In many cases the stroke is fatal, in other patients various types and degrees of disability are ... more In many cases the stroke is fatal, in other patients various types and degrees of disability are available, most often affecting the cognitive, motor and perceptive functions. Some vascular and circulation diversions concerning the function of the central nervous system may result in parenchymal lesions that could be hemorrhagic, ischemic or mixed. Modifiable risk factors are hypertonia, metabolic syndrome, changes in blood coagulation status, etc. Non-modifiable are age, gender, race and genetic predisposition. Consequences of the stroke could affect different motor and/or cognitive functions. Stroke rehabilitation includes any procedure that aims to facilitate the optimal functioning of individuals experiencing or likely to have post-stroke difficulties in interacting with their environment. Neurorehabilitation after stroke is based on neurorecovery, neuroplasticity and neuroregeneration. The main goals are directed to prevention and reduction the severity of stroke, death risk and long-term consequences. Development of interdisciplinary social programs is necessary. Elimination of the risk factors would be the best prophylactic.
Our aim is to verify whether serum IgG antiganglioside antibodies (anti-GM1, anti-GD1a and anti-G... more Our aim is to verify whether serum IgG antiganglioside antibodies (anti-GM1, anti-GD1a and anti-GM3) can be used as markers for patients with ischemic stroke. We showed the results of the antibody titer obtained by ELISA technique. There were 21 sera from patients under 75 years of age with ischemic strokes received between 2017-2020 from University First MHAT-Sofia, "St. John Krastitel". Three patients had a very weak indication for demyelination. Five had indication that correlates with possible metabolic abnormalities. The conclusion that could be drawn was that the antiganglioside antibodies titer was not decisive and could not be used as biomarker in stroke patients, and did not directly correlate with stroke severity, size of the ischemic area (reported by computed tomography data), and likely recovery prognosis. In ischemic strokes none of the patients had an indication of neuronal damage, as in the case with severe attacks of multiple sclerosis or Alzheimer's disease.

Some of the neurological disorders (stroke, epileptic status, multiple sclerosis etc.) account fo... more Some of the neurological disorders (stroke, epileptic status, multiple sclerosis etc.) account for the largest number of hospital admissions in Bulgaria. They need specific nursing cares and activities. This article aims to help students and nurses develop their skills in caring for patients’ accommodation as well as specific in cares and activities for neurological patients. After theoretical introduction about needs and classification, there are presented algorithms for: patients’ accommodation in the neurological department and intensive care sector, acute cerebrovascular accident, multiple sclerosis and epileptic status. The last part focuses attention on the nursing care for the neurorehabilitation of stroke patients.

As a solesymptom, ictal pain is extremely rare and it is uncommon for temporal lobe epilepsy. In a casereport we present a patient with temporal lobe seizures with the basic symptoms of severe left legpain and muscles cramps. A 29-year-old man was admitted to the hospital because of probableepilepsy. After sport trauma (left leg injury) 10 years ago, he had seizures of severe pain 1–4 timesdaily, muscle cramps spread throughout his left leg, often “saw stars” after physical overfatigue andwhen he was feeling cold. Seizures lasted one to two minutes. He had never lost consciousness.Seven years ago he was misdiagnosed as having orthopaedic disorder. In the last two years thesecomplaints were rarely preceded by lower lip trembling. He had no family history of epilepsy. Thetreatment with carbamazepine (CBZ) monotherapy, valproate (VPA) monotherapy and combinationCBZ+VPA did not show any seizure reduction. Routine interictal EEG recordings showed normalbackground activity. Video-EEG revealed ictal onset consisted of rhythmic 7.5 Hz activity of thetawaves over the left midtemporal region, which spread to the right midtemporal region, followed bydiffuse spike-waves with 7 Hz, but predominated on the left midtemporal. It was the clinical start ofthe seizure. During the seizure the patient had a tortured face and screamed, because of the pain.There was no consciousness impairment and postictal confusion. The seizure lasted one minute.Fluid-attenuated inversion recovery (FLAIR) magnetic resonance and interictal SPECT demonstrateleft hippocampus changes. We started treatment with CBZ 600 mg/daily and levetiracetam (LEV)2500 mg/daily, seizures reduced to only once a month. This case shows that seizure semiology oftemporal lobe epilepsies could be quite different and often misdiagnosed as other disorders.

Neurological diseases (stroke, epilepsy, multiple sclerosis, Parkinson’s disease, dementia, etc) ... more Neurological diseases (stroke, epilepsy, multiple sclerosis, Parkinson’s disease, dementia, etc) are the third leading cause of death for the Bulgarian population (after heart and malignant diseases). Having in mind that the patients with neurological diseases account for 20% of the total number of hospitalized, a little attention is paid to the needs and care of these patients. The article aims at developing nursing students and qualified nurses’ knowledge and skills in caring for patients with neurological diseases. The first section addresses assessment and prioritization of care needs, while the rest four sections focus on specific patient needs and cares related to respiration, skin maintenance, nutrition, urination, defecation and proper bed positioning.

