We compared our new technique with the splint wear, of which the efficacy in CTS has been proven with many studies. Eighty-four patients between 31 and 65 years of age were included in the study. The patients were divided into two equal groups. In the first group, splint and ''Madenci'' hand massage technique were applied, and in the second group only splint was applied. A splint was provided for all patients with tendon and nerve gliding exercises, and also when needed analgesic drugs were given. When the pretreatment and posttreatment parameters were compared via repetitive measurement analysis, it was found that PGA and MDPGA were significantly decreased in both groups (P = 0.001), whereas grip strength was significantly increased (P = 0.001). While no statistically significant difference was found between the groups regarding pretreatment values (P [ 0.05), the posttreatment PGA, MDPGA, and grip strength scores were significantly improved in Group I compared to Group II (P \ 0.05). To the best our knowledge, the present study is the first and largest study in the literature conducted on the massage technique that will contribute to the treatment of CTS. As this new massage technique is easy for self-application, cheap, and practical, every patient with CTS can apply the massage to him/ herself easily.

Methods: Sixty-six patients with active myofascial trigger points in the trapezius muscle were randomized into treatment groups with US and three sessions of ESWT. Efficacy of the therapies were evaluated prior to therapy at 3 weeks and at 3 months of therapy using the Patient Global Assessment and Physician's Global Assessment scales, Neck Pain and Disability Scale, Nottingham Health Profile, and Hamilton Anxiety Scale. Results: No severe complications were encountered with US and ESWT, and patients tolerated the therapies well overall. Statistically significant improvement was determined in the number of trigger points, pain, quality of life, and anxiety scores in post-therapy evaluations [p50.01]. Efficacy of therapies in inter-group comparison was evaluated in terms of improvement in the same scores, and the ESWT group had greater improvement when the anxiety scores were excluded [p50.05]. Conclusion: Our results indicated that both US and ESWT were effective and safe treatment modalities in myofascial pain syndrome. Three sessions of low dosage ESWT was more effective compared to US therapy that could be used as an effective and safe modality in the treatment protocols of myofascial pain syndrome.

ABSTRACT Objective: For the purpose of this study, we assessed the frequency of hand deformities ... more ABSTRACT Objective: For the purpose of this study, we assessed the frequency of hand deformities in rheumatoid arthritis (RA) as well as the impact of these deformities on the quality of life of the patients.Method: A group of 87 consecutive patients who had been admitted to the Department of Physical Medicine and Rehabilitation of our University Hospital between 2000 and 2002 were included in the study. 66 (76%) were females and, 21 (24%) were males with definite or classical RA. Patients who had undergone articular surgery or had warm and swollen joints were excluded. A protocol was designed to record age, dominant hand, duration of disease, and results of the physical examination. Hand disability was assessed by using a special hand disability index, which was composed by seven items.Result: There was hand deformity in 71 (81.5%) patients. The most common hand deformity was flexor tenosynovitis (64.5%). The other deformities were ulnar deviation (39.5%), boutonniere deformity (29.5%) and swan neck deformity (27.5%). There was a significant correlation between severity of the hand deformity, and disability index and disease duration. All patients were right handed but there was no significant difference between the right and the left hand of the patients regarding either the number of deformities or the grip strength. Eighty-one (93%) of the patients had hand disability. Hand disability was mild in 44%, moderate in 46%, and severe in 10%.Conclusion: RA is an important disease that can cause functional disabilities. Therefore, understanding these deformities is necessary for the proper management of these patients to improve their quality of life. This study is representative for the RA related hand deformities and quality of life of the patients in this country.

Twenty-four female patients with FM syndrome (FMS) were included in the study. The assessments were based on history, physical examination, audiometry, bithermal caloric testing and auditory brainstem response (ABR) testing. Dizziness was the most common complaint of the patients, and was followed by tinnitus, hearing loss and vertigo. Almost 50% of the patients had some sort of otologic symptoms. Dix-Halpike maneuver proved positional rotary vertigo in 5 (20.8%) patients. The audiometry results of 23 patients were normal. None of the patients had abnormal bithermal caloric testing. Although there were a variety of ABR abnormalities, the ABR results of the patients with and without cochleovestibular symptoms were not significantly different (p < 0.05). In conclusion, FM patients can complain otologic symptoms even though they do not have any clinically or audiologically detectable ear disease. A neural disintegration or some other events related to neural mediators may be the mechanisms involved in the pathogenesis of otoneurologic as well as systemic manifestations of the disease that possibly leads to abnormal perception of the stimuli coming from internal or external environment.

