The patient MM, 62 years old, was admitted in the Institute of Gastroenterology and Hepatology, I... more The patient MM, 62 years old, was admitted in the Institute of Gastroenterology and Hepatology, Iasi, for the investigation of a liver tumor identified at ultrasound examination and accusing: dispneea, sporadic diarrhea, cutaneous flush and a significant weight loss of recent history. The clinical examination revealed a hepatomegaly and a tumour in the lower right quadrant of the abdomen. Laboratory usual analysis were normal. At ultrasound imaging there was identified a solid unomogenous liver tumor invading the middle hepatic vein. Colonoscopy was normal. Contrast computedtomography revealed a terminal ileum tumor with regional enlarged limph nodes and liver metastasis. Suspecting a carcinoid tumor the level of serum serotonine was 5 times higher than normal. The evolution was marked by an intestinal occlusion wich required surgery (ileo-right hemicolectomy with termino-terminal anastomosis) but in the 13-th day the patient died by pulmonar embolism.

The clinical course combines elements of severe pancreatitis and obstructive jaundice with angiocolitis. Complications are very frequent in the absence of spontaneous, interventional or surgical desobstruction. The diagnosis is based upon clinical features, biochemical paramethers with cholestasis, hyperamilasemia, hyperamilasuria, hyperlipasemia, metabolic disorders. Imagistic assay is undoubtful necessary for the diagnosis; it allows identifying most of the complications, and some of these investigations are very useful in the desobstruction procedures, done at proper moment.

We present a 28 years old patient admitted for pain in the lower right quadrant of the abdomen, fever, chills, weight loss and anorexia. Clinical examination revealed a tumor in the lower right quadrant and moderate fever. Biochemical and hematological investigations identified a nonspecific inflammatory syndrome. Carcinoembrionar antigen was negative. The yellowish discoloration of the palms and skin is reported under different terms: xanthodermia... more The yellowish discoloration of the palms and skin is reported under different terms: xanthodermia, hypercarotenemia, carotenemia, carotenodermia. Yellow discoloration of the skin may be associated with carotenemia, hypothyroidism, diabetes mellitus, hyperlipoproteinemia, liver disease, and renal disease, meaning that carotenemia is not synonym with yellow skin, but rather one of the cause. We presented an 8 year‐old boy with a yellow discoloration of the palms and soles, observed by the mother 3 weeks prior to medical examination. The discoloration was uniformly distributed, rather symmetrically, no nails changes, just a mild xerosis palmaris on the right hypotenar area. In lab investigations only were: hypercholesterolemia. The final diagnosis was xanthodermia in context of hyperlypoproteinemia type II A.

We present the case of a 68 year old patient, in medical evidence for ten years with a Child A ethanolic liver cirrhosis, who was admitted in emergency for hematemesis and melena. Clinical examination and the laboratory findings at the admittance revealed signs of decompensated cirrhosis and severe anemia. Ultrasound examination showed a cirrhotic liver with portal hypertension signs and a multinodular mass in the right lobe of the liver with portal, biliary and right hepatic vein invasions extended to inferior caval vein. In upper digestive endoscopy stage IV esophageal varices were evidenced with signs of recent bleeding (sclerotherapy was performed) along with gastric varices and portal gastropathy. Transabdominal ultrasound (US) is one of the most useful methods in the diagnosis of biliary ston... more Transabdominal ultrasound (US) is one of the most useful methods in the diagnosis of biliary stones. To appreciate the value of US in the diagnosis of bile duct stones. 989 patients, explored in the Institute of Gastroenterology and Hepatology Iaşi by US compared with ERCP, MRCP, operative diagnosis and the final diagnostic. The sensitivity of US in the detection of bile duct stones was 78.39% and specificity was 72.22% in the first group of 650 patients, and bile duct stones were identified in 139 cases of 339 of the second group where US could not identify the cause of bile duct dilatations. The results were comparable with the most recent studies in the literature. US is an efficient method for the diagnosis of bile duct stones in many cases and remains the first step in the diagnosis.

