This study adds to the knowledge base by comparing the efficacy of Culturally Informed and Flexible Family Treatment for Adolescents (CIFFTA) to an Individually Oriented Treatment-As-Usual (ITAU) in its ability to retain Latino youth and families in treatment, reduce internalizing and externalizing child symptoms, and improve family functioning. CIFFTA uses an adaptive/flexible approach to deliver individual therapy, family therapy, and psycho-educational modules tailored to each family's unique clinical and cultural characteristics. Two hundred Latino adolescents 11-14 years of age completed a baseline assessment, were randomly assigned to CIFFTA or ITAU, then were assessed again after 16 weeks of intervention. Results show that CIFFTA had significantly higher retention (83%) than ITAU (71%), OR = 2.05, p = .036. Youth in both conditions showed significant reductions in youth and parent reported externalizing and internalizing behaviors, and there were no differences in change between conditions. Parents in CIFFTA reported significantly greater reductions in family conflict, d = 0.38, p = .025 than in ITAU. In CIFFTA, children of less acculturated Latino parents showed more improvement than the children of more acculturated parents. In ITAU, the reverse was true, children of more acculturated parents reported more improvement. This evidence of CIFFTA's impact on retention, family conflict, and differential effect depending on cultural values and behaviors, has important implications for the field of Latino psychology and family treatment.

Purpose of review The comorbidity between psychiatric and substance use disorders remains an impo... more Purpose of review The comorbidity between psychiatric and substance use disorders remains an important phenomenon to understand, and an active area of investigation. The purpose of this review is to highlight key 2011 issues and novel findings on psychiatric and substance disorders comorbidity from the National Epidemiologic Survey on Alcohol and Related Conditions (NESARC), a large national survey of the US general population. Recent findings Topics of active investigation included the internalizing/externalizing meta-structure of common mental disorders; the 10 Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition (DSM-IV) personality disorders; the 3-year incidence and persistence of disorders; treatment of major depression; and many other topics not as easily categorized. Summary Meta-structure may increasingly offer a parsimonious way of addressing comorbidity, although adding new disorders adds complexity and the value of etiologic analyses utilizing broad dimensions of psychopathology rather than individual disorders is not yet fully known. Expanding the range of personality disorders beyond antisocial personality disorder appears essential in understanding the incidence and persistence of substance use disorders. Substance use disorders have low rates of treatment relative to major depression, but increase the likelihood of depression treatment among comorbid cases, a phenomenon that needs to be understood. These comorbidity studies provide much novel information, and indicate many potentially fruitful directions for new research.

Current literature supports both adaptive treatments and technology-assisted interventions as highly promising innovations for treating at-risk adolescents. The purpose of this investigation was to develop and test the efficacy of a computer-assisted version of an established office-based multicomponent family therapy. Eighty Hispanic and Black Non-Hispanic adolescents and their families participated in the study and were randomized to either Immediate Computer-Assisted CIFFTA or Delayed Computer-Assisted CIFFTA. Significant between-groups effects were found from baseline to posttreatment showing the superiority of the Immediate CA CIFFTA condition on both the Conduct Disorder (B = -5.17, SE = 1.73, p < .01, CI [-8.55, -1.79]) and Socialized Aggression (B = -2.04, SE = .83, p < .05, CI [-3.67, -.41]) subscales of the Revised Behavior Problem Checklist, on the Youth Self Report Externalizing scale (B = -4.22, SE = 1.40, p < .01, CI [-6.95, -1.48]), and on both the parent (B = 1.34, SE = .50, p < .01, CI [.36, 2.32]) and adolescent (B = 1.31, SE = .46, p < .01, CI [.41, 2.21]) reports of the Family Environment Scale’s family cohesion subscale. Baseline to 6-weeks posttreatment (T1-T3) analyses showed that these significant within-subjects effects were sustained for the treatment group. Results highlight that adolescent behavior problems can be significantly impacted by a computer-assisted intervention that replaces psychoeducational face-to-face meetings with computer-delivered modules.

La bibliografia muestra una escasez de intervenciones de prevencion para ITS-VIH orientadas a esta poblacion de alto riesgo y un conjunto unico de barreras para recibir mensajes de prevencion. Las intervenciones basadas en Internet estan prometiendo proporcionar intervenciones de prevencion para ITS-VIH y evitar barreras a los servicios. Objetivos: El estudio tenia por finalidad desarrollar una intervencion de prevencion de ITS-VIH basada en Internet y culturalmente informada para mujeres chilenas entre 18 y 24 anos, investigar su factibilidad y aceptabilidad y recopilar recomendaciones sobre lo que haria la intervencion mas aceptable y viable para estas mujeres. Metodos: El desarrollo de la intervencion a traves de Internet fue facilitado por un proceso que incluyo la consulta con expertos en contenido y tecnologia. Se utilizo un diseno pre-postest para probar la aceptabilidad y la factibilidad de la intervencion con 40 mujeres jovenes chilenas de entre 18 y 24 anos de edad. Resultados: El sitio web de la intervencion constaba de cuatro modulos de contenidos y actividades para apoyar el aprendizaje. La intervencion fue factible y aceptable para mujeres chilenas entre 18 y 24 anos de edad. Discusion y conclusion: Este estudio demostro el valor de la implicacion de multiples paneles de expertos para desarrollar intervenciones culturalmente informadas y basada en tecnologia. Los resultados de este estudio apoyan la factibilidad y la aceptabilidad de la conduccion de una intervencion basada en Internet con sesiones multiples, produciendo altas tasas de partcipacion en una poblacion en la que existen barreras a la exposicion de prevencion de ITS-VIH y a contenidos relacionados con el sexo. Implicaciones para la enfermeria y la politica sanitaria: Los resultados tienen implicaciones para la educacion y la practica clinica de la enfermeria y pueden ser utilizados por sistemas legales y judiciales para promover o reforzar politicas que fomenten estrategias de prevencion de ITS-VIH en mujeres.

This study investigated the efficacy of brief strategic family therapy (BSFT) with Hispanic behav... more This study investigated the efficacy of brief strategic family therapy (BSFT) with Hispanic behavior problem and drug using youth, an underrepresented population in the family therapy research literature. One hundred twenty-six Hispanic families with a behavior problem adolescent were randomly assigned to 1 of 2 conditions: BSFT or group treatment control (GC). Results showed that, compared to GC cases, BSFT cases showed significantly greater pre-to post-intervention improvement in parent reports of adolescent conduct problems and delinquency, adolescent reports of marijuana use, and observer ratings and self reports of family functioning. These results extend prior findings on the efficacy of family interventions to a difficult to treat Hispanic adolescent sample. Disruptive behaviors and substance use continue to be among the most common presenting problems associated with child and adolescent referrals to mental health services (Kazdin, 1991; Substance Abuse and Mental Health Services Administration [SAMHSA], 2001). These problems are also among the most frequently diagnosed conditions in both outpatient and inpatient mental health facilities for children (Loeber & Stouthamer-Loeber, 1998).
