A 60-year-old male endured multiple erythematous white-scaled maculopapules over his trunk, back, and four extremities after 2 months of the 6-weekly intravesical instillation of BCG. Skin biopsy revealed parakeratotic hyperkeratosis, intracorneal neutrophilic Munro's abscess, and acanthosis. Guttate psoriasis was diagnosed, and he was treated successfully with topical steroid and narrow band UV-B phototherapy. We present the first Asian case of guttate psoriasis associated with intravesical BCG immunotherapy and review the literatures.

AIM: To investigate the effect of epidermal growth factor (EGF) on mucosal healing in rats with d... more AIM: To investigate the effect of epidermal growth factor (EGF) on mucosal healing in rats with duodenal ulcer. METHODS: Male Sprague-Dawley rats were randomly divided into sham operation without EGF, sham operation with EGF, duodenal ulcer without EGF, or duodenal ulcer with EGF groups. Additionally, normal rats without operation served as the control group. Duodenal ulcer was induced in rats by 300 mL/L acetic acid. Rats with EGF were orally administered at a dose of 60 µg/kg/day in drinking water on the next day of operation (day 1). Healing of duodenal ulcer was detected by haematoxylin and eosin staining. Cell growth of damaged mucosa was determined by the contents of nucleic acids and proteins. The level of EGF in duodenal mucosa was measured by ELISA. RESULTS: The pathological results showed that duodenal ulcer rats with EGF improved mucosal healing compared with those without EGF after day 5. Duodenal ulcer rats with EGF significantly increased duodenal DNA content compared with those without EGF on day 15 (6.44±0.54 mg/g vs 1.45±0.52 mg/g mucosa, P<0.05). Duodenal RNA and protein contents did not differ between duodenal ulcer rats with and without EGF during the experimental period. Sham operation and duodenal ulcer rats with EGF significantly increased duodenal mucosal EGF content compared with those without EGF on day 5 (76.0±13.7 ng/g vs 35.7±12.9 ng/g mucosa in sham operation rats, and 68.3±10.9 ng/g vs 28.3±9.2 ng/g mucosa in duodenal ulcer rats, P<0.05). CONCLUSION: Oral EGF can promote mucosal healing of the rats with duodenal ulcer by stimulating mucosal proliferation accompanied by an increase in mucosal EGF content.

The study investigated the association between lower urinary tract symptoms (LUTS) and sexual dys... more The study investigated the association between lower urinary tract symptoms (LUTS) and sexual dysfunction in ageing men. It was a cross-sectional study in an unselected consecutive sample of 398 men aged &amp;gt;40 years attending a urology clinic. LUTS and sexual function were assessed by validated symptom scales, including the International Prostate Symptom Score (IPSS) and the International Index of Erectile Function-5 (IIEF-5). Clinically the severity of total and obstructive IPSS showed no significant correlation with age, but irritative IPSS is statistically correlated with age (P &amp;lt; 0.05). The prevalence of moderate to severe ED (IIEF-5 &amp;lt; 12) was significantly associated with LUTS severity (P &amp;lt; 0.05) and the severity of IIEF-5 correlated significantly with age (P &amp;lt; 0.01). A consistent inverse correlation was found between IIEF-5 and IPSS severity across the age groups, with the strongest effect within aged 60 to 69 years (r = -0.286, P &amp;lt; 0.01). The irritative IPSS showed a significant correlation with IIEF-5 severity across all age groups. These results reveal a significant correlation between LUTS and the severity of ED, especially on the irritative domain.

Our aim is to compare the magnetic resonance imaging (MRI) appearances of urinary bladder in recreational ketamine users with lower urinary tract symptoms (LUTS) and healthy subjects. Materials and Methods: MRI of pelvis was performed for 10 consecutive ketamine abusers with KC and 16 healthy young adults at a medical center in the northern Taiwan from January to December, 2011. The thickness of the bladder wall was measured and analyzed. All patients with KC had been diagnosed based on their history, clinical features and urodynamic study. Results: The mean bladder wall thickness of KC patients was signi cantly thicker than the control group (1.45±0.23 cm vs. 0.62 ± 0.50 cm, p < 0.001). There is signi cant positive correlation between the duration of ketamine usage and the thickness of bladder wall (r = 0.891, p =0.001); whereas the dosage of ketamine had weak positive correlation with the thickness of bladder wall (r = 0.413, p =0.235). Furthermore, the thickness of bladder wall was also strong correlated with maximum urethral closure pressure (MUCP; r = 0.855, p =0.002). Conclusions: Ketamine-associated cystitis causes increased bladder wall thickness which may be related to the bladder outlet obstruction. MRI is a useful tool to evaluate the bladder wall thickness and contributes to the diagnosis of ketamine associated cystitis.

