A assistência farmacêutica bem planejada e conduzida é importante para o adequado atendimento às necessidades de saúde da população, facilitando o acesso aos medicamentos essenciais e promovendo seu uso racional.O objetivo deste estudo é descrever a importância da assistência farmacêutica e o papel do farmacêutico no uso racional de medicamentos no paciente idoso.Trata-se de uma revisão da literatura, realizada em artigos científicos e documentos governamentais publicados entre os anos de 2010 a 2020. This study aimed to verify the effect of expressive therapies (ET) on the number of medical appointments and on mental status in elderly FH. This is a quasi-experimental study, with a sample of 69 elderly people attended at the PHC of Granja do Torto, Federal District, divided into: intervention group (IG), with elderly FH; and control group (CG), with elderly people not HF. The elderly HF were submitted to ET for 24 weeks, twice / week. In the data analysis, Pearson's chi-square test, multivariate analysis of variance for repeated MANOVA measures and Wilks' Lambda test were used. The IG showed a mean age of 73.97 years (SD=6.67) and the CG 69.78 years (SD=6.78), with a significant difference between the groups (p =0.04).In the post-intervention, the IG showed a significant decrease in depression (p <0.001), anxiety (p =0.05), loneliness (p =0.01) and stress (p =0.006).There was a significant decrease in the number of consultations in the IG (p <0.001) during the year following the intervention. In HF elderly people, the TE intervention provided positive effects, reducing the number of medical appointments and improving the mental state (depression, anxiety, stress and loneliness) of this group, allowing them to experience alternatives to improve their quality of life. The use of ET minimized health problems from the perspective of resolving low-cost care, in a holistic approach to comprehensive care for the elderly in PHC.

Nesse contexto, os anti-inflamatórios não esteroidais (AINEs), amplamente utilizados, devem ser evitados por aumentar a incidência de reações adversas e interações medicamentosas no idoso. O objetivo do estudo foi descrever as reações adversas em uso dos anti-inflamatórios não esteroidais; descrever o mecanismo de ação dos AINES e sua relação com as reações adversas; enumerar as interações medicamentosas envolvendo os AINES e outros medicamentos mais utilizados por essa população; e descrever o cuidado farmacêutico e a sua importância na segurança e eficácia do tratamento no paciente idoso. Trata-se de um estudo de revisão narrativa de literatura, utilizado como estratégias de busca as bases de referências SCIELO, LILACS, PUBMED E BVS. O estudo evidenciou que o uso dos AINEs no paciente idoso deve ser cauteloso, devido os riscos de reações adversas e interações medicamentosas, sendo de grande importância o acompanhamento farmacoterapêutico e a utilização do critério de Beers para garantir a segurança e o uso racional de medicamento. Conclui-se que o farmacêutico é o profissional mais habilitado a realizar o acompanhamento farmacoterapêutico de forma a promover o uso racional dos medicamentos, assim orientando o paciente quanto aos riscos do consumo inapropriado dos mesmos.

The objective of this study was to verify the association of the classic APOB Xba I polymorphism (rs693) with atherosclerotic risk factors in a segment of the Brazilian elderly population considering their usual dietary intake. Clinical and biochemical characteristics as well as total caloric and fat intake data were determined from 644 elderly individuals. Polymorphism analysis was performed by conventional polymerase chain reaction followed by enzyme restriction. Statistical analyses compared measures and proportions according to different APOB genotypic combinations. Statistically significant association was found between Xba I polymorphism and serum LDL, total cholesterol, and total lipid levels, with important elevations among T homozygotes compared to the other genotypes. There was homogeneity in all other parameters analyzed (including intake pattern), with a tendency for reduced levels of circulating apolipoprotein B among TT individuals. Our results pointed out that genetic variation in APOB affected the lipemic profile of elderly individuals in a context not biased by diet, generating a pattern suggestive of secretory disorder of lipoprotein particles, with possible implication in atherosclerotic risk.

