The first thought that comes to the mind of an expecting woman regarding her delivery is the pain of labour. The major role and responsibility of the nurse is in identifying the problems of the woman in labour, providing appropriate information regarding the alternative modalities of pain relief during labour. A pre-experimental study was conducted to evaluate the effectiveness of back massage on pain among pregnant women in first stage of labour pains in a selected Hospital, Ludhiana, Punjab. The objectives of the study were to assess the pre-test level of pain in first stage of labour pains among pregnant women, to administer the back massage in first stage of labour pains, to assess the post-test level of pain and to compare the pre-test and post-test level of pain in first stage of labour pains among pregnant women and to determine the relationship of pre-test and post-test level of pain with the selected variables. Methods: Conceptual framework was based on General system theory by Ludwig Von Bertanlanffy. Modified Labour Pain Relief Tool and Participants Opinionnarie were used to assess the effectiveness of back massage. Results: Findings of the study were in the pre-test mean score was 5.83 and post-test mean score was 3.75 which was found statistically highly significant at p<0.01 level. Gravida had significant impact on level of pain. Back massage had impact on level of pain among pregnant women. Conclusions: Present study revealed that in the pre-test mean score was 5.83 and post-test mean score was 3.75 which was found statistically highly significant at p<0.01 level. Age, education, mother's occupation, period of gestation and any history of abortion had no significant relationship with pain, and gravida had statistically significant relationship with pain. Back massage had impact on pain level. Therefore it was concluded that back massage was effective to reduce the level of pain.

To date only 23 cases of synovial sarcoma of the hypopharynx have been reported in the literature. An additional case in an 18-year-old male is presented. This is the first case of synovial sarcoma in the hypopharynx to be reported in Singapore. The presentation was that of a mass in the hypopharynx; progressive dysphagia, intermittent hoarseness and gradual airway compromise. A CT scan was valuable in determining the site of origin and extent of the lesion. Histopathology was diagnostic. Treatment comprised of wide surgical excision of the tumour and post-operative radiotherapy.

A 35-year-old male presented to us with a painless, gradually progressive mass in the right ear o... more A 35-year-old male presented to us with a painless, gradually progressive mass in the right ear of 2 years duration. There was no history of ear discharge or hearing loss and no history suggestive of any similar lesion elsewhere in the body. Examination revealed a yellowish-brown, nontender, firm, and polypoidal mass measuring approximately 5 × 5 mm apparently arising from the floor of the external auditory canal just lateral to the bony cartilaginous junction almost completely obliterating the external auditory canal (Fig. 1). The mass was excised with adequate margins and subjected to pathological evaluation. A normal and intact tympanic membrane could be visualized after excision of the mass. Gross pathologic evaluation revealed a single, yellow to dirty brown colored, skin-lined mass measuring 5 × 5 × 7 mm (Fig. 2). Histologically, a low power view [50×, hematoxylin and eosin (H&E) staining] showed a large conglomerate of multiple pilosebaceous units with keratotic plugging (Fig. 3). High power view (250×, H&E staining) showed foci with nests of basaloid cells along with immature hair follicles (Fig. 4). With this histopathological evaluation the final diagnosis of the lesion was given as nevus sebaceous. The patient has been under follow-up for almost 5 years and is free of any recurrence or any other lesion at the site. DisCussion Nevus sebaceous has been described by Jadassohn as a congenital organoid mass that contains an excess of glandular and epidermal structures. 1 It usually presents as a yellow-orange-colored, waxy, pebble-like papule or plaque. 2 The incidence of this lesion is approximately 0.68% with equal incidence among males and females without any racial predilection. 3,4 Nevus sebaceous primarily occurs on the face, scalp, and periauricular region. 4 The presence of this lesion in the external auditory canal is extremely rare and a review of pertinent English

The patient was planned for excision under general anesthesia and took fitness for the same. Under general anesthesia, the foreign body was palpated and an approximately 4 cm long horizontal incision was given over it (Fig. 2). The wound site was explored and after dissecting the superficial soft tissue, the black-colored metallic foreign body was traced (Fig. 3). Further dissection all around the metallic body allowed an in toto retrieval of the foreign body (Fig. 4). Following a drain insertion at the wound site, the surgeon completed closure. Postoperative antibiotic and analgesic were given. The patient's postoperative recovery was uneventful.

