Nasal irrigation clears mucus and allergens, blood clots, crusts and debris from the nose and sin... more Nasal irrigation clears mucus and allergens, blood clots, crusts and debris from the nose and sinuses and improves the healing process in post-operative FESS. The direct mucosal cleansing, the removal of antigens, biofilms or inflammatory mediators resolve inflammation and improved mucociliary function. It is safe and inexpensive, representing a non-pharmacological form of treatment. The aim of nasal irrigation is to carry out distribution of the irrigating solution effectively and reliably to all sinuses. This depends upon the surgical state, delivery device, head position, technique, nasal anatomy, respiratory cycle, and carrier vehicle [1]. Nasal irrigation has been practiced around the world since a long time, for a range of purposes. It is also known as saline sniffs, nasal douche, nasal lavage, sinus flush, nasal irrigation, nasal washout, and nasal toileting. Malignant tumors of external auditory canal (EAC) constitute less than 0.2% of all head and neck ... more Malignant tumors of external auditory canal (EAC) constitute less than 0.2% of all head and neck cancers. The incidence of carcinoma of the EAC is estimated to be between one in six per million populations. Majority of cancers of EAC are squamous cell carcinomas and basal cell carcinomas. Some rare and unusual tumors do occur within the ear canal including malignant melanoma, merkel cell carcinoma, angiosarcoma, lymphoma and adnexal carcinomas like ceruminous adenocaricinoma and adenoid cystic carcinoma. Ceruminous glands tumors constitute about 5% of all external auditory canal tumors. Carcinoma of the external auditory canal is a difficult diagnosis unless the tumors presents as a fungating mass protruding from the external auditory canal. Syringocystadenocarcinoma Papilliferum (SCACP) is an extremely rare cutaneous adnexal neoplasm. Syringocystadenoma papilliferum (SCAP) is thought to be precursor of SCACP. About 50 cases of SCACP have been reported in literature all over the body. The diagnosis is difficult and excisional biopsy becomes mandatory for diagnosis and treatment. We present a case of SCACP in the external auditory canal in a middle-aged female. To the best of our knowledge and belief, this is the first case of SCACP in the external auditory canal in the English literature. This prompted us to report this case.

A 65-year-old man presented with a 3-day history of progressive left-sided ear pain followed by i... more A 65-year-old man presented with a 3-day history of progressive left-sided ear pain followed by ipsilateral facial droopiness. He denied febrile nor hearing loss. There were no apparent past medical and family histories. Physical examination revealed a left peripheral facial paralysis accompanied with incomplete left eye closure (Figure 1A), in addition to multiple exudative vesicles on the left cavum conchae associated with an erythematous swollen helix (Figure 1B). The left tympanic membrane and ear canal were otoscopically normal. Routine blood and biochemical analyses were unremarkable. The characteristic triad of acute Bell's palsy, ipsilateral auricular vesicles and otalgia were diagnostic for Ramsay Hunt syndrome type II (herpes zoster oticus), which was caused by the re-activation of preexisting varicella zoster virus in the geniculate ganglion and subsequently affecting the facial and vestibulocochlear nerves. Knowledges of these clinical findings and an early diagnosis, depending on the time at which acyclovir-corticosteroid therapy is administered, are crucial as prognostic factors of the cranial nerves damage. 1,2 After the combination therapy with 1-week intravenous acyclovir 750 mg three times daily plus oral prednisolone 40 mg twice daily, he had complete neurologic recovery at a 6-month follow-up in the outpatient department.

Comparisons of microscopic and endoscopic techniques present in the literature use different routes of access and cannot be compared head-on. Objective: To compare endoscopic and microscopic myringoplasty results when both are performed via the transcanal route. Methods: We present a retrospective study done at a single center to compare endoscopic and microscopic type I tympanoplasties both done via the transcanal route. A total of 70 patients, 30 operated with endoscope and 40 with microscope, were included in the analysis. Patients of either sex, between 18 and 60 years of age, with a dry perforation and air-bone gap (ABG) of ≤30 dB, were included. Results: Perforations were completely closed in 93% of the endoscopic group and 92.5% of the microscopic group. The ABG closure was 12.89 dB in endoscopic and 11.97 dB in the microscopic groups. There was no association of the site or size of perforation with failure of surgery. The time taken for surgery was also equivalent. The endoscope had the advantage of avoiding a canaloplasty and looking into the hidden areas of the middle ear. Conclusion: For transcanal, minimally invasive type I tympanoplasty, both techniques provide equivalent results. We recommend the use of an endoscope as the primary or an accessory tool for better visualization.

Meningoencephalic herniation into the middle ear is a rare condition that can be life threatening... more Meningoencephalic herniation into the middle ear is a rare condition that can be life threatening for the patient due to eventual infective intracrunial complications. It is characterized by a bony and dural defect localized in the tegmen through which meninges and encephalic tissue can herniate. Meningoencephalic hernia of the middle ear can appear as an insidious complication of Chronic mastoiditis (infective) and its surgery, trauma or spontaneous. High resolution computed tomography (CT scan) and Magnetic resonance imaging (MRI) must be done to evaluate the extent and the contents of the disease. Early and effective surgical repair, the only appropriate treatment, is necessary to avoid complication and recurrence. Middle cranial fossa, mastoid, combined approaches and obliteration of mastoid; external auditory canal and middle ear are the surgical procedures for the repair of hernias. Lymphangiomas are rare benign, hamartomatous, congenital malformations of the lymphatic system in... more Lymphangiomas are rare benign, hamartomatous, congenital malformations of the lymphatic system involving the skin and subcutaneous tissues of head, neck and oral cavity. Occasional adult onset cases occur, this condition is thought to be a developmental malformation of lymph vessels which have poor communication with normal lymph system. Most of these malformations are present at birth or appear within two years of life. 75 % of cases occur in head and neck area, submandibular and parotid being the most affected parts. Lymphangioma arising in nasopharynx and in an adult has not been reported in english literature. This prompted us to report the very first case of Lymphangioma Nasopharynx.

Synovial sarcoma is a rare malignant tumor. It derives from a mesenchymal precursor stem cell tha... more Synovial sarcoma is a rare malignant tumor. It derives from a mesenchymal precursor stem cell that is unrelated to mature synovial tissue. Synovial sarcoma classically affects lower limbs between the ages of 15 and 40 years and the proportion of male-to-female patients is 3:2. It is very rare in the head and neck region especially in laryngopharynx. Till date, only six cases of synovial sarcoma involving laryngopharynx have been reported in the English literature. Painless mass, hoarseness, upper respiratory distress, and dysphagia characterize the original complaints in laryngopharyngeal synovial sarcoma. Because head and neck synovial sarcoma in clinical practice is so uncommon, early diagnosis is difficult and the treatment protocol is unclear. Therefore, every case report should include complete information on presentation and management. Also, long-term prognostic indices need to be evaluated. We hereby report a case of large laryngopharyngeal synovial sarcoma confirmed by histopathology and immunohistochemistry with review of literature.

Ameloblastoma or Adamantinoma of the jaw has attracted a great interest among all the tumours of ... more Ameloblastoma or Adamantinoma of the jaw has attracted a great interest among all the tumours of the oral cavity. Ameloblastoma accounts for 1% of all the tumours and cysts of the jaws..lt usually occur in the lower jaw. It's incidence in the upper jaw is approximately 20%. They start in the cancellous bone, producing painless expansion. Early signs and symptoms go unnoticed. They are slow growing and seldom metastasize. Radical excision (enblock resection) is the treatment of choice and ensures 90-100% cure rate. The clinical outcome of a particular adamantinoma depends mainly on the practability of radical excision.