Measurement of inflammatory markers for risk stratification of vascular disorders has been the fo... more Measurement of inflammatory markers for risk stratification of vascular disorders has been the focus of numerous investigations worldwide, and usually reveals augmented levels of circulating cytokines/ chemokines among carriers of classic risk factors for atherosclerosis. Nonetheless, this low-grade inflammatory milieu detected in aged individuals tends to be influenced by body composition. Moreover, cardiovascular risk factors have a complex genetic etiology, and disregarding the genetic heritage may produce spurious results owing to interethnic differences. In this complex scenario, our study was designed to verify the existence and strength of the association between selected mediators of systemic inflammation and classic risk factors of cardiovascular diseases (CVD). Methods: In a sample of post-menopausal older women, correlation analyses explored the association of circulating levels of IL1a, IL1b, IL8, IL10 and IL12 with atherosclerosis-related clinical/metabolic parameters, using age, body mass index (BMI), genetic ancestry estimates as standard correction factors. Further adjustment for use of therapeutic agents was applied when appropriate. Results: Our analyses revealed association of log 10-transformed IL-12 titers with VLDL-c levels (r = .192; p = .002) and with SBP (rÀ.185; p = .003), and of log 10-transformed IL-8 titers with GLY (r = .235; p < .001). Conclusion: Interpretation to the results account to a possible dysregulation of the PPAR signaling pathway to explain the association of IL12 and VLDL-c, and to IL8-driven mechanisms to promote dysglycemia. No previous report sought to investigate the relationship between this set of inflammatory markers and classic risk factors for atherosclerosis correcting for the heterogeneity in genetic admixture and body composition of Brazilian post-menopausal women.

To describe the prevalence of stress urinary incontinence (SUI) among women older than 20 years participating in the Family Health Program (FHP) of the municipality of Dourados (MS), Brazil, and to correlate it with the following variables: age, body mass index (BMI), parity, number of pregnancies, hysterectomy, smoking, and diabetes mellitus. Methods. Cross-sectional household survey. We interviewed 336 women older than 20 years who had no exclusion criteria. Two forms were used during the interviews: an evaluation sheet containing demographic data and the International Consultation on Incontinence Questionnaire-Short Form (ICIQ-SF) assessing the impact on quality of life (QOL). For the statistical analysis, we used the chisquare test, Student's t test, the corrected coefficient of contingency and a forward stepwise logistic regression model. results. Prevalence of SUI was 21.4%. In terms of risk factors, age (p = 0.113), smoking (p = 0.796), and diabetes mellitus (p = 0.221) showed no statistically significant association; however, BMI (p = 0.007), number of pregnancies (p = 0.018), parity (p = 0.032), and hysterectomy (p = 0.024) showed a weak association. Using logistic regression, only pairing of weight and hysterectomy was able to predict the outcome (SUI). Most women with SUI (63.9%) had very severe impairment in the QOL. conclusIon. The prevalence of SUI was similar to that found in other studies; BMI, parity, number of pregnancies, and hysterectomy were associated with this disease, which seriously impairs the QOL.

