To assess the Natalizumab efficacy and safety in cohort of MS patients treated in Eastern region ... more To assess the Natalizumab efficacy and safety in cohort of MS patients treated in Eastern region of Abu Dhabi Emirate. Methods Retrospective charts review of MS patients at Tawam hospital from January 2010 till May 2019. Results They were 34 females and 20 males, mean age 33 years, received Natalizumab in Tawam hospital. Natalizumab was a first DMT for twelve patients, while others used from 1 to 4 DMT before Natalizumab. Six patients had positive JCV antibodies before starting Natalizumab. The number of infusions ranged from 1 to 104 (an average 34). Annualized relapse rate before starting the drug was 1.12 (0-4) which decreased to 0.032 on Natalizumab. Brain MRI follow up revealed six patients (11.1%) who had new or active lesions after first 6 months of treatment. The average EDSS remain the same (2.66 before and after DMT), but in eleven patients (20.4%) EDSS improvement ranged from 0.5 to 3.5 points. Five patients (9.3%) had a clinical MS relapse on Natalizumab and nine (16.7%) had a progression in EDSS (from 1 to 6 points), most of them progressed to SPMS (average EDSS before DMT was 4.3 and after 6.3). Conclusion DMT with Natalizumab in our MS cohort revealed a high efficacy in patients who were carefully selected for treatment.

Tiesa, dabartinis medicininiais kriterijais pagrįstas negalios vertinimas stokoja ne tik visaverčio žmogaus kaip asmenybės įvertinimo, bet ir nerodo specifinių žmonių, turinčių negalią, poreikių bei vis dar esančių galimybių dalyvauti visuomeniniame gyvenime, kurias galima įgyvendinti, pritaikius tam tikras priemones. Tokia tarptautiniu mastu pripažinta negalios vertinimo priemonė kaip Tarptautinė funkcionavimo, neįgalumo ir sveikatos klasifikacija gali būti naudojama negalią vertinti biopsichosocialiniu požiūriu. Džiugu, jog Lietuvoje jau pradėta negalios reforma, kurios tikslas – pagerinti asmenų, turinčių negalią, individualių poreikių, pagalbos, informacijos teikimo prieinamumą, išvengti diskriminacijos bei palengvinti įsitraukimą į darbo rinką. Background: Multiple sclerosis (MS) is an inflammatory and degenerative disease of the central ne... more Background: Multiple sclerosis (MS) is an inflammatory and degenerative disease of the central nervous system and is triggered by several environmental factors in genetically predisposed people. Objectives: To explore which evaluation battery items used for evaluation of work capacity at baseline can best predict MS progression at 1 year follow-up. Methods: In this prospective single-centre study, participants with MS were recruited consecutively when visiting a neurologist for referral for the determination work capacity status at the Disability and Working Capacity Assessment Office. At baseline, a neurologist assessed patients using the following evaluation scales: Fatigue selfassessment, Fatigue Descriptive Scale (FDS), Memory self-assessment, Brief International Cognitive Assessment for Multiple Sclerosis (BICAMS), Short Form 36 (SF-36), and the Brief International Classification of Functioning and Disability (ICF) core set for MS. The Expanded Disability Status Scale (EDSS) was evaluated by neurologists at baseline and one year later. An increase in EDSS by 0.5 points after one year was defined as MS progression. Results: During the one year period among 72 participants, 21 fulfilled the criteria for MS progression. In more than 75% of these participants, impairments were found in the following ICF subitems at baseline: "energy and drive functions", "muscle and power functions", and "moving around". Greater impairments were identified in progressing participants. Progressing participants scored higher on the FDS and scored lower on the BICAMS and SF-36. Regression analysis indicated that the FDS sum score predicted MS progression one year later. Conclusions: Increased fatigue might indicate worsening in MS one year later.

Pastarąjį dešimtmetį sergančiųjų išsėtine skleroze (IS) darbingumo lygiui nustatyti taikoma biopsichosocialiniu požiūriu paremta Tarptautinė funkcionavimo, neįgalumo ir sveikatos klasifikacija (TFK) ne tik prisideda prie sergančiųjų būklės nustatymo, bet ir siūlo priemones geresnei jų įtraukčiai į visuomenę. Visgi Lietuvoje TFK principai darbingumo lygiui nustatyti taikomi fragmentiškai, dažniau remiamasi biologiniais ir fiziniais asmens sveikatos būklės parametrais. Šio darbo tikslas – atlikti išsamų Lietuvos sergančiųjų IS biopsichosocialinį vertinimą ir nustatyti, kaip jų darbingumo lygis siejasi su TFK įvairių kategorijų rezultatais. Tiriamieji ir tyrimo metodai.
