The main clinical presentation is progressive solid food and pill dysphagia, with subsequent weight loss, halitosis, and regurgitation, complicated by the occurrence of several aspiration and recurrent pneumonia when extended into the chest [2,3]. Indeed, only rare few cases of massive mediastinal extension were reported in the literature [3].

Introduction—Although auditory brainstem response (ABR) testing is among the most frequently used... more Introduction—Although auditory brainstem response (ABR) testing is among the most frequently used investigations in pediatric audiology and it often requires sedation or general anesthesia. In recent years, melatonin has been successfully used as an alternative way of inducing sleep, particularly in children undergoing magnetic resonance imaging (MRI) or electroencephalography (EEG). Purpose—To assess the effectiveness of orally administered melatonin as an alternative to sedation or general anesthesia during ABR testing. Method—In total, 33 children with suspected hearing loss underwent ABR tests in melatonin-induced sleep. Each patient received an initial dose of 5 mg, which was re-administered in case of failure to obtain sleep. Click-induced ABR tests were performed on both ears. Results—ABR tests were successfully performed in 72.7% of the patients. The average total length of time needed to obtain sleep and complete the ABR testing was 45 min. INTRODUCTION: First branchial cleft anomalies (FBCA) are rare. They have an estimated incidence o... more INTRODUCTION: First branchial cleft anomalies (FBCA) are rare. They have an estimated incidence of 1 in 100,000. Type I are those that embryologically duplicate the membrane (cutaneous) external auditory canal. The aim of this case is to describe an unusual path of a type II first branchial cleft fistula tract in a 3 years old child and its surgical management in the academic hospital of Casablanca. CASE PRESENTATION: This case is about a 3 year old girl who presented to the Ear Nose Throat (ENT) consultation for recurrent right lateral cervical infection. Clinical examination found an unsightly scar attached to an orifice giving pus located near the right mandibular angle suggesting type II first branchial cleft anomaly. Surgical excision was performed under general anesthesia by the superficial parotidectomy approach, the facial nerve was identified and preserved. The fistula cord was dissected and followed, it went under the facial nerve and the parorid gland to end under the digastric muscle where we tied it up. The postoperative checkup did not show any complications. The follow-up period was 12 months; the clinical examination did not find any sign of recurrence. CONCLUSION: First branchial cleft fistula are rare and can be in form of cyst or fistula. Its management is surgical excision keeping the tract cyst of the fistula intact with facial nerve preservation.

Tympanoplasty is one of the most performed procedures in ENT. The aggressiveness of its microscop... more Tympanoplasty is one of the most performed procedures in ENT. The aggressiveness of its microscopic approach has led otologists to adopt the endoscopic approach as a less invasive alternative. The purpose of this work is to appreciate the advantages and disadvantages of this surgical technique. We conducted a prospective descriptive cross-sectional study on 20 interventions within the ENT department of August the 20th 1953 Hospital of Casablanca from April 2019 to June 2019. The average age of operated patients was 36.3 years. Perforations were unilateral in (71%) of the cases with a predominance of the anterior (29%) and subtotal (36%) locations. The tympanoplasties were performed by 3 different senior otologic surgeons, and were left in (57%). The mean operating time was (59.5 min) and the mean anesthesia duration was 75.1 min. The aim of this study was to translate and verify the psychometric properties of the CES (Chronic... more The aim of this study was to translate and verify the psychometric properties of the CES (Chronic Ear Survey Score) in the Moroccan Arabic dialect as well as the evaluation of the quality of life of patients suffering from suppurative chronic otitis media. A cross-sectional study was carried out between 1st April 2016 and 10 January 2017 in ENT department of the 20 August hospital Casablanca. The data were collected using an anonymous questionnaire administered by the physician. The CES scale used has been translated according to international recommendations. Internal Reliability was evaluated by the Cronbach alpha coefficient and validity was calculated through the Spearman coefficient of correlations. Statistical analysis was done using SPSS 20.0. A total of 107 patients were enrolled in the study with an average age of 35 years (standard deviation = 13 years) with a sex ratio of 0.70. About 35.5% of patients perceived their disease as severe. The mean CES was 48 (standard deviation = 15) indicating a significant decrease in the quality of life of patients suffering from CSOM. Good internal reliability was noted (alpha = 0.72). CES scores increased significantly with perceived severity (p < 0.001). The Arabic version of the CES has good psychometric properties attesting to its reliability and validity. Its use would make it possible to better evaluate the quality of life of patients suffering from chronic otitis media and thus be able to compare and choose the optimal therapeutic methods based on the results of this questionnaire especially before and after surgery.

INTRODUCTION: Ethmoidal fibrosarcoma is an extremely rare tumor which represent less than one per... more INTRODUCTION: Ethmoidal fibrosarcoma is an extremely rare tumor which represent less than one percent of all malignancies in this anatomical area, the case of a 13-year-old is reported here. PRESENTATION OF CASE: A 13-year-old boy, his medical history was marked exclusively before presented to our hospital by an exophthalmia. Rhinoscopic examination showed well-defined pink mass at the level of ethmoid. Computed tomography was performed to showed local and regional extension. Treatment was an endoscopic excision of the tumor using a navigation system. The final diagnosis was made on pathologic examination. Radiotherapy was recommended by multidisciplinary staff but the patient refused. DISCUSSION: These tumors occur in extremities most frequently and Ethmoidal localization is extremely rare and have no specifics symptoms, so the rhinoscopic examination is necessary for any chronic nasal symptom. Its treatment is not codified because of its rarity and its prognosis is unknown. CONCLUSION: It's important knowledge about this pathology because of its rarity and its therapeutic and prognostic difficulties.

BACKGROUND: Epitheliod hemandioendothelioma (EHE) is a rare vascular tumor which was first report... more BACKGROUND: Epitheliod hemandioendothelioma (EHE) is a rare vascular tumor which was first reported by Weiss and Enzinger in 1982. It can be seen in many locations whose paranasal sinus is extremly rare. Its main treatment is surgery. To our best knowledge, no report of EHE treated by radiation therapy and chemotherapy has been described. CASE SUMMARY: A 18 years old man presented to our hospital with a 3 months history of right intermittent epistaxis, permnanent nasal obstruction and right hearing loss. The physical exam found a right exophtalmia, swelling deformatted right hemifacia and a bulky whitish tumor filling the right nasal cavity. The magnetic resonance imaging (MRI) showed an extensive hyperascularized nasopharyngeal process filling the right nasal cavity whose histopathological exam revealed an epitheloid hemangioendothelioma. Due to intracranial extension, surgery was contraindicated. The patient received 65 Gy of radiation therapy and chemotheray (cisplatin). After 18 months of follow up, the exopthalmous regressed and the MRI showed a 50% regression of the tumour size. CONCLUSION: Extensive EHE can be treated by radiation therapy and chemotherapy when surgery can't be perfomed.