Fibromyalgia Syndrome (FMS) is one of the most frequent rheumatologic conditions, its main charac... more Fibromyalgia Syndrome (FMS) is one of the most frequent rheumatologic conditions, its main characteristic being chronic diffuse musculoskeletal pain and hyperalgesia, likely due to altered central processing of pain (1). Fibromyalgia symptoms are not restricted to pain. Besides the painful setting, these patients often complain of fatigue, sleep disturbances, morning stiffness, and paresthesia on the extremities, subjective edema sensations and cognitive disturbances. Many patients experience cognitive dysfunction known as "fibrofog" which may be characterized by impaired concentration. Cognitive symptoms in these patients may be exacerbated by the presence of depression, anxiety, sleep problems, endocrine disturbances, and pain. Association with other comorbidities is often found, contributing to the suffering and decline in quality of life of these patients. Among the most frequent comorbidities found, it can be mentioned depression, anxiety, chronic fatigue syndrome, myofascial syndrome, irritable bowel syndrome and nonspecific urethral syndrome (2). Various researches show that there were relation between chronic pain and cognitive dysfunction in FM patients. Managing chronic pain may take some cognitive effort and this may interfere with performance on cognitive tasks. This is important because even though FMS patients report cognitive symptoms, physicians and scientists must consider the possibility that because FM patients experience many symptoms, there may be a tendency to mistake normal, everyday lapses on cognition as something more serious (3). 2. Relational issues and alexithyma It has been argued that alexithymia is one of the major disturbances about cognitive difunction. The core disturbance in alexithymia is a deficit in the cognitive processing and regulation of emotions. Thus, some of the emotional and behavioural disturbances associated with fibromyalgia may reflect the presence of alexithymia. (4) The high rate of alexithymia as well as anxiety and depression may be seen in FMS patients. Several reports reveal the existence of a relationship between alexithymia, depression and anxiety. Hendryx et al. (5) suggested that alexithymia is multidimensional feature. They also reported that alexithymia was related to generalized anxiety and depression. Anxiety and depression are often precedes or manifest after fibromyalgia.

Our objective was to investigate the association between the bone mineral density and risk factor... more Our objective was to investigate the association between the bone mineral density and risk factors predisposing to osteoporosis as well as impact of regional factors on bone mineral density by comparing the data obtained from patients who lived in Gaziantep (a province in the south east of this country) and Trabzon (a province in the north east of this country). Included in the study were 318 patients, of whom 162 were from Gaziantep and 156 from Trabzon. Bone mineral density of the patients was measured with DEXA, and those who had a t score below (-2.0) SD on bone mineral density measurement in the back and hip regions were included in the study. An osteoporosis follow up questionnaire that was modified from MEDOS study questionnaire was filled. The patients who lived in Gaziantep were fatter than those who lived in Trabzon (p<0.001). The height and age of the patients were also different significantly (p>0.05). The bone mineral density (L2-4 femur (total) and Wards triangle) of the patients who lived in Trabzon was significantly lower than in Gaziantep (p<0.001). There was no statitical difference between the two groups regarding hair color, coffee and dressing habits, daily milk consumption, history of maternal milk ingestion, and alcohol and cigarette abuse (p>0.05). White skin color and high tea consumption were more common in the second group (p<0.001). Some parameters like dark or wheat skin and black eye color, birth and abortion rate, and sun bathing habis were more common in the first group (p<0.001). In conclusion, fist and cost effective option to estimate whether the patient carries a high risk is to evaluate the place where the patient lives as well the habits and traditions. The proceeding bone mineral density measurement will help to diagnose the disease. We believe that this approach will help not only for early diagnosis of osteoporosis but also useful economically.

