiety and depression symptoms were screened by the Hospital Anxiety and Depression scale, a questionnaire that was filled-in by the patients themselves. Also, patients were questioned about their place of residence, level of education, diagnosis, CAM modality types, application reasons, and outcomes. Chi-square test was used to analyze categorical data. Parametric data were analyzed using Student t-test, and nonparametric data were analyzed using Mann-Whitney U test. RESULTS Thirty-two of our patients had applied to CAM modalities (phytotherapy [34.45%], cupping therapy [21.8%], acupuncture [12.5%], hirudotherapy [12.5%], food supplement [12.5%], and ozone treatment [6.25%]). Only 31.3% of the patients informed their doctors about CAM applications. 47.8% of fibromyalgia patients and 29.2% of patients with inflammatory rheumatic diseases had applied to CAM. Gender, working status, income level, smoking, and alcohol habits were not associated with the application to CAM. However, none of the residents of the village, 14.3% of the residents of the district center, and 41.1% of the residents of the city center had applied to CAM modality. The rate of applying to CAM was 18.2% for those who cannot read and write. The application ratio of CAM is over 40% among secondary school, high school, and university graduates. CONCLUSION Among patients with rheumatic diseases, application to CAM is quite common. Very few patients inform their physicians about applying to CAM. Contrary to what is presumed, the rate of applying CAM applications is lower among those living in rural areas and with low education levels.

Aim: Rheumatoid arthritis (RA) is a progressive, inflammatory, autoimmune disease that particular... more Aim: Rheumatoid arthritis (RA) is a progressive, inflammatory, autoimmune disease that particularly affects the joints. Tofacitinib is the first oral Janus kinase inhibitor approved for RA treatment. We aimed to analyze the 6-month drug survival rate and factors affecting the discontinuation of tofacitinib in RA patients. Materials and Methods: Age, gender, rheumatoid factor (RF) and anti-cyclic citrullinated peptide (anti-CCP) levels, whether to continue treatment tofacitinib, if treatment is not continued what treatment is applied, use of biological agent before tofacitinib treatment were retrospectively recorded from the patient data. Results: 30 RA patients included in the study (29 female, 1 male) with a mean age of 50.5 ± 11.3 years. At the 6th month of treatment of tofacitinib, the drug survival rates were 50%. There was no significant difference between CCP positive and negative patients as well as between RF positive and negative patients in terms of drug survival rates (p = 0.92 and p = 0.90, respectively). The drug survival rates were alike in tofacitinib monotherapy and combined therapy of tofacitinib with any conventional DMARD (p = 0.36). The tofacitinib survival rates were similar in biologically naïve patients and in patients who had received at least one previous biological DMARD (p = 0.70). Conclusion: Half of the RA patients receiving tofacitinib treatment continue their treatment at the 6th month. The drug survival rate was not associated with co-treatment with conventional DMARD, auto-antibody positivities, and previously used biological DMARD therapy. These findings support that tofacitinib shows similar efficacy when used as combined or monotherapy, in seronegative or seropositive patients, and in biologic resistant or naive patients.

Ginger is used for food and medicine to treat arthralgia, sprains, and muscle aches. Anti-inflammatory effects of ginger have been observed. The aim of our study was to detect the effects of ginger on experimentally induced inflammatory arthritis. METHODS Female Wistar albino rats (n = 21) were randomly separated into three groups (control, arthritis, and arthritis + ginger). Arthritis was generated by an appropriate method using type 2 collagen and Freund's adjuvant (collagen-induced arthritis model). The ginger group was treated starting at the first collagen injection with ginger root extract for 32 days by oral gavage (50 mg/kg/daily). Interleukin (IL)-6, IL-17, tumor necrosis factor-α (TNF-α), sclerostin, dickkopf-related protein-1 (DKK-1), and obestatin serum levels were studied by enzyme-linked immunosorbent assay method. Tissue TNF-α, IL-17, cyclooxygenase-2 (COX-2), and nuclear factor kappa B (NF-κB) levels were detected using the Western blot method. RESULTS Mean arthritis score and serum levels of TNF-α, IL-6, and IL-17 were significantly decreased in ginger group than in the arthritis group. Increased sclerostin serum level and decreased DKK-1 serum levels were detected in ginger group compared with arthritis group. The decreases of IL-17, TNF-α, COX-2, and NF-κB tissue levels were statistically significant in the ginger group compared with arthritis group. Histopathological evaluation of the ginger group showed a decrease in the inflammation score compared to arthritis group. CONCLUSION It can be concluded that ginger has protective properties in the development of inflammatory arthritis. The antiarthritic acts of ginger are related to NF-κB activity and Wnt pathway. Thus, it may be suggested that ginger is a candidate to research in human RA treatment.

