Few reports on panic disorder in children are available, despite the retrospectively documented o... more Few reports on panic disorder in children are available, despite the retrospectively documented onset in childhood of about 20% of the cases of adult panic disorder. The authors report on six prepubertal children, aged 8 to 13 years, who met DSM-IlI-R criteria for adult-type panic disorder. Hyperthyroidism, cardiologic, and respiratory problems were excluded as well as abuse of caffeine or other drugs. The first panic attack occurred between 5 to 11 years of age, with an average interval of 3 years between onset of the disorder and diagnosis. Mitral valve prolapse was documented in two cases. Family history was always positive for panic disorder. Although not common, panic disorder should be considered in children with school phobia and positive family history. As it is in adults, mitral valve prolapse may be associated with panic disorder in children.

SummaryAbnormal sexual behaviour in pre-pubescent children is uncommon even in a psychiatrically ... more SummaryAbnormal sexual behaviour in pre-pubescent children is uncommon even in a psychiatrically referred population. Among 516 referrals there were 6 boys and 10 girls, the girls starting younger. The boys differed little in history or background from other conduct-disordered boys, but the girls were more commonly victims of sexual abuse and lacking a father in the home. The personalities of the girls suggested continued risk of sexual experimentation and possible development of personality disorder.

These are the results of a retrospective study in which we reviewed the charts of 38 consecutive ... more These are the results of a retrospective study in which we reviewed the charts of 38 consecutive aggressive mentally retarded persons who were treated with lithium carbonate. All subjects were residents of a university operated program that serves mentally retarded persons with behavioral disorders. Over 63% of these subjects evidenced a greater than 30% reduction in the frequency of aggressiveness subsequent to the start of lithium therapy. Subjects with higher serum lithium levels had a more favorable response to lithium, along with those exhibiting higher levels of hyperactivity and violence before treatment.

Blood and breath acetaldehyde levels were measured following ethanol ingestion (0.5 ml/kg) in 11 ... more Blood and breath acetaldehyde levels were measured following ethanol ingestion (0.5 ml/kg) in 11 boys familially at risk for alcoholism and 11 age-matched controls. No significant differences were found between groups for acetaldehyde, objective, or subjective measures of intoxication. Previous reports of acetaldehyde as a marker of risk for alcoholism were not confirmed. Baseline behavioral state predicted response to alcohol. Children tended to have a subjective response in a direction opposite from the baseline mood state.

We question their conclusions concerning prevention of adolescent suicide and management of those who have attempted suicide. Successful suicide by adults is almost always associated with a readily diagnosed condition: depressive illness, alcoholism, dementia, schizophrenia, or antisocial personality.1,2 Unfortunately it is not yet clear which disorders in children or adolescents are likely to lead to suicide, although Shaffer3 has reported a strong association with conduct disorder.

The revision of DSM-IlI and ch6:nges in policies of third-party payers have sensitized clinicians to the presence of psychopathology among mentally retarded persons. The authors discuss the relationship between mental illness and mental retardation and review recent research on the diagnosis of specific psychiatric disorders in these patients. Some problems, such as behavioral disruptiveness, psychoses, and phobias, are more prevalent among mentally retarded persons than among other populations, whereas other problems, such as akobolism and suicide, may be less common. Mentally retarded patients test the diagnostic and therapeutic standards and skills ofpsychiatrists. Many such patients have little insight or do not talk.
