Subcoracoid dislocation of the outer end of the clavicle in association with an acromial fracture... more Subcoracoid dislocation of the outer end of the clavicle in association with an acromial fracture is a rare injury usually produced by high energy trauma that moves the clavicle posterior to the conjoined tendon and inferior to the coracoid process. To restore the acromioclavicular joint, open reduction is needed. After the reduction is accomplished, the clavicle has to be stabilized with a coracoclavicular screw. The acromial fracture can be reduced and held with nonabsorbable sutures placed through the bone. To avoid osteolysis of the distal end of the clavicle and to be able to repair the torn ligaments, treatment should not be delayed.

The importance of the preservation of the subacromial arch has been stressed recently, especially... more The importance of the preservation of the subacromial arch has been stressed recently, especially in irreparable lesions of the rotator cuff to prevent anterosuperior migration of the humeral head. The purpose of this article is to describe the surgical technique of a modified open anterior acromioplasty performed through an intra-acromial osteotomy that increases the subacromial space and preserves the insertion of the coracoacromial ligament on the undersurface of the acromion. To compare this new technique with classical acromioplasty, a prospective but nonrandomized study was performed including 20 patients undergoing open anterior acromioplasty and 22 patients undergoing a modified open anterior acromioplasty. At a mean follow-up of 18 months, no differences related to shoulder function as evaluated by the Constant score were found between these two groups. This modified acromioplasty increases the subacromial space, preserving the anatomy of the subacromial arch, and provides functional results as good as those obtained with classical open acromioplasty.

Objetivo Determinar la influencia de la versión protésica y la consolidación de la tuberosidad ma... more Objetivo Determinar la influencia de la versión protésica y la consolidación de la tuberosidad mayor en el resultado funcional final de las hemiartroplastias de hombro por fractura del tercio proximal de húmero. Material y método Estudio prospectivo de 32 hemiartroplastias con un seguimiento medio de 43,51 meses. Se ha efectuado un control radiológico en el postoperatorio inmediato y al final del seguimiento para evaluar la reducción obtenida del troquíter. También al final del seguimiento se ha realizado un estudio mediante tomografía axial computarizada (estudio de retroversión del implante), una evaluación funcional (escala funcional de Constant) y una valoración del estado de salud (EuroQol-5D). Resultados El valor global medio de la escala de Constant de la serie fue de 61,12. En cuanto a la retroversión, en 8 pacientes se obtuvo un valor entre 20-40°, en 20 pacientes un valor entre 0-20° y en 4 pacientes anteversión superior a 0°. We report on the concordance and reproducibility of the evaluation of radiolucent lines in the hu... more We report on the concordance and reproducibility of the evaluation of radiolucent lines in the humeral component of shoulder arthroplasty. Thirty-two shoulder prostheses were assessed independently, on two occasions, by five observers. The level of inter- and intra-observer agreement was calculated using the kappa statistic. Intra-observer agreement: the overall kappa values ranged from 0 to 0.6, meaning poor, fair and moderate agreement levels. Inter-observer agreement: when the anteroposterior (AP) views were analysed, the values obtained for the bone–cement interface ranged from 0.290 to 0.539, meaning a poor-to-moderate agreement. For the cement–implant interface, the values ranged from 0.064 to 0.684, meaning a poor-to-good agreement. When radiolucent lines of the humeral component were analysed, inter-observer agreement proved to be as low as that obtained when total hip or knee components were analysed. Intra-observer agreement showed better results.

Several intrinsic and extrinsic factors have been advocated in the pathogenesis of rotator cuff t... more Several intrinsic and extrinsic factors have been advocated in the pathogenesis of rotator cuff tears, but it is still unclear whether the origin of the tear is related to tendon degeneration itself or induced by several morphologic changes. The purpose of this study is to determine the relationship between the acromial coverage of the humeral head and the presence of a cuff tear. We evaluated 148 shoulders, including 45 that underwent surgical rotator cuff repair (group I), 26 with documented rotator cuff tears treated conservatively (group II), and 77 with no cuff pathology as a control group (group III). The mean acromial coverage index was 0.68 in group I, 0.72 in group II, and 0.59 in group III, giving a highly significant difference (P Ͻ .0001) between the control group and both cuff tear groups. Patients with a cuff tear have a significantly higher acromial coverage index than the control group.

Robotics has shown its potential not only in assisting the surgeon during an intervention but als... more Robotics has shown its potential not only in assisting the surgeon during an intervention but also as a tool for training and for surgical procedure's evaluation. Thus, robotics can constitute an extension of simulators that are based on the high capabilities of computer graphics. In addition, haptics has taken a first step in increasing the performance of current virtual reality systems based uniquely on computer simulation and their corresponding interface devices. As a further step in the field of training and learning in surgery, this work describes a robotic experimental workstation composed of robots and specific measuring devices, together with their corresponding control and monitoring strategies for orthopaedic surgery. Through a case study, humerus arthroplasty, experimental evaluation shows the possibilities of having a test bed available for repetitive and quantifiable trials, which make a reliable scientific comparison between different surgical strategies possible.
