In an experimental model using the rat, the role of invagination and double invagination for effi... more In an experimental model using the rat, the role of invagination and double invagination for efficient closure of the caecum was studied. A standardized opening of the tip of the caecum was closed in three different ways, thus placing the animals in three groups. I: Closed with simple ligation. II: Closed with simple ligation and a purse-string suture invagination. III: Closed with simple ligation and a purse-string suture and a Z-suture causing double invagination. The caecum was filled with methylene blue and obstructed by a balloon catheter introduced via a caecostomy. Pressure inside the obstructed caecum was measured through a channel opening at the tip of the catheter. The pressure was increased with 4 cm of water every 10 minutes. The maximal pressure reached before leakage of methylene blue from the caecal closure or a drop in pressure in the lumen, was seen to be significantly higher in the groups with invagination and double invagination of the caecal closure, respectively...

We wish to thank the authors of the Letter to the Editor for their attention to our study [1] and... more We wish to thank the authors of the Letter to the Editor for their attention to our study [1] and contributing to the discussion. Our replies to their comments are summarized below. The study [2] should not be excluded because of an age range of 2–19 years even though the included criteria of the meta-analysis (MA) mentioned boys aged < 18 years. The authors of the publication [2] state that “Between 1995 and 2001, Snodgrass-type repair was done in 32 primary and in 24 recurrent hypospadias cases (total 56 patients, mean age 5.8, age range 3–17 years), and Mathieu-type repair was done in 33 primary and in 21 recurrent distal hypospadias cases (total 54 patients, mean age 5.2, age range 2–19 years). There was no statistically significant difference between groups regarding demographics”. We are aware of this age range and do not consider it appropriate to exclude a wellconducted study based on a 1-year discrepancy alone mostly because there were no statistically significant demographic differences between the groups. Furthermore, the study [3] should not be excluded due to a minor discrepancy in the limited number of cases of distal and mid-shaft hypospadias. The surgical methods employed were the same and performed by the same group of surgeons. These differences are discussed in the summary as a potential bias to the study [1]. We do not agree that one of the studies [4] should be excluded from the overall analysis, since two different groups (2010–2012 and 2013–2015) of hypospadias patients underwent the Mathieu repair method or tubularized incised plate (TIP) urethroplasty in the original study. This was a well-conducted comparative study of two different methods for hypospadias repair. In this case [4], the same surgeons preformed the interventions using the same techniques. Thus, we believe that inclusion of this report was appropriate. We acknowledge that there are some data inconsistencies with the original text [5], as compared with the MA [1]. For example, in Fig. 5, the number of urethra strictures was included in the MA [1] even though the original publication reported only three cases [5]. We reasoned that a MA conducted by Wang et al. [6] included that study [5] for analysis in the original paper. Thus, we consider that the study included the most appropriate information. In one study [7], three groups (Mathieu, TIP, and TIP + Mathieu) were compared and were assigned to either the TIP or Mathieu group (Mathieu and TIP + Mathieu) in the MA [1], which we believe is appropriate, since the modified TIP procedure (TIP + Mathieu) could also be included in the TIP group. We are aware of the fact that different surgeons performed the modified Mathieu and TIP procedures. We acknowledge differences in details between the different surgeons when discussing the two methods. The relationship of acute appendicitis occurring previous to cancer in colon and rectum was studi... more The relationship of acute appendicitis occurring previous to cancer in colon and rectum was studied in the consecutive records of 561 patients, of 40 years of age and older, operated upon with an appendectomy because of acute appendicitis. Sixteen (2.9%) of these patients were readmitted within three years because of a carcinoma in colon or rectum. The incidence of carcinoma in the colon and rectum in the population, of the same age, is only 0.1%, according to the Sweden Cancer Registry (1). This difference is statistically significant. Where acute appendicitis and colon carcinoma co-exist, the danger is that the carcinoma may be missed. Therefore, any patient over the age of 40 presenting with acute appendicitis should be carefully checked for carcinoma in the colon.

