Aim: In patients with Coronavirus disease 2019 (COVID-19) infection, a situation called cytokine ... more Aim: In patients with Coronavirus disease 2019 (COVID-19) infection, a situation called cytokine storm and an increase in proinflammatory cytokines such as interleukin-6 (IL-6), interleukin-8 (IL-8), interleukin-1 (IL-1) in the blood has been observed and it has been found that this is clinically related to the development of severe disease. Therefore, tocilizumab (TCZ) therapy that blocks IL-6 will reduce the immunological response and thus potentially harm caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). The aim of this study is to determine the effect of TCZ treatment on length of hospital stay, need for invasive mechanical ventilation and mortality in COVID-19 patients followed in the tertiary intensive care unit.Material and Method: This retrospective cross-sectional study was conducted among patients hospitalized with the diagnosis of COVID-19 pneumonia between 01.09.2020 and 01.01.21 in intensive care units. Dünyada yılan ısırmaları ciddi morbidite ve mortalite ile sonuçlanabilmektedir. Türkiye'de sıklık... more Dünyada yılan ısırmaları ciddi morbidite ve mortalite ile sonuçlanabilmektedir. Türkiye'de sıklıkla Güney ve Güney Doğu Anadolu Bölge'lerinde karşılaşılan bir durumdur. Burada Ege Bölge'sinde Vipera xanthina (Şeritli Engerek) tarafından sağ el ikinci parmaktan ısırılan 41 yaşında erkek olgu sunulmuştur. Trombositopeni ile komplike olan hastanın ısırılan bölgesinde şiddetli ağrı, ödem ve ekimoz mevcuttu. Şiddetli zehirlenme olgusu olarak kabul edilen hasta yoğun bakım yatışı, medikal tedavi ve antivenom uygulamasından sonra komplikasyonsuz olarak yatışının yedinci gün şifa ile taburcu edildi.

Günümüzde venöz port kateter sistemleri kanser hastalarının uzun süreli kemoterapi tedavisinde ar... more Günümüzde venöz port kateter sistemleri kanser hastalarının uzun süreli kemoterapi tedavisinde aralıklı infüzyon için yaygın olarak kullanılmaktadır. Bu girişimsel işlemin uygulama yerine bağlı olarak enfeksiyon, hematom, kataterin kırılması, arter yaralanmaları, kateterin yer değiştirmesi, pnömotoraks, venöz tromboz gibi komplikasyonlar ortaya çıkmaktadır. Subklavian venöz port kateter işlemi diğer anatomik alanlara göre daha fazla tek taraflı pnömotoraks riski taşımaktadır. Mide malign neoplazm tanısı olan 37 yaşında kadın hasta acil servise üç gündür devam eden bilateral yan ağrısı ve dizüri şikayeti ile başvurdu. Dört gün önce bilateral subklavian venöz port girişim öyküsü olan hastada nefes darlığı ve oksijen satürasyon düşüklüğü saptanmadı. Burada atipik klinikle acil servise başvuran, insidental olarak tanı konan iyatrojenik bilateral masif pnömotoraks olgusunu sunmayı amaçladık.

InTrodUcTIon: It was observed that the new coronavirus disease had a different clinical course in... more InTrodUcTIon: It was observed that the new coronavirus disease had a different clinical course in children compared to adults. Fewer cases and deaths have been reported in children. MATerIAL And MeTHods: All children with suspected COVID-19 who applied to a secondary health care centre were included in this study and blood parameters were compared according to Reverse Transcriptase Polymerase Chain Reaction (RT-PCR) results. This study is a retrospective, cross-sectional study and was conducted by accessing the hospital records of 40 paediatric patients aged 0-16 years. RT-PCR test was performed by isolating Viral RNA with RNA Isolation System after oropharyngeal and nasopharyngeal samples were taken from patients with appropriate sterile swabs and transferred to tubes containing SNP Viral Inactivation Solution. All statistical calculations were done with SPSS 23.0 (SPSS for Windows, Chicago, IL, SA). Mean values of continuous variables were compared between groups using the Mann-Whitney U test. resULTs: The results were evaluated as 95% confidence interval and p-value < 0.05 and were considered statistically significant. In the light of the blood results of a small number of paediatric cases who applied, the authors tried to determine the parameters that could increase the success in diagnosis in asymptomatic paediatric cases. C-Reactive Protein, ID-dimer (quantitative), leukocytes, monocytes, sodium, mean platelet volume levels were found to be significantly different between the patients with positive and negative COVID test results. concLUsIons: The results of this study reveal that the use of leukocytopenia, monocytopenia and mean platelet volume elevation as diagnostic markers may increase diagnostic success, especially in asymptomatic paediatric COVID-19 patients.
