We aimed to present the diagnosis and treatment of foreign bodies in pediatric age group. Material and Methods: Files of 510 patients aged 16 years and below that underwent rigid bronchoscopy due to suspicion of FB aspiration between June 1994 and November 2017 were retrospectively reviewed. 263 cases were included to the study that underwent foreign body removal. The cases were assessed in terms of gender, age, type and localization of the foreign body, clinical complaint, radiological findings, treatment method, complication and mortality. Results: Among the patients 45.6% were male and 54.4% were female. The aspirated foreign bodies varied from dried nuts to plastic, from hijab needles to beads. The most aspirated foreign body were dried nuts. 51% of them were removed from the right main bronchus, 31.9% from the left main bronchus, and 17.1% from the trachea. 97.8% of the foreign bodies were removed via bronchoscope. Thoracotomy was performed in 6 cases since the foreign bodies were not extractable with a bronchoscope or due to developing complications. No mortality occurred. Conclusions: FB aspiration can lead to life-threatening complications in children aged 3 years or below. Bronchoscopy should be performed in all cases of foreign body aspiration, including suspicious cases. Notably, the bronchoscopy procedure should be carried out by a well-trained, experienced anesthesiologist-endoscopy team and with appropriate tools.

We present a case of traumatic hydropneumothorax due to hydatid cyst rupture in a 10 year-old gir... more We present a case of traumatic hydropneumothorax due to hydatid cyst rupture in a 10 year-old girl. The patient was suspected to have a bronchial rupture because of prolonged massive air leak and she underwent cystotomy via right posterolateral thoracotomy. The patient was readmitted with dyspnea and chest X-ray revealed a lung collapse 13 months postoperatively. Lung collapse was treated by using a Heimlich valve. Pulmonary hydatid cyst can be asymptomatic for a long time unless a complication occurs. Because of the high incidence of hydatid disease in our country, this condition should be considered in cases with hydropneumothorax. The use of a Heimlich valve may be a good choice in the management of persistent air leak and may reduce the need for surgery.

We report a rare occurrence of trapped lung associated with Wermer's syndrome and cutis l... more We report a rare occurrence of trapped lung associated with Wermer's syndrome and cutis laxa. A 25-year-old man with bilateral trapped lung presented with respiratory distress. Additional investigation revealed adenoma of the parathyroid and hypophysis, a neuroendocrine abdominal tumor, and cutis laxa. Successful treatment of the respiratory symptoms was achieved through a median sternotomy with bilateral decortication, and excision of the bilateral fibrous pleural bands and bullae on the right upper lobe. Octreotide therapy was initiated and a percutaneous ultrasound-guided alcohol injection to the parathyroid adenoma was done. The patient has gynecomastia, acromegaly, and kyphoscoliosis. The patient remains well at 19 months after treatment.

Chylothorax is a rare but serious complication of cardiac surgery, with a poor prognosis unless t... more Chylothorax is a rare but serious complication of cardiac surgery, with a poor prognosis unless treated properly. We report the case of 66-year-old woman who developed chylothorax after coronary artery bypass grafting. The chylothorax was successfully treated in 8 days by means of subcutaneous octreotide administration and a diet that contained medium-chain triglycerides. Octreotide, a long-acting somatostatin analog, is an effective and safe agent for the treatment of postoperative chylothorax and may reduce the need for reoperation.

In this study, patients with spontaneous rib fracture were analyzed according to age, sex, underlying pathology, treatment and complications. MATERIALS AND METHODS: 12 patients who presented between February 2009 and February 2011 with spontaneous rib fracture were analyzed retrospectively. The patients were evaluated according to anamnesis, physical examination and chest X-rays. RESULTS: 7 patients (58.4%) were male. The age of the patients ranged from 34 to 77 years (mean ± SD, 55.91 ± 12.20). All patients had severe cough and chest pain. Multiple rib fractures were detected in five patients (41.7%). The fracture were detected most frequently between 4th and 9th ribs. 8 (66.66%) patients had COPD, 2 (16.66%) patients had bronchial asthma and 2(16.66%) patients had osteoporosis as well. The patients with COPD and bronchial asthma were treated with high dose steroids for over a year. Bone densitometry revealed high risk of bone fracture in all patients. CONCLUSION: Spontaneous rib fracture due to severe cough may occur in patients with osteoporosis or with COPD and bronchial asthma receiving long-term steroid therapy. If these patients have severe chest pain, chest radiography should be evaluated for bone lesions particularly.

