Objetivo: As tenólises dos tendões flexores na zona 2 são procedimentos difíceis e um verdadeiro ... more Objetivo: As tenólises dos tendões flexores na zona 2 são procedimentos difíceis e um verdadeiro desafio na cirurgia da mão. Com o objetivo de comparar os resultados obtidos entre as tenólises com despertar intraoperatório, realizadas sob anestesia locorregional (grupo 1), daqueles obtidos com as tenólises convencionais, realizadas sob anestesia geral ou bloqueio total do plexo braquial (grupo 2), os autores realizam um estudo prospectivo e controlado. Métodos: Foram avaliados 22 pacientes com 39 dedos portadores de lesão de tendão flexor na zona 2 que evoluíram para aderências. Todos os pacientes foram operados após três meses e antes de um ano da sutura tendinosa primária. Todos apresentavam limitação da movimentação ativa sem melhora com a reabilitação. Os grupos 1 e 2 mostraram-se homogêneos em relação à idade dos pacientes, sexo, comprometimento pré-operatório e ausência de lesões ou patologias associadas. Todos os pacientes foram avaliados de acordo com a movimentação ativa (TAMs) no período pré-operatório e com seis meses de pós-operatório. Resultados: A análise estatística dos dados obtidos nos grupos 1 e 2 demonstra que as tenólises realizadas pelas duas técnicas proporcionam bons resultados. Ao comparar os resultados da movimentação ativa total após seis meses da tenólise, observa-se que os pacientes do grupo 1, tratados pela técnica do despertar intraoperatório, apresentavam melhores resultados. Conclusões: As tenólises dos flexores na zona 2 proporcionam bons resultados em termos de ganho de amplitude articular. A técnica de despertar intraoperatório com anestesia locorregional proporciona resultados mais satisfatórios que a técnica convencional.

Introduction The radiographic and surgical findings, and treatment of radiocarpal fracture disloc... more Introduction The radiographic and surgical findings, and treatment of radiocarpal fracture dislocations, were analyzed retrospectively in 40 patients. Materials and Methods All patients were classified according to Dumontier´s radiological classification and compared with the surgical findings. Based on this analysis, a new classification and treatment are proposed. Results From 1995 to 2018, 40 patients with radiocarpal fracture dislocation underwent surgery. Thirty-six were males and four were females. The mean age was twenty-four years (range: 18-45). Three dislocations were volar dislocations and 37 were displaced dorsally. Initially, 8 (20%) patients were classified as group I, 29 (72.5%) as group II, and 3 (7.5%) remained unclassified. The main variations occurred in group II. Seven fractures were stable after radial styloid fixation and 6 remained unstable. Purpose: The purpose of this study is describe a osteotomy technique of the radius with insertion... more Purpose: The purpose of this study is describe a osteotomy technique of the radius with insertion of a trapezoidal wedge and present our clinical experience with this method. Methods: Eight female patients in the 12to 22-year age range (mean age 15.6 years) with bilateral Madelung’s deformity underwent surgery between 1997 and 2008, with a total of 12 operated wrists. The postoperative period varied between 3 and 83 months, with an average of 40.3 months. Results: An improvement was noticed in all parameters analyzed, with an average increase in the degree of amplitude of the joint in terms of flexion, extension, pronation and supination. Conclusion: All patients had improvement in gain of motion and reported being satisfied with the improvement in the appearance of their deformities.

Relatam as tecnicas das reconstrucoes realizadas e referem os resultados obtidos, imediatamente e tardios. A maioria das amputacoes ocorreu ao nivel da articulacao metacarpofalangiana. O indice de sucesso em termos de sobrevida e resultado funcional satisfatorio foi de 64 por cento. Concluem que, mesmo nestas situacoes de alta complexidade, os reimplantes do polegar sempre devem ser realizados (AU)

Arthroscopy is a surgical technique whose indication for wrist injuries has grown in recent years... more Arthroscopy is a surgical technique whose indication for wrist injuries has grown in recent years. Athletes are subject to traumatic injury to the wrist due to training overload or the intensity of the activity during competition.The need of a quick return to sports practice makes arthroscopy a very useful minimally invasive technique in these situations. The authors present indications of sports-related injuries to the wrist that can be treated by arthroscopy. A literature review is also presented.

Hand reconstruction in epidermolysis bullosa Digital contractures and pseudosyndactyly are freque... more Hand reconstruction in epidermolysis bullosa Digital contractures and pseudosyndactyly are frequently found in patients with epidermolysis bullosa, leading to a severe functional hand impairment. Due to permanent blistering, such deformities, if left untreated, become worse with time. Surgical treatment associated with postoperative splitting may minimize the progression of the finger contractures and pseudosyndactyly formation. The technique of surgery and immediate postoperative splitting differs from those previously reported. The authors present the results of treatment in 20 patients, in a total of 30 operated hands. Mean follow-up was 20 months (range, seven to thirty months). Partial recurrence of the deformities was observed in two patients.
