La enfermedad hepática grasa no alcohólica (EHGNA) es una condición altamente prevalente y usualm... more La enfermedad hepática grasa no alcohólica (EHGNA) es una condición altamente prevalente y usualmente relacionada con el sobrepeso y la obesidad, pero afecta al 10% de la población no obesa en el mundo, con tendencia al aumento. Mecanismos diferentes a los tradicionalmente relacionados con el exceso de grasa corporal, como algunos polimorfismos genéticos y la exposición a ciertos factores ambientales, se han asociado con el desarrollo de esta entidad. Comparados con individuos sanos, las personas con EHGNA tienen con mayor frecuencia desarreglos metabólicos, como resistencia a la insulina y síndrome metabólico. El curso de la enfermedad hepática pareciera ser más benigno en los individuos no obesos en comparación con las personas obesas, pero el riesgo de desarrollar enfermedades como la diabetes mellitus pareciera ser mayor. To study the incidence, clinical presentation, management, complications and outcome of tuberculo... more To study the incidence, clinical presentation, management, complications and outcome of tuberculosis in pediatric liver transplant recipients. Methods. A retrospective review of the medical records of children who underwent liver transplantation between 1991 and 1998. Results. Mycobacterium tuberculosis infection occurred in 6 of 254 (2.4%) children undergoing liver transplantation between 1991 and 1998. Cough, pyrexia and poor appetite were common presentations; one-half had normal chest radiographs. The median time to confirmation of diagnosis was 8 months (range, 1 to 17 months). Tests contributing to diagnosis included: Ziehl-Neelsen (ZN) stain (2 patients), M. tuberculosis polymerase chain reaction (1 patient), Mantoux test (1 patient) and histopathology (4 patients). Family health screening was productive for 4 patients. Duration of treatment varied from 9 to 18 months. Isoniazid-induced hepatitis was observed in 2 patients but resolved with dose reduction. Two patients died while receiving treatment, one of Klebsiella spp. septicemia and the other of pulmonary hemorrhage. Conclusions. Tuberculosis after liver transplantation has a significant morbidity and mortality. Pretransplantation a personal and family history of tuberculosis must be sought, and screening of patients and their families should be considered. Standard regimens incorporating isoniazid and rifampin are effective, but regular monitoring of liver function is essential to detect drug-induced hepatotoxicity.

This is partly due to the lack of established criteria for the diagnosis of this resistance. However, some clinical aspects may suggest that the diagnosis does not necessarily have to be positive insulin receptor antibodies as such patients could respond to immunosuppressive treatment. Methods. We describe a case and have performed a literature review on PubMed/MEDLINE, EMBASE, and Google Scholar bibliographic databases to identify all case reports. All available studies from January 1975 through December 2020 were included. Data collected were tabulated, and outcomes were analyzed cumulatively. Results. Khorama score as a predictive tool for thromboembolic events in high risk patients Background: Th... more Khorama score as a predictive tool for thromboembolic events in high risk patients Background: There is an established relation between cancer and the development of thromboembolic venous disease. Khorana et al developed a predictive score using clinical characteristics and laboratory values to stratify patients according to their risk. Aim: To characterize using Khorana score, patients with active cancer and a diagnosis of thromboembolic disease during hospitalization in an oncology hospital. Material and Methods: Review of records of the pharmacy of the hospital, selecting patients who received heparin in therapeutic doses during their hospitalization. Using laboratory values available in the medical records, the Khorama score was calculated. Results: Twenty seven patients with thromboembolic events, aged 60 ± 2 years (56% males) were selected for the study. Eighty percent of them were catalogued in the intermediate and high risk group according to Khorana score. Conclusions: Khorana score is a simple scale that might be useful to establish early prophylactic measures in patients with high risk for thromboembolic events, especially those with cancer.