Among various surgical modalities, surgical transaction/excision of posterior aspect of the vocal folds is an established modality. The objective of the present study is to evaluate the effectiveness and safety of the technique using Coblation technology. Study Design: This was a retrospective review. Setting: This study was conducted in a tertiary health care center. Materials and Methods: Fourteen consecutive patients meeting inclusion criterion undergoing posterior cordectomy using Coblator were included in this study. Preoperative subjective voice analysis was done and compared with measurements at 6 months postprocedure. Parameters studied included voice handicap index (VHI), the need for repeat procedures, effect of decannulation, and subjective improvement in respiratory distress. Results: There was a significant improvement in the airway as adjudged by the postoperative subjective improvement in respiratory distress (in 2 nontracheostomized patients) and uneventful decannulation (in 12 tracheostomized patients). However, the postoperative VHI scores showed no statistically significant difference with the preoperative assessment. One of the patients required the procedure to be repeated owing to recurrence of respiratory distress. No untoward incident was reported in any of the surgical procedures. Conclusion: Posterior cordectomy using a Coblator offers a safe and efficacious option for the management of compromised airway secondary to bilateral abductor paralysis of vocal folds. It offers a significant airway improvement without any significant effect on voice.

The aim of the present study is to compare the vocal outcomes of microflap based vocal cord reduction using cold knife and CO2 laser for managing patients with Reinke’s edema. The study involves a retrospective analysis of 24 consecutive patients meeting inclusion criterion undergoing vocal cord reduction for managing Reinke’s edema using cold knife or CO2 laser in our institution. The pre and postoperative VHI and VQE scores were measured and compared. There was a significant improvement in the postoperative scores in both the cold knife as well as the CO2 laser group. This study aims to compare endoscopic myringoplasty with microscopic myringoplasty surgery based on uptake of graft, hearing improvement in terms of pure tone audiograms performed preoperatively and postoperatively, complications in the postoperative period, and subjective cosmetic results. Materials and methods: Between January 2019 and December 2019, 100 patients were subjected to myringoplasty, 50 performed endoscopically and 50 performed microscopically. Results were compared 3 months following the surgery. Results: This article reveals that there was no statistically significant difference between the two groups regarding the success rate. In this study, the endoscopic myringoplasty group shows fewer incidences of postoperative pain and better cosmetic results. Conclusion: Most of the disadvantages of the microscope can be overcome by an endoscope by virtue of its wide-angled telescopic and magnified view. However, the endoscope has the disadvantages of a comparative lack of depth perception and one-handed technique. These can be easily overcome with practice. Thus, endoscopic myringoplasty may be a good alternative to microscopic myringoplasty. Clinical significance: The study gives significant insight into the comparative outcomes of endoscopic and microscopic myringoplasty.

Isolated lingual cysticercosis is a rare entity due to the high muscular activity and metabolic rate of tongue which prevents the development of larva. We present a series of three patients with isolated lingual cysticercosis. One patient was treated by medical management while the other two were treated with surgical excision followed by medical therapy. All patients are asymptomatic after repeated follow ups.