The study aimed to determine effects of a ketogenic diet on metabolic dysfunction, testicular ant... more The study aimed to determine effects of a ketogenic diet on metabolic dysfunction, testicular antioxidant capacity, apoptosis, inflammation, and spermatogenesis in a high-fat and high-cholesterol diet-induced obese mice model. Forty-two male C57BL/6 mice were fed either a normal diet (NC group) or a high-fat and high-cholesterol (HFC) diet (HFC group) for 16 weeks, and mice from the HFC group were later randomly divided into two groups: the first were maintained on the original HFC diet, and the second were fed a medium-chain triacylglycerol (MCT)-based ketogenic diet for 8 weeks (KD group). A poor semen quality was observed in the HFC group, but this was eliminated by the ketogenic diet. Both the HFC and KD groups exhibited enhanced apoptosis protein expressions in testis tissue, including caspase 3 and cleaved PARP, and higher inflammation protein expressions, including TNF-α and NF-κB. However, the KD group exhibited a statistically-significant reduction in lipid peroxidation and an increased glutathione peroxidase level as compared with the HFC group. The HFC diet induced obesity in mice, which developed body weight gain, abnormal relative organ weights, metabolic dysfunction, and liver injury. Overall, the results showed that a ketogenic diet attenuated oxidative stress and improved the semen quality reduced by the HFC diet.

Objective This study investigated the effect of resistant maltodextrin (RMD) on reproduction in s... more Objective This study investigated the effect of resistant maltodextrin (RMD) on reproduction in streptozotocin (STZ)-induced type 2 diabetic male rats. Methods Forty male rats were induced with diabetes by a single intraperitoneal injection of STZ (50 mg/kg) and nicotinamide (100 mg/kg). Five groups were analyzed in total: normal, Diabetic rats without RMD, diabetic rats with RMD 1.2 g/100 g diet (1×), with RMD 2.4 g/100 g (2×), and with RMD 6.0 g/100 g (5×). Results The groups of diabetic rats with the RMD supplement, compared to those without supplement, showed improved plasma glucose control, attenuated insulin resistance and recovery of testosterone level & spermatogenesis stage. The STZ-induced diabetes mellitus (DM) caused a significant reduction in serum testosterone, testis androgen receptor (AR), steroidogenic acute regulatory protein (StAR) and 3β-hydroxysteroid dehydrogenase (3β-HSD) protein, but a statistical recovery in each of these was observed in the 5× group. TUNEL-positive cells were observed in the Diabetic without RMD group and RMD treatment reduced apoptotic germ cells. The expression of Bax/Bcl2 was induced in the diabetic group and also significantly reduced in the 5× group. Conclusions Dietary RMD may improve metabolic control in STZ-induced diabetic rats and attenuate hyperglycemia-related impaired male reproduction and testicular function.

In upper urinary tract (UUT), squamous cell carcinoma (SCC) and urothelial carcinoma (UC) are two... more In upper urinary tract (UUT), squamous cell carcinoma (SCC) and urothelial carcinoma (UC) are two different disease in its etiology, incidence and prognosis. Due to paucity of the study in SCC, we treat it just as UC. However, we believe the therapeutic and following protocols for the two diseases are different. In this study, we compare the bladder recurrence rate in these two malignancies. Materials and Methods: From January 2000 to December 2007, 327 patients were diagnosed as upper urinary tract malignancy. Their clinical and pathological characteristics were retrospectively reviewed. There were 294 pure UC (90.0%), 24 UC with squamous differentiation (SqD) (7.3%) and 9 pure SCC (2.7%) patient. These patients all received RNU initially. Kaplan-Meier method was used to estimate bladder recurrence free survival (RFS). The recurrence curve is compared by log rank test. The statistical significance was set as p&0.05. Results: The 5-year RFSs of bladder were 67.9%, 70.4 and 100% in UC, UC with SqD and SCC groups respectively. Pairwise comparison was done., and here was no significant difference in bladder RFS between two groups. The p-value of log-rank test comparing UC and SCC is 0.061. Although not statistically significant, there was a trend that patients with UUT-SCC had less bladder recurrence after initially operative management. Conclusion: There was a statistical trend that patients with UUT-SCC had less bladder recurrence after initially operative management. It did not reach significance because of the small number of patients with SCC.

