Firearm injuries are among one of the essential causes for renal trauma. Overall, trauma is the leading cause of death and accounts for approximately 50% of mortality in children over 1 year of age. Therefore, the current study aims to determine the frequency of morbidity, mortality and renal trauma in blunt and firearm accidents in pediatric population. Methodology: A total of 180 children, aged up to 15 years with blunt and penetrating trauma, including firearm injuries, were enrolled in our study. All patients with in the inclusion criteria of the study, who were presented in the emergency department were evaluated and resuscitated by a pediatric surgeon. The development of Transurethral Resection of Prostate (TURP) skills has shifted to simulator-based learning from being directly on patients. We evaluated the outcome of simulation-based teaching of TURP procedural skills on virtual reality simulator and objectively assessed the operative skills on the Global Rating Scale (GRS). Methodology: In this prospective comparative study, high fidelity (VirtaMed | UroSim™) simulator was used. A total of 17 participants were enrolled in this study, 4 were urology faculty members, and 13 were post-graduate urology trainees. All participants performed five attempts of real-time TURP complete procedure and were allowed to resect the maximum possible prostate tissue (goal ≥ 85%). Objective: The purpose of this study was to assess the knowledge of patients regarding anesthesia, their attitudes towards basic types of anesthesia techniques and effect of previous experience of anesthesia on the awareness. Study design: Descriptive observational study. Place and duration of study: This descriptive, prospective study was conducted in the isra university hospital Hyderabad, Sindh, Pakistan from 2nd August 2013 upto 8th March 2014. Patients and Methods: Two hundred thirty one patients were included in the study after taking verbal informed consent. All patients who were scheduled for elective surgery were interviewed while patients who had refused from participation, have language barrier, psychiatric disease, had lack of sufficient mental capacity due to poor health status and patients undergoing emergency surgeries were excluded from the study. Patients were interviewed in their local language upon arrival in the preoperative area of operation theatre. Self made questionnaire was filled. All the data was entered on SPSS version 16. Frequency and percentages were calculated to show the results. Mean with SD of patients age was computed. Chi-square test was applied to assess the affect of previous experience of surgery on patient's knowledge regarding anesthesia. P value less than 0.05 was taken as significant. Results: Total 231 patients were included in the study. Mean age of the patients was 36 ± SD 1.01. Fifty one (22.1%) patients were illiterate; while 54 (23.4%) patients were graduate. 103(44.6%) patients had previous experience of surgery/ anesthesia while 128(55.4%) patients had no experience. Out of 231 patients, 111 (48.1%) patients were aware of different type of anesthesia techniques while 120(51.9%) patients were not aware. Out of 103 patients, who had previous exposure of anesthesia, 64 patients answered correctly about different types of anesthesia techniques while 81 patients who had no previous exposure of anesthesia, answered incorrectly (P value = 0.000). Fifty three patients who had previous exposure of anesthesia answered correctly that anesthetist is a qualified doctor, while 62 patients answered incorrectly who had no previous exposure (P value=0.546). Out of 231 patients, 91 (39.39%) patients were in favor of having general anesthesia while 52(22.51%) patients were in favor of regional anesthesia, while 88(38.0%) had left the choice on surgeon. Conclusions: Results of the study show poor knowledge of patients regarding anesthesia and anesthesiologists. Majority of the patients were in favor of having general anesthesia rather regional anesthesia. There is need to correct the misconceptions of patients and to educate the public.

Methodology: A total of 162 patients presenting with acute urinary retention were divided into two groups. Objective: To compare the outcome of internal optical urethrotomy versus laser urethrotomy for ma... more Objective: To compare the outcome of internal optical urethrotomy versus laser urethrotomy for management of male patients with urethral stricture. Methodology: This Randomized Controlled Trial was carried out at Department of Urology, Liaquat University of Medical and Health Sciences, Jamshoro from March 8 to September 7, 2017. Total 60 patients having urethral strictures were included and divided into Group-A (laser urethrotomy) and Group-B (internal optical urethrotomy). Operative time was noted. After 1 month, peak flow rate was recorded. Comparison for mean operative time and peak flow rate by using t-test was done. P-value ≤0.05 was considered as significant. Results: Mean stricture duration in Group-A was 7.36±1.06 weeks and 6.66±1.21 in group-B. Mean stricture length in Group-A was 10.30±2.98 mm while in Group-B 11.90±3.00 mm. Mean operative time in Group-A was 17.10±6.17 minutes and 29.76±8.89 minutes in group-B while mean postoperative peak flow rate in Group-A was 24.13±3.17 ml/sec and 22.06±2.92 ml/sec in group-B. There was significant mean difference in mean operative time and mean postoperative peak flow rate among groups. Conclusion: Laser Urethrotomy was effective in providing relief by significant shorter operative time and increased flow rate.

