[1,2] The etiology of liver disease is multi-factorial. However, the growing epidemiological evidence indicates that a number of risk factors like excessive alcohol drinking, hepatitis B and C infection, obesity, family history etc., modulate Background: Areca nut (AN) is one of the addictive substances known to cause deleterious effects on oral cavity as well as on various body organs including liver. But, scanty information is available reporting the adverse effects of AN chewing on the liver. Aim: To study the risk of liver disease in AN habitual based on the relevant published data. Methods: The literature search was performed by an electronic search of the PubMed/Medline, Scopus and Google Scholar databases using proper MESH headings and retrieved the articles published from 1998 to 2021. The eligibility criteria included: Human studies, AN habitual as study participants, use of controls and articles published in English. Data were extracted regarding characteristics of studies, characteristics of AN exposure, effect estimate and outcome of the studies. Results: Total 253 articles were identified from various databases and 15 studies were selected that met the inclusion criteria. Among these, thirteen studies showed an association between AN habit and attenuation of risk of liver disease as determined by relative risk/odds ratio/hazard ratio. Eleven studies described additive effect of AN and HBsAg and/or Anti hepatitis C virus status on development of liver disease. However, two of the studies showed opposite results. The heterogeneity in the study designs, exposure characteristics, outcomes and confounders precluded further meta-analysis. Conclusion: The association between AN chewing and an increased risk of developing liver disease is noted which necessitates the need for AN cessation campaign.

Areca nut (AN) consumption is a practice of great antiquity in India and reported to be consumed ... more Areca nut (AN) consumption is a practice of great antiquity in India and reported to be consumed by 20 to 30% of the Indian population in last 2-3 decades. Various mechanisms have been reported with reference to its adverse effects on the liver, one of which could be ingestion of aflatoxin infected AN, as aflatoxin B1 is a known hepatotoxin. Reports are available stating that the most of the ANs available in the market are substandard and contaminated with cancer- causing fungus and chewing of such poor quality, infested ANs would definitely pose severe health problems on human beings in a long time. But, to our knowledge scanty literature is available reporting the role of aflatoxin infested AN on risk of liver cancer. The evolution of such risk factors provides insight into understanding of the future burden of the disease.

It involves any part of the oral cavity from the anterior to posterior oropharyngeal region and sometimes upper aerodigestive tract. Slowly progressive chronic inflammatory change from epithelium to underlying connective tissue causes the deposition of abnormal Introduction: Areca nut is deeply rooted sociocultural habit in India. Areca nut reported to be infested by fungi during the field and storage conditions. Areca nut alkaloids, nitrosamines, tobacco and aflatoxin are cytotoxic, immunotoxic to red blood cell and epithelial cell. Hence, the present study was conducted to assess the serum aflatoxin B 1 (AFB 1) antibody titer, percent hemolysis and transaminases in oral submucous fibrosis (OSMF) patients. Materials and Methods: In this study, 128 participants of which 88 were suffering from OSMF. Twenty participants were areca nut habitual without OSMF (habitual control) and 20 participants without any habit (healthy control). For the detection of AFB 1 antibody titer, AFB 1 antigen (Sigma) A6636 from Aspergillus flavus was used. Percent hemolysis was estimated as per the procedure described by Mathuria and Verma. Serum Glutamic oxalo acetic transaminase (SGOT) and Serum Glutamic pyruvic transaminase (SGPT) were estimated by the optimized ultraviolet method using the enzyme-linked immunosorbent assay kit. Results: Mean SGOT, SGPT, percent hemolysis and AFB 1 antibody titer were significantly higher in participants with OSMF than the habitual and healthy controls. AFB 1 antibody titer and % hemolysis in both OSMF and habitual without OSMF were showed significant correlation, i.e., increased AFB 1 antibody titer with increased % hemolysis. Conclusions: A study result demonstrates that aflatoxin causes increase in serum transaminases which is indicative of liver damage in OSMF. The combined toxic effects of areca nut alkaloids, tobacco and AFB 1 on red blood cell (RBC) cell wall might be responsible for increased percent hemolysis in OSMF and habitual control.