Our objective was to assess the impact of treatment with risedronate on the bone mineral density ... more Our objective was to assess the impact of treatment with risedronate on the bone mineral density and quality of life in the women who had postmenopausal osteoporosis (PMO). Sixty-five patients with PMO were included in the study. Forty-five of them were selected randomly, and constituted the study group while the remaining 20 served as control. Forty-five patients comprising the study group were treated with 5 mg risedronate and 1000 mg calcium for 1 year while 20 control patients were treated only with 1000 mg calcium. The quality of life of the patients was evaluated with Notthingham Health Profile (NHP) scale both at the beginning and at the end of the study. Bone mineral densities were measured in the vertebral and femoral regions using DEXA at the beginning, and at 6th and 12th months of the treatment. The results of patients and controls were not different significantly after 6 months follow up (p>0.05). At the end of 1 year follow up, the bone mineral density of the treatment group increased significantly while it decreased significantly in the controls (p<0.05). Quality of life increased significantly in the treatment group (p<0.05) while it did not change in the control group (p>0.05). The parameters related to the limitation of physical activity did not change significantly in the treatment group (p>0.05). In conclusion, by increasing the bone mineral density, risedronate increases the quality of bone, decreases the risk of fracture and pain, and positively effects the quality of life.

Objective: In the rehabilitation period of patients with spinal cord injuries (SCI), factors that... more Objective: In the rehabilitation period of patients with spinal cord injuries (SCI), factors that can affect the success of rehabilitation programs include the caregiver’s adaptation to the new situation, his/her motivation and attitude towards possible challenges. This study was conducted for the determine the related factors with burden of caregivers of patients with SCI. Material and Methods: Forty patients with SCI aged 18 to 65 and 40 caregievers were enrolled to the study, that they admitted to Gaziantep University Medical Faculty, Physiotherapy and Rehabilitation Clinic. American Spinal Injury Association (ASIA) and Functional Independence Measurement (FIM) were used for clinical assessment of patients; the Beck anxiety and depression inventory was used for psychiatric assessment; and the Zarit caregiver burden scale was used for assessment of caregivers’ status. Results: 14 caregivers (10 female and 4 male) were the patients’ spouses. Other caregivers consisted of the patients' parents (11), siblings (7), children (5) and other relatives (3). A significant positive correlation was identified between the SCI clinical stage, anxiety and depression scores with caregiver burden. However a significant negative correlation was identified between the patient’s functional status with caregiver burden scores. While reviewing the relation between the patients’ ASIA grades and caregiver burden levels, it was determined that the average care burden level decreased when moving from ASIA grade A to D. Conclusion: We think that timely determination of the potential risk factors such as anxiety and depression, poor clinic and functional status factor for caregiver burden in SCI, as well as providing an appropriate therapy support, may contribute to higher quality in treatment and care of patients with SCI.

This investigation was carried out between April 1999 and March 2000 by the Karadeniz Technical University Medical Faculty Department of Physical Medicine and Rehabilitation. A total of 1825 persons over 20 years old chosen by the sampling method: 959 (52.5%) females and 866 males (47.5%), were included in the study. With the assistance of questionnaires prepared according to the ARA-1987 criteria and the laboratory results, a diagnosis of RA was made in a total of 69 (3.7%) subjects: 53 women (76.8%) and 16 men (23.2%). The male/female ratio was 1/3.3. The RA prevalence in our community was higher than expected, but multicenter studies are necessary.

Objective: Osteoporosis is a chronic disease that can take a long time to treat and is an importa... more Objective: Osteoporosis is a chronic disease that can take a long time to treat and is an important public health issue since it increases the risk of fractures and it has negative effects on quality of life with its social, psychological, economic and physical dimensions. It was aimed to evaluate the effect of patient follow-up program on drug compliance and quality of life. Materials and Methods: This study included 233 patients diagnosed with osteoporosis and the study was continued with 200 patients (173 females with postmenopausal osteoporosis, 27 males with senile osteoporosis). Patients were divided into 3 groups as daily, weekly and monthly follow-up groups. Patients were called by telephone with the help of patient follow-up program. Monthly Medication Possession Ratio (MPR) was calculated. Qualeffo-41 quality of life data were assessed at baseline, at the 3 rd and 6 th months. Results: From the baseline within 6 months, MPR levels were significantly increased in all groups (p<0.05). Monthly follow-up group was found the most compatible group compared to others. Significant changes in quality of life were observed (p<0.05). Conclusions: Dose intervals, especially long dose interval regimens and patient drug reminder programs are highly effective on compliance and quality of life would be better as the compliance increases. It should be bear in mind that if treatment incompliance occurs it may lead to medical harms and economical losses.

