Background: Diabetic foot ulcer is the commonest cause of hospitalization in diabetic patients. A... more Background: Diabetic foot ulcer is the commonest cause of hospitalization in diabetic patients. Amputation is one of its dreaded complications, which deteriorates quality of life and increases mortality. Most of amputations are preventable and it is essential to define standard and efficient approach to treat diabetic foot ulcer in a timely manner. The first step is to define correct grade of diabetic foot ulcer and its risk of developing complications. Aim was to assess the outcome of treatment of diabetic foot ulcer with the given PEDIS score and to find the role of PEDIS score in predicting the outcome. Methods: This is a prospective cohort study. PEDIS score was calculated after all variables were categorized for a give patient and documented. PEDIS score was recorded for each patient. Patients followed up for 6 months and outcome was categorized as healed, unhealed, amputed, or death. Study was carried out till adequate sample size was achieved. Results: High PEDIS score in diabetic foot ulcer was associated with peripheral neuropathy. Adverse outcome like amputation and unhealed ulcer were more seen with high PEDIS score. All amputed patients were having high PEDIS score. Conclusions: PEDIS score is more useful in clinical practice for diabetic foot ulcer grading and can be uniformly applied to compare outcome all over the world.

A 63 year old male patient presented with a hard mass of 2×2cm in the right axilla. Excisional biopsy showed an adenocarcinoma histopathologically which is similar to breast carcinoma originating in an accessory mammary gland. There is no evidence of primary lesion in both the mammary glands or in the other organs.

Large ventral hernia is a challenge to operate in tier 3 cities of India, because of economic con... more Large ventral hernia is a challenge to operate in tier 3 cities of India, because of economic constrains of local population. Here author reports three large ventral hernias operated with use of Da-Silva, Malmo peritoneal flap technique. Peritoneal flap should be integral part of abdominal wall repair procedures. It reduces cost, morbidity and mortality of a large ventral hernia repair. It is a useful method for repair of large ventral and incisional hernia when primary fascial closer is not achievable.

Femoral hernias are rare compared to inguinal hernias. Here, there were varied clinical presentations of all femoral hernias. Obstruction, strangulation and groin swelling were the presentations observed. Management of all varies according to presentation. Because of narrow neck and stiff femoral canal chances of complications are more. Use of minimal access techniques may reduce the morbidity of surgical treatment in emergency.

Most common cause is diffuse signet ring adenocarcinoma, metastatic disease from breast and lungs can also lead to Linitis plastica and also caustic burns. In Linitis plastica the stomach becomes rigid and pipe like (leather bottle). We are presenting a rare case of Linitis plastica of terminal ileum. Ours is a 55-year female patient presenting in casualty with abdominal pain, distension, vomiting and bilious vomiting. Her terminal ileum had stricturous rigid tube-like lesion with multiple mesenteric lymphadenitis. We did curative resection (ileoascending colon) and anastomosis. She had uneventful recovery. Though small bowel malignancies are rare, one should have high index of suspicion when one encounters rigid pipe like strictures lesion of ileum, as these can be malignant lesion and one should perform curative resection. This is an uncommon case of a 55- year-old male paraplegic patient, who had urethrocutaneous fistula with distal urethral diverticulum and proximal urethral stricture. The patient was successfully managed by single-stage diverticulectomy and urethroplasty.