The functions of tri-peptide Glutathione (GSH) as tumor suppressor, neuroprotector and endocrine regulatos, but also its role in the regulation of cellular growth, proliferation, differentiation and death, as well as of inflammatory and pro-inflammatory processes has been proved. In the current study, total extracts (controls) from rat brain and rat pancreas were prepared. Separate amounts were passed through GSH-agarose columns to select molecules, possessing affinity to GSH. The average titers of gangliosides and of anti-ganglioside antibodies in the so prepared biological samples were determined by enzyme-linked immunosorbent assay (ELISA). Initial myeloid and lymphoid differentiation of hematopoietic and non-hematopoietic stem/progenit... more Initial myeloid and lymphoid differentiation of hematopoietic and non-hematopoietic stem/progenitor cells by appropriate ex vivo-and in vitro-incubation was evaluated. So, hematopoietic stem/progenitor cells from mouse spleen were ex vivo-incubated in the presence of different interleukins, as well as of various combinations of them. Non-hematopoietic mouse embryonic stem cells (mESCs) were in vitroincubated in the presence of GM-CSF and malignant antigens of human cervical carcinoma cells HeLa. Analogically, mouse embryonic fibroblasts from line 3T3 were pre-incubated in the presence of malignant antigens from mouse myeloma cells. Possibilities for derivation of myeloid and lymphoid cells by hematopoietic and non-hematopoietic cellular progenitors were proposed. The literature ability for production of immune molecules, including membrane glycoprotein receptors and antibodies/immunoglobulins by initial myeloid and lymphoid cells, derived from hematopoietic, as well as by non-hematopoietic cellular progenitors was accepted when appropriate factors are available. Because the produced antibodies are out of the germinative centers of the specialized lymphoid tissues and organs, control of their function is very important, for escape of chronic inflammatory processes. Realistically, this control proved the role of small ions and molecules, by direct and/or indirect influence on different intra-and extra-cellular inter-molecular interactions in various cascade regulatory pathways.

We examined the role of uptake mechanisms and ~2-adrenoceptors on prejunctional muscarinic recept... more We examined the role of uptake mechanisms and ~2-adrenoceptors on prejunctional muscarinic receptor modulation of stimulationevoked 3H-overflow from rabbit isolated aorta preloaded with 3Hnoradrenaline. Acetylcholine (10-S-3xl0-6M) added cumulatively reduced the 3H-overflow by up to 80%. The inhibitory effect of acetylcholine (10-S-3xl0-~M) was inversely related to frequency of stimulation (1-10 Hz). The maximal inhibition (%) was 79 (1 Hz), 54 (3 Hz) and 14 (10 Hz). The inhibitory effect of acetylcholine (10-6M) and earbachol (10-SM) reached a maximum 15 rain after addition and remained almost constant for the next 90 rain. The inhibitory effect of acety!choline (10-8-3x10-6M) was endotheliumindependent. Cocaine (3xl0-SM) did not alter the inhibitory effect of acetylcholine (10-8-3x10-6M). ' Cocaine (3x10-SM) plus corticosterone (4x10-SM) and corticosterone (4xlO-SM) alone attenuated the inhibition by acetylcholine. Desipramine (10-8-10-6M) and atropine (10-9-10-7M) competitively antagonized the inhibition. Rauwolscine (10-6M) enhanced the inhibitory effect of acetylcholine (10-8-3x10-6M). This was also the case when the stimulation current was reduced from 200 to 100 mA. We conclude that there is an interaction between prejunctional a2-adrenoceptors and muscarinic receptors and that cocaine, but not desipramine, is suited as a neuronal uptake inhibitor when prejunctional muscarinic receptors are studied. Introduction: High-grade gliomas are the most common primary brain tumors. Dismal prognosis for t... more Introduction: High-grade gliomas are the most common primary brain tumors. Dismal prognosis for the patients with a marked deterioration of the quality of life and a mean length of survival of 9-18 months are typical characteristics for this pathology. The aim of this study is to assess the most appropriate questionnaire for measuring the quality of life (QOL) in this group of patients. Material and methods: For the period 01. 2003-05. 2005 quality of life was assessed in 100 patients using SF-36 (n = 95), WHOQOL-100 (n = 35) and QLQ-C30 of EORTC (n = 40) questionnaires. After that using a visual analog scale patients were asked five additional questions in regard to the easiness of completion and clarity of the questionnaires. Results: Comparison of the answers of the additional questions revealed SF-36 and QLQ-C30 to be the most suitable scales for QOL measurement in patients with high grade gliomas. There were no difficulties met by the patients during the initial filling of the questionnaires as well as during the follow up of the QOL with the progression of the disease. WHOQOL-100 proved to be a little complicated for a group of patients, probably due to the large group of questions included and the additional time and concentration needed for its completion. Conclusion: The SF-36 and QLQ-C30 questionnaires are suitable for patients with high grade gliomas. WHOQOL-100 proves to be a little complicated for patients with high grade gliomas probably due to the large number of questions included. It is suitable for application in a selected group of patients.