Dizziness was the most common complaint of the patients, and was followed by tinnitus, hearing loss and vertigo. Almost 50% of the patients had some sort of otologic symptoms. Dix-Halpike maneuver proved positional rotary vertigo in 5 (20.8%) patients. The audiometry results of 23 patients were normal. None of the patients had abnormal bithermal caloric testing. Although there were a variety of ABR abnormalities, the ABR results of the patients with and without cochleovestibular symptoms were not significantly different (p < 0.05). In conclusion, FM patients can complain otologic symptoms even though they do not have any clinically or audiologically detectable ear disease. A neural disintegration or some other events related to neural mediators may be the mechanisms involved in the pathogenesis of otoneurologic as well as systemic manifestations of the disease that possibly leads to abnormal perception of the stimuli coming from internal or external environment.

Fibromyalgia symptoms are not restricted to pain. Besides the painful setting, these patients often complain of fatigue, sleep disturbances, morning stiffness, and paresthesia on the extremities, subjective edema sensations and cognitive disturbances. Many patients experience cognitive dysfunction known as "fibrofog" which may be characterized by impaired concentration. Cognitive symptoms in these patients may be exacerbated by the presence of depression, anxiety, sleep problems, endocrine disturbances, and pain. Association with other comorbidities is often found, contributing to the suffering and decline in quality of life of these patients. Among the most frequent comorbidities found, it can be mentioned depression, anxiety, chronic fatigue syndrome, myofascial syndrome, irritable bowel syndrome and nonspecific urethral syndrome (2). Various researches show that there were relation between chronic pain and cognitive dysfunction in FM patients. Managing chronic pain may take some cognitive effort and this may interfere with performance on cognitive tasks. This is important because even though FMS patients report cognitive symptoms, physicians and scientists must consider the possibility that because FM patients experience many symptoms, there may be a tendency to mistake normal, everyday lapses on cognition as something more serious (3). 2. Relational issues and alexithyma It has been argued that alexithymia is one of the major disturbances about cognitive difunction. The core disturbance in alexithymia is a deficit in the cognitive processing and regulation of emotions. Thus, some of the emotional and behavioural disturbances associated with fibromyalgia may reflect the presence of alexithymia. (4) The high rate of alexithymia as well as anxiety and depression may be seen in FMS patients. Several reports reveal the existence of a relationship between alexithymia, depression and anxiety. Hendryx et al. (5) suggested that alexithymia is multidimensional feature. They also reported that alexithymia was related to generalized anxiety and depression. Anxiety and depression are often precedes or manifest after fibromyalgia.

Our objective was to investigate the association between the bone mineral density and risk factor... more Our objective was to investigate the association between the bone mineral density and risk factors predisposing to osteoporosis as well as impact of regional factors on bone mineral density by comparing the data obtained from patients who lived in Gaziantep (a province in the south east of this country) and Trabzon (a province in the north east of this country). Included in the study were 318 patients, of whom 162 were from Gaziantep and 156 from Trabzon. Bone mineral density of the patients was measured with DEXA, and those who had a t score below (-2.0) SD on bone mineral density measurement in the back and hip regions were included in the study. An osteoporosis follow up questionnaire that was modified from MEDOS study questionnaire was filled. The patients who lived in Gaziantep were fatter than those who lived in Trabzon (p<0.001). The height and age of the patients were also different significantly (p>0.05). The bone mineral density (L2-4 femur (total) and Wards triangle) of the patients who lived in Trabzon was significantly lower than in Gaziantep (p<0.001). There was no statitical difference between the two groups regarding hair color, coffee and dressing habits, daily milk consumption, history of maternal milk ingestion, and alcohol and cigarette abuse (p>0.05). White skin color and high tea consumption were more common in the second group (p<0.001). Some parameters like dark or wheat skin and black eye color, birth and abortion rate, and sun bathing habis were more common in the first group (p<0.001). In conclusion, fist and cost effective option to estimate whether the patient carries a high risk is to evaluate the place where the patient lives as well the habits and traditions. The proceeding bone mineral density measurement will help to diagnose the disease. We believe that this approach will help not only for early diagnosis of osteoporosis but also useful economically.

