Ninety-one patients with nasopharyngeal carcinoma (NPC), and 164 age-matched healthy controls wer... more Ninety-one patients with nasopharyngeal carcinoma (NPC), and 164 age-matched healthy controls were tested for presence of IgA antibodies to Epstein-Barr virus capsid antigen (VCA) and early antigen (EA) in their sera by indirect ELISA using "EBViral DETECT" commercial test kit. IgA anti-VCA was found in 76 (83.5%) of NPC patients and 16 (9.8%) of the controls. Meanwhile, IgA anti-EA was found in 72 (79.1%) of NPC patients and 21 (12.8%) of the controls. In a parallel study by indirect immunofluorescence test (IIF), IgA anti-VCA was found in 77 of 91 (84.6%) NPC patients and 22 of 142 (15.5%) controls. The prevalence rates of anti-VCA as screened by ELISA and IIF were very similar suggesting that neither one of the two tests can be used alternatively depending on the purpose and facilities in each individual laboratory. IgA antibodies to VCA and EA were more prevalence in NPC patients than those in the controls, the finding which again supported the association between EBV and NPC as was suggested in many other reports.

A retrospective review of T and T2 glottic squamous cell carcinoma patients undergoingfrontolateral laryngectomy in the department of Otolaryngology, Faculty of Medicine Siriraj Hospital between 1986 and 2006 with a minimum of five years follow-up period Out of 55 patients who underwent frontolateral laryngectomy, 50 cases that fitted the criteria were recruited for the present study. After surgery, all patients but one had tracheotomy decannulation and recovered good respiratory function. Forty-three successfully decannulated patients (87.80%) had tube decannulation within 14 days. The mean duration offeeding via nasogastric tube was 11.4 days (ranged from 5 to 22 days). Choanal atresia is caused by failure of resorption of the bucco-pharyngeal membrane during embryo... more Choanal atresia is caused by failure of resorption of the bucco-pharyngeal membrane during embryonic development. The atresia can be membranous or bony in nature, but is usually mixed in most cases. When the atresia is bilateral, newborns can have severe airway distress and cyanosis is alleviated by crying. Bilateral choanal atresia is managed with an oropharyngeal airway. Flexible nasal endoscopy and computed tomography can confirm the diagnosis. Surgery is the definitive treatment with two main approaches, namely transnasal or transpalatal. The transnasal route is currently the preferred procedure and can be performed in a minimally invasive fashion with endoscopic instrumentation. It is a safe and rapid procedure even in very young children, with no complications and a high rate of success. Before 1920, 1.4 and 6.5 per cent of all adenoids and tonsils removed from asymptomatic patients were infected by tuberculosis. After the introduction of antituberculous chemotherapy and BCG vaccination, this disease was considered uncommon and sporadic cases were reported in the medical literature. Recently, tuberculosis has begun to increase again due to the high global HIV-infected rate and antituberculous drug resistance among these people. To describe and highlight the clinical features of this condition, fifteen Thai patients (7 males and 8 females) from the Department of Otolaryngology, Siriraj Hospital, Bangkok, Thailand were reviewed. A total of 34 tissue biopsies were collected from nasopharyngeal carinoma (NPC) patients and 5 co... more A total of 34 tissue biopsies were collected from nasopharyngeal carinoma (NPC) patients and 5 controls with non-NPC. Extracted DNA from tissue biopsies were analyzed for presence of specific gene sequences to EBV type A and type B, and HHV-6 by polymerase chain reaction (PCR). The different sequences of EBV type A and B were parts from the highly divergent forms of the EBV nuclear antigen 2 (EBNA 2). The PCR amplified products for EBNA 2A and EBNA 2B were 115 and 119 base pairs respectively whereas that of HHV-6 DNA was 776 base pairs. The results demonstrated that EBV DNA was detected in 32 of 34 cases (94.1%) : 28 (82.3%) with type A, 2 (5.9%) with type B, and 2 (5.9%) with both types. EBV DNA of type A could be detected I (20%) of5 controls. HHV-6 DNA was in 5 of 34 samples (14.7%) whereas HHV-6 DNA was not detectable in biopsy tissues from controls. The results show that in the NPC patient group, A type of EBV is predominant. Detection of HHV-6 DNA in patients group only might be resulted from reactivation of a latent infection or association with EBV induction of NPC.

