Synchronous multicentric osteosarcoma is a very rare condition that might be confused with metast... more Synchronous multicentric osteosarcoma is a very rare condition that might be confused with metastatic osteosarcoma. We report the first pediatric case of synchronous multicentric osteosarcoma reported in the Puerto Rican. The child presented with multiple osteoblastic bone lesions followed by right upper lid ptosis. Orbital CT-Scan showed a lytic lesion involving the sphenoid bone. Management consisted of chemotherapy for forty-two weeks after which she remained without new secondary bone lesions.

Patients or Programs: A 47-year-old woman. Program Description: The patient with medical history ... more Patients or Programs: A 47-year-old woman. Program Description: The patient with medical history significant for hypertension and multiple rib fractures of unknown etiology presented to the outpatient rehabilitation clinic with a 5-year history of muscle soreness, generalized weakness, progressive gait dysfunction and severe bone pain. The patient was referred with the diagnosis of arthritis, fibromyalgia, and osteogenesis imperfecta. The patient was hospitalized for the management of severe bone pain. During the hospital stay multiple radiographs revealed generalized osteomalacia and healed fracture sites. Her laboratory work showed low serum phosphorus and low 1,25 hydroxyvitamin D and 25-OH vitamin D. An octreotide bone scan was positive for an occult tumor in the left popliteal fossa. An extensive tumor workup revealed this tumor to be a phosphaturic mesenchymal tumor. Setting: Outpatient physical medicine and rehabilitation clinic. Results: The tumor was excised, and the patient experienced full symptom resolution. Discussion: This oncogenic hypophosphatemic osteomalacia is caused by a rare phosphaturic mesenchymal tumor. Extracts of this mesenchymal tumor produce fibroblast growth factor 23 messenger RNA, leading to increased levels of FGF-23 hormone. FGF-23 inhibits phosphate re-absorption by the sodium phosphate co-transporter in the kidney resulting in renal phosphate wasting. The end result is phosphaturia, hypophosphatemia, and reduced calcitriol production. FGF-23 also inhibits the synthesis of 1,25-dihydroxyvitamin-D leading to decreased intestinal phosphate absorption. Phosphate ions are essential for normal bone mineralization because calcium and phosphate are required for the formation of hydroxyapatite and other mineral-phase components. Conclusions: Oncogenic hypophosphatemic osteomalacia is a rare and serious condition that can be reversed if the tumor is identified and excised. Idiopathic fractures in the setting of bone pain with weakness in a younger patient should alert the clinician to the possibility of a tumor-induced osteomalacia. Appropriate and timely diagnosis of this disorder can save the patient months of unnecessary pain and debility.
Objective: This review article describes the etiology, pathogenesis, clinical characters and mana... more Objective: This review article describes the etiology, pathogenesis, clinical characters and management of low back pain (LBP) caused by myofascial trigger points (MTrPs), i.e. myofascial low back pain. Findings: Based on the currently available knowledge and our clinical experience, we analyzed the basic and clinical aspects of myofascial LBP. Most cases of myofascial LBP are related to injuries, either current or previous. Active MTrPs that cause LBP are usually activated as a consequence of other etiological lesions. Therefore, it is important to determine and treat the underlying pathological lesions in order to avoid recurrence of myofascial LBP. If the underlying pathological lesion is unable to be identified and the pain in MTrPs is very severe, we may still have to suppress the active MTrP for pain control. To inactivate MTrPs, effective approaches include manual therapy, physical therapy modalities, and needling including acupuncture and MTrP injection. Conclusion: It is important to find out the underlying etiological lesion, which causes LBP, and to provide appropriate management based on our best knowledge. Although many psychologists support prescription privileges, the historical training paradigm in ... more Although many psychologists support prescription privileges, the historical training paradigm in psychology comprises limited scientific education that is directly relevant to prescribing medications. Issues related to prescriptive authority for psychologists, including training gaps, attitudes, accreditation, and regulation are discussed. Current proposals for training psychologists to prescribe deleted the prerequisite coursework in the biological and physical sciences that had been identified by the American Psychological Association's Ad Hoc Task Force on Psychopharmacology. Current proposals do not delineate clear requirements for several key aspects of supervised practical training. Such training limitations raise legitimate questions about how much additional scientific and medical training would be necessary to ensure that psychologists could provide an acceptable quality of pharmacologic care.

Panic disorder is a common illness with significant impact on function and, at times, disabling c... more Panic disorder is a common illness with significant impact on function and, at times, disabling consequences. Advances over recent years have yielded potential pathophysiologic mechanisms, including neurotransmitter systems and neural substrates. This article provides an overview of various models and hypotheses regarding the etiology of panic disorder. Because its symptoms overlap with the symptoms of various medical disorders, this discussion reviews the comorbidity of panic disorder with medical conditions, especially cardiac and respiratory disorders.

As some psychologists advocate for prescription privileges, the need for closer analysis of the d... more As some psychologists advocate for prescription privileges, the need for closer analysis of the differences between psychologists and psychiatrists grows. Our data reveal key gaps in psychologists' training and significant limitations in their knowledge pertaining to prescribing relative to psychiatrists. Attitudes toward prescribing and estimates of psychologists' competence in prescribing are presented. The authors believe that psychologists' deficits in training and pertinent knowledge constitute major hurdles to competent prescribing. Caution is warranted about expanding psychologists' scope of practice to include prescribing.

The treatment of psychiatric illnesses, prevalent in the general hospital, requires broadly train... more The treatment of psychiatric illnesses, prevalent in the general hospital, requires broadly trained providers with expertise at the interface of psychiatry and medicine. Since each hospital operates under different economic constraints, it is difficult to establish an appropriate ratio of such providers to patients. Objective: The authors sought to determine the current staffing patterns and ratios of Psychosomatic Medicine practitioners in general hospitals, to better align manpower with clinical service and educational requirements on consultation-liaison psychiatry services. Method: Program directors of seven academic Psychosomatic Medicine (PM) programs in the Northeast were surveyed to establish current staffing patterns and patient volumes. Survey data were reviewed and analyzed along with data from the literature and The Academy of Psychosomatic Medicine (APM) fellowship directory. Results: Staffing patterns varied widely, both in terms of the number and disciplines of staff providing care for medical and surgical inpatients. The ratio of initial consultations performed per hospital bed varied from 1.6 to 4.6. Conclusion: Although staffing patterns vary, below a minimum staffing level, there is likely to be significant human and financial cost. Efficient sizing of a PM staff must be accomplished in the context of a given institution's patient population, the experience of providers, the presence/absence and needs of trainees, and the financial constraints of the department and institution. National survey data are needed to provide benchmarks for both academic and nonacademic PM services.

A series of465 consecutive psychiatric consultations to medical-surgical services was examined to... more A series of465 consecutive psychiatric consultations to medical-surgical services was examined to compare consultations designated as emergencies with those requested on a routine basis. Twenty-three percent ofthe consultation requests were designated as emergencies. and psychotic thinking and suicidality were noted more frequently in this group. Emergencies. however. were invoked less frequently in patients 60 years ofage or older. The consultant's approach to emergency consultations demonstrates a more active role. with more recommendations for initiation ofpsychotropic drugs. more follow-up visits. greater numbers ofrecommendations for disposition. and greater numbers ofcontacts with outside sources ofinformation effected. Notably. concordance with diagnostic and psychotropic drug recommendations and representation ofpsychiatric diagnosis in discharge summaries did not differ significantly between the two groups. The results underscore the usefulness ofclosely monitoring consultation outcomes and the need to explore and implement interventions that will enhance consultee responses in situations requiring acute psychiatric interventions.
