Objective: The present study proposes a psychological preparation program with a cognitive behavi... more Objective: The present study proposes a psychological preparation program with a cognitive behavioral intervention to reduce levels of anxiety and depression in addition to modifying the coping style of patients with heart disease who will undergo a surgical intervention with cardiopulmonary bypass. Method: A quasi-experimental pre-test-posttest study was carried out with a control group. 91 patients were evaluated prior to undergoing cardiac cardiopulmonary bypass surgery, of which 16 were selected for this study, 8 participated in psychological treatment and 8 patients were selected for the control group. Instruments: Hospital Anxiety and Depression Scale and the Questionnaire for Coping with Injuries and Health Problems. Results: Differences were found in the anxiety and depression scores of the experimental group, although these results were not statistically significant. On the other hand, a significant difference was found as a treatment effect between the groups in favor of the experimental group in the greater use of the Palliative coping style (p = 0.05) and less predominance of the Emotional style (p = 0.05). Conclusion: It is essential that the psychologist specialized in behavioral cardiology develops and carries out interventions so that the patient develops cognitive, emotional and coping strategies for the management of anxiety and depression.

Anomalous Aortic Origin of Coronary Arteries is a coronary disorder uncommon but occasionally let... more Anomalous Aortic Origin of Coronary Arteries is a coronary disorder uncommon but occasionally lethal. The possibility of sudden death is the indication for surgery. We inform a case of a patient with anomalous aortic origin of right coronary artery which emerged from left sinus of Valsalva, catalogued as type III. He was previously treated by CBAG, however the bypass was occluded and therefore he was sent to our service. We made a correction using the unroofing technique. Unroofing technique is a reproducible technique with good results, ultimately solving the problem. The outcome was excellent, 6 months later he was reported asymptomatic.

Advances in the diagnosis and treatment of heart disease have enabled patients to reach adulthood... more Advances in the diagnosis and treatment of heart disease have enabled patients to reach adulthood thus reproductive age. In this matter, when women with congenital heart disease get pregnant, it is vital that the multidisciplinary health team includes-of course-an obstetrician, but also a cardiologist and a geneticist. Such teamwork will be able to provide a comprehensive care hence increase the chances of reaching term delivery and, moreover, the probability of reducing the rate of complications for both the mother and the fetus. It is therefore important to acknowledge the physiological changes that occur during pregnancy and the possible complications according to the type of heart disease that the mother presents. It is also responsibility of this multidisciplinary group to provide advice and guidance to terminate the pregnancy when the risk is high.

This historic moment includes: 1. the description and analysis of the function of blood and its components; 2. the description of the normal and abnormal Anatomy of the human heart and its vessels; 3. the anatomic and functional correlation: Foundation of the deductive thinking, and 4. the anatomic and functional integration with the clinic. Finally, the third wave, which is living today, is the stage of the technological explosion that begins with procedures as thoracoscopic surgery with the concept of reducing surgical trauma through minimum approach surgery. Also the use of robotics to solve some of the alterations in the CC, another is hybrid procedures and finally the use of fetal cardiac surgery.

Background: The aim of this study was to identify if the interaction of myocardial revascularizat... more Background: The aim of this study was to identify if the interaction of myocardial revascularization methods increases the functional vascular area. Methods: A 4x3 factorial design was performed in 11 groups, five rats per group, ten samples per rat, evaluated at 45 days postoperative, with different surgical combinations. The magnitude of the interaction was evaluated by immunoexpression of vascular endothelium-derived growth factor, fibroblast growth factor and tyrosine receptor, to allow the activity of vascular endothelium-derived growth factor, fibroblast growth factor and thrombin (Flk-1), as well as vascular area measurement; Both measures were performed by computerized morphometry. Results: An increase in immunohistochemical expression and vascular area in direct proportion to the interaction was identified; It can be affirmed (ANOVA p < 0.0001), that with the interaction of all the maneuvers the maximum effect is achieved. Conclusions: It is demonstrated that indirect myocardial revascularization has a specific weight within the integral myocardial revascularization with a real impact on cost-benefit and cost-effectiveness.

A review of the history of cardiac surgery around the world is made divided into three stages, th... more A review of the history of cardiac surgery around the world is made divided into three stages, the first since the beginning of humanity until 300 years BC; the second moment shows how comes the platform that would give the anatomical and functional bases of the cardiovascular system. This historic moment includes: 1. the description and analysis of the function of blood and its components; 2. the description of the normal and abnormal Anatomy of the human heart and its vessels; 3. the anatomic and functional correlation: Foundation of the deductive thinking, and 4. the anatomic and functional integration with the clinic. Finally, the third wave, which is living today, is the stage of the technological explosion that begins with procedures as thoracoscopic surgery with the concept of reducing surgical trauma through minimum approach surgery. Also the use of robotics to solve some of the alterations in the CC, another is hybrid procedures and finally the use of fetal cardiac surgery.

The majority of infants born with congenital heart disease are now expected to reach adulthood. A... more The majority of infants born with congenital heart disease are now expected to reach adulthood. As a group, however, those with defects of moderate or great complexity remain at risk for arrhythmias, heart failure, repeat interventions, and premature mortality in adulthood. The experience of living with a lifelong medical condition may contribute to unique psychosocial experiences and an increased risk of psychological distress, particularly depression and anxiety. Other potential challenges include impaired social interactions, managing the transition from pediatric to adult health care, and advance care planning. Given these psychosocial challenges, it is important to develop and evaluate strategies to optimize the quality of life of adults with congenital heart disease.