From April 2007 to April 2008, there were 28 patients enrolled in the present study. The temperature-controlled radiofrequency (RF) was applied at inferior turbinate on both sides. To compare before and at the 8th weeks after RF treatment, the patients were given Rhinoconjuctivitis Quality of Life questionnaires (Rcq-36) and questionnaires using visual analog scales (0-10) to assess subjective symptoms such as the frequency and severity of nasal obstruction, nasal discharge, hyposmia, and sneezing. The acoustic rhinometry and active rhinomanometry were also done to evaluate objective outcomes. A retrospective review of patients with benign non-toxic thyroid disease undergoing hemithyroidectomy between April 2004 and November 2008 in the Department of Otorhinolaryngology, Siriraj Hospital was conducted All patients were in euthyroid state preoperatively. Thyroid specimens were examined for pathological diagnosis and degree of lymphocytic infiltration in thyroid tissue, and thyroid function was evaluated again six weeks after surgery. One hundred patients who received hemithyroidectomy were recruited for the present study. All had normal preoperative thyroid function. To assess the health-related quality of life (HRQOL) of Thai patients with obstructive sleep diso... more To assess the health-related quality of life (HRQOL) of Thai patients with obstructive sleep disordered breathing (OSDB) and their responses to continuous positive airway pressure (CPAP) treatment. Thirty-eight healthy volunteers, 35 primary snorers (PS), and 108 patients with obstructive sleep apnea (OSA) diagnosed with polysomnography (PSG) were asked to fill questionnaires including Thai version of Short Form-36 (SF-36), and Epworth sleepiness scale (ESS). Thirty-two subjects with OSA who had been using CPAP adequately were asked to complete SF-36 twice, before and three months after treatment initiation. The role-physical and general health dimensions of SF-36 in OSA patients and PS were significantly lower than healthy volunteers (p < 0.05). Other dimensions were not significantly different among groups of subjects. By lifting the soft tissue from the midportion of the face, many extensive benign tumors such as inverted papilloma, nasopharyngeal angiofibroma and some limited malignant lesions could be safely removed. This procedure could be combined with other approaches such as transtemporal, intracranial approach to resect more extensive tumors around the orbit, central skull base, as well. This technique was applied in 40 cases, ranging from 12-72 years of age and in a variety of diseases in Siriraj Hospital from 1991 to 2000. Snoring is an initial end of sleep disorder breathing (SDB) spectrum. It can lead to terminal end... more Snoring is an initial end of sleep disorder breathing (SDB) spectrum. It can lead to terminal end, obstructive sleep apnea syndrome. Treatments comprise of non-surgical and surgical treatments, and surgical treatment is an alternative when non-surgical treatment fails. The novel radiofrequency procedures are technically simple, minimally invasive and associated with minimal postoperative pain and discomfort compared to traditional surgical procedures. It can be performed as an outpatient basis under local anesthesia with a low complication rate and excellent therapeutic results. A result of heating by radiofrequency produces ablation to collateral tissue surrounding the target tissue site. Over a course of several weeks following the initial treatment, firmer fibrous tissues are formed and they reduce the tissue volume with less vibration. Temperatured controlled radiofrequency (TCRF) has advantages in monitoring tissue impedance and temperature during application and results in a precision of lesion dimension control. However, the radiofrequency generator with monitoring equipments and specific needle electrodes are very expensive. We have used non-temperatured controlled radiofrequency (NTCRF) to perform radiofrequencyassited uvulopalatoplasty (RAUP) and/ or volumetric tissue reduction of nasal mucosa, soft palate, and base of tongue under local anesthesia or as an adjunct with uvulopalatopharyngoplasty (UPPP) under general anesthesia in our institute successfully for several years without serious complications. Its major advantage over TCRF is cost-effectiveness. It is relatively inexpensive, safe, well-tolerated, and effective in the treatment of patients with obstructive SDB. We present our technique in performing RAUP, volumetric tissue reduction of nasal mucosa, soft palate, and base of tongue by using NTCRF in this study.

