In 2019, an outbreak of hand, foot and mouth disease (HFMD)occurred in Maldives and many patients... more In 2019, an outbreak of hand, foot and mouth disease (HFMD)occurred in Maldives and many patients presented with onychomadesis following HFMD. Here we present a study of 33 cases out of which a total of 06 cases developed onychomadesis. Patients varied in clinical presentations ranging from mild to severe form of HFMD and during follow up 06 cases presented with peroidic shedding of nails involving both fingernails and toenails. All the patients were observed 4 to 6 weeks after diagnosis of HMFD was made. It was observed that onychomadesis developed was not related to the severity of disease. Patients required only symptomatic treatmentfor HFMD and observation with assurance for onychomadesis as course was self limited. Nailchanges were temporary in nature and spontaneous regrowth occurred after shedding of the nails. Parental counselling played a cruicial role in management.

We report a case of recurrent mucinous carcinoma of the breast in a 60 year old female who presen... more We report a case of recurrent mucinous carcinoma of the breast in a 60 year old female who presented with two separate lumps on the left chest wall for 1 year. She gave the history of left sided mastectomy 15 years ago and hysterectomy 10 years ago. The patient did not seek any medical attention for the lumps since she noticed them a year ago. On examination, there were two lumps larger one measuring 7.5 x 5 cm and other smaller one measuring 3 x 2.5 cm. Core needle biopsy showed ductal carcinoma. CT scan was done which showed post-operative status of left side breast cancer with left chest wall mass and metastatic left axillary mass. Ultrasonography of whole abdomen and bony scan was done and no metastasis was seen. She underwent en-bloc excision of the tumor with involved muscle. Histopathology was done from two different centers both revealed Mucinous carcinoma of the breast. She was advised to take adjuvant chemotherapy.

The incidence of placenta accreta spectrum (PAS) disorders has increased over the last decades du... more The incidence of placenta accreta spectrum (PAS) disorders has increased over the last decades due to increase in cesarean deliveries, resulting in increase in Cesarean hysterectomies,maternal mortality and morbidity but since last few years there has been a gradual shift towards the idea of conservative management. Conservative management of PAS is known to reduce major obstetric hemorrhage and salvage hysterectomy.We present a case of placenta accreta diagnosed by ultrasound where management of post-partum hemorrhage was accomplished by conservative surgery. The concise steps taken in management of placenta accreta before and during cesarean section were: Availability of 4 donors with cross match; Stark cesarean section; atraumatic clamps around uterine arteries; ureterotonic drugs; external (B-Lynch suture); and application of diathermy where required. This experience indicates that few selected cases of PAS could be managed conservatively who are at risk of intra-partum hemorrhage and post-partum hemorrhage.

A 3-year-old boy presented to our atoll hospital in H.A Alif Dhidhoo, with severe pallor, jaundic... more A 3-year-old boy presented to our atoll hospital in H.A Alif Dhidhoo, with severe pallor, jaundice, easy fatigability and recurrent episodes of passage of dark-colored urine for past 3 days. He was born mature at 39 weeks of gestation with no past significant medical history. Recent history revealed the consumption of 2 cans of fava beans and application of some medicinal herbs. On admission, physical examination revealed fever of 101 degree Fahrenheit, severe pallor, jaundice, cervical lymphadenopathy and mild hepatomegaly. Laboratory investigation results showed a hemoglobin level of 5.4 g/dl with a hemolytic blood picture and serum Bilirubin of 6mg/dl. The patient's G6PD level was measured which showed marked deficiency. Other causes of hemolytic anemia were excluded. Patient required urge nt packed RBC transfusion and antibiotics for infection. He responded well to the treatment and was discharged in a stable condition. Parents were appropriately advised on the condition and the importance of avoiding certain foods and medication. Folic acid was prescribed for maintaining normal hemoglobin concentration. This is a first case report in North Maldives of G6PD presenting with severe hem olytic anemia requiring blood transfusion.

Objective: The goal of our study was to determine the magnitude, prevalence and severity of iron ... more Objective: The goal of our study was to determine the magnitude, prevalence and severity of iron deficiency anemia among children in Doda (J&K) India. Methods: It was a prospective, cross-sectional, multistage hospital-based and outpatient department (OPD) study of apparently healthy children who visited our Hospital/OPD clinic for follow-up from different regions of district Doda from February 1, 2021, to February 28, 2022. Five hundred children aged 6 months-12 years were included in our study. The sociodemographic data of subjects were collected using a structured questionnaire. 5 mL of venous blood sample was collected for complete blood count. Hemoglobin level was estimated, along with indices. Anemia was classified into mild (≤12 g/dl->10 g/dl), moderate (≤10 g/dl->7 g/dl), and severe (≤7 g/dl), respectively. Microcytic hypochromic picture using indices was considered for labeling iron deficiency. The dietary pattern of children was assessed using questionnaire and anthropometric measurements were done. Data were analyzed using SPSS software version 20. Results: Overall, the prevalence of anemia was 77.6%. Out of the total vegetarians, 60.1% were anemic. Poor family status particularly families with low income <5000 Rs. per month and less education were more prone to develop anemia. Another important predictor of IDA was no anti-parasitic medications in 60.4%. Conclusion: Iron deficiency anemia in children seems less severe public health problem; however, its consequences are disastrous and a huge burden on health, wealth and workforce. Parents need special counseling sessions for understanding its magnitude. Recommendations to put preventive strategies in place and mass awareness of people about the consequences of iron deficiency anemia should be made.

Prognosis is guarded with current management only symptomatic and supportive. There is no effective treatment, however early gene therapy has improved the quality of life of patients. Laboratory investigation results showed a hemoglobin level of 5.4 g/dl with a hemolytic blood picture and serum Bilirubin of 6mg/dl. The patient's G6PD level was measured which showed marked deficiency. Other causes of hemolytic anemia were excluded. Patient required urgent packed RBC transfusion and antibiotics for infection. Here we present a study of 33 cases out of which a total of 06 cases developed onychomadesis. Patients varied in clinical presentations ranging from mild to severe form of HFMD and during follow up 06 cases presented with peroidic shedding of nails involving both fingernails and toenails. All the patients were observed 4 to 6 weeks after diagnosis of HMFD was made. It was observed that onychomadesis developed was not related to the severity of disease. Patients required only symptomatic treatmentfor HFMD and observation with assurance for onychomadesis as course was self limited. Nailchanges were temporary in nature and spontaneous regrowth occurred after shedding of the nails. Parental counselling played a cruicial role in management.

The incidence of placenta accreta spectrum (PAS) disorders has increased over the last decades du... more The incidence of placenta accreta spectrum (PAS) disorders has increased over the last decades due to increase in cesarean deliveries, resulting in increase in Cesarean hysterectomies,maternal mortality and morbidity but since last few years there has been a gradual shift towards the idea of conservative management. Conservative management of PAS is known to reduce major obstetric hemorrhage and salvage hysterectomy.We present a case of placenta accreta diagnosed by ultrasound where management of post-partum hemorrhage was accomplished by conservative surgery. The concise steps taken in management of placenta accreta before and during cesarean section were: Availability of 4 donors with cross match; Stark cesarean section; atraumatic clamps around uterine arteries; ureterotonic drugs; external (B-Lynch suture); and application of diathermy where required. This experience indicates that few selected cases of PAS could be managed conservatively who are at risk of intra-partum hemorrhage and post-partum hemorrhage.