IT-based teaching innovations can be effective in introducing sustainability problems to management students, but little is known about the research activity on IT-based teaching innovations focusing on sustainability in the context of business education. This study maps the literature on IT-based teaching innovations that can make management students motivated to solve sustainability problems. Fifty-eight studies are included in the review. A series of thematic analyses are conducted about IT’s roles in teaching activities, used learning models, sustainability themes, and learning objectives. Results indicate that most of the teaching innovations use IT to transform the learning process. On the other hand, an analysis of the learning models shows that there is still potential for future research on IT-based teaching innovations to enable collaborative and experiential learning in a realistic environment

In the construction sector, progress payments and approximate cost calculations can be prepared faster by package programs. The choice of package programs which perform similar calculations in the market is a multi-criteria decision-making problem. In this study, the most appropriate package selection problem is solved by using DEMATEL method, which determines the interaction between the variables that affect the decision in the multi criteria decision making environment.

Bu çalışmada, Avrupa Birliği Ülkelerinde ve Ülkemizdeki inşaat ve yıkıntı atıklarının tekrar kull... more Bu çalışmada, Avrupa Birliği Ülkelerinde ve Ülkemizdeki inşaat ve yıkıntı atıklarının tekrar kullanımı, geri dönüşümü ve yıkıntı malzemelerin geri kazanımıyla ilgili genel bilgiler verilmiş ve kamu binalarının hurda karşılığı yıktırılması süreci analiz edilerek buradan elde edilecek kamu gelirinin artırılmasına yönelik çözüm önerilerinin sunulması amaçlanmıştır. Kamu idarelerinin 2016 ile 2020 yılları arasında yapmış oldukları tüm hurda karşılığı yıkım işleri incelenmiştir. İhaleye çıkılan iki yüz beş (205) iş için ilgili kamu idareleriyle temasa geçilerek yapmış oldukları işler ile ilgili sözleşme bedelleri ile bu işlerin muhammen bedel hesaplama yöntemine ilişkin bilgiler talep edilmiştir. Elli dokuz işle ilgili elde edilmiş verilerin değerlendirilmesi sonucunda muhammen bedelin hesaplanmasında farklı yöntemler kullanıldığı görülmüştür. Bunlar; metraj hesabı yapılmış ve kamu kurumlarının yayımladıkları birim fiyatlar kullanılmış, metraj hesabı yapılmış ve birim fiyatlar için piyasa araştırması yapılmış, sadece piyasa araştırması yapılmış veya ihaleye çıkacak idare kendi tecrübesine dayanarak tahminde bulunmuştur. İhalesi yapılıp sözleşmeye bağlanan işlerde muhammen bedel ile sözleşme bedeli arasında ciddi dalgalanmalar olduğu ancak bu durumun muhammen bedel hesaplama yöntemiyle bir ilişkisinin olmadığı görülmüştür. Yaptığımız değerlendirmede, muhammen bedelin değerinden çok altında tahmin edilmesi ve ihalede yeterli rekabetin oluşmaması durumunda kamu gelirinde ciddi kayıplar meydana gelebileceği sonucuna ulaşılmıştır.

Selection of technical personnel is generally made on the basis of a decision maker’s subjective evaluation, and includes fuzzy linguistic variables; therefore, the best suitable candidate cannot be seen easily. The aim of this study is to develop a fuzzy logic evaluation model to be able to select appropriate technical personnel. For this purpose, it is determined decision criteria, the evaluation fuzzy set and the affect fuzzy set in a construction project for selecting site chief. Thus, a construction project will be able to be completed in time, within desired quality and budget by using this model.

