ABSTRACT Over the last 20 years psychotherapy and family therapy have been inundated with a pleth... more ABSTRACT Over the last 20 years psychotherapy and family therapy have been inundated with a plethora of empirically validated treatments for particular disorders. That trend will increase. Psychotherapists will increasingly be exhorted and ultimately required to integrate empirical data and multicultural competence into their practice. Additionally, individual psychotherapy's default dominance of psychotherapeutic discourse needs to integrate and come to theoretical and clinical terms with the implications of the growing body of research demonstrating the validity and value of a multisystemic perspective. This article (and its companion article) presents a comprehensive, integrative, multisystemic, and empirically informed psychotherapeutic perspective to help therapists and psychotherapy trainers successfully address these challenges—Integrative Problem Centered Metaframeworks (IPCM) Therapy. This first article presents and illustrates IPCM's theoretical foundation, core concepts, and “case formulating” components. It delineates a Blueprint for the practice and teaching of 21st century psychotherapists who can meld science and art into best practice. Durante los últimos 20 años la psicoterapia y la terapia familiar se han visto inundadas de una plétora de tratamientos validados empíricamente para trastornos particulares. Esta tendencia irá en aumento. Cada vez más se les recomendará, y a la larga se les exigirá, a los terapeutas que integren los datos empíricos y la competencia multicultural en su práctica. Por fin el dominio predeterminado del discurso psicoterapéutico en la psicoterapia individual necesita integrarse y reconciliarse a nivel teórico y clínico con las implicancias del conjunto cada vez mayor de investigaciones que demuestran la validez y el valor de una perspectiva multisistémica. En este artículo (y en el artículo que lo acompaña) se presenta una perspectiva psicoterapéutica exhaustiva, integradora, multisistémica y con fundamentos empíricos para ayudar a los terapeutas y a los capacitadores en psicoterapia a abordar satisfactoriamente estas dificultades (terapia de metaestructuras integradoras centradas en el problema, MICP). En este primer artículo se presentan y se ponen de manifiesto los fundamentos teóricos de las MICP, los conceptos básicos y los componentes de la “formulación de casos.” También se esboza un proyecto para la práctica y la capacitación de los psicoterapeutas del siglo veintiuno que pueden fusionar la ciencia y el arte en una práctica adecuada. This study describes the development of a standardized strategy for evaluating pediatric resident... more This study describes the development of a standardized strategy for evaluating pediatric residents' mastery of observational, conceptual, and executive skills central to the practice of behavioral pediatrics (BP). The study focused on assessment of family context and dynamics as they relate to the child's symptom, and the pediatrician's interaction with the family as it relates to case treatment/management. A videotape was developed from actual case material to simulate a pediatrician conducting a family interview in which one child has been identified as eneuretic. A multiple-choice test that addressed the learning domains of observational, conceptual, and executive skills was also developed. Equivalent experimental and control groups of residents were given the tape/test either before and after a one-month rotation of BP training or after a no-training condition. The results indicated modestly significant improvement by the experimental group in its total scores and in scores for the conceptual domain, as measured by the test. No significant differences were found for the observational or executive domains. The implications are discussed in the context of the usefulness of the tape/test for training and evaluation and the need for further evaluative research on the optimal matching of learning tasks to residents' receptivity by the year of residency.