Various surgical options have been proposed for managing this condition. The present study aims to share our experience of using two established surgical modalities in managing this condition. Study design: Retrospective review. Setting: Tertiary health care center. Materials and methods: Twenty-three consecutive patients meeting inclusion criteria undergoing posterior cordotomy using CO 2 laser (n = 9) and posterior cordectomy using coblator (n = 14) were included in this study. Preoperative subjective voice analysis was done and compared with measurements at 6 months post procedure. Parameters studied included voice handicap index (VHI), the need for repeat procedures, the effect of decannulation and subjective improvement in respiratory distress. Results: In all the patients undergoing these surgical procedures, there was a significant improvement in the airway as adjudged by the postoperative subjective improvement in respiratory distress (in five non-tracheostomized patients) and uneventful decannulation (in 18 tracheostomized patients). However, there was no statistically significant difference in either of the two techniques. The postoperative VHI scores showed no statistically significant difference with the preoperative assessment using either of the techniques. Two of the patients in the CO 2 laser group and one patient in the coblator group required the procedure to be repeated owing to recurrence of respiratory distress. Conclusion: Both CO 2 laser-assisted posterior cordotomy and coblator assisted posterior cordectomy are safe and efficacious options for the management of compromised airway secondary to bilateral abductor paralysis of vocal folds with comparable outcomes.

Complete surgical excision of the tumour was performed and the histological evaluation of the excised specimen was consistent with schwannoma. A brief discussion of the case along with the differential diagnosis of such lesions is discussed.

To determine the effectiveness of educational film regarding awareness for PPIUCD as a method of family planning, 3. To find out the association of knowledge with their selected demographic variables. Method: A pre experimental study was conducted to evaluate the effectiveness of educational video regarding awareness for PPIUCD among 40 antenatal women which were selected using purposive sampling technique and pre and post test were administered which consists of structured knowledge questionnaire. Results: The mean pre-test and post-test score were 4.775 and 11. Since (t) =11.21 > t0.05= 3.3128 (table value) which concludes that there is significant difference between mean post test knowledge score and mean pre test knowledge score. Thus, H1 is accepted. There is significant association of Occupation and economic status at 0.05 level of significance. Educational Status, Previously used contraception, future pregnancy, Age in years and Religion and type of family has no significant association. Conclusion: The study concluded that there is need to motivate people for family planning through use of AV Aids so that antenatal women are aware of the latest modalities including PPIUCD so they could use PPIUCD as method of family planning.

Prospective study. Tertiary health care centre. We studied 180 patients with TCA who were managed in the Department of Otolaryngology between December 2004 and June 2007. There were 76 male patients (42%) and 104 females (58%), with ages ranging from 5 to 58 years. All patients presented with single or multiple neck swellings. Constitutional symptoms such as fever were present in 52 patients (29%), whereas cough, weight loss, and reduced appetite were seen in 40 patients (22%). Forty-seven patients (26%) had bilateral lymph node involvement. Only 23 patients (13%) had chest radiographs suggestive of pulmonary tuberculosis. Fine-needle aspirates from 168 patients (93%) showed granulomas with caseating necrosis. However, acid-fast bacilli were seen in only 45 (25%) aspirate smears. Among various surgical modalities, surgical transaction/excision of posterior aspect of the vocal folds is an established modality. The objective of the present study is to evaluate the effectiveness and safety of the technique using Coblation technology. Study Design: This was a retrospective review. Setting: This study was conducted in a tertiary health care center. Materials and Methods: Fourteen consecutive patients meeting inclusion criterion undergoing posterior cordectomy using Coblator were included in this study. Preoperative subjective voice analysis was done and compared with measurements at 6 months postprocedure. Parameters studied included voice handicap index (VHI), the need for repeat procedures, effect of decannulation, and subjective improvement in respiratory distress. Results: There was a significant improvement in the airway as adjudged by the postoperative subjective improvement in respiratory distress (in 2 nontracheostomized patients) and uneventful decannulation (in 12 tracheostomized patients). However, the postoperative VHI scores showed no statistically significant difference with the preoperative assessment. One of the patients required the procedure to be repeated owing to recurrence of respiratory distress. No untoward incident was reported in any of the surgical procedures. Conclusion: Posterior cordectomy using a Coblator offers a safe and efficacious option for the management of compromised airway secondary to bilateral abductor paralysis of vocal folds. It offers a significant airway improvement without any significant effect on voice.