Our objective was to assess the impact of treatment with risedronate on the bone mineral density ... more Our objective was to assess the impact of treatment with risedronate on the bone mineral density and quality of life in the women who had postmenopausal osteoporosis (PMO). Sixty-five patients with PMO were included in the study. Forty-five of them were selected randomly, and constituted the study group while the remaining 20 served as control. Forty-five patients comprising the study group were treated with 5 mg risedronate and 1000 mg calcium for 1 year while 20 control patients were treated only with 1000 mg calcium. The quality of life of the patients was evaluated with Notthingham Health Profile (NHP) scale both at the beginning and at the end of the study. Bone mineral densities were measured in the vertebral and femoral regions using DEXA at the beginning, and at 6th and 12th months of the treatment. The results of patients and controls were not different significantly after 6 months follow up (p>0.05). At the end of 1 year follow up, the bone mineral density of the treatment group increased significantly while it decreased significantly in the controls (p<0.05). Quality of life increased significantly in the treatment group (p<0.05) while it did not change in the control group (p>0.05). The parameters related to the limitation of physical activity did not change significantly in the treatment group (p>0.05). In conclusion, by increasing the bone mineral density, risedronate increases the quality of bone, decreases the risk of fracture and pain, and positively effects the quality of life.

This study was conducted for the determine the related factors with burden of caregivers of patients with SCI. Material and Methods: Forty patients with SCI aged 18 to 65 and 40 caregievers were enrolled to the study, that they admitted to Gaziantep University Medical Faculty, Physiotherapy and Rehabilitation Clinic. American Spinal Injury Association (ASIA) and Functional Independence Measurement (FIM) were used for clinical assessment of patients; the Beck anxiety and depression inventory was used for psychiatric assessment; and the Zarit caregiver burden scale was used for assessment of caregivers’ status. Results: 14 caregivers (10 female and 4 male) were the patients’ spouses. Other caregivers consisted of the patients' parents (11), siblings (7), children (5) and other relatives (3). A significant positive correlation was identified between the SCI clinical stage, anxiety and depression scores with caregiver burden. However a significant negative correlation was identified between the patient’s functional status with caregiver burden scores. While reviewing the relation between the patients’ ASIA grades and caregiver burden levels, it was determined that the average care burden level decreased when moving from ASIA grade A to D. Conclusion: We think that timely determination of the potential risk factors such as anxiety and depression, poor clinic and functional status factor for caregiver burden in SCI, as well as providing an appropriate therapy support, may contribute to higher quality in treatment and care of patients with SCI.

This investigation was carried out between April 1999 and March 2000 by the Karadeniz Technical University Medical Faculty Department of Physical Medicine and Rehabilitation. A total of 1825 persons over 20 years old chosen by the sampling method: 959 (52.5%) females and 866 males (47.5%), were included in the study. With the assistance of questionnaires prepared according to the ARA-1987 criteria and the laboratory results, a diagnosis of RA was made in a total of 69 (3.7%) subjects: 53 women (76.8%) and 16 men (23.2%). The male/female ratio was 1/3.3. The RA prevalence in our community was higher than expected, but multicenter studies are necessary.

Objective: Osteoporosis is a chronic disease that can take a long time to treat and is an importa... more Objective: Osteoporosis is a chronic disease that can take a long time to treat and is an important public health issue since it increases the risk of fractures and it has negative effects on quality of life with its social, psychological, economic and physical dimensions. It was aimed to evaluate the effect of patient follow-up program on drug compliance and quality of life. Materials and Methods: This study included 233 patients diagnosed with osteoporosis and the study was continued with 200 patients (173 females with postmenopausal osteoporosis, 27 males with senile osteoporosis). Patients were divided into 3 groups as daily, weekly and monthly follow-up groups. Patients were called by telephone with the help of patient follow-up program. Monthly Medication Possession Ratio (MPR) was calculated. Qualeffo-41 quality of life data were assessed at baseline, at the 3 rd and 6 th months. Results: From the baseline within 6 months, MPR levels were significantly increased in all groups (p<0.05). Monthly follow-up group was found the most compatible group compared to others. Significant changes in quality of life were observed (p<0.05). Conclusions: Dose intervals, especially long dose interval regimens and patient drug reminder programs are highly effective on compliance and quality of life would be better as the compliance increases. It should be bear in mind that if treatment incompliance occurs it may lead to medical harms and economical losses.
