Its exact diagnosis is challenging for surgeons and difficult for pathologists. There are only a few cases reported in the international literature. Case: We report a case of primary pure ovarian angiosarcoma with coexisting chylothorax which is, to the best of our knowledge, the first reported case. An extensive review of the literature analyzing all clinical and pathological parameters related to this condition is presented. Result: In spite of all therapeutic efforts, surgical and medical, prognosis of ovarian angiosarcoma remains very poor in most cases. Conclusion: Primary ovarian angiosarcoma is a rare and aggressive malignancy. The report of such cases is interesting in order to exchange knowledge and experience, and possibly to further improve our diagnostic and therapeutic capabilities.

Two of the carcinomas occurred in postmenopausal women 54 and 71 years old and the third in a menstruating woman 42 years old. The two women with well-differentiated squamous cell carcinomas, who had extra ovarian extension of the tumor, died four and eight months postoperatively. The third woman with a poorly-differentiated squamous cell carcinoma, which was confined to the ovary, was alive two years after the operation. The special importance in the prognosis and spreading of tumors is emphasized.

Colostrum from 51 women living in the city of Thessaloniki (exposed to increased air lead concentration, 0.54 micrograms/m3) and from 40 women living in rural areas (exposed to significantly lower air lead concentrations) was analyzed by atomic absorption spectrometry. Urban women showed slightly higher lead concentrations (mean +/- SD: 0.090 +/- 0.029 micrograms/ml) than rural women (mean +/- SD: 0.084 +/- 0.024 micrograms/ml). This difference was not statistically significant. These results suggest that the lead content of human milk is not influenced by the concentrations of this environmental pollutant in the air.

Research paper thumbnail of Histopathological findings of the endometrium in patients with dysfunctional uterine bleeding

PubMed, 1996

The histopathological examination of the endometrium in cases of abnormal functional uterine blee... more The histopathological examination of the endometrium in cases of abnormal functional uterine bleeding frequently reveals the type of functional disturbance and assists correct treatment. On this basis, 7,000 endometrial curettages from patients with abnormal uterine bleeding were studied. The 1,282 cases with no endometrium in phase, organic changes, or systemic disorders were defined as dysfunctional uterine bleeding; the endometrial lesions were then classified and the histological findings correlated with the patients' ages. The endometrial curettages revealed an anovulatory cycle in 984 (77%) of the patients, and an ovulatory cycle in 298 (23%). Of the cases with an anovulatory cycle, 446 patients (47.5%) showed endometrial hyperplasia, 412 (41.86%) showed abnormal endometrial proliferation due to prolonged persistence of a follicle, while 106 (10.77%) showed deficient endometrial proliferation. Three hundred and thirty four cases (71.67%) of endometrial hyperplasia were (simple) cystic hyperplasias, 124 (26.60%) were (complex) adenomatous hyperplasia, and 8 (1.71%) were atypical hyperplasia. Of the cases with an ovulatory cycle, 252 (84.56%) showed deficient endometrial secretion due to a prolonged proliferative phase, while the other 46 (15.43%) showed deficient secretion due to luteal phase defect. Seven hundred and thirty six patients with an anovulatory cycle and 212 with an ovulatory cycle-i.e. 948 (74%) of the 1282 patients studied-were at the climacteric. The conclusions may be summarised as follows. i) Dysfunctional abnormal uterine bleeding was found more often at the climacteric and chiefly in the form of an anovulatory endometrium; ii) 88.14% of cases (1130 patients) presented histological signs of oestrogen influence in the form of either an anovulatory endometrium or an ovulatory endometrium; iii) Cystic (simple) hyperplasia was the most common form of endometrial hyperplasia.

The aim of this study was to investigate the relationship between specific obstetric adverse outcomes and factor V Leiden and prothrombin G20210A mutations. Forty-five women with adverse pregnancy outcome defined as severe pre-eclampsia, abruptio placentae, intrauterine growth restriction and stillbirth, were tested for factor V Leiden and prothrombin G20210A mutations. The control group comprised 100 women with at least one normal pregnancy and no history of thrombosis. Overall, 13 women with one or more of the above-mentioned pregnancy complications (28%) had either thrombophilic mutation, as compared with six in the control group (6%) (p < 0.001, odds ratio (OR) 6.1; 95% confidence interval (CI) 1.9-20). The factor V Leiden mutation was detected in ten of the women with complicated pregnancies (22%) and in four of the controls (4%) (p < 0.001, OR 6.6; 95% CI 1.7-27.2). The prothrombin G20210A mutation was detected in three women in the group with complications (6%) and in two of the controls (2%) (p = 0.17, OR 3.4; 95% CI 0.4-30.5). Compared to controls, the prevalence of the factor V Leiden mutation was significantly higher in the subgroups of severe pre-eclampsia, abruptio placentae and fetal growth restriction. The prevalence of the prothrombin G20210A mutation does not appear to be significantly different from that in the controls in any of the groups studied. Our data suggest that inherited thrombophilia, and specifically the factor V Leiden mutation, may be associated with adverse pregnancy outcome. The role of the prothrombin G20210A mutation remains to be elucidated.

Plasma ceramides are biomarkers of cardiovascular disease, type 2 diabetes mellitus, Alzheimer’s disease, various autoimmune conditions and cancer. During pregnancy, ceramides play an important role as stress mediators, especially during implantation, delivery and lactation. Based on the current literature, plasma ceramides could be potential biomarkers of obstetrical adverse outcomes, although their role in metabolic pathways under such conditions remains unclear. This review aims to present current studies that examine the role of ceramides during pregnancy and obstetrical adverse outcomes, such as pre-eclampsia, gestational diabetes mellitus and other complications.

Research paper thumbnail of PRL, TSH and Their Response to the TRH Test in Patients with Endometriosis before, during, and after Treatment with Danazol

For evaluation of prolactin (PRL) and TSH secretion, a TRH test was carried out in the follicular phase before treatment, during the 6th month of treatment and after the reappearance of the second menses. The same test was also performed, only once, in the follicular phase of 10 normal women. The absence of a significant difference in the basal PRL levels as well as its response to the TRH test between controls and patients with endometriosis before and after danazol treatment supports the view that hyperprolactinemia should not be considered a cause of infertility in endometriotic women. Furthermore, there were no statistically significant changes in the basal levels of serum TSH during danazol treatment. On the contrary, in the TRH test, the response of serum TSH was significantly higher during danazol treatment than before or after treatment.

Clinical and histological features were typical of impetigo herpetiformis (IH). Systemic steroids resulted in an unstable condition, with no resolution of lesions. Resistance to the above therapeutic scheme served as a stimulus to discuss the use of cyclosporine as a therapeutic option in this condition. Reviewing the limited literature, cyclosporine seems to serve not as a monotherapy in the management of IH but as an additional medication, in order to achieve a stable course of the disease and avoid high doses of systemic steroids.

This review aims to present current studies that examine the role of ceramides during pregnancy and obstetrical adverse outcomes, such as pre-eclampsia, gestational diabetes mellitus and other complications.

Gynecologic and Obstetric Investigation, 1996

Cases Journal, 2008

Inversion of the uterus through the uterine lower segment incision during a caesarean section is ... more Inversion of the uterus through the uterine lower segment incision during a caesarean section is an extremely rare obstetric incident. It consists, though, an emergency complication that is potentially life-threatening, especially in cases of prolonged inversion, because haemodynamic instability and shock may occur. Prompt diagnosis and immediate uterine reversion are the key actions in the management of this serious complication.

Research paper thumbnail of Cyclosporine in the management of impetigo herpetiformis: a case report and review of the literature