This study aims to compare endoscopic myringoplasty with microscopic myringoplasty surgery based on uptake of graft, hearing improvement in terms of pure tone audiograms performed preoperatively and postoperatively, complications in the postoperative period, and subjective cosmetic results. Materials and methods: Between January 2019 and December 2019, 100 patients were subjected to myringoplasty, 50 performed endoscopically and 50 performed microscopically. Results were compared 3 months following the surgery. Results: This article reveals that there was no statistically significant difference between the two groups regarding the success rate. In this study, the endoscopic myringoplasty group shows fewer incidences of postoperative pain and better cosmetic results. Conclusion: Most of the disadvantages of the microscope can be overcome by an endoscope by virtue of its wide-angled telescopic and magnified view. However, the endoscope has the disadvantages of a comparative lack of depth perception and one-handed technique. These can be easily overcome with practice. Thus, endoscopic myringoplasty may be a good alternative to microscopic myringoplasty. Clinical significance: The study gives significant insight into the comparative outcomes of endoscopic and microscopic myringoplasty.

One patient was treated by medical management while the other two were treated with surgical excision followed by medical therapy. All patients are asymptomatic after repeated follow ups.

Results: In all the patients undergoing these surgical procedures, there was a significant improvement in the airway as adjudged by the postoperative subjective improvement in respiratory distress (in five non-tracheostomized patients) and uneventful decannulation (in 18 tracheostomized patients). However, there was no statistically significant difference in either of the two techniques. The postoperative VHI scores showed no statistically significant difference with the preoperative assessment using either of the techniques. Two of the patients in the CO 2 laser group and one patient in the coblator group required the procedure to be repeated owing to recurrence of respiratory distress. Conclusion: Both CO 2 laser-assisted posterior cordotomy and coblator assisted posterior cordectomy are safe and efficacious options for the management of compromised airway secondary to bilateral abductor paralysis of vocal folds with comparable outcomes.

Complete surgical excision of the tumour was performed and the histological evaluation of the excised specimen was consistent with schwannoma. A brief discussion of the case along with the differential diagnosis of such lesions is discussed.

To determine the effectiveness of educational film regarding awareness for PPIUCD as a method of family planning, 3. To find out the association of knowledge with their selected demographic variables. Method: A pre experimental study was conducted to evaluate the effectiveness of educational video regarding awareness for PPIUCD among 40 antenatal women which were selected using purposive sampling technique and pre and post test were administered which consists of structured knowledge questionnaire. Results: The mean pre-test and post-test score were 4.775 and 11. Since (t) =11.21 > t0.05= 3.3128 (table value) which concludes that there is significant difference between mean post test knowledge score and mean pre test knowledge score. Thus, H1 is accepted. There is significant association of Occupation and economic status at 0.05 level of significance. Educational Status, Previously used contraception, future pregnancy, Age in years and Religion and type of family has no significant association. Conclusion: The study concluded that there is need to motivate people for family planning through use of AV Aids so that antenatal women are aware of the latest modalities including PPIUCD so they could use PPIUCD as method of family planning.

Prospective study. Tertiary health care centre. We studied 180 patients with TCA who were managed in the Department of Otolaryngology between December 2004 and June 2007. There were 76 male patients (42%) and 104 females (58%), with ages ranging from 5 to 58 years. All patients presented with single or multiple neck swellings. Constitutional symptoms such as fever were present in 52 patients (29%), whereas cough, weight loss, and reduced appetite were seen in 40 patients (22%). Forty-seven patients (26%) had bilateral lymph node involvement. Only 23 patients (13%) had chest radiographs suggestive of pulmonary tuberculosis. Fine-needle aspirates from 168 patients (93%) showed granulomas with caseating necrosis. However, acid-fast bacilli were seen in only 45 (25%) aspirate smears.