Bir çok yazar tarafından faydalı olduğu iddia edilmekle birlikte bu konuda tam bir görüş birliği henüz sağlanamamıştır. İyi seçilmiş vakalarda üç alanlı lenfadenektomi kabul edilebilir morbidite ve mortalite oranlarıyla uygulanabilmektedir.

We report a case of myasthenia gravis and pancytopenia in a 53-year-old man with lymphoepithelial thymoma and interstitial lung disease. Preoperative examination revealed neither hematologic abnormality nor myasthenia gravis. The patient had enteritis prior to thymomectomy, sternal infection in the first month of operation, and urinary infection at the third month. About three months after thymomectomy, he required mechanical ventilation support due to myasthenia gravis-related respiratory failure. One month later, a rapidly progressing pancytopenia developed. The patient died within two weeks of overwhelming septicemia unresponsive to treatment with antibiotics and steroids. The possible onset of myasthenia gravis or pancytopenia after thymomectomy should be kept in mind during follow-up. Recurrent infections in the early stages of thymomectomy may suggest a lethal onset of pancytopenia.

Aim of the study was the assessment of thoracotomies performed for spine surgery. All patients un... more Aim of the study was the assessment of thoracotomies performed for spine surgery. All patients undergoing spine surgery with thoracotomy between December 2004 and October 2007 were included and evaluated with regard to their age, gender, etiology, the level of the intervention and the types of procedures performed. A total of 29 patients undergoing spine surgery were included. Mean age was 45 years (range: 6-75) and the female to male ratio was 9 : 20. Trauma (14 patients, 48 %) and malignancy (5 patients, 17 %) were the most frequent reasons for surgery. A total of 7, 8 and 14 patients were operated at T3-T6, T7-T10, and T11-L2 levels, respectively. A left thoracotomy was performed in all but three patients (n = 26). The most frequent procedure was corpectomy and stabilization (n = 25, 86 %). An anterior approach with thoracotomy is needed for spine surgery in subjects with conditions such as lung cancer, trauma, hydatid cyst, etc. A good preoperative assessment of the vertebrae requiring intervention and the use of appropriate techniques for separating the diaphragm during thoracotomy may provide a good exposure up to the L3 level.

The aim of this study is to assess patients treated for esophageal foreign bodies. The charts of ... more The aim of this study is to assess patients treated for esophageal foreign bodies. The charts of patients (n=177), between January 1994 and April 2009, were investigated retrospectively. Patients' age and gender, locations and types of foreign bodies (FBs) and interventions were taken into consideration. Fifty-seven percent of the patients were male. The youngest patient was 6 months, whereas the oldest was 83 years old. The median age was 9 years. Half of the patients were in their first decade. Treatment took place 11 h (ranging from 1 to 120 h) after impaction of the FB. One hundred fifty-two FBs were removed in 177 patients. Our negative esophagoscopy (n=25) rate was 14%. The FBs were radiopaque in 75% (n=114) and were commonly (71%; n=109) located in the cervical esophagus. Metallic coins (n=81-53%) were the most commonly observed inorganic FB while bones and/or meat impaction (n=54-35%) were the most frequent organic FB. A total of 182 endoscopic interventions were performed on these patients. One hundred eleven of them were esophagoscopy and the remaining 71 were direct laryngoscopy. The FB was pushed into the stomach in 11 patients. Our morbidity rate was 1.6% (n=3). Iatrogenic perforation occurred in two patients. There was no mortality. Esophageal FBs may vary in type: sharp or round objects, metallic, plastic or organic material. FBs are commonly found at pharyngoesophageal junction and usually removed by McGill forceps. Rigid esophagoscopy is used for children and adults because of its large working channel. Rigid instruments are considered reliable and safe for extracting foreign bodies.