This study explored the effects of coenzyme Q 10 (CoQ 10) on the testicular functions of male mic... more This study explored the effects of coenzyme Q 10 (CoQ 10) on the testicular functions of male mice exposed to cigarette smoke. Eight-week-old BALB/c male mice were divided into the following groups: the AV group (air with a vehicle), the AQ group (air with CoQ 10), the SV group (smoke with a vehicle), and the SQ group (smoke with CoQ 10). The results showed that the CoQ 10 concentrations in the sera and testes were decreased in the groups subjected to smoke but they were improved after the administration of CoQ 10. Neither smoke nor CoQ 10 supplementation affected the serum or testis testosterone concentrations. Regarding the antioxidant system in the testis, the exposure to smoke induced malondialdehyde and hydrogen peroxide production and decreased the catalase and glutathione peroxidase activities. Oral CoQ 10 administration reversed the oxidative damage. In apoptosis, the cytochrome c, c-caspase 9, and c-caspase 3 proteins were increased in the groups exposed to smoke but they were decreased after the CoQ 10 administration. In mitochondrial biogenesis, smoke exposure led to decreases in the PGC1-α, NRF1, and NRF2 levels, but CoQ 10 increased the expressions of these proteins. Additionally, oral CoQ 10 administration improved the mitochondrial copy numbers that were reduced following the exposure to smoke. In summary, CoQ 10 administration reduces smoke-induced testicular damage by regulating the antioxidant capacity, the cell apoptosis, the mitochondrial biogenesis, and the copy numbers in the testes.

At present, robot-assisted radical prostatectomy (RARP) is a gold standard in radical prostatecto... more At present, robot-assisted radical prostatectomy (RARP) is a gold standard in radical prostatectomy. The aim of this study was to evaluate the incidence, risk factors, and timing of occurrence of inguinal hernia (IH) after RARP. We included 427 patients with prostate cancer who underwent RARP by a single surgeon from February 2006 to August 2017. Incidence, clinical, and pathological factors were investigated to assess relationship with the development of IH. Postoperative IH occurred in 29 cases (6.79% of all RARP patients), whereas 22 cases (75.9% of all IH patients) occurred within the first 2 years. The median follow-up period was 5.2 years, and the median age of patients was 65 years. Postoperative IH occurrence was significantly associated with body mass index (BMI), smoking history, and low surgeon experience (P = .036, .023, and .048, respectively). However, low surgeon experience did not reach statistical significance after multivariate analysis. The overall incidence of IH after RARP was significantly associated with BMI and smoking history. With obvious incidence of IH within the first 2 years after operation which was not observed at the open prostatectomy, RARP itself may play a role in the development of IH. Herein, we report a rare case of malignant glomus tumor arising from kidney. Case report: A 46 years old man with history of nasopharynx cancer, status post chemoraiation therapy, has right renal tumor found incidentally on a pulmonary computed tomography scan for following of nasopharynx cancer. A contrast enhanced computed tomography of abdomen showed a tumor about 4.4cm with heterogenous enhancement over medial upper pole of right kidney. The patient undertook laparoscopic right radical nephrectomy with right adrenalectomy and recovered well after the operation. According to the histologic and immunohistochemical examination, the tumor is diagnosed as malignant glomus tumor. Conclusion: Glomus tumors resemble glomus body, which is associated with body temperature regulation. Glomus tumors usually occur in the extremities, most common the subungual region of the fingers. Visceral organs are uncommon site involving development of glomus tumors. Glomus tumors arising from kidney are extremely rare, with a few cases described before. It is difficult to differentiate glomus tumor from other renal malignany preoperatively through imaging study. Histolgoy and immunohistochemical study are necessary to make a diagnosis of glomus tumor of the kidney. Renal glomus tumors are usually benign with good outcome and prognosis, with one exception of malignant glomus tumor been reported. We report a case of malignant glomus tumor of kidney. To our knowledge, this is the second case of malignant renal glomus tumor reported. Given the small number of cases reported to date and relative short-term follow up, close clinical follow-up is necessary.

Herein we present a rare complication of extraperitoneal abscess owing to intermittent urinary catheterization in a 62-year-old male who had cervical spine injury and was treated with intermittent urethral catheterization for neurogenic lower urinary tract dysfunction. Treatment and a literature review are also described.

