Cart ilage grafting is an interesting option for rinoplasties refinements. Aim: to compare butyl-... more Cart ilage grafting is an interesting option for rinoplasties refinements. Aim: to compare butyl-2-cyanocrylate, compound gelatin-resorcin-formaldeyide (GRF) and suture control to determine the efficacy of these tissue glue preparations in securing grafted cartilage. Study Design: Experimental. Methods: Fifteen male adult New Zealand rabbits were submitted to a surgical procedure to harvest 6 auricular cartilage grafts from each animal. 2 of these grafts in each animal were glued together with butyl-2-cyanocrylate, 2 with compound gelatin-resorcin-formaldehyde and 2 sewn together with nylon suture. These sandwich grafts were then glued or sewn to the periosteum of the calvaria. After 2, 6 and 12 weeks, groups of 5 animals were sacrificed and histological analysis for inflammation was performed. Cartilage graft migration, adhesion and deformities of the grafts were also evaluated. Results: there was less migration of the cartilages glued with GRF than with cyanoacrylate and suture. There was no statistical difference between the 3 materials of graft stabilization in relation to the inflammatory reaction in all evaluated periods. There wasn't detachment neither deformity in the cartilage sandwiches sewed with suture. The majority of detachment and deformed cartilages were found among those glued with cyanoacrylate. The number of deformed cartilages was directly related to the number of detached cartilages. The data were statistically significant (p< 0.05). Conclusion: the compound gelatin-resorcin-formaldehyde revealed to be a stabilization material for cartilage grafts in rabbits better than butyl-2-cyanoacrylate. The compound gelatin-resorcin-formaldeyide was also better than the suture when comparing it's fixation to the periosteum.

MÉTODOS: Foram selecionadas 24 pacientes submetidas ao tratamento cirúrgico de endometriose de reto e sigmoide com técnica de ressecção segmentar, seguido de anastomose mecânica término-terminal, com grampeador circular, no período de janeiro de 2005 a dezembro de 2007. Neste estudo incluímos mulheres no menacme que se submeteram a tratamento cirúrgico por endometriose profunda infiltrativa com acometimento do reto e sigmoide, atingindo o nível da camada muscular, submucosa ou mucosa. Para o grupo de estudo foram utilizados 24 fragmentos de reto e sigmoide com endometriose confirmada histologicamente, sendo um fragmento de cada uma das 24 pacientes selecionadas. Para o grupo controle, utilizou-se um fragmento da margem distal da ressecção, denominado anel de anastomose, de cada uma das 24 pacientes selecionadas e incluídas no estudo. As amostras foram agrupadas em blocos de Tissue Micro Array (TMA) e submetidas à reação imunoistoquímica para avaliar a expressão do fator de necrose tumoral alfa (TNF-α), do fator de crescimento neural (NGF), do neuropeptídeo Y (NPY) e do peptídeo intestinal vasoativo P (VIP), e posterior análise semiquantitativa da imunomarcação por meio da leitura da densidade ótica relativa (DO). RESULTADOS: Observou-se maior densidade ótica relativa da imunomarcação para TNF-α e NGF no grupo de estudo (amostras com endometriose intestinal), DO= 0,01, respectivamente, para as duas proteínas (p<0,05), em relação aos controles sem endometriose. Não houve diferença estatística na densidade ótica da imunomarcação do NPY e VIP. CONCLUSÃO: Identificou-se o aumento da imunomarcação dos anticorpos TNF-α e NGF em fragmentos de reto e sigmoide comprometidos por endometriose em relação aos controles livres da doença. Não identificamos diferença estatística na imunomarcação das proteínas NPY e VIP.

Objective: We report a case of an atypical presentation of a Cytomegalovirus meningoencephalitis ... more Objective: We report a case of an atypical presentation of a Cytomegalovirus meningoencephalitis in a person who did not have recently immunodeficience causes. Background: A 62-year-old woman with 2-week-long headache with slightly disorientation. The medical history was constituted by Non-Hodgkin’s Lymphoma 10 years earlier. Ceftriaxone was administered as empiric antibiotic coverage after a CT scan showing sphenoid sinusitis. After admission, the confusion progressed and the patient experienced memory impairment and motor agitation. Initial CSF revealed a cloudy aspect, no organisms were observed on Gram stain. CSF cell count was 616×106/L, with lymphocytic predominance. CSF protein 1,77 g/L and glucose 32 mg/dL. Latex agglutination tests, serology and oncotic cytology were negative. MRI of the brain showed a leptomeningeal inflammatory process. Lumbar puncture was repeated 20 days after, showing 160×106/L cells and protein 0,98 g/L. The CSF was sent for BK PCR analysis, as well as for flow cytometry immunophenotypic who turn negative. One month later, the patient experienced motor agitation and speech difficulties. A new MRI showed reduction of the anomalous leptomeningeal impregnation. Although it has been a characterization of multiple foci of cytotoxic edema. Immunohistopathology of brain byopsy revealed Cytomegalovirus inclusions. The patient was began on Ganciclovir for a total of 3 months and achieved significant improvement. Design/Methods: Not applicable Results: Not applicable Conclusions: Some studies in CMV-infected patients have shown that CSF analysis results tend to be variable, including normal findings. The diagnosis of CMV encephalitis is often difficult because of the variable clinical presentation and lack of CSF findings. An interesting and unusual observation in our patient was that CMV infections involving the central nervous system are uncommon in previously healthy individuals, and, whilst meningitis findings were improving in CSF analysis, isolated CMV parenchymal lesions were emerging. Introduction: Breast cancer (BC) is an austere public health problem, representing the most incid... more Introduction: Breast cancer (BC) is an austere public health problem, representing the most incident and prevalent malignancy among women worldwide. Objective: To evaluate the health-related quality of life of women with breast cancer undergoing initial and final stages of chemotherapy treatment. Methods: It was descriptive, correlational, analytical, prospective cross-sectional study that included 80 women diagnosed with breast cancer undergoing chemotherapy treatment. Health-related quality of life was assessed in two phases. The first one was on the initial day of the treatment and the second, 120 days after started the chemotherapy. Results: Eighty adult women were part of the study, whose average age was 52.54. Overall quality of life decreased due to chemotherapy. It was noted the negative impact that the emotional role, the sexual function and the concern about hair loss on health-related quality of life. It was observed that fatigue, nausea and vomiting, appetite loss, diarrhea were frequent symptoms in comparison between the phases. Sexual function and satisfaction were dissatisfying and worsened during chemotherapy. Conclusion: The quality of life related to the health of patients is negatively influenced during chemotherapy treatment, hence, requiring interventions in oncology and research in this sphere.