We present three rare entities of mucinous tumors: appendiceal mucinous adenomas, enteroid mucino... more We present three rare entities of mucinous tumors: appendiceal mucinous adenomas, enteroid mucinous cyst and pseudomyxoma peritonei, the latter as a developmental course or separate idiopathic etiology of mucinous tumors. We attempted to clarify the term of pseudomyxoma peritonei, a poorly understood condition, characterized by a diffuse intraperitoneal collection of gelatinous fluid with mucinous tumoral implants on the peritoneal surfaces. With this rare condition it is often difficult to establish the histological and developmental malignant or benign characteristics. We analyzed 4 patients admitted during the period of February 2000 - February 2002 in the First Surgical Clinic of St. Spiridon Hospital and in addition we referred to the current approach in the recent literature. Using a decision-analytic model, we evaluated the effectiveness and cost-effectiveness of surveil... more Using a decision-analytic model, we evaluated the effectiveness and cost-effectiveness of surveillance for hepatocellular carcinoma (HCC) in individuals with cirrhosis. Separate cohorts with cirrhosis due to alcoholic liver disease, hepatitis B and hepatitis C were simulated. Results were also combined to approximate a mixed aetiology population. Comparisons were made between a variety of surveillance algorithms using a-foetoprotein (AFP) assay and/or ultrasound at 6-and 12-monthly intervals. Parameter estimates were obtained from comprehensive literature reviews. Uncertainty was explored using one-way and probabilistic sensitivity analyses. In the mixed aetiology cohort, 6-monthly AFP þ ultrasound was predicted to be the most effective strategy. The model estimates that, compared with no surveillance, this strategy may triple the number of people with operable tumours at diagnosis and almost halve the number of people who die from HCC. The cheapest strategy employed triage with annual AFP (incremental cost-effectiveness ratio (ICER): d20 700 per quality-adjusted life-year (QALY) gained). At a willingness-to-pay threshold of d30 000 per QALY the most cost-effective strategy used triage with 6-monthly AFP (ICER: d27 600 per QALY gained). The addition of ultrasound to this strategy increased the ICER to d60 100 per QALY gained. Surveillance appears most cost-effective in individuals with hepatitis B-related cirrhosis, potentially due to younger age at diagnosis of cirrhosis. Our results suggest that, in a UK NHS context, surveillance of individuals with cirrhosis for HCC should be considered effective and cost-effective. The economic efficiency of different surveillance strategies is predicted to vary markedly according to cirrhosis aetiology.

The role of oxidative stress in inflammatory bowel diseases (IBD) has been extended lately from a... more The role of oxidative stress in inflammatory bowel diseases (IBD) has been extended lately from a simple consequence of inflammation to a potential etiological factor, but the data are still controversial. Active disease has been characterized before by an enhanced production of reactive oxygen species and the increased peroxidation of lipids, but patients in remission were generally not considered different from healthy people in terms of oxidative stress. We evaluated the antioxidant defense capacity and lipid peroxidation status in the serum of patients with active and non-active disease compared with healthy matched control subjects. The study included 20 patients with confirmed IBD in clinical and biological remission, 21 patients with active disease, and 18 controls. We determined the serum levels of two antioxidant enzymes, superoxide dismutase (SOD) and glutathione peroxidase (GPX), and a lipid peroxidation marker, malondialdehyde (MDA). Active disease patients had an increased activity of both SOD and GPX, as well as significant high values of MDA versus controls. Furthermore, patients being in remission had significantly lower values of antioxidant enzymes (SOD and GPX) and increased lipid peroxidation measured by MDA serum levels, as compared with healthy control subjects. Our study confirmed the presence of high oxidative stress in active IBD. More importantly, we have demonstrated a lower antioxidant capacity of patients in remission versus control group. This may represent a risk factor for the disease and can be an additional argument for the direct implication of oxidative stress in the pathogenesis of IBD.