Background: This study aimed to investigate the presence of circulating tumor cells (CTCs) in pat... more Background: This study aimed to investigate the presence of circulating tumor cells (CTCs) in patients with penile squamous cell carcinoma (PSCC). Methods: CTCs were isolated from 14 patients with PSCC, 6 patients with balanoposthitis, and 6 healthy individuals. CTCs were enriched based on cell surface markers and filtered through the IsoFlux device, followed by identification according to cell morphology and immunofluorescence studies. Results: CTCs were found in all PSCC blood samples but not in balanoposthitis samples and samples from healthy individuals. Immunofluorescence studies confirmed the tumor origin. When the patients with PSCC were stratified according to metastatic inguinal lymph node status, a statistically significant difference was observed in the number of detected CTCs. Conclusion: Our study showed that CTCs in PSCC may represent a valuable marker for differentiating PSCC from other tumors. Based on the correlation with some clinical parameters, CTC analysis is possibly relevant for noninvasive monitoring of disease progression and prognosis. The results also suggested a potential role of CTCs in preventing overtreatment, such as inguinal lymph node dissection.

Low-molecular-weight heparins (LMWHs) have been widely used in many clinical settings. LMWH-assoc... more Low-molecular-weight heparins (LMWHs) have been widely used in many clinical settings. LMWH-associated spontaneous retroperitoneal hematoma (SRH) has been reported mostly in patients treated with enoxaparin. We here present a 76-year-old man with renal insufficiency treated with dalteparin for acute myocardial infarction, who developed catastrophic SRH with intra-abdominal hypertension (IAH) and hemothorax. Because of the high risk of surgical intervention, we performed ultrasound-guided abdominal paracentesis and thoracentesis to relieve IAH and respiratory distress. Vigorous supportive care with massive blood product transfusions was given to stabilize his hemodynamics. Finally, the bleeding was stopped with a powerful hemostatic agent, recombinant factor VIIa. Surgical evacuation of a 3,600 mL blood clot was performed successfully 1 month later.

The STZ-induced diabetes mellitus (DM) caused a significant reduction in serum testosterone, testis androgen receptor (AR), steroidogenic acute regulatory protein (StAR) and 3β-hydroxysteroid dehydrogenase (3β-HSD) protein, but a statistical recovery in each of these was observed in the 5× group. TUNEL-positive cells were observed in the Diabetic without RMD group and RMD treatment reduced apoptotic germ cells. The expression of Bax/Bcl2 was induced in the diabetic group and also significantly reduced in the 5× group. Conclusions Dietary RMD may improve metabolic control in STZ-induced diabetic rats and attenuate hyperglycemia-related impaired male reproduction and testicular function.

Due to paucity of the study in SCC, we treat it just as UC. However, we believe the therapeutic and following protocols for the two diseases are different. In this study, we compare the bladder recurrence rate in these two malignancies. Materials and Methods: From January 2000 to December 2007, 327 patients were diagnosed as upper urinary tract malignancy. Their clinical and pathological characteristics were retrospectively reviewed. There were 294 pure UC (90.0%), 24 UC with squamous differentiation (SqD) (7.3%) and 9 pure SCC (2.7%) patient. These patients all received RNU initially. Kaplan-Meier method was used to estimate bladder recurrence free survival (RFS). The recurrence curve is compared by log rank test. The statistical significance was set as p&0.05. Results: The 5-year RFSs of bladder were 67.9%, 70.4 and 100% in UC, UC with SqD and SCC groups respectively. Pairwise comparison was done., and here was no significant difference in bladder RFS between two groups. The p-value of log-rank test comparing UC and SCC is 0.061. Although not statistically significant, there was a trend that patients with UUT-SCC had less bladder recurrence after initially operative management. Conclusion: There was a statistical trend that patients with UUT-SCC had less bladder recurrence after initially operative management. It did not reach significance because of the small number of patients with SCC.

Eight-week-old BALB/c male mice were divided into the following groups: the AV group (air with a vehicle), the AQ group (air with CoQ 10), the SV group (smoke with a vehicle), and the SQ group (smoke with CoQ 10). The results showed that the CoQ 10 concentrations in the sera and testes were decreased in the groups subjected to smoke but they were improved after the administration of CoQ 10. Neither smoke nor CoQ 10 supplementation affected the serum or testis testosterone concentrations. Regarding the antioxidant system in the testis, the exposure to smoke induced malondialdehyde and hydrogen peroxide production and decreased the catalase and glutathione peroxidase activities. Oral CoQ 10 administration reversed the oxidative damage. In apoptosis, the cytochrome c, c-caspase 9, and c-caspase 3 proteins were increased in the groups exposed to smoke but they were decreased after the CoQ 10 administration. In mitochondrial biogenesis, smoke exposure led to decreases in the PGC1-α, NRF1, and NRF2 levels, but CoQ 10 increased the expressions of these proteins. Additionally, oral CoQ 10 administration improved the mitochondrial copy numbers that were reduced following the exposure to smoke. In summary, CoQ 10 administration reduces smoke-induced testicular damage by regulating the antioxidant capacity, the cell apoptosis, the mitochondrial biogenesis, and the copy numbers in the testes.

