The Authors report a retrospective review of 224 cases of transitional cell carcinoma of the blad... more The Authors report a retrospective review of 224 cases of transitional cell carcinoma of the bladder observed between 1986 and 1990. Squamous metaplasia was present in 45 case. This histological feature was found especially in association with poorly differentiated and invasive tumours: the presence of extensive areas of squamous metaplasia (metaplastic areas = or greater than 50% of neoplastic surface) was always found in association with high grade neoplasms and with a disease-free period shorter than transitional cell carcinoma. The Authors conclude that squamous metaplasia in transitional cell carcinoma of the bladder is a really unfavourable prognostic factor.

The Authors report their personal experience of pure squamous cell carcinoma of the bladder. Betw... more The Authors report their personal experience of pure squamous cell carcinoma of the bladder. Between 1984 and 1988 they have observed 389 tumors of the bladder; squamous cell carcinoma was present in fifteen cases (3.8%). Epidemiology, pathology (gross configuration of tumor, histologic grading, staging according to the criteria of the UICC of 1987), presenting symptoms, treatment and survival have been considered. The Authors underline the importance of a careful histologic examination of the vesical tumors.

The case of a 56 year-old male patient with asynchronous isolated metastases to the ureteral stum... more The case of a 56 year-old male patient with asynchronous isolated metastases to the ureteral stump and bladder from renal cell carcinoma is presented. Ureteral metastasis was discovered 2 years after nephrectomy. Cystoscopy and MRI were performed. An excision of the ureteral stump with a cuff of bladder was carried out. No other metastasis was observed. Six months later a transurethral resection was performed on account of metastasis of the right wall of the bladder. Pathological examination demonstrated a renal cell carcinoma metastasis. After 6 months a new vesical metastasis from renal cell carcinoma was found. The small nodule of the lower part of the right wall of the bladder was removed. The literature is reviewed and the presentation, metastatic pathways, management of ureteral stump and bladder metastases from renal cell carcinoma are discussed.

Primary renal sarcomas are rare and constitute about 3 % of all malignant renal neoplasms. They a... more Primary renal sarcomas are rare and constitute about 3 % of all malignant renal neoplasms. They are malignant mesenchymal tumors of the kidney with a variety of histological types. In the past, these were variously classified as fibrosarcoma, leiomyosarcoma, angiosarcoma, liposarcoma, etc., but often they were also grouped under the common term sarcoma. As a result much confusion prevailed regarding the incidence of each of these entities. With recent advancements in histochemical techniques classification has been improved. We treated three cases of leiomyosarcoma and tree of fibrosarcoma. CLINICAL MATERIAL. The series included six cases of sarcomas seen at our division between 1973 and 1985. There were four women and two men. Patient age ranged from 38 to 74 years, with an average of 54 years. The right side was involved in 5 patients and the left side in one. IVP was performed in all cases, ultrasonography in four, CT scan in two and we required the use of selective arteriography in only one case. A definite diagnosis could not be made following these investigations. Retrospective immunohistochemical studies of neoplastic tissue fixed in formaline and set in paraffin were done to test the presence of cytocheratin, vimentin, desmin, and actin, in an attempt to identify the cell types of the neoplastic lesions. All patients underwent surgical treatment only. No radiotherapy or chemiotherapy was given. The cases histories are presented. CASES REPORT. Case 1. P.B.A. a 38-year old female, was admitted complaining of left flank pain of three weeks duration with spontaneous emission of a small-stone. She denied other symptoms. On physical examination she had pain during palpation of the abdomen, but no mass was found. Chest-X ray was negative. Laboratory findings (except for a urea level of 6,4 mg/dl; n. v.: 2,4-5,7 mg/dl) were within normal limits. IVP showed a soft tissue mass occupying the lower pole of the left kidney. In two cases the neoplasm was an adrenal adenoma, one causing a Cushing syndrome, the other clinically asymptomatic and associated with renal tuberculosis. In two patients the neoplasm was a clinically unsuspected pheochromocytoma and in one case it proved to be a malignant tumor.

We report a case of sarcomatoid renal cell carcinoma and simultaneous transitional cell carcinoma... more We report a case of sarcomatoid renal cell carcinoma and simultaneous transitional cell carcinoma of the renal pelvis in a 77 year old man admitted for uroseptic fever persisting for two months. Seven years earlier he underwent cystectomy with ureterosigmoidostomy for transitional cell carcinoma of the bladder. CT scan described a severe hydronephrosis with dilated pelvis, several pseudocystic formations with renal parenchima thinning and absence of contrast excretion. Radical nephrectomy was performed consequent to a clinical diagnosis of uroseptic fever in secondary hydronephrosis due to stenosis of ureterosigmoidostomy. Tumors were suspected on cut section and confirmed by histological examination.

To assess effectiveness and durability of Solifenacin (SS) versus tibial nerve stimulation (PTNS)... more To assess effectiveness and durability of Solifenacin (SS) versus tibial nerve stimulation (PTNS) versus combination therapy (PTNS + SS) in women with overactive bladder syndrome (OAB). Materials and Methods: 105 women with OAB were divided randomly into three groups of 35 patients each. In group A women received SS, in group B women underwent PTNS, in group C women underwent combination of PTNS + SS. Improvements in OAB symptoms were assessed with OABSS questionnaire; patients' quality of life was assessed with OAB-q SF questionnaire. Evaluation of effectiveness of treatments was performed with PGI-I questionnaire. OABSS and PGI-I were also assessed monthly for ten months. Results: All treatments were effective on symptoms. PTNS showed a greater effectiveness than SS, but PTNS + SS was more effective than SS and PTNS. Furthermore, PTNS + SS showed a greater duration of effectiveness than PTNS and SS. Conclusions: Combination of PTNS with SS showed more effectiveness and more durability than PTNS and SS alone.

STUDY DESIGN A total of 49 women hospitalized for uterine myoma were eval- uated in this prospective study. Patients underwent- a detailed clinical evaluation and ukodynamic study. Uri- nary symptoms were as- sessed using 3-day micturi- tion diary and Patient Perception of Intensity of Urgency scale questionnaire. The dimensions of fibroma- tous uterus and single large myomas were evaluated by both transabdominal and transvaginal ultrasound. Urodynamic parameters and urinary symptoms were assessed before and 4 months after surgery. RESULTS A total of 44 patients were evaluable at the end of the study: 32 out of 44 (72.8%) had urinary symptoms; 26 women underwent hysterectomy, and 18 had abdominal myomectomy.