A síndrome de Klüver-Bucy, descrita em modelos experimentais 14. tem sido pouco reconhecida em hu... more A síndrome de Klüver-Bucy, descrita em modelos experimentais 14. tem sido pouco reconhecida em humanos 4. Entretanto tem sido descrita no homem 4 , em patologias de natureza variada, desde que as lesões predominem nos lobos temporais bilateralmente 10^8. Esta síndrome foi descrita por Klüver e Bucy 14 em 1937, em macacos submetidos a lobectomia bilateral. As características essenciais da síndrome compreendem: (a) "agnosia" visual, porque embora os animais fossem capazes de agarrar com precisão pequenos objetos eles perdiam a capacidade de identificá-los pela visão; (b) exagero do comportamento oral, fenômeno que impulsiona o animal a levar todos os objetos à boca; (c) hiperfagia, manifestação que determina a ingestão maciça de alimentos (voracidade); (d) hipermetamorfose, condição que torna o animal sensível a todos os estímulos visuais, manifestada pela sua tendência a agarrá-los; (e) comportamento sexual exagerado (hipersexualidade), sendo em alguns casos inadequado (atividade auto, hetero e homosexual; (./) alteração do comportamento emocional, caracterizada pela diminuição da agressividade (o animal torna-se plácido) e perda da reação ao medo. Julgamos de interesse o relato do presente caso pelos seus aspectos clínicos, eletrencefalográficos, tomográficos e anátomo-patológicos.

Aim: To evaluate the influence of identifying victims of air disasters in São Paulo on experts' q... more Aim: To evaluate the influence of identifying victims of air disasters in São Paulo on experts' quality of life (QoL). Methods: QoL was evaluated using the abbreviated version of the World Health Organization (WHO) quality of life questionnaire (WHOQOL-bref). We assessed 29 forensic experts who worked in air disasters in São Paulo and 29 experts who have not worked. The results were analyzed with Student's t-tests; we compared the QoL scores of individuals at the time of the accident with their current QoL scores, and the scores of the control group were compared with the current scores of the disaster group. Results: Statistical analyses revealed a significant decrease in forensic expert QoL when they worked at the accident site, and this result was evident in all WHOQOL-bref domains. No significant difference was observed between the experts' current QoL scores and those of the control group. Conclusions: The identification of air disaster victims in the city of São Paulo significantly decreased expert health-related QoL (HRQoL) with regard to physical and psychological aspects, social relationships and environment domains. This disturbance on the QoL was not persistent over the years.

Image guided stereotactic biopsy (SB) provides cerebral tissue samples for histological analysis ... more Image guided stereotactic biopsy (SB) provides cerebral tissue samples for histological analysis from minimal lesions or those that are located in deep regions, being crucial in the elaboration of therapeutic strategies, as well as the prevention of unnecessary neurosurgical interventions. Method: Sixty patients with central nervous lesions underwent SB from November 1999 to March 2008. They were followed up to 65 months. Preoperative diagnosis was based on clinical presentation and neuro-radiological features, pathologic diagnosis, clinical outcome. The compatibility of these findings with the pathologic diagnosis was analyzed. Results: Considering diagnosis confirmation when inflammatory hypothesis were made, our accuracy was of 76%, with 94% of those cases having clinic-pathological correspondence after an average of 65.2 months of follow up. Considering diagnosis confirmation with the preoperative hypothesis of neoplasm, our accuracy was of 69% with 90% of these cases having clinic-pathological correspondence after an average of 47.3 months of follow-up. Morbidity rate was of 5% and mortality was zero. The diagnosis rate was 95%. Conclusion: Stereotactic biopsy represents a safe and precise method for diagnosis. Anatomic and histopathological analyses have high compatibility with long-term clinical outcome.