Rutin pratikte hasta ve hekimini en çok zorlayan, en sık mortalite ve morbidite nedeni olan solunum sistemi tutulum örneği interstisyel akciğer hastalığıdır (İAH). İdiyopatik pulmoner fibroz (İPF) sık karşılaşılan İAH nedenlerinden bir diğeridir. BDH-İAH ile İPF arasındaki patojenik, klinik ve radyolojik bulgular bakımından önemli oranlarda benzerlikler iki hastalığın ayrımını güçleştirmektedir. Ancak, BDH-İAH ile İPF'nin tedavisi çok farklıdır. Bu nedenle, her ikisinin ayrımını yapmak oldukça önemlidir. İAH, histopatolojik ve radyolojik olarak, usual interstisyel pnömoni (UİP), non-spesifik interstisyel pnömoni (NSİP), lenfositik interstisyel pnömoni (LİP) ve organize pnömoni olarak gruplandırılır. NSİP paterninde BDH-İAH olan hastalarda immünosüpresif tedaviye yanıt UİP paterni olan hastalardan daha iyidir. UİP paterni olan hastalarda antifibrotik tedavinin etkinliği daha belirgindir. İPF UİP paterninde iken, BDH-İAH sıklıkla NSİP paternindedir. Ancak, romatoid artrit UİP, Sjögren sendromu ise LİP paterninde olabilir.

OBJECTIVE The knowledge of physicians about complementary and alternative medicine (CAM) applicat... more OBJECTIVE The knowledge of physicians about complementary and alternative medicine (CAM) applications is limited. However, especially in chronic diseases, patients and their relatives can often refer to CAM applications. Rheumatic diseases are chronic in nature presenting with a wide clinical spectrum. Despite developing treatment options, achieving treatment goals may not always be possible. For this reason, patients seek different treatment and use traditional and complementary medicine. The aim of this study was to investigate causes, consequences, and the frequency of applying to CAM in rheumatic diseases. METHODS Ninety-five patients admitted to the rheumatology outpatient clinic were enrolled in the study. Health assessment questionnaire and short-form-36 were used to determine the quality of life of patients. Anxiety and depression symptoms were screened by the Hospital Anxiety and Depression scale, a questionnaire that was filled-in by the patients themselves. Also, patients were questioned about their place of residence, level of education, diagnosis, CAM modality types, application reasons, and outcomes. Chi-square test was used to analyze categorical data. Parametric data were analyzed using Student t-test, and nonparametric data were analyzed using Mann-Whitney U test. RESULTS Thirty-two of our patients had applied to CAM modalities (phytotherapy [34.45%], cupping therapy [21.8%], acupuncture [12.5%], hirudotherapy [12.5%], food supplement [12.5%], and ozone treatment [6.25%]). Only 31.3% of the patients informed their doctors about CAM applications. 47.8% of fibromyalgia patients and 29.2% of patients with inflammatory rheumatic diseases had applied to CAM. Gender, working status, income level, smoking, and alcohol habits were not associated with the application to CAM. However, none of the residents of the village, 14.3% of the residents of the district center, and 41.1% of the residents of the city center had applied to CAM modality. The rate of applying to CAM was 18.2% for those who cannot read and write. The application ratio of CAM is over 40% among secondary school, high school, and university graduates. CONCLUSION Among patients with rheumatic diseases, application to CAM is quite common. Very few patients inform their physicians about applying to CAM. Contrary to what is presumed, the rate of applying CAM applications is lower among those living in rural areas and with low education levels.

Tofacitinib is the first oral Janus kinase inhibitor approved for RA treatment. We aimed to analyze the 6-month drug survival rate and factors affecting the discontinuation of tofacitinib in RA patients. Materials and Methods: Age, gender, rheumatoid factor (RF) and anti-cyclic citrullinated peptide (anti-CCP) levels, whether to continue treatment tofacitinib, if treatment is not continued what treatment is applied, use of biological agent before tofacitinib treatment were retrospectively recorded from the patient data. Results: 30 RA patients included in the study (29 female, 1 male) with a mean age of 50.5 ± 11.3 years. At the 6th month of treatment of tofacitinib, the drug survival rates were 50%. There was no significant difference between CCP positive and negative patients as well as between RF positive and negative patients in terms of drug survival rates (p = 0.92 and p = 0.90, respectively). The drug survival rates were alike in tofacitinib monotherapy and combined therapy of tofacitinib with any conventional DMARD (p = 0.36). The tofacitinib survival rates were similar in biologically naïve patients and in patients who had received at least one previous biological DMARD (p = 0.70). Conclusion: Half of the RA patients receiving tofacitinib treatment continue their treatment at the 6th month. The drug survival rate was not associated with co-treatment with conventional DMARD, auto-antibody positivities, and previously used biological DMARD therapy. These findings support that tofacitinib shows similar efficacy when used as combined or monotherapy, in seronegative or seropositive patients, and in biologic resistant or naive patients.

