This study was aimed to investigate the association between serum Vitamin B12 (vitB12), body mass index (BMI), and nutritional status among obese women. Methods: This cross-sectional study enrolled consecutive female subjects. The consumptions of red meat, fish, bovine liver, egg, and mushroom were recorded. According to the Dietary Reference Intakes, the patients were categorized as insufficiency and sufficiency. Three cutoff points were defined for vitB12 status: (1) Deficiency if vitB12 is <200 pg/mL; (2) insufficiency if vitB12 is 250-350 pg/mL, and (3) sufficient if vitB12 is ≥350 pg/mL. According to BMI, the patients were assigned to nonobese and obese groups. BMI, serum vitB12 level, consumptions of red meat, fish, bovine liver, egg, and mushroom were evaluated and compared between two groups. Results: Mean level of vitB12 was 247.8 ± 10.4 pg/mL and significantly associated with consumption of egg (P = 0.031), bovine liver (P = 0.004), mushroom (P = 0.040), and red meat (P = 0.003). VitB12 was significantly higher in nonobese than obese group (282.5 ± 106.8 vs. 242.5 ± 107.5 pg/mL, P = 0.001). The ratio of vitB12 deficiency was significantly higher in obese than nonobese group (37.6% vs. 24.7%; P = 0.019). VitB12 level was negatively correlated with BMI (r = −0.155; P < 0.001), but not insulin resistance (r = −0.172; P = 0.062). Conclusion: Obesity was associated with low level of vitB12 in obese women, and more likely to be vitB12 deficient. Consumption of certain types of food contributes to increase vitB12 level.

Gastrointestinal, cardiovascular, pulmonary and neurological symptoms are well known symptoms in mad-honey intoxication. Hypnic headache is a rare form of primary headaches. It is usually seen in older people and occurs during night sleeping. Its pathogenesis is not exactly known. Herein, we present a case of mad-honey intoxication mimicking hypnic headache due to ingestion of mad-honey, which has been commonly used for healing among people living Black Sea region. The patient was admitted to our clinic with throbbing headache during night sleeping. It was diagnosed with hypnic headache, based on her anamnesis, neurological examination, and laboratory findings. The patient implied that her headache had occurred whenever she ingested some amount of mad-honey before sleeping, disappeared with its cessation and reoccurred with its ingestion, so we considered that headache was induced by intake of mad-honey. In this report, we aimed to highlight the importance of history taking about traditional foods triggering headache during consultation with patient.

Research paper thumbnail of Effects of prehypertension on arterial stiffness and wave reflections

Clinical and Experimental Hypertension, 2010

The Seventh Report of the Joint National Committee on the Prevention, Detection, Evaluation, and ... more The Seventh Report of the Joint National Committee on the Prevention, Detection, Evaluation, and Treatment of High Blood Pressure classifies blood pressure (BP) as normal, prehypertension, and hypertension. Although it has been shown that there is a relationship between hypertension and arterial stiffness, there is not sufficient data about arterial stiffness in patients with prehypertension. The present study was designed to evaluate arterial stiffness and wave reflections in subjects with prehypertension. We evaluated arterial stiffness and wave reflections of 45 subjects with prehypertension and an age-matched control group of 40 normotensive individuals, using applanation tonometry (Sphygmocor, AtCor Medical, Sydney, Australia). Aortic pulse wave velocity (PWV) was measured as indices of elastic-type aortic stiffness. The heart rate-corrected augmentation index (AIx@75) was estimated as a composite marker of wave reflections and arterial stiffness. Aortic PWV (10 +/- 2.5 vs. 8.6 +/- 1.7, m/s, p = 0.004) and AIx@75 (21 +/- 8.3 vs. 10 +/- 9.1, %, p = 0.0001) were significantly higher in subjects with prehypertension than in the control group. In multiple linear regression analysis, we found that the presence of the prehypertension was a significant predictor of aortic PWV (beta = 0.26, p = 0.009) and AIx@75 (beta = 0.46, p = 0.0001). Our results suggest that arterial functions were impaired even at the prehypertensive stage.