Summary Esophageal atresia with or without tracheoesophageal fistula (EA/TEF) is the most common ... more Summary Esophageal atresia with or without tracheoesophageal fistula (EA/TEF) is the most common congenital malformation of the upper digestive tract. This study represents the first genome-wide association study (GWAS) to identify risk loci for EA/TEF. We used a European case-control sample comprising 764 EA/TEF patients and 5,778 controls and observed genome-wide significant associations at three loci. On chromosome 10q21 within the gene CTNNA3 (p = 2.11 × 10−8; odds ratio [OR] = 3.94; 95% confidence interval [CI], 3.10–5.00), on chromosome 16q24 next to the FOX gene cluster (p = 2.25 × 10−10; OR = 1.47; 95% CI, 1.38–1.55) and on chromosome 17q12 next to the gene HNF1B (p = 3.35 × 10−16; OR = 1.75; 95% CI, 1.64–1.87). We next carried out an esophageal/tracheal transcriptome profiling in rat embryos at four selected embryonic time points. Based on these data and on already published data, the implicated genes at all three GWAS loci are promising candidates for EA/TEF development. We also analyzed the genetic EA/TEF architecture beyond the single marker level, which revealed an estimated single-nucleotide polymorphism (SNP)-based heritability of around 37% ± 14% standard deviation. In addition, we examined the polygenicity of EA/TEF and found that EA/TEF is less polygenic than other complex genetic diseases. In conclusion, the results of our study contribute to a better understanding on the underlying genetic architecture of ET/TEF with the identification of three risk loci and candidate genes.

Social networks are frequently used by university students, as well as by the rest of the populat... more Social networks are frequently used by university students, as well as by the rest of the population worldwide to communicate, collect information, and share information and images. The aim of this report was to determine if there was substantial evidence in the literature that demonstrated the benefits of using social networks to enhance the learning process, as measured by students' performance on examinations. The main outcome measures were any measurable difference between students who used social networks as part of their education compared to those who did not. A systemic literature search was performed in the PubMed database using predefined search terms, exclusion/inclusion criteria, and primary/secondary criteria. The results disclosed in total, 636 publications were identified; however, only 77 articles met the criteria for inclusion and exclusion. After applying secondary filter criteria, 18 publications were identified that included randomized comparative studies, review articles, and meta-analyses. A review of the list of references in these publications revealed an additional seven articles. The findings demonstrate that although social media represents an important source of medical information that is widely used in education and in everyday lives, no studies have reported that it has a significant impact in enhancing the learning process. Additional comparative studies are needed on this topic.

The aim of this study was to identify short-term complications after video-assisted gastrostomy i... more The aim of this study was to identify short-term complications after video-assisted gastrostomy in children and to examine the association between complications and underlying disease. Methods: This was a retrospective study at a single institution. Data for all children who underwent video-assisted gastrostomy during 12 years was collected. Complications occurring within three months postoperatively were analyzed. Results: Among 421 children undergoing videoassisted gastrostomy, 402 were included in the study. The median age at surgery was two years (range one month-14 years). The most common underlying disorder was neurological disease (49%). There were two major postoperative complications and 78% had minor complications. Minor complications included granulation tissue (43%), leakage (16%), wound infection (14%), vomiting, dislodgement and pain. There was no significant difference in frequency of the various complications when compared between the underlying diseases (p=0.10-0.82). Wound infection was registered in overall 14% and occurred least frequently in children with neurologic disease and syndromes (10% and 9% respectively). Conclusion: Serious complications after videoassisted gastrostomy are rare, while minor complications occur in 78% of the children. There is no association between the frequency of complications and underlying disease according to this study.

Studying whole cell preparations with intact mitochondria and respiratory complexes has a clear b... more Studying whole cell preparations with intact mitochondria and respiratory complexes has a clear benefit compared to isolated or disrupted mitochondria due to the dynamic interplay between mitochondria and other cellular compartments. Platelet mitochondria have a potential to serve as a source of human viable mitochondria when studying mitochondrial physiology and pathogenic mechanisms, as well as for the diagnostics of mitochondrial diseases. The objective of the present study was to perform a detailed evaluation of platelet mitochondrial respiration using high-resolution respirometry. Further, we aimed to explore the limits of sample size and the impact of storage as well as to establish a wide range of reference data from different pediatric and adult cohorts. Our results indicate that platelet mitochondria are well suited for ex-vivo analysis with the need for minute sample amounts and excellent reproducibility and stability.

Charts of all children with anorectal malformations (ARM) reconstructed with PSARP, limited PSARP, or PSARVUP at a tertiary centre of paediatric surgery between 2001 and 2016 were reviewed. Before reconstruction of the esophagus, a bronchoscopy is performed to visualize the tracheal opening of the tracheoesophageal fistula. If the fistula can be identified, a guidewire is passed via the bronchoscope through the fistula, into the distal esophagus, and then into the stomach. The tracheoesophageal guidewire can then serve as a landmark to facilitate the esophageal reconstruction.