Objetivo. Morbilidad y defunciones por el reemplazo quirúrgico de la válvula pulmonar. Material y... more Objetivo. Morbilidad y defunciones por el reemplazo quirúrgico de la válvula pulmonar. Material y métodos. Cincuenta y siete pacientes con tetralogía de Fallot y otras cardiopatías con afectación de la válvula pulmonar, que ameritaron el reemplazo de esta válvula fueron seguidos. Resultados. Treinta y uno tenían tetralogía de Fallot, 31 atresia pulmonar, 14 (24.6%) con comunicación interventricular, cinco (8.8%) con tronco común, (7%) con doble vía de salida del ventrículo derecho con comunicación interventricular cuatro y un caso, respectivamente de enfermedad de Ebstein, transposición de los grandes vasos y comunicación interventricular. En 38 (67%) se implantó prótesis pulmonar, 34 biológicas. En 19 (33%) se realizó cirugía de Rastelli. En 11 (19%) hubo complicaciones tempranas, con falla cardiaca cuatro (7%), hipertensión arterial pulmonar dos (3.5%). En complicaciones tardías dos (3.5%) tuvieron falla del conducto valvulado. Fallecieron seis pacientes (10.5%) por choque cardiogénico. El tiempo promedio de seguimiento de los pacientes después de la cirugía fue de 37.6 meses (3-59) no requiriendo reoperación 49 casos, el 96% de los sobrevivientes. Conclusiones. Los resultados del seguimiento a corto, mediano y largo plazo de la serie de pacientes pediátricos intervenidos de sustitución de la válvula pulmonar, ya sea mediante prótesis mecánica, biológica o cirugía de Rastelli, muestran una morbilidad y mortalidad bajas, independientemente de la etiología.

Las fístulas arteriovenosas, también llamadas malformaciones arteriovenosas pulmonares (MAVP), co... more Las fístulas arteriovenosas, también llamadas malformaciones arteriovenosas pulmonares (MAVP), corresponden a un reducido grupo de patologías vasculares que en un 85% se originan de la comunicación de una arteria aferente con una vena eferente. Las congénitas son las menos frecuentes. Se presenta el caso de una adolescente de 13 años, asintomática, con soplo continuo y saturación periférica de 88%. Mediante estudios de imagen se corrobora la presencia de una fistula arteriovenosa a nivel pulmonar, que es cerrada por intervencionismo con dispositivo oclusor. Una vez tratado el defecto, la evolución de la paciente es satisfactoria. Conclusiones: Mediante el caso se abordan las herramientas clínicas y diagnósticas de las MAVP, así como las indicaciones de los modos terapéuticos.

Objetivo. Morbilidad y defunciones por el reemplazo quirúrgico de la válvula pulmonar. Material y... more Objetivo. Morbilidad y defunciones por el reemplazo quirúrgico de la válvula pulmonar. Material y métodos. Cincuenta y siete pacientes con tetralogía de Fallot y otras cardiopatías con afectación de la válvula pulmonar, que ameritaron el reemplazo de esta válvula fueron seguidos. Resultados. Treinta y uno tenían tetralogía de Fallot, 31 atresia pulmonar, 14 (24.6%) con comunicación interventricular, cinco (8.8%) con tronco común, (7%) con doble vía de salida del ventrículo derecho con comunicación interventricular cuatro y un caso, respectivamente de enfermedad de Ebstein, transposición de los grandes vasos y comunicación interventricular. En 38 (67%) se implantó prótesis pulmonar, 34 biológicas. En 19 (33%) se realizó cirugía de Rastelli. En 11 (19%) hubo complicaciones tempranas, con falla cardiaca cuatro (7%), hipertensión arterial pulmonar dos (3.5%). En complicaciones tardías dos (3.5%) tuvieron falla del conducto valvulado. Fallecieron seis pacientes (10.5%) por choque cardiogénico. El tiempo promedio de seguimiento de los pacientes después de la cirugía fue de 37.6 meses (3-59) no requiriendo reoperación 49 casos, el 96% de los sobrevivientes. Conclusiones. Los resultados del seguimiento a corto, mediano y largo plazo de la serie de pacientes pediátricos intervenidos de sustitución de la válvula pulmonar, ya sea mediante prótesis mecánica, biológica o cirugía de Rastelli, muestran una morbilidad y mortalidad bajas, independientemente de la etiología.

Las fístulas arteriovenosas, también llamadas malformaciones arteriovenosas pulmonares (MAVP), co... more Las fístulas arteriovenosas, también llamadas malformaciones arteriovenosas pulmonares (MAVP), corresponden a un reducido grupo de patologías vasculares que en un 85% se originan de la comunicación de una arteria aferente con una vena eferente. Las congénitas son las menos frecuentes. Se presenta el caso de una adolescente de 13 años, asintomática, con soplo continuo y saturación periférica de 88%. Mediante estudios de imagen se corrobora la presencia de una fistula arteriovenosa a nivel pulmonar, que es cerrada por intervencionismo con dispositivo oclusor. Una vez tratado el defecto, la evolución de la paciente es satisfactoria. Conclusiones: Mediante el caso se abordan las herramientas clínicas y diagnósticas de las MAVP, así como las indicaciones de los modos terapéuticos.