Ginger is used for food and medicine to treat arthralgia, sprains, and muscle aches. Anti-inflammatory effects of ginger have been observed. The aim of our study was to detect the effects of ginger on experimentally induced inflammatory arthritis. METHODS Female Wistar albino rats (n = 21) were randomly separated into three groups (control, arthritis, and arthritis + ginger). Arthritis was generated by an appropriate method using type 2 collagen and Freund's adjuvant (collagen-induced arthritis model). The ginger group was treated starting at the first collagen injection with ginger root extract for 32 days by oral gavage (50 mg/kg/daily). Interleukin (IL)-6, IL-17, tumor necrosis factor-α (TNF-α), sclerostin, dickkopf-related protein-1 (DKK-1), and obestatin serum levels were studied by enzyme-linked immunosorbent assay method. Tissue TNF-α, IL-17, cyclooxygenase-2 (COX-2), and nuclear factor kappa B (NF-κB) levels were detected using the Western blot method. RESULTS Mean arthritis score and serum levels of TNF-α, IL-6, and IL-17 were significantly decreased in ginger group than in the arthritis group. Increased sclerostin serum level and decreased DKK-1 serum levels were detected in ginger group compared with arthritis group. The decreases of IL-17, TNF-α, COX-2, and NF-κB tissue levels were statistically significant in the ginger group compared with arthritis group. Histopathological evaluation of the ginger group showed a decrease in the inflammation score compared to arthritis group. CONCLUSION It can be concluded that ginger has protective properties in the development of inflammatory arthritis. The antiarthritic acts of ginger are related to NF-κB activity and Wnt pathway. Thus, it may be suggested that ginger is a candidate to research in human RA treatment.

This study maps the literature on IT-based teaching innovations that can make management students motivated to solve sustainability problems. Fifty-eight studies are included in the review. A series of thematic analyses are conducted about IT’s roles in teaching activities, used learning models, sustainability themes, and learning objectives. Results indicate that most of the teaching innovations use IT to transform the learning process. On the other hand, an analysis of the learning models shows that there is still potential for future research on IT-based teaching innovations to enable collaborative and experiential learning in a realistic environment

Kamu idarelerinin 2016 ile 2020 yılları arasında yapmış oldukları tüm hurda karşılığı yıkım işleri incelenmiştir. İhaleye çıkılan iki yüz beş (205) iş için ilgili kamu idareleriyle temasa geçilerek yapmış oldukları işler ile ilgili sözleşme bedelleri ile bu işlerin muhammen bedel hesaplama yöntemine ilişkin bilgiler talep edilmiştir. Elli dokuz işle ilgili elde edilmiş verilerin değerlendirilmesi sonucunda muhammen bedelin hesaplanmasında farklı yöntemler kullanıldığı görülmüştür. Bunlar; metraj hesabı yapılmış ve kamu kurumlarının yayımladıkları birim fiyatlar kullanılmış, metraj hesabı yapılmış ve birim fiyatlar için piyasa araştırması yapılmış, sadece piyasa araştırması yapılmış veya ihaleye çıkacak idare kendi tecrübesine dayanarak tahminde bulunmuştur. İhalesi yapılıp sözleşmeye bağlanan işlerde muhammen bedel ile sözleşme bedeli arasında ciddi dalgalanmalar olduğu ancak bu durumun muhammen bedel hesaplama yöntemiyle bir ilişkisinin olmadığı görülmüştür. Yaptığımız değerlendirmede, muhammen bedelin değerinden çok altında tahmin edilmesi ve ihalede yeterli rekabetin oluşmaması durumunda kamu gelirinde ciddi kayıplar meydana gelebileceği sonucuna ulaşılmıştır.