It is frequently engaged by different pathologies of tumoral, traumatical or inflammatory origin, leading to spinal instability. The advanced knowledge about the anatomical details, the physiology, the mechanism of injury, the nature of the processes and their classification are crucial for the adequate therapeutic approach when dealing with pathologies in this region. In the actual study we are reviewing the anatomical landmarks, the classification of the pathological processes and the diagnostic options and criterion when dealing with pathologies involving the cranio-cervical region. We are presenting our experience with the posterior screw fixation for the treatment of the craniocervical junction instabilities.

The functions of protein Secretagogin (SCGN) and tri-peptide Glutathione (GSH) as tumor suppressors, neuroprotectors and endocrine regulators are known. The participation of these molecules in the regulation of cellular growth, proliferation, differentiation and death, but also of inflammatory and pro-inflammatory processes has also been proved. The role of SCGN and GSH in brain and pancreas of rat origin was investigated in the current study. Total lysates from rat brain and pancreas were prepared (controls). Equal volumes were passed through GSH-agarose columns to select molecules with affinity to GSH. ABSTRACT Neuropathic Pain (NP) as reported by IASP (International Association Study of Pain) is a... more ABSTRACT Neuropathic Pain (NP) as reported by IASP (International Association Study of Pain) is a “Pain arising as a direct consequence of a lesion or disease affecting the somatosensory system”. In clinical practice NP is divided in peripheral and central pain and it is characterised by positive and negative symptoms. It is not so easy to plan out an effective therapy for this kind of pain. In this article we try to suggest a method to adapt drugs according to pain mechanisms and somatosensory pathways damages. In this “mechanisms based therapy”, drugs and invasive procedures are used according to a complete examination (bedside and instrumental) of the patient’s signs and symptoms. We describe characteristics of NP symptoms in order to individualise our therapeutic target and then we list the possible therapeutic drugs for these kinds of symptoms.

Conclusion: The SF-36 and QLQ-C30 questionnaires are suitable for patients with high grade gliomas. WHOQOL-100 proves to be a little complicated for patients with high grade gliomas probably due to the large number of questions included. It is suitable for application in a selected group of patients.

It is frequently engaged by different pathologies of tumoral, traumatical or inflammatory origin, leading to spinal instability. The advanced knowledge about the anatomical details, the physiology, the mechanism of injury, the nature of the processes and their classification are crucial for the adequate therapeutic approach when dealing with pathologies in this region. In the actual study we are reviewing the anatomical landmarks, the classification of the pathological processes and the diagnostic options and criterion when dealing with pathologies involving the cranio-cervical region. We are presenting our experience with the posterior screw fixation for the treatment of the craniocervical junction instabilities.

The functions of protein Secretagogin (SCGN) and tri-peptide Glutathione (GSH) as tumor suppressors, neuroprotectors and endocrine regulators are known. The participation of these molecules in the regulation of cellular growth, proliferation, differentiation and death, but also of inflammatory and pro-inflammatory processes has also been proved. The role of SCGN and GSH in brain and pancreas of rat origin was investigated in the current study. Total lysates from rat brain and pancreas were prepared (controls). Equal volumes were passed through GSH-agarose columns to select molecules with affinity to GSH. ABSTRACT Neuropathic Pain (NP) as reported by IASP (International Association Study of Pain) is a... more ABSTRACT Neuropathic Pain (NP) as reported by IASP (International Association Study of Pain) is a “Pain arising as a direct consequence of a lesion or disease affecting the somatosensory system”. In clinical practice NP is divided in peripheral and central pain and it is characterised by positive and negative symptoms. It is not so easy to plan out an effective therapy for this kind of pain. In this article we try to suggest a method to adapt drugs according to pain mechanisms and somatosensory pathways damages. In this “mechanisms based therapy”, drugs and invasive procedures are used according to a complete examination (bedside and instrumental) of the patient’s signs and symptoms. We describe characteristics of NP symptoms in order to individualise our therapeutic target and then we list the possible therapeutic drugs for these kinds of symptoms.
