La cirugía de aumento mamario es uno de los procedimientos más realizados a nivel mundial de índo... more La cirugía de aumento mamario es uno de los procedimientos más realizados a nivel mundial de índole estético reconstructivo. Dentro de las indicaciones para la realización del procedimiento se encuentran alteraciones primarias o secundarias asociadas a la forma y volumen mamario. En la valoración inicial se debe enfatizar en la anamnesis y un examen físico adecuado permitiendo dimensionar metas y expectativas que permitan prospectivamente determinar el grado posible de conformidad posoperatorio. Existen diversos métodos para la selección del implante que a través del tiempo han permitido la aplicación de tecnología para lograr mayor exactitud. Diversos materiales se han desarrollado, siendo la silicona hasta el momento la más utilizada mundialmente con tasas bajas de complicaciones dependiendo de cada paciente y la técnica usada. Se considera una de las cirugías estéticas reconstructivas con mayor grado de aceptación dentro de la población general que se somete a este tipo de procedimiento.

La terapia con presion negativa topica (PNT) es una manera de actuar positivamente en el ambiente... more La terapia con presion negativa topica (PNT) es una manera de actuar positivamente en el ambiente de la herida aplicando presion subatmosferica, acelerando el proceso de curacion de la misma. Se realiza una presentacion actualizada del tratamiento con PNT, su desarrollo, fundamentos teorico-practicos y aplicaciones clinicas. A pesar que aun faltan estudios rigurosos que permitan evaluar la superioridad de la PNT versus otros metodos terapeuticos, la experiencia internacional ganada con su aplicacion es vasta y positiva. Se presenta la experiencia con el uso de la terapia con PNT en Uruguay, ilustrandola con un caso clinico.

The abdominal wall is a multilayered, complex structure that is frequently operated on with recon... more The abdominal wall is a multilayered, complex structure that is frequently operated on with reconstructive and cosmetic objectives. A better understanding of the surgical implications of its components is essential to improve techniques and results. Anatomic and physiological aspects pertinent to cosmetic and transverse rectus abdominis musculo-cutaneous (TRAM) flap donor site closure is reviewed both in the superficial and musculoaponeurotic layers. Anatomic elements are examined as components of a functional system rather than as isolated units. The superficial anatomy is described as related to cosmetic procedures. Several underlying components of the abdominal wall are reviewed. Current concepts on the subcutaneous fat and Scarpa's fascia are presented. At the musculoaponeurotic plane, the importance of the rectus sheath and linea alba as the dynamic center of the abdominal wall is underlined, and consequently, their importance in terms of TRAM flap closure. The transversus muscle as a circular compressor of the abdomen is discussed, as well as its potential use for better waist definition.

Since 1984, we have used the buccinator muscle mucosal flap for the treatment of mucosal defects ... more Since 1984, we have used the buccinator muscle mucosal flap for the treatment of mucosal defects after tumor resection, osteomyelitis of the mandible, closure of cleft palate fistulas, primary closure of very wide cleft palates, and lengthening of the soft palate. A study was made in the facial regions of 14 cadavers, and a comparison was made to descriptions found in the anatomic literature. It was confirmed in our dissections that the buccal artery, which reaches the posterior half of the muscle, is the major arterial pedicle of the buccinator and that it runs very close to the buccal nerve. Several veins originating from the lateral aspect of the muscle make the venous drainage of the buccinator even richer than its arterial supply. The abundant vascular supply from multiple interconnected pedicles supports the blood supply of the buccal mucosa. The motor innervation of the buccinator muscle comes from the facial nerve. The buccinator is considered to be a part of the sphincteric muscular system involving the functions of sucking, whistling, propelling food during mastication, and voiding the buccal cavity. The flap was utilized clinically in 38 patients: 24 to close primary cleft palates that required palate lengthening, 12 to close palatal fistulas, 1 to treat a mandibular osteitis, and 1 to repair the palate after tumor resection. We had three small fistulas as complications in our series owing to technical mistakes.

Different problems can be encountered when planning a lower blepharoplasty, such as festoons, pro... more Different problems can be encountered when planning a lower blepharoplasty, such as festoons, prominent fat bags, and a conspicuous transition from eyelid to cheek. Frequently, they occur simultaneously. In such cases, a larger amount of skin excision is needed to achieve an adequate correction. This carries an increased risk of eyelid retraction when dealing with hypotonic eyelids. Orbicularis muscle suspension can provide vertical support to the eyelid and so achieve a safer, more effective correction. Several techniques have been reported to obtain this effect. We have found a laterally based transposition orbicularis flap to be a safe end effective method to transmit a controlled amount of traction to the lower lid.