In this scenario, estimating ancestry proportions in recently admixed populations is a strategy that can help increasing knowledge about the genetic structure of this complex trait. Our purpose was to assess mean ancestry estimates for the three main parental contributors to the Brazilian contingent (European, African and Amerindian) using a panel of 12 ancestry informative markers. Outpatients with the late-onset form of AD (n = 120) were compared for ancestry levels with non-cognitively impaired subjects (n = 412) in the Midwest Brazil, controlling for classic clinical, social and anthropometric risk factors. OBJECTIVE: The purpose was to assess the effect of Tai Chi Chuan (TCC) on functional capacity (FC... more OBJECTIVE: The purpose was to assess the effect of Tai Chi Chuan (TCC) on functional capacity (FC) and quality of life (QoL) in elderly men with low bone mineral density (BMD). METHODS: This quasi-experimental, controlled blinded trial evaluated 41 senior men (≥ 60 years), divided into two groups: control group 1-G1 (TCC; n = 20; 69.2 ± 6.2 years) and control group 2-G2 (control; n = 21; 69.0 ± 5.7 years). The BMD (of lumbar spine and femur neck) was assessed by dual energy x-ray absorptiometry (DEXA). FC was assessed for aerobic endurance, for upper and lower limbs strength and flexibility, for static, dynamic and functional balance testes, and for risk of falls. QoL was assessed by the SF-36 questionnaire (MOS 36-item Short-Form Health Survey). G1 practiced the 24-form Yang style of TCC, for 12 weeks twice a week on low intensity; G2 did not practice oriented physical activity. RESULTS: TCC practitioners had significantly higher scores for aerobic endurance, upper and lower limbs strength, and dynamic balance. In terms of QoL, values were significantly better in total score and aspects such as FC, general health, vitality, and mental health. DISCUSSION: TCC was proven beneficial to FC and QoL scores, which adds evidence for its practice by elderly men with low BMD. CONCLUSION: TCC training is effective in improving FC and QoL in elderly men with low BMD. KEYWORDS: tai chi chuan; aged; osteoporosis; quality of life. RESUMO OBJETIVO: Avaliar efeitos do Tai Chi Chuan (TCC) na capacidade funcional (CF) e na qualidade de vida (QV) de homens idosos com baixa densidade mineral óssea (DMO). MÉTODO: Estudo quasi-experimental, controlado, cego, com 41 idosos (≥ 60 anos) divididos em dois grupos: grupo experimental-G1 (TCC; n = 20; 69,2 ± 6,2 anos) e grupo controle-G2 (controle; n = 21; 69,0 ± 5,7 anos). A DMO (coluna lombar e colo do fêmur) foi avaliada com raios-x de dupla energia (DEXA). Para aferir CF foram utilizados: testes de resistência aeróbia (RA); de força e flexibilidade de membros superiores e inferiores; de equilíbrio em condições estática, dinâmica e funcional; e de alcance funcional, assim como para risco de quedas. A QV foi pesquisada pelo questionário SF-36 (MOS 36 ltem Short-Form Health Survey). O G1 praticou TCC estilo Yang, com 24 movimentos por 12 semanas, 2 vezes por semana, em intensidade leve; o G2 não praticou atividade física orientada. RESULTADOS: Praticantes de TCC apresentaram valores significativamente maiores em resistência aeróbia, em força de membros superiores e inferiores, e para equilíbrio dinâmico. Ao aferir a QV, surgiu significância para escore total, assim como para as dimensões de capacidade funcional, estado geral de saúde, vitalidade e saúde mental. DISCUSSÃO: Constataram-se efeitos benéficos do TCC em CF e QV, o que agrega evidências à sua utilização em homens idosos com baixa DMO. CONCLUSÃO: O TCC é eficaz na melhora da CF e da QV em homens idosos com baixa DMO. PALAVRAS-CHAVE: tai chi chuan; idoso; osteoporose; qualidade de vida.

Os aspectos discutidos na literatura foram: sobre a aceitação, sobre o uso e sobre as variáveis que poderiam interferir nas colocações pré citadas. Considerando estudos nacionais e internacionais, as discussões acerca do tema, de forma geral, foram igualitárias. Fatores como medo, receio, limitações ao acesso das tecnologias e motivação, foram determinantes no resultado do estudo. This study aimed to verify the effect of expressive therapies (ET) on the number of medical appointments and on mental status in elderly FH. This is a quasi-experimental study, with a sample of 69 elderly people attended at the PHC of Granja do Torto, Federal District, divided into: intervention group (IG), with elderly FH; and control group (CG), with elderly people not HF. The elderly HF were submitted to ET for 24 weeks, twice / week. In the data analysis, Pearson's chi-square test, multivariate analysis of variance for repeated MANOVA measures and Wilks' Lambda test were used. The IG showed a mean age of 73.97 years (SD=6.67) and the CG 69.78 years (SD=6.78), with a significant difference between the groups (p =0.04).In the post-intervention, the IG showed a significant decrease in depression (p <0.001), anxiety (p =0.05), loneliness (p =0.01) and stress (p =0.006).There was a significant decrease in the number of consultations in the IG (p <0.001) during the year following the intervention. In HF elderly people, the TE intervention provided positive effects, reducing the number of medical appointments and improving the mental state (depression, anxiety, stress and loneliness) of this group, allowing them to experience alternatives to improve their quality of life. The use of ET minimized health problems from the perspective of resolving low-cost care, in a holistic approach to comprehensive care for the elderly in PHC.

Nesse contexto, os anti-inflamatórios não esteroidais (AINEs), amplamente utilizados, devem ser evitados por aumentar a incidência de reações adversas e interações medicamentosas no idoso. O objetivo do estudo foi descrever as reações adversas em uso dos anti-inflamatórios não esteroidais; descrever o mecanismo de ação dos AINES e sua relação com as reações adversas; enumerar as interações medicamentosas envolvendo os AINES e outros medicamentos mais utilizados por essa população; e descrever o cuidado farmacêutico e a sua importância na segurança e eficácia do tratamento no paciente idoso. Trata-se de um estudo de revisão narrativa de literatura, utilizado como estratégias de busca as bases de referências SCIELO, LILACS, PUBMED E BVS. O estudo evidenciou que o uso dos AINEs no paciente idoso deve ser cauteloso, devido os riscos de reações adversas e interações medicamentosas, sendo de grande importância o acompanhamento farmacoterapêutico e a utilização do critério de Beers para garantir a segurança e o uso racional de medicamento. Conclui-se que o farmacêutico é o profissional mais habilitado a realizar o acompanhamento farmacoterapêutico de forma a promover o uso racional dos medicamentos, assim orientando o paciente quanto aos riscos do consumo inapropriado dos mesmos.