OBJECTIVE: To assess the effectiveness of honey dressing in Fournier's Gangrene against conventio... more OBJECTIVE: To assess the effectiveness of honey dressing in Fournier's Gangrene against conventional Edinburgh University solution of lime (EUSOL) dressing. METHODOLOGY: This was a prospective experimental study conducted in Liaquat National Medical College and Hospital Karachi for duration of 5 and half years. Total 44 patients having gangrene and no co morbid conditions were included in this study. Patients who had co-morbid conditions like deranged coagulation profile and CVA, who were not willing to participate or loss to follow up were excluded from the study. After getting baseline blood workup (i.e. CBC, S. Creatinine, S. Electrolytes, Urine D/R, C/S and Diabetic workup) done and taking consent, the patients were immediately shifted to operation theatre, the incision and drainage was performed, and the necrotic tissues were debrided. Pus cultures were sent, and patients were started on routine intravenous antibiotics (3 rd generation Cephalosporin and metronidazole). A Dressing of wound was done randomly by honey in Group A and by Eusol in Group B. Patients were discharged after wound was completely healed. The data were obtained and analyzed by using SPSS version 20. Mann-Whitney U-test and chi-square test was applied to find out the significance. RESUTS: The patients in Group A got slough cleared in4.82±0.96 days, on average requiring 3.22±0.75 number of debridement's, complete wound healing and duration of hospital stay being 9.54±0.74 days. The patients in Group B got slough cleared in 8±0.87 days, requiring 5.32±0.72 number of debridement's, complete wound healing and duration of hospital stay 13.68±1.09 days.There was a significant difference in slough clearance, number of debridement's required, time taken for complete wound healing and hospital stay between Group A and Group B (P-value <0.001). CONCLUSION: This study predicted that honey is very valuable in the management of Fournier's gangrene as it ensured early clearance of slough, required lesser number of debridement's, had early wound healing and had relatively shorter duration of stay in hospital than those treated with conventional EUSOL as dressing material

About sixty patients with benign prostatic hyperplasia and correlated lower urinary tract symptoms were included. The participants were distributed into two groups (Group A was prescribed combined therapy while Group B was prescribed monotherapy). The therapy was considered as successful if there was an improvement in the International Prostate symptom scoring of more than 3 points from the baseline to 12 weeks. Statistical analysis was performed using Excel sheet and SPSS v. 26. Results: In group 1 (combined therapy), 22 (73.3%) (22/30) of the patients reported a significant reduction in International Prostate symptom scores after taking Tamsulosin with Sildenafil as compared to Group 2, where only 13 (43.3%) patients who were treated with Tamsulosin only reported improvement in the International Prostatic symptom scoring at 12 weeks of treatment. Conclusion: The current study indicates that Tamsulosin in combination with Sildenafil is more effective in relieving the lower tract urinary symptoms in patients with benign prostate hyperplasia as compared to monotherapy.

Methodology: This cross sectional study was carried in Department of Urology, Liaquat National Hospital, Karachi, from August 10, 2011 to August 20, 2017 using non probability convenient sampling technique. After informed consent, 150 female patients presented with sign and symptoms of vesicovaginal fistula were included in the study. Data were analyzed using SPSS version 20. Results: Out of 150 patients, 67(44.56%) were of less than 20 years while 43(28.66%) belonged to 20-30 age group; 91(60.66%) were illiterate. OBJECTIVE: To assess the effectiveness of honey dressing in Fournier's Gangrene against conventio... more OBJECTIVE: To assess the effectiveness of honey dressing in Fournier's Gangrene against conventional Edinburgh University solution of lime (EUSOL) dressing. METHODOLOGY: This was a prospective experimental study conducted in Liaquat National Medical College and Hospital Karachi for duration of 5 and half years. Total 44 patients having gangrene and no co morbid conditions were included in this study. Patients who had co-morbid conditions like deranged coagulation profile and CVA, who were not willing to participate or loss to follow up were excluded from the study. After getting baseline blood workup (i.e. CBC, S. Creatinine, S. Electrolytes, Urine D/R, C/S and Diabetic workup) done and taking consent, the patients were immediately shifted to operation theatre, the incision and drainage was performed, and the necrotic tissues were debrided. Pus cultures were sent, and patients were started on routine intravenous antibiotics (3 rd generation Cephalosporin and metronidazole). A Dressing of wound was done randomly by honey in Group A and by Eusol in Group B. Patients were discharged after wound was completely healed. The data were obtained and analyzed by using SPSS version 20. Mann-Whitney U-test and chi-square test was applied to find out the significance. RESUTS: The patients in Group A got slough cleared in4.82±0.96 days, on average requiring 3.22±0.75 number of debridement's, complete wound healing and duration of hospital stay being 9.54±0.74 days. The patients in Group B got slough cleared in 8±0.87 days, requiring 5.32±0.72 number of debridement's, complete wound healing and duration of hospital stay 13.68±1.09 days.There was a significant difference in slough clearance, number of debridement's required, time taken for complete wound healing and hospital stay between Group A and Group B (P-value <0.001). CONCLUSION: This study predicted that honey is very valuable in the management of Fournier's gangrene as it ensured early clearance of slough, required lesser number of debridement's, had early wound healing and had relatively shorter duration of stay in hospital than those treated with conventional EUSOL as dressing material

About sixty patients with benign prostatic hyperplasia and correlated lower urinary tract symptoms were included. The participants were distributed into two groups (Group A was prescribed combined therapy while Group B was prescribed monotherapy). The therapy was considered as successful if there was an improvement in the International Prostate symptom scoring of more than 3 points from the baseline to 12 weeks. Statistical analysis was performed using Excel sheet and SPSS v. 26. Results: In group 1 (combined therapy), 22 (73.3%) (22/30) of the patients reported a significant reduction in International Prostate symptom scores after taking Tamsulosin with Sildenafil as compared to Group 2, where only 13 (43.3%) patients who were treated with Tamsulosin only reported improvement in the International Prostatic symptom scoring at 12 weeks of treatment. Conclusion: The current study indicates that Tamsulosin in combination with Sildenafil is more effective in relieving the lower tract urinary symptoms in patients with benign prostate hyperplasia as compared to monotherapy.

Methodology: This cross sectional study was carried in Department of Urology, Liaquat National Hospital, Karachi, from August 10, 2011 to August 20, 2017 using non probability convenient sampling technique. After informed consent, 150 female patients presented with sign and symptoms of vesicovaginal fistula were included in the study. Data were analyzed using SPSS version 20. Results: Out of 150 patients, 67(44.56%) were of less than 20 years while 43(28.66%) belonged to 20-30 age group; 91(60.66%) were illiterate.
