The impact of diabetes on bypass graft patency, development of new lesions and the rates of re-operation, re-intervention w ere assessed angiographically in 1 O 1 diabetic and 309 non-diabetic patients who had been operated between 1992-2001. The mean period of control angiography was 53.4 ±21.±2 vs 54.0 ±22.6 months. C ompared w ith nondiabetic patients, the group w ith diabetes was older (61.3 ±10. 7 years versus 59.4 ±11.2 years), comprised more women (26.7% versus 11.7% p=O,OOJ ), had more comman triple-vessel disease (55.4 % versus 41.7%, p=0,02) and had lower ejectionfractions (54.5 ±8.9 versus 56.6 ±8.5, p=0.03). Coronary artery aneurysms are rare entities most commonly detected during coronary angiography in... more Coronary artery aneurysms are rare entities most commonly detected during coronary angiography incidentally. The incidence has increased during the recent years due to widespread use of drug eluting stents. Stasis with the aneurysm may lead to thrombosis and occurrence of myocardial ischemic symptoms. Percutaneous intervention strategies and surgical options are available. Here, we report a case with distal left main coronary artery aneurysm presented with acute coronary syndrome and treated successfully with surgery.

Ischaemic heart disease and fistulous communication between the LAD artery and the main pulmonary artery was diagnosed in two brothers aged 48 and 49. They had coronary artery bypass grafting and also the fistulae were ligated in both. This seems to be the first report of congenital coronary artery fistula and obstructive coronary artery disease in two brothers. Detection of coronary artery is important when considering surgical treatment of a congenital heart disease (CHD) such as tetralogy of Fallot (TF). Cardiac catheterization plays an important supplementary role in the evaluation of patients with CHD. In a few reports, it has been proposed that multidetector computed tomography (MDCT) can be helpful for the visualization of coronary arteries. We sought to demonstrate the feasibility and usefulness of MDCT angiography for anatomical evaluation of coronary arteries in CHD patients with suspected coronary artery anomalies. Materials and Methods. A total of 10 patients, 9 pediatric and 1 adult, underwent MDCT angiography for the investigation of coronary artery anomalies and mediastinal vascular structures. Seven patients had TF; 5 of these patients were suspected of having coronary artery anomalies and 2 were suspected of having pulmonary artery atresia or a nonconfluent pulmonary artery. The other 3 patients had truncus arteriosus and severe left pulmonary artery stenosis (n = 1), double outlet right ventricle (n = 1), and Kawasaki disease (n = 1) with suspected coronary artery aneurysms. The entire heart, major vascular structures, and coronary artery anomalies were preoperatively scanned in patients with cyanotic heart disease. Examinations were performed by 16-MDCT with 1-mm slice thickness. A breath-holding test was performed in 5 patients. Nonionic iodinated contrast material (2 cc/kg) was administered by a power injector. Results. Major vascular structures and the proximal part of the right and left coronary arteries were visualized successfully in all patients. Mid and/or distal segments of the coronary arteries were visualized in 5 patients with TF. Pulmonary vascular bed findings were also confirmed during surgery in patients with TF and in one patient with truncus arteriosus type I and severe left pulmonary artery stenosis. Kawasaki disease was diagnosed by the presence of aneurysms in one patient. Pulmonary artery atresia was confirmed in one patient and diameter of the pulmonary arteries (4 mm and 4.5 mm) was determined in the other 2 patients by MDCT. Conclusion. The advantage of MDCT for cardiac imaging is the shortened scanning time for imaging the entire heart without long breath-holding times. Selective conventional coronary angiography is invasive and technically difficult in pediatric patients. We suggest that MDCT angiography can be performed as a noninvasive method in patients with CHD for the evaluation of coronary artery anatomy and anomalies and mediastinal vascular structures.

Chlamydia pneumoniae, which is a Gram(-) intracellular bacteria, besides being a respiratory path... more Chlamydia pneumoniae, which is a Gram(-) intracellular bacteria, besides being a respiratory pathogen, is thought to play an active role in the progress of acute myocardial infarction and chronic coronary artery disease. In this study we aim to determine the frequency of C. pneumoniae in coronary artery lesions of Turkish people. The atherosclerotic material taken from 8 cases by directional atherectomy and from 23 cases by surgical endarterectomy and examined by indirect immunofluorescence (IIFA) test and polymerase chain reaction (PCR). C. pneumoniae positivity was 32.3% (10/31) by IIFA and 29.0% (9/31) by PCR while the evaluation of the methods together yield a positivity of 35.5% (11/31). A statistically significant difference could not be established between C. pneumoniae positive and negative groups according to age and the classical atherosclerotic risk factors such as diabetes mellitus, smoking, hypercholesterolaemia, hypertension, family history; besides, a statistically significant difference could not be found between the presence of C. pneumoniae and the severity and clinical picture of coronary artery disease.