The objective of this study was to verify the association of the classic APOB Xba I polymorphism (rs693) with atherosclerotic risk factors in a segment of the Brazilian elderly population considering their usual dietary intake. Clinical and biochemical characteristics as well as total caloric and fat intake data were determined from 644 elderly individuals. Polymorphism analysis was performed by conventional polymerase chain reaction followed by enzyme restriction. Statistical analyses compared measures and proportions according to different APOB genotypic combinations. Statistically significant association was found between Xba I polymorphism and serum LDL, total cholesterol, and total lipid levels, with important elevations among T homozygotes compared to the other genotypes. There was homogeneity in all other parameters analyzed (including intake pattern), with a tendency for reduced levels of circulating apolipoprotein B among TT individuals. Our results pointed out that genetic variation in APOB affected the lipemic profile of elderly individuals in a context not biased by diet, generating a pattern suggestive of secretory disorder of lipoprotein particles, with possible implication in atherosclerotic risk.

Measurement of inflammatory markers for risk stratification of vascular disorders has been the fo... more Measurement of inflammatory markers for risk stratification of vascular disorders has been the focus of numerous investigations worldwide, and usually reveals augmented levels of circulating cytokines/ chemokines among carriers of classic risk factors for atherosclerosis. Nonetheless, this low-grade inflammatory milieu detected in aged individuals tends to be influenced by body composition. Moreover, cardiovascular risk factors have a complex genetic etiology, and disregarding the genetic heritage may produce spurious results owing to interethnic differences. In this complex scenario, our study was designed to verify the existence and strength of the association between selected mediators of systemic inflammation and classic risk factors of cardiovascular diseases (CVD). Methods: In a sample of post-menopausal older women, correlation analyses explored the association of circulating levels of IL1a, IL1b, IL8, IL10 and IL12 with atherosclerosis-related clinical/metabolic parameters, using age, body mass index (BMI), genetic ancestry estimates as standard correction factors. Further adjustment for use of therapeutic agents was applied when appropriate. Results: Our analyses revealed association of log 10-transformed IL-12 titers with VLDL-c levels (r = .192; p = .002) and with SBP (rÀ.185; p = .003), and of log 10-transformed IL-8 titers with GLY (r = .235; p < .001). Conclusion: Interpretation to the results account to a possible dysregulation of the PPAR signaling pathway to explain the association of IL12 and VLDL-c, and to IL8-driven mechanisms to promote dysglycemia. No previous report sought to investigate the relationship between this set of inflammatory markers and classic risk factors for atherosclerosis correcting for the heterogeneity in genetic admixture and body composition of Brazilian post-menopausal women.

To describe the prevalence of stress urinary incontinence (SUI) among women older than 20 years participating in the Family Health Program (FHP) of the municipality of Dourados (MS), Brazil, and to correlate it with the following variables: age, body mass index (BMI), parity, number of pregnancies, hysterectomy, smoking, and diabetes mellitus. Methods. Cross-sectional household survey. We interviewed 336 women older than 20 years who had no exclusion criteria. Two forms were used during the interviews: an evaluation sheet containing demographic data and the International Consultation on Incontinence Questionnaire-Short Form (ICIQ-SF) assessing the impact on quality of life (QOL). For the statistical analysis, we used the chisquare test, Student's t test, the corrected coefficient of contingency and a forward stepwise logistic regression model. results. Prevalence of SUI was 21.4%. In terms of risk factors, age (p = 0.113), smoking (p = 0.796), and diabetes mellitus (p = 0.221) showed no statistically significant association; however, BMI (p = 0.007), number of pregnancies (p = 0.018), parity (p = 0.032), and hysterectomy (p = 0.024) showed a weak association. Using logistic regression, only pairing of weight and hysterectomy was able to predict the outcome (SUI). Most women with SUI (63.9%) had very severe impairment in the QOL. conclusIon. The prevalence of SUI was similar to that found in other studies; BMI, parity, number of pregnancies, and hysterectomy were associated with this disease, which seriously impairs the QOL.