We aimed to evaluate knowledge levels about the health effects of smoking and attitudes toward smoking and tobacco control activities among primary care providers. Methods: In the cross-sectional and primary care-based study, self-administered surveys modified from the WHO Global Health Professional Survey 5A steps of smoking cessation practice (Ask, Advise, Assess, Assist and Arrange) were provided to primary care physicians (PCPhs) and nurses (PCNs). Results: Respondents included 1182 PCPhs and 1063 PCNs. The proportions of current and former smokers were significantly higher among PCPhs than among PCNs (34.4 vs. 30.7 % and 14.0 vs. 10.1 %, respectively; both P < 0.001). We observed that 77.2 % of PCPhs and 58.4 % of PCNs always or rarely practiced an "Ask" step about their patients' smoking status (P < 0.001). One-third of PCPhs (33.8 %) stated that they always practiced an "Ask" step, whereas only 27.6 % of PCNs always did so in their practice (P < 0.001). A small minority of primary care providers had advised patients to quit smoking, although there was a significant difference in this between PCNs and PCPhs (8.4 vs. 15.6 %; P < 0.001). Most PCPhs considered themselves competent in advising about smoking interventions, but only a minority of PCNs did so (75.1 vs. 17.3 %; P < 0.001). Among barriers to tobacco intervention measures, lack of time was the item most commonly cited by PCPhs, whereas low patient priority was most commonly cited by PCNs (35.9 and 35.7 %; P < 0.001). Conclusions: Smoking intervention practice by primary care nurses was quite low. Lack of time and low patient priority were identified as barriers by primary care providers. Strategies by which primary care providers could improve tobacco control should be established.

Apoptosis, is a necessary physiological process for cell elimination which is very important both cellular homeostasis and cell proliferation and differantiation. This also leads to uncontrolled cell growth and tumor development. Survivin, a member of the IAP family, plays a key role in promotion of cell proliferation as well as inhibition of apoptosis in cancer cells. The aim of this study was to investigate whether specific genetic polymorphisms of Survivin could be associated with colon cancer development and progression in a Turkish population. Our study is the first to our knowledge to investigate the relationship between colon cancer risk and Survivin gene polymorphisms. Methods: The relation between colon cancer and survivin -31 G/C (rs9904341), -241 C/T (rs17878467) and -625 C/G (rs8073069) polymorphism in promotor site of survivin gene associated with apoptosis was investigated by using polymerase chain re...

Research paper thumbnail of A Comparison of Error Correcting Output Coding Methods for Multiclass Classification by Using Support Vector Machine: The Prediction of Self-Monitoring of Blood Sugar

International Journal of Statistics in Medical Research, 2013

However, in many real-world problems, particularly pattern recocnation studies, aimed to determine the distinctive features of large number of class or group. For this reason, a number of methods to generate multiclass SVMs from binary SVMs have been proposed by researchers and this is still a continuing research topic. In this study we aimed to compare classification accuracy and computational cost of four multiclass approaches using a original and simulated data sets. Error Corrected Output Coding (ECOC) based multiclass approaches that is used in this study creates many binary classifiers and combines their results to determine the class label of a test pixel. As a result of the comparisons for all conditions examined in this study, it's found that the classification accuracy and computational cost of One vs. One multiclass approach is better than the other multiclass approaches. In classification or pattern recognization problems, some of supervised machine learning methods or algorithms can be easily extended to multiclass problems. However, some other powerful and popular classifiers, such as AdaBoost and Support Vector Machines, do not extend to multiclass easily. In those situations, the usual way to proceed is to reduce the complexity of the multiclass problem into multiple simpler binary classification problems.

Moreover, multiple sclerosis (MS) is the most frequent demyelinating disease and has been associated with various chronic inflammatory diseases. However, its association with JRA has not been frequently described. Autoimmunity in both JRA and MS has been documented in the scientific literature, although there has been no definitive finding that patients with JRA are prone to the development of MS. An increasing frequency of MS resulting from an increased use of antitumor necrosis factor agents in the treatment of rheumatoid arthritis and other chronic inflammatory diseases has been reported recently. In this study, we report on the development of MS in a patient with JRA who did not have a history of antitumor necrosis factor use.

Yöntem: Bu tanımlayıcı ve kesitsel çalışmaya, 70'i (%60) erkek, 47'si (%40) kadın 26-34 yaş aralığında 117 araştırma görevlisi katıldı.

We aimed to investigate association between severity of metabolic syndrome (MetS) and anthropometric measurements, and to evaluate correlation of MetS and its components with metabolic deterioration and inflammatory indexes. The cross-sectional study enrolled 1474 patients with obesity and overweight. The patients were grouped as MetS and Non-MetS, and were sub-grouped as group 1 (three criteria), 2 (four criteria) and 3 (≥ five criteria) according to NCEP ATP III. Mean age was 38.7 ± 11.9 years and BMI was 35.1 ± 6.3 kg/m(2). Lipid profile, anthropometric and blood pressure measurements, liver function tests, bioelectric impedance body fat compositions, insulin resistance and HbA1c, and spot urinary albumin-creatinine ratio were significantly different between groups of MetS and Non-MetS. Age, lipid profile, bioelectric impedance fat analyses, BMI, blood pressure values, glucose, insulin resist...