The aim of this study was to evaluate the incidence, risk factors, and timing of occurrence of inguinal hernia (IH) after RARP. We included 427 patients with prostate cancer who underwent RARP by a single surgeon from February 2006 to August 2017. Incidence, clinical, and pathological factors were investigated to assess relationship with the development of IH. Postoperative IH occurred in 29 cases (6.79% of all RARP patients), whereas 22 cases (75.9% of all IH patients) occurred within the first 2 years. The median follow-up period was 5.2 years, and the median age of patients was 65 years. Postoperative IH occurrence was significantly associated with body mass index (BMI), smoking history, and low surgeon experience (P = .036, .023, and .048, respectively). However, low surgeon experience did not reach statistical significance after multivariate analysis. The overall incidence of IH after RARP was significantly associated with BMI and smoking history. With obvious incidence of IH within the first 2 years after operation which was not observed at the open prostatectomy, RARP itself may play a role in the development of IH. Herein, we report a rare case of malignant glomus tumor arising from kidney. Case report: A 46 years old man with history of nasopharynx cancer, status post chemoraiation therapy, has right renal tumor found incidentally on a pulmonary computed tomography scan for following of nasopharynx cancer. A contrast enhanced computed tomography of abdomen showed a tumor about 4.4cm with heterogenous enhancement over medial upper pole of right kidney. The patient undertook laparoscopic right radical nephrectomy with right adrenalectomy and recovered well after the operation. According to the histologic and immunohistochemical examination, the tumor is diagnosed as malignant glomus tumor. Conclusion: Glomus tumors resemble glomus body, which is associated with body temperature regulation. Glomus tumors usually occur in the extremities, most common the subungual region of the fingers. Visceral organs are uncommon site involving development of glomus tumors. Glomus tumors arising from kidney are extremely rare, with a few cases described before. It is difficult to differentiate glomus tumor from other renal malignany preoperatively through imaging study. Histolgoy and immunohistochemical study are necessary to make a diagnosis of glomus tumor of the kidney. Renal glomus tumors are usually benign with good outcome and prognosis, with one exception of malignant glomus tumor been reported. We report a case of malignant glomus tumor of kidney. To our knowledge, this is the second case of malignant renal glomus tumor reported. Given the small number of cases reported to date and relative short-term follow up, close clinical follow-up is necessary.

Herein we present a rare complication of extraperitoneal abscess owing to intermittent urinary catheterization in a 62-year-old male who had cervical spine injury and was treated with intermittent urethral catheterization for neurogenic lower urinary tract dysfunction. Treatment and a literature review are also described.

Background: This study aimed to investigate the presence of circulating tumor cells (CTCs) in pat... more Background: This study aimed to investigate the presence of circulating tumor cells (CTCs) in patients with penile squamous cell carcinoma (PSCC). Methods: CTCs were isolated from 14 patients with PSCC, 6 patients with balanoposthitis, and 6 healthy individuals. CTCs were enriched based on cell surface markers and filtered through the IsoFlux device, followed by identification according to cell morphology and immunofluorescence studies. Results: CTCs were found in all PSCC blood samples but not in balanoposthitis samples and samples from healthy individuals. Immunofluorescence studies confirmed the tumor origin. When the patients with PSCC were stratified according to metastatic inguinal lymph node status, a statistically significant difference was observed in the number of detected CTCs. Conclusion: Our study showed that CTCs in PSCC may represent a valuable marker for differentiating PSCC from other tumors. Based on the correlation with some clinical parameters, CTC analysis is possibly relevant for noninvasive monitoring of disease progression and prognosis. The results also suggested a potential role of CTCs in preventing overtreatment, such as inguinal lymph node dissection.

LMWH-associated spontaneous retroperitoneal hematoma (SRH) has been reported mostly in patients treated with enoxaparin. We here present a 76-year-old man with renal insufficiency treated with dalteparin for acute myocardial infarction, who developed catastrophic SRH with intra-abdominal hypertension (IAH) and hemothorax. Because of the high risk of surgical intervention, we performed ultrasound-guided abdominal paracentesis and thoracentesis to relieve IAH and respiratory distress. Vigorous supportive care with massive blood product transfusions was given to stabilize his hemodynamics. Finally, the bleeding was stopped with a powerful hemostatic agent, recombinant factor VIIa. Surgical evacuation of a 3,600 mL blood clot was performed successfully 1 month later.