In this scenario, estimating ancestry proportions in recently admixed populations is a strategy that can help increasing knowledge about the genetic structure of this complex trait. Our purpose was to assess mean ancestry estimates for the three main parental contributors to the Brazilian contingent (European, African and Amerindian) using a panel of 12 ancestry informative markers. Outpatients with the late-onset form of AD (n = 120) were compared for ancestry levels with non-cognitively impaired subjects (n = 412) in the Midwest Brazil, controlling for classic clinical, social and anthropometric risk factors. METHODS: This quasi-experimental, controlled blinded trial evaluated 41 senior men (≥ 60 years), divided into two groups: control group 1-G1 (TCC; n = 20; 69.2 ± 6.2 years) and control group 2-G2 (control; n = 21; 69.0 ± 5.7 years). The BMD (of lumbar spine and femur neck) was assessed by dual energy x-ray absorptiometry (DEXA). FC was assessed for aerobic endurance, for upper and lower limbs strength and flexibility, for static, dynamic and functional balance testes, and for risk of falls. QoL was assessed by the SF-36 questionnaire (MOS 36-item Short-Form Health Survey). G1 practiced the 24-form Yang style of TCC, for 12 weeks twice a week on low intensity; G2 did not practice oriented physical activity. RESULTS: TCC practitioners had significantly higher scores for aerobic endurance, upper and lower limbs strength, and dynamic balance. In terms of QoL, values were significantly better in total score and aspects such as FC, general health, vitality, and mental health. DISCUSSION: TCC was proven beneficial to FC and QoL scores, which adds evidence for its practice by elderly men with low BMD. RESUMO OBJETIVO: Avaliar efeitos do Tai Chi Chuan (TCC) na capacidade funcional (CF) e na qualidade de vida (QV) de homens idosos com baixa densidade mineral óssea (DMO). MÉTODO: Estudo quasi-experimental, controlado, cego, com 41 idosos (≥ 60 anos) divididos em dois grupos: grupo experimental-G1 (TCC; n = 20; 69,2 ± 6,2 anos) e grupo controle-G2 (controle; n = 21; 69,0 ± 5,7 anos). A DMO (coluna lombar e colo do fêmur) foi avaliada com raios-x de dupla energia (DEXA). Para aferir CF foram utilizados: testes de resistência aeróbia (RA); de força e flexibilidade de membros superiores e inferiores; de equilíbrio em condições estática, dinâmica e funcional; e de alcance funcional, assim como para risco de quedas. A QV foi pesquisada pelo questionário SF-36 (MOS 36 ltem Short-Form Health Survey). O G1 praticou TCC estilo Yang, com 24 movimentos por 12 semanas, 2 vezes por semana, em intensidade leve; o G2 não praticou atividade física orientada. RESULTADOS: Praticantes de TCC apresentaram valores significativamente maiores em resistência aeróbia, em força de membros superiores e inferiores, e para equilíbrio dinâmico. Ao aferir a QV, surgiu significância para escore total, assim como para as dimensões de capacidade funcional, estado geral de saúde, vitalidade e saúde mental. DISCUSSÃO: Constataram-se efeitos benéficos do TCC em CF e QV, o que agrega evidências à sua utilização em homens idosos com baixa DMO. CONCLUSÃO: O TCC é eficaz na melhora da CF e da QV em homens idosos com baixa DMO. PALAVRAS-CHAVE: tai chi chuan; idoso; osteoporose; qualidade de vida.

O objetivo deste estudo foi analisar, com base na literatura, a aceitação e o uso das tecnologias... more O objetivo deste estudo foi analisar, com base na literatura, a aceitação e o uso das tecnologias assistivas pelos idosos. Trata-se de estudo de revisão sistemática em que foi realizada busca por artigos nas bases de dados PubMed/Medline, Lilacs, e Scielo. Os aspectos discutidos na literatura foram: sobre a aceitação, sobre o uso e sobre as variáveis que poderiam interferir nas colocações pré citadas. Considerando estudos nacionais e internacionais, as discussões acerca do tema, de forma geral, foram igualitárias. Fatores como medo, receio, limitações ao acesso das tecnologias e motivação, foram determinantes no resultado do estudo. Notou-se por fim a necessidade de maior informação aos idosos sobre a existência das tecnologias, dos tipos, das categorias e dos benefícios que podem trazer ao idoso, fomentando o interesse no uso, para que a inserção dos idosos seja plena no mundo tecnológico.
