Malgré les nombreux progrès réalisés dans la connaissance et les traitements de cette maladie, la... more Malgré les nombreux progrès réalisés dans la connaissance et les traitements de cette maladie, la survenue d'une épilepsie chez un jeune enfant et l'annonce de ce diagnostic à ses parents peuvent avoir des répercussions importantes sur sa construction psychique, à la fois du fait des troubles neurologiques et neuropsychologiques qui lui sont associés, et du fait des perturbations des interactions précoces parents-enfant qu'elle induit. Peur des crises et angoisse de mort, peur des conséquences sur le cerveau, secret et honte risquent de majorer les difficultés et de perturber le fonctionnement familial. Une bonne connaissance des différents aspects de cette pathologie complexe doit permettre aux professionnels concernés d'accompagner de façon adaptée l'enfant tout au long de son parcours personnel affectif, social, scolaire, et d'aider sa famille à retrouver un équilibre au fil du temps. An evolutionary diagram that takes into account the development of personality and its structurin... more An evolutionary diagram that takes into account the development of personality and its structuring or restructuring was developed thanks to a better understanding of autistic disorders from a psychodynamic point of view through long term psychoanalytic treatment of autistic children. This grid is organised around the major stages of the formation of the bodily ego which autistic children helped us understand better. The construction of space and the capacities of cognitive instrumentation are logically within this line of structuring. These major stages are defined: the first is the "successful" autistic state; the second is the stage where the primary skin is recovered (feeling of a circular envelope); the third is the symbiotic phase which includes vertical splitting then horizontal splitting of the image of the body; finally, the fourth is the phase of individuation/separation into a whole body. In this article the authors report insights into autism developed through their extensive experie... more In this article the authors report insights into autism developed through their extensive experience of psychoanalytic therapy with children with autism. The first stages of body psychic development are seriously disrupted by this pathology, resulting in primitive anxieties of falling and of being liquefied. These anxieties are connected to the fragile development of body ego and of its related spatiotemporal organisation. The changes in children observed by the authors during the therapeutic process lead them to offer a psychodynamic assessment tool, which revolves principally around the development of body ego. After the initial state of 'severe autism', the authors describe three stages: the stage of 'recovery of the skin' (Bick); the established 'symbiotic phase', subdivided into 'vertical then horizontal splitting of the body ego'; and finally the stage of 'individuation'. First, the authors describe the principal psychoanalytic approaches to autism and reflect on the links possible with non-psychoanalytic work.
