BACKGROUND In expanding follicles the anti-Mullerian hormone (AMH) is selected from granulosa-cel... more BACKGROUND In expanding follicles the anti-Mullerian hormone (AMH) is selected from granulosa-cell secretions as the best endocrine marker that can measure ovarian reserve. Systemic Lupus Erythematosus (SLE) is an autoimmune illness of several systems. It is frequent in young women of reproductive age and may have a negative impact on their fertility due to SLE activity and therapy. METHODS This case - control retrospective study conducted on 50 patients with SLE and 50 healthy controls who fulfilled the 1997 ACR criteria. By measuring AMH levels, assessing an ovarian volume (OV) and total antral follicle count (AFC) by transvaginal sonar (TVS) in comparison to healthy controls. An analysis was carried out on the association of SLE disease activity with AMH, OV and AFC. RESULTS SLE patients had decreased levels of AMH, OV and AFC, correlating with age, activity and damage levels of SLE. Displaying a negative consequence of SLE. In the SLE Group, the CD4+Tcells and NK cells were significantly reduced (p<0.05). CONCLUSION AMH levels and AFC were decreased in SLE women with autoimmune activity and CD8+ T cell activation, used as useful markers and accounting for a high proportion of women with low ovarian reserve.

Enthesitis may be considered as a sign of increased disease burden due to its association with several clinical aspects. Therefore, the aim of the study was to investigate the effect of clinical enthesitis on quality of life and work productivity in PsA patients. Results Enthesitis was detected in 50% of patients. There was statistically significant difference between the studied groups as regard disease activity index for psoriatic arthritis (DAPSA) score, psoriatic arthritis impact of the disease 12 (PsAID-12), Health Assessment Questionnaire disability index (HAQ-DI), and Spondyloarthritis Research Consortium of Canada index (SPARCC) with higher mean score in patients with enthesitis. Background: Lupus nephritis (LN) is one of the most dangerous manifestations of systemic lupus er... more Background: Lupus nephritis (LN) is one of the most dangerous manifestations of systemic lupus erythematosus (SLE). LN is a complex interplay between genetics, immunological, and environmental factors. Objective: Our study aimed to evaluate tumor necrosis factor α-induced protein-3 (TNFAIP3) mRNA expression level as a noninvasive predictive test of LN and to assess its correlations with clinic-pathologic characteristics as well as disease activity of SLE. Subjects and Methods: Among 150 studied subjects; 80 had SLE and 70 were healthy controls. Patients were stratified into LN group (n=35) and the non-LN group (n=45). TNFAIP3 mRNA expression level was measured using a quantitative real-time PCR. Results: TNFAIP3 mRNA expression level was upregulated in the SLE group compared to the control group. While TNFAIP3 mRNA expression level was downregulated in the LN group of SLE patients compared to the non-LN group. Our results show that the lowest values of TNFAIP3 mRNA expression level were in Class V compared to Class IV, Class III, and Class II. According to the current study results, the effectiveness and strength of TNFAIP3 mRNA expression level for differentiating SLE a from the control group we applied ROC curve, the sensitivities and specificities were 96.8% and 83.3%, respectively. Regards discriminating LN among SLE the sensitivities and specificities were 91.7% and 82.2%, respectively. Thus, TNFAIP3 mRNA expression level could be a useful diagnostic test to discriminate between SLE patients in particular LN patients. Conclusion: Non-LN group had statistically significant higher values of TNFAIP3 mRNA expression level compared to LN and control groups. However, the values decreased with more damage to kidney tissues and progression of SLE activity thus, TNFAIP3 mRNA expression level could be used as a genetic marker of LN susceptibility and severity.

Methods: An exploratory study was conducted on RA patients with ongoing combined methotrexate/ abatacept therapy and reporting a case who developed small-vessel vasculitis. Follow-up visits with thorough clinical examination and routine laboratory investigations were done every 3-6 months. Disease activity was assessed by the Clinical Disease Activity Index (CDAI). Functional assessment was done by Health Assessment Questionnaire (HAQ) score. Results: Sixteen biologic-naive female patients with newly diagnosed moderate-to-severe RA under abatacept therapy (mean age, 44 ± 10.86 years) were enrolled in this study. The mean disease duration was 4.63 ± 2.20, mean duration of methotrexate/abatacept therapy was 22.75 ± 9.40 months. Fibromyalgia (FM) is a diffuse, chronic pain syndrome associated with tender body areas and somat... more Fibromyalgia (FM) is a diffuse, chronic pain syndrome associated with tender body areas and somatic complaints. A high prevalence of undetected carpal tunnel syndrome (CTS) in patients with FM has been reported and it seems to affect their functional level. The aim of the work: This study was designed to compare between the presence of CTS in FM patients and control subjects and to find out if the severity of CTS is correlated with the functional impairment questionnaire (FIQ) score. Patients & methods: The study was carried out on 100 females, divided into two groups: 40 patients with fibromyalgia, their ages ranged from 20 to 45 years and 60 healthy controls, their ages ranged from 20 to 42 years. All subjects were subjected to full history taking, full locomotor examination, calculation of body mass index (BMI) and electrodiagnosis for CTS. Patients were asked to complete the FIQ. Results: FM patients had a significantly higher frequency of CTS as compared to the control group. Moderate CTS was more frequent among the patients. There was a highly statistically significant correlation between the severity of CTS and the FIQ score. Also there was a highly statistically significant difference as regards the FIQ score between the two subgroups of patients. Conclusion: CTS is more frequent in patients with FM than the normal population. Correlations between the severity of CTS and the FIQ score prove that CTS is one of the main causes of impairment in patients with FM.