Methods A retrospective study was conducted on children with EA (Gross type C) who underwent direct repair between January 2008 and March 2015 at a single tertiary center of pediatric surgery. Endoscopic signs of stricture were indications for dilatation because the endoscopy provides more reliable information than X-ray imagining methods. Results Among our cohort of 49 children with EA, 19 (39%) required at least one esophageal dilatation. All children required initial dilatation within the first year of life and none was older than 1 year during initial dilatation (p < 0.01). This is a description of the technique used for laparoscopy-assisted colostomy in neonates with a... more This is a description of the technique used for laparoscopy-assisted colostomy in neonates with anorectal malformations (ARM), using 2 laparoscopic ports only. The technique is easy to use and has the advantages of enabling identification of the sigmoideum, the internal genitals and to establish a stoma leaving only minimal scars in the umbilicus. In our departments, we routinely perform stoma operations using this technique. We do not claim any benefits or disadvantages of LAC compared with the traditional open technique.

The aim of this study was to map gastroscopies performed at a single tertiary pediatric surgery centre to investigate indications, complications, outcomes, and ethical aspects. Material and Methods. A retrospective study of gastroscopies performed during two time periods (2001-2004 and 2011-2014) was conducted. Data regarding indications, outcomes, and complications of pediatric gastroscopies were analysed from a prospectively collected database. Results. The indications for gastroscopies changed over time. Therefore, 376 gastroscopies performed from 2011 through 2014 were studied separately. The median patient was four years old. The predominant indications were laparoscopic gastrostomy (40%), investigation of gastroenterological conditions (22%), obstruction in the upper gastrointestinal tract (20%), gastroesophageal reflux disease (GERD) (15%), and other indications (3%). The aim of this report is to describe a helpful new technique for use in the management of congen... more The aim of this report is to describe a helpful new technique for use in the management of congenital esophageal atresia with tracheoesophageal fistula. Before reconstruction of the esophagus, a bronchoscopy is performed to visualize the tracheal opening of the tracheoesophageal fistula. If the fistula can be identified, a guidewire is passed via the bronchoscope through the fistula, into the distal esophagus, and then into the stomach. The tracheoesophageal guidewire can then serve as a landmark to facilitate the esophageal reconstruction.

Methods A retrospective study was conducted on children with EA (Gross type C) who underwent direct repair between January 2008 and March 2015 at a single tertiary center of pediatric surgery. Endoscopic signs of stricture were indications for dilatation because the endoscopy provides more reliable information than X-ray imagining methods. Results Among our cohort of 49 children with EA, 19 (39%) required at least one esophageal dilatation. All children required initial dilatation within the first year of life and none was older than 1 year during initial dilatation (p < 0.01). A median of three dilatations (range: 1–13) took place per patient, with 87% performed during the first postoperative year. Methods: This was a retrospective study at a single institution. Data for all children who underwent video-assisted gastrostomy during 12 years was collected. Complications occurring within three months postoperatively were analyzed. Results: Among 421 children undergoing videoassisted gastrostomy, 402 were included in the study. The median age at surgery was two years (range one month-14 years). The most common underlying disorder was neurological disease (49%). There were two major postoperative complications and 78% had minor complications. Minor complications included granulation tissue (43%), leakage (16%), wound infection (14%), vomiting, dislodgement and pain. This is a description of the technique used for laparoscopy-assisted colostomy in neonates with a... more This is a description of the technique used for laparoscopy-assisted colostomy in neonates with anorectal malformations (ARM), using 2 laparoscopic ports only. The technique is easy to use and has the advantages of enabling identification of the sigmoideum, the internal genitals and to establish a stoma leaving only minimal scars in the umbilicus. In our departments, we routinely perform stoma operations using this technique. We do not claim any benefits or disadvantages of LAC compared with the traditional open technique.

The aim of this study was to map gastroscopies performed at a single tertiary pediatric surgery centre to investigate indications, complications, outcomes, and ethical aspects. Material and Methods. A retrospective study of gastroscopies performed during two time periods (2001-2004 and 2011-2014) was conducted. Data regarding indications, outcomes, and complications of pediatric gastroscopies were analysed from a prospectively collected database. Results. The indications for gastroscopies changed over time. Therefore, 376 gastroscopies performed from 2011 through 2014 were studied separately. The median patient was four years old. The predominant indications were laparoscopic gastrostomy (40%), investigation of gastroenterological conditions (22%), obstruction in the upper gastrointestinal tract (20%), gastroesophageal reflux disease (GERD) (15%), and other indications (3%).