OBJECTIVE To assess the efficacy of dexpanthenol nasal spray compared with normal saline spray in... more OBJECTIVE To assess the efficacy of dexpanthenol nasal spray compared with normal saline spray in the postoperative treatment of patients with chronic rhinosinusitis (CRS) who underwent endoscopic sinus surgery (ESS). MATERIALS AND METHOD A prospective, randomized controlled study was conducted in CRS patients who underwent ESS. The enrolled patients had never been operated intranasally. These patients received either dexpanthenol or normal saline nasal spray intranasally four times a day for six weeks post-operatively. RESULTS Fifty CRS patients were recruited in the present study. Age ranged from 23 to 63 years (means 43.4 +/- 11.2 years). Forty-four percent of patients were diagnosed as CRS without nasal polyps (NP) (CRSs NP) and 56% were CRS with NP (CRSw NP). Methods: A total of 105 healthy subjects (80 males and 25 females) were recruited. Inclusion criteria were healthy people age ≥ 18 years with body mass index (BMI) of < 30 kg/m2, normal visual harmony of facial profile, no history of snoring or witnessed apnea, and no excessive daytime sleepiness (Epworth Sleepiness Scale ≤ 8). All subjects must have a regular sleep pattern with total sleep duration of 7-9 hours and must have no complaint of difficulty sleeping. Thirty-eight healthy volunteers, 35 primary snorers (PS), and 108 patients with obstructive sleep apnea (OSA) diagnosed with polysomnography (PSG) were asked to fill questionnaires including Thai version of Short Form-36 (SF-36), and Epworth sleepiness scale (ESS). Thirty-two subjects with OSA who had been using CPAP adequately were asked to complete SF-36 twice, before and three months after treatment initiation. The role-physical and general health dimensions of SF-36 in OSA patients and PS were significantly lower than healthy volunteers (p < 0.05). Other dimensions were not significantly different among groups of subjects. By lifting the soft tissue from the midportion of the face, many extensive benign tumors such as inverted papilloma, nasopharyngeal angiofibroma and some limited malignant lesions could be safely removed. This procedure could be combined with other approaches such as transtemporal, intracranial approach to resect more extensive tumors around the orbit, central skull base, as well. This technique was applied in 40 cases, ranging from 12-72 years of age and in a variety of diseases in Siriraj Hospital from 1991 to 2000. It can lead to terminal end, obstructive sleep apnea syndrome. Treatments comprise of non-surgical and surgical treatments, and surgical treatment is an alternative when non-surgical treatment fails. The novel radiofrequency procedures are technically simple, minimally invasive and associated with minimal postoperative pain and discomfort compared to traditional surgical procedures. It can be performed as an outpatient basis under local anesthesia with a low complication rate and excellent therapeutic results. A result of heating by radiofrequency produces ablation to collateral tissue surrounding the target tissue site. Over a course of several weeks following the initial treatment, firmer fibrous tissues are formed and they reduce the tissue volume with less vibration. Temperatured controlled radiofrequency (TCRF) has advantages in monitoring tissue impedance and temperature during application and results in a precision of lesion dimension control. However, the radiofrequency generator with monitoring equipments and specific needle electrodes are very expensive. We have used non-temperatured controlled radiofrequency (NTCRF) to perform radiofrequencyassited uvulopalatoplasty (RAUP) and/ or volumetric tissue reduction of nasal mucosa, soft palate, and base of tongue under local anesthesia or as an adjunct with uvulopalatopharyngoplasty (UPPP) under general anesthesia in our institute successfully for several years without serious complications. Its major advantage over TCRF is cost-effectiveness. It is relatively inexpensive, safe, well-tolerated, and effective in the treatment of patients with obstructive SDB. We present our technique in performing RAUP, volumetric tissue reduction of nasal mucosa, soft palate, and base of tongue by using NTCRF in this study.

OBJECTIVE To assess the efficacy of dexpanthenol nasal spray compared with normal saline spray in... more OBJECTIVE To assess the efficacy of dexpanthenol nasal spray compared with normal saline spray in the postoperative treatment of patients with chronic rhinosinusitis (CRS) who underwent endoscopic sinus surgery (ESS). MATERIALS AND METHOD A prospective, randomized controlled study was conducted in CRS patients who underwent ESS. The enrolled patients had never been operated intranasally. These patients received either dexpanthenol or normal saline nasal spray intranasally four times a day for six weeks post-operatively. RESULTS Fifty CRS patients were recruited in the present study. Age ranged from 23 to 63 years (means 43.4 +/- 11.2 years). Forty-four percent of patients were diagnosed as CRS without nasal polyps (NP) (CRSs NP) and 56% were CRS with NP (CRSw NP). Methods: A total of 105 healthy subjects (80 males and 25 females) were recruited. Inclusion criteria were healthy people age ≥ 18 years with body mass index (BMI) of < 30 kg/m2, normal visual harmony of facial profile, no history of snoring or witnessed apnea, and no excessive daytime sleepiness (Epworth Sleepiness Scale ≤ 8). All subjects must have a regular sleep pattern with total sleep duration of 7-9 hours and must have no complaint of difficulty sleeping.