Regular bite remains the standard of care in most hospitals. Laparotomies were done for elective and emergency cases.Methods: There were two techniques used to close abdominal wall fascia. The present study has evaluated small bite abdominal closer verses regular bite closer. Author and his team had used small bite fascia closer technique for all laparotomies, while other group of surgeons utilized regular bite fascia closer.Results: Out of 26 laparotomies in short bite closer, 8 had surgical site infections and surgical site occurrences, while 26 laparotomies in regular bite closer group had 15 patients with surgical site infections and surgical site occurrences. This declaration, signed by an interdisciplinary task force of 234 experts from 83 different coun... more This declaration, signed by an interdisciplinary task force of 234 experts from 83 different countries with different backgrounds, highlights the threat posed by antimicrobial resistance and the need for appropriate use of antibiotic agents and antifungal agents in hospitals worldwide especially focusing on surgical infections. As such, it is our intent to raise awareness among healthcare workers and improve antimicrobial prescribing. To facilitate its dissemination, the declaration was translated in different languages. A ntimicrobial resistance (AMR) has emerged as one of the principal public health problems of the 21st century. This has resulted in a public health crisis of international concern, which threatens the practice of modern medicine, animal health, and food security. The substantial problem of AMR is especially relevant to antibiotic resistance (ABR), although antifungal resistance is increasing at an alarming rate. Although the phenomenon of ABR can be attributed to many factors, there is a well-established relationship between antibiotic prescribing practices and the emergence of resistant bacteria. This declaration, signed by an interdisciplinary task force of 234 experts from 83 different countries with different backgrounds, highlights the threat posed by AMR and the need for appropriate use of antibiotic agents and antifungal agents in hospitals worldwide especially focusing on surgical infections.

Background: Antimicrobial Stewardship Programs (ASPs) have been promoted to optimize antimicrobia... more Background: Antimicrobial Stewardship Programs (ASPs) have been promoted to optimize antimicrobial usage and patient outcomes, and to reduce the emergence of antimicrobial-resistant organisms. However, the best strategies for an ASP are not definitively established and are likely to vary based on local culture, policy, and routine clinical practice, and probably limited resources in middle-income countries. The aim of this study is to evaluate structures and resources of antimicrobial stewardship teams (ASTs) in surgical departments from different regions of the world. Methods: A cross-sectional web-based survey was conducted in 2016 on 173 physicians who participated in the AGORA (Antimicrobials: A Global Alliance for Optimizing their Rational Use in Intra-Abdominal Infections) project and on 658 international experts in the fields of ASPs, infection control, and infections in surgery. Results: The response rate was 19.4%. One hundred fifty-six (98.7%) participants stated their hospital had a multidisciplinary AST. The median number of physicians working inside the team was five [interquartile range 4-6]. An infectious disease specialist, a microbiologist and an infection control specialist were, respectively, present in 80.1, 76.3, and 67.9% of the ASTs. A surgeon was a component in 59.0% of cases and was significantly more likely to be present in university hospitals (89.5%, p < 0.05) compared to community teaching (83.3%) and community hospitals (66.7%). Protocols for pre-operative prophylaxis and for antimicrobial treatment of surgical infections were respectively implemented in 96.2 and 82.3% of the hospitals. The majority of the surgical departments implemented both persuasive and restrictive interventions (72.8%). The most common types of interventions in surgical departments were dissemination of educational materials (62.5%), expert approval (61.0%), audit and feedback (55.1%), educational outreach (53.7%), and compulsory order forms (51.5%). Conclusion: The survey showed a heterogeneous organization of ASPs worldwide, demonstrating the necessity of a multidisciplinary and collaborative approach in the battle against antimicrobial resistance in surgical infections, and the importance of educational efforts towards this goal.

Most common cause is diffuse signet ring adenocarcinoma, metastatic disease from breast and lungs can also lead to Linitis plastica and also caustic burns. In Linitis plastica the stomach becomes rigid and pipe like (leather bottle). We are presenting a rare case of Linitis plastica of terminal ileum. Ours is a 55-year female patient presenting in casualty with abdominal pain, distension, vomiting and bilious vomiting. Her terminal ileum had stricturous rigid tube-like lesion with multiple mesenteric lymphadenitis. We did curative resection (ileoascending colon) and anastomosis. She had uneventful recovery. Though small bowel malignancies are rare, one should have high index of suspicion when one encounters rigid pipe like strictures lesion of ileum, as these can be malignant lesion and one should perform curative resection. Surgery improves survival rate along with adjuvant chemotherapy as required.