Pedicle preparation and pharmacological vasodilatory treatment vary greatly. This clinical study was performed in order to define the effectiveness of two different applications of sodium nitroprusside as vasodilating agent. Methods: Eighty-six consecutive patients whose left ITA was mobilized only by one surgeon for elective coronary artery bypass graft operations were randomly divided into two groups. A secundum atrial septal defect is the most common congenital heart defect. Transcatheter treatme... more A secundum atrial septal defect is the most common congenital heart defect. Transcatheter treatment of secundum atrial septal defects is a popular and less invasive alternative to surgery. Procedural complications may occur in a wide spectrum, particularly device embolus as the most emergent one, but luckily they do not commonly occur in the clinical setting. Mortality from adverse events related to transcatheter treatment strategies is twentyfold higher than that of primary elective surgical closure. Here, we report an Amplatzer device embolus in a secundum atrial septal defect patient. The device was successfully removed with surgery, postoperative course was uneventful, and the patient was discharged from the hospital on the 5 th postoperative day.

The aim of this retrospective study is to investigate the safety and efficacy of endovascular rep... more The aim of this retrospective study is to investigate the safety and efficacy of endovascular repair of aortic lesions with the Medtronic Talent stent-graft system and to present mid-term results of endovascular aortic repair performed in our center. Between December 2002 and March 2007, 54 patients (6 women) with aortic (14 thoracic and 40 abdominal) lesions underwent treatment with Talent stent-graft. The average age of the patients was 64.8 (20-88) years. Duration of follow-up period ranged from 1 to 49 months (average 21 months). Indications for endovascular repair were degenerative aneurysm in 45, degenerative aneurysm and penetrating ulcer in 2, only penetrating ulcer in 1, traumatic thoracic isthmic transsection in 4 and vasculitic aneurysm secondary to Behcet's disease in 2 patients. Repair was performed with the tubular (16), aortouniiliac (1) or bifurcated (37) stent-grafts. Technical success rate was 100%. No death, major complication or need of immediate conversion to open repair was seen. Endoleak rate was 18.5% at 1 month follow-up period. Thirty-day mortality was 1.8% and morbidity (other than endoleaks) rate was 12.9%. During the follow-up period, secondary intervention was required in 12.9% of patients. Iliac limb occlusion was detected in 1 patient (1.8%). Graft migration causing type 1 endoleak and requiring open surgical treatment was seen in 1 patient (1.8%). Four patients (7.4%) are still under follow-up for type-2 endoleaks that do not require intervention. No graft infection or death due to aneurysm rupture was detected. Endovascular treatment of aortic lesions in selected patients with comorbid conditions using the Talent stent-graft exhibits a high degree of technical success with a low perioperative morbidity and mortality rate. The major disadvantage of endovascular aortic repair is necessity of life-long imaging follow-up and secondary interventions.

Purpose: The aim of this study is to compare the long-term graft patency between patients who had... more Purpose: The aim of this study is to compare the long-term graft patency between patients who had sequential or individual right posterior descending artery (RPDA) anastomosis. Materials and Methods: Two hundred and forty-two patients underwent coronary artery bypass grafting (CABG) between June 1994 and December 2003. They were examined retrospectively with respect to coronary angiograghic data. [Group 1] Individually right system grafts in RPDA position (n=139). [Group 2] RPDA anastomosis sequentially with left system (n=103). Patency rates for posterior descending arteries in each group were separately calculated for each vessel quality category. Results: The mean interval from operation to angiography was 50.6ȗ48.9 months in group 1 vs 57.5ȗ39.2 months in group 2 respectively. The overall patency rate was 66.2% (92/139) in group 1 and 78.6% (81/103) in group 2 (p=0.04). When the RPDA has good runoff capacity, the patency rate was 69.1% in group 1 and 85.2% in group 2. Conclusion: When the RPDA has good runoff capacity, snake grafts show excellent results and right and left coronary systems could be anastomosed sequentially.

We designed this study to compare long-term results of St. Jude Medical and Car-boMedics mechanic... more We designed this study to compare long-term results of St. Jude Medical and Car-boMedics mechanical heart valves in the aortic position. We retrospectively analyzed the results of 174 consecutive patients who received either a St. Jude (n=80) or a Car-boMedics (n=94) mechanical aortic valve from March 1992 through October 2004. The follow-up rate was 97.7%. The mean follow-up duration for the St. Jude group was 79.3 ± 35.0 and for the CarboMedics group, 70.0 ± 34.3 months. The cumulative follow-up was 523.8 and 530.1 patient-years, respectively. The 30-day mortality rates for the St. Jude and CarboMedics patients were 1.3% and 3.2%, respectively. The actuarial survival rate for the St. Jude group at 138.0 ± 4.7 months was 75.9% ± 0.1% and for the CarboMedics group at 130.8 ± 4.8 months was 69.8% ± 0.1% (P=NS). There was no structural valve deterioration in either group. Freedom from thromboembolic events was 87.7% for the St. Jude group and 83.0% for the CarboMedics group (P= NS). Freedom from bleeding events for the St. Jude group was 93.6% and for the Car-boMedics group, 89.7% (P=NS). The results obtained from this study indicate that standard St. Jude Medical and Car-boMedics aortic valve prostheses offer similar excellent clinical performance. Definitive judgment must await trials that are extensive, randomized, and prospective.

The results obtained from this study indicate that standard St. Jude Medical and Car-boMedics aortic valve prostheses offer similar excellent clinical performance. Definitive judgment must await trials that are extensive, randomized, and prospective.