Methods: Four weeks of glucose data were collected from 21 participants with type 1 diabetes using the c-rtCGM and FSL: two weeks with a single initial calibration (uncalibrated) and two weeks with a daily calibration (calibrated). Accuracy and agreement evaluation included mean absolute relative difference (MARD), the %20/20 rule, Bland-Altman plots, and the Consensus Error Grid analysis. Results: Values reported by the c-rtCGM system compared with the FSL resulted in an overall MARD of 12.06% and 84.71% of the results falling within Consensus Error Grid Zone A when the device is calibrated. BACKGROUND: Atherosclerotic disease and its complications are the leading causes of death in peop... more BACKGROUND: Atherosclerotic disease and its complications are the leading causes of death in people over 45 years in Colombia and LDL cholesterol levels have an exponentially relation with the risk of acute myocardial infarction. It is generally recommended to calculate the value of LDL cholesterol using the Friedewald formula. The aim of this study was to estimate in a population of the city of Medellin the correlation between the value of LDL cholesterol by direct measurement with the LDL cholesterol calculated by the Friedewald formula. In an exploratory manner estimate the correlation with the formula proposed by Cordova and Cordova. METHODS: This is a descriptive study of the correlation between LDL cholesterol value measured and calculated by the Friedewald formula in persons between 18 and 75 years with triglyceride levels below 400 mg. RESULTS: Data from 454 persons were obtain, 61% were women, mean age was 50.7 years (SD±16.2), the mean measured LDL cholesterol was 123mg/dL (SD±36) and the calculated by the Friedewald formula was 119 mg/dL (SD±36), the correlation maintain between the different groups. CONCLUSIONS: In a population of Medellín both formulas show a very good correlation with the measured cholesterol and this is maintained regardless of the value of triglycerides, age and sex.

Dentro de las vacunas desarrolladas, las primeras en ser aprobadas con una tecnología nueva en el campo de la vacunación, fueron las vacunas basadas en ARNm (ácido ribonucleico mensajero), que lograron tasas de efectividad cercanas al 95 % para la prevención de la enfermedad COVID-19 grave. Los eventos adversos comunes son reacciones locales leves, pero ha habido varios informes de pacientes que desarrollaron tiroiditis subaguda y disfunción tiroidea después de recibir la vacuna contra SARS-CoV-2. Este artículo presenta dos casos de tiroiditis subaguda poco después de recibir la vacuna contra COVID-19.

Khorana et al developed a predictive score using clinical characteristics and laboratory values to stratify patients according to their risk. Aim: To characterize using Khorana score, patients with active cancer and a diagnosis of thromboembolic disease during hospitalization in an oncology hospital. Material and Methods: Review of records of the pharmacy of the hospital, selecting patients who received heparin in therapeutic doses during their hospitalization. Using laboratory values available in the medical records, the Khorama score was calculated. Results: Twenty seven patients with thromboembolic events, aged 60 ± 2 years (56% males) were selected for the study. Eighty percent of them were catalogued in the intermediate and high risk group according to Khorana score. Conclusions: Khorana score is a simple scale that might be useful to establish early prophylactic measures in patients with high risk for thromboembolic events, especially those with cancer.

Methods: Four weeks of glucose data were collected from 21 participants with type 1 diabetes using the c-rtCGM and FSL: two weeks with a single initial calibration (uncalibrated) and two weeks with a daily calibration (calibrated). Accuracy and agreement evaluation included mean absolute relative difference (MARD), the %20/20 rule, Bland-Altman plots, and the Consensus Error Grid analysis. BACKGROUND: Atherosclerotic disease and its complications are the leading causes of death in peop... more BACKGROUND: Atherosclerotic disease and its complications are the leading causes of death in people over 45 years in Colombia and LDL cholesterol levels have an exponentially relation with the risk of acute myocardial infarction. It is generally recommended to calculate the value of LDL cholesterol using the Friedewald formula. The aim of this study was to estimate in a population of the city of Medellin the correlation between the value of LDL cholesterol by direct measurement with the LDL cholesterol calculated by the Friedewald formula. In an exploratory manner estimate the correlation with the formula proposed by Cordova and Cordova. METHODS: This is a descriptive study of the correlation between LDL cholesterol value measured and calculated by the Friedewald formula in persons between 18 and 75 years with triglyceride levels below 400 mg. RESULTS: Data from 454 persons were obtain, 61% were women, mean age was 50.7 years (SD±16.2), the mean measured LDL cholesterol was 123mg/dL (SD±36) and the calculated by the Friedewald formula was 119 mg/dL (SD±36), the correlation maintain between the different groups. CONCLUSIONS: In a population of Medellín both formulas show a very good correlation with the measured cholesterol and this is maintained regardless of the value of triglycerides, age and sex.

Dentro de las vacunas desarrolladas, las primeras en ser aprobadas con una tecnología nueva en el campo de la vacunación, fueron las vacunas basadas en ARNm (ácido ribonucleico mensajero), que lograron tasas de efectividad cercanas al 95 % para la prevención de la enfermedad COVID-19 grave. Los eventos adversos comunes son reacciones locales leves, pero ha habido varios informes de pacientes que desarrollaron tiroiditis subaguda y disfunción tiroidea después de recibir la vacuna contra SARS-CoV-2. Este artículo presenta dos casos de tiroiditis subaguda poco después de recibir la vacuna contra COVID-19.