Background: It is essential to search for an efficient and reliable remedy for treating oral subm... more Background: It is essential to search for an efficient and reliable remedy for treating oral submucous fibrosis taking into account the shortcomings of present treatment modalities to either provide a complete relief or treating the patients at the cost of adverse effects. Objective: Assessment of utility of Tulsi and Turmeric in treatment of oral submucous fibrosis: a clinical study. Material and methods: The study included 30 individuals. All the clinical parameters including mouth opening and burning sensation were evaluated and recorded in specially designed proforma. The burning sensation and inter-incisal distance were measured on visual analogue scale (VAS) and vernier calibre (in millimetres), respectively. The patients were instructed to apply the gel (combination of tulsi and turmeric) to the affected areas of oral cavity five times in a day. Patients were recalled after every fifteen days for continuous three months (total 6 visits for each patient). On each visit, the mouth opening and burning sensation were recorded. Results: The difference between the mouth opening before and after the treatment was found to be statistically significant. Similar significant findings were observed with reference to the reduction in burning sensation in the present study. The mean of the burning sensation before treatment was 5.43±2.02 and after treatment was 0.20±.76 as assessed by VAS. On correlation of the findings of mouth opening and burning sensation in the subjects of different grades, the results were found to statistically significant. No clinically eminent adverse reactions to the therapy were seen in any patient. Conclusion: The combination of Tulsi and Turmeric can be considered as a better alternative to the modern treatment modality in the management of all grades of OSMF..

Methods: This prospective study was carried out on Orthopantomograms of 1000 subjects between the age group ranges of 18-70 years to evaluate the prevalence of root dilacerations in Vidarbh region using orthopantomograms. Angulation of each tooth root was measured using annotation software in the CR system. A single examiner analyzed all radiographs root angulations in mesial or distal direction starting from 50° were measured. Mini-CEX is an assessment approach which focuses on trainees’ encounters with a variety of real p... more Mini-CEX is an assessment approach which focuses on trainees’ encounters with a variety of real patient issues, in there, routine clinical practice. 60 mini-CEX encounters were carried out involving 15 Final BDS students. They took focused history, performed clinical examination of submandibular lymph node on a patient with apical periodontitis with mandibular first molar. This was done during their clinical posting of Oral Medicine. Four assessors observed students, rated the performance, by scoring trainee on a structured form and provided an immediate feedback. Efficacy of mini-CEX method was rated on a 9-point scale. Perception of all participants and assessors regarding mini-CEX method of evaluation was taken. The mean scores of all the 15 students from 1 st to 4 th encounters given by assessors were 4.20 +_0.67, 5.20+_0.56, 7.06+_0.59 and 8.60+_0.50 respectively. Serum Glutamic oxalo acetic transaminase (SGOT) and Serum Glutamic pyruvic transaminase (SGPT) were estimated by the optimized ultraviolet method using the enzyme-linked immunosorbent assay kit. Results: Mean SGOT, SGPT, percent hemolysis and AFB 1 antibody titer were significantly higher in participants with OSMF than the habitual and healthy controls. AFB 1 antibody titer and % hemolysis in both OSMF and habitual without OSMF were showed significant correlation, i.e., increased AFB 1 antibody titer with increased % hemolysis. Conclusions: A study result demonstrates that aflatoxin causes increase in serum transaminases which is indicative of liver damage in OSMF. The combined toxic effects of areca nut alkaloids, tobacco and AFB 1 on red blood cell (RBC) cell wall might be responsible for increased percent hemolysis in OSMF and habitual control.

Objective: Assessment of utility of Tulsi and Turmeric in treatment of oral submucous fibrosis: a clinical study. Material and methods: The study included 30 individuals. All the clinical parameters including mouth opening and burning sensation were evaluated and recorded in specially designed proforma. The burning sensation and inter-incisal distance were measured on visual analogue scale (VAS) and vernier calibre (in millimetres), respectively. The patients were instructed to apply the gel (combination of tulsi and turmeric) to the affected areas of oral cavity five times in a day. Patients were recalled after every fifteen days for continuous three months (total 6 visits for each patient). On each visit, the mouth opening and burning sensation were recorded. Results: The difference between the mouth opening before and after the treatment was found to be statistically significant. Similar significant findings were observed with reference to the reduction in burning sensation in the present study. The mean of the burning sensation before treatment was 5.43±2.02 and after treatment was 0.20±.76 as assessed by VAS. On correlation of the findings of mouth opening and burning sensation in the subjects of different grades, the results were found to statistically significant. No clinically eminent adverse reactions to the therapy were seen in any patient. Conclusion: The combination of Tulsi and Turmeric can be considered as a better alternative to the modern treatment modality in the management of all grades of OSMF..

They took focused history, performed clinical examination of submandibular lymph node on a patient with apical periodontitis with mandibular first molar. This was done during their clinical posting of Oral Medicine. Four assessors observed students, rated the performance, by scoring trainee on a structured form and provided an immediate feedback. Efficacy of mini-CEX method was rated on a 9-point scale. Perception of all participants and assessors regarding mini-CEX method of evaluation was taken.