Inflammatory bowel diseases (IBD) are chronic conditions of unknown etiology with increasing inci... more Inflammatory bowel diseases (IBD) are chronic conditions of unknown etiology with increasing incidence in South-East Europe. The epidemiological characteristics, clinical, biological and endoscopic profiles are very different worldwide, with little data existing from Moldova region. This study aims to characterize IBD in North-East Romania from the mentioned points of view. All patients with Crohn's disease or ulcerative colitis (UC) admitted to a tertiary hospital during a three year interval were retrospectively analyzed in terms of epidemiological characteristics. All IBD patients hospitalized during 2010 were complexly analyzed by noting the type and form of disease, the biological picture, the clinical and endoscopic scores. UC represents 77% of all cases (one of the highest values of the recent studies). Age at onset is higher in the studied region, with the average over 40 years. All the imagistic (barium enema and computer tomography) and endoscopic (left colonoscopy -there was no possibility to reach the right colon) investigations could not established the diagnostic, but these investigations identified the intestinal tumor involving cecum and terminal ileon, with mostly benign characters and loco-regional enlarged limph nodes. The surgical exploration showed an inflammatory tumor of the terminal ileon and cecum with ileo-rectal fistula; extended enterectomy, right colectomy, excision of the ileo-rectal fistula and ileo-colic anastomosis were performed The pathological exam revealed Crohn`s disease but with three nests of malignant cells. The postoperative course was uneventful and the patient was discharged in 7 days.

Using a decision-analytic model, we evaluated the effectiveness and cost-effectiveness of surveil... more Using a decision-analytic model, we evaluated the effectiveness and cost-effectiveness of surveillance for hepatocellular carcinoma (HCC) in individuals with cirrhosis. Separate cohorts with cirrhosis due to alcoholic liver disease, hepatitis B and hepatitis C were simulated. Results were also combined to approximate a mixed aetiology population. Comparisons were made between a variety of surveillance algorithms using a-foetoprotein (AFP) assay and/or ultrasound at 6-and 12-monthly intervals. Parameter estimates were obtained from comprehensive literature reviews. Uncertainty was explored using one-way and probabilistic sensitivity analyses. In the mixed aetiology cohort, 6-monthly AFP þ ultrasound was predicted to be the most effective strategy. The model estimates that, compared with no surveillance, this strategy may triple the number of people with operable tumours at diagnosis and almost halve the number of people who die from HCC. The cheapest strategy employed triage with annual AFP (incremental cost-effectiveness ratio (ICER): d20 700 per quality-adjusted life-year (QALY) gained). At a willingness-to-pay threshold of d30 000 per QALY the most cost-effective strategy used triage with 6-monthly AFP (ICER: d27 600 per QALY gained). The addition of ultrasound to this strategy increased the ICER to d60 100 per QALY gained. Surveillance appears most cost-effective in individuals with hepatitis B-related cirrhosis, potentially due to younger age at diagnosis of cirrhosis. Our results suggest that, in a UK NHS context, surveillance of individuals with cirrhosis for HCC should be considered effective and cost-effective. The economic efficiency of different surveillance strategies is predicted to vary markedly according to cirrhosis aetiology.

Yellow discoloration of the skin may be associated with carotenemia, hypothyroidism, diabetes mellitus, hyperlipoproteinemia, liver disease, and renal disease, meaning that carotenemia is not synonym with yellow skin, but rather one of the cause. We presented an 8 year-old boy with a yellow discoloration of the palms and soles, observed by the mother 3 weeks prior to medical examination. The discoloration was uniformly distributed, rather symmetrically, no nails changes, just a mild xerosis palmaris on the right hypotenar area. In lab investigations only were: hypercholesterolemia. The final diagnosis was xanthodermia in context of hyperlypoproteinemia type II A.

The role of oxidative stress in inflammatory bowel diseases (IBD) has been extended lately from a... more The role of oxidative stress in inflammatory bowel diseases (IBD) has been extended lately from a simple consequence of inflammation to a potential etiological factor, but the data are still controversial. Active disease has been characterized before by an enhanced production of reactive oxygen species and the increased peroxidation of lipids, but patients in remission were generally not considered different from healthy people in terms of oxidative stress. We evaluated the antioxidant defense capacity and lipid peroxidation status in the serum of patients with active and non-active disease compared with healthy matched control subjects. The study included 20 patients with confirmed IBD in clinical and biological remission, 21 patients with active disease, and 18 controls. We determined the serum levels of two antioxidant enzymes, superoxide dismutase (SOD) and glutathione peroxidase (GPX), and a lipid peroxidation marker, malondialdehyde (MDA). Active disease patients had an increased activity of both SOD and GPX, as well as significant high values of MDA versus controls. Furthermore, patients being in remission had significantly lower values of antioxidant enzymes (SOD and GPX) and increased lipid peroxidation measured by MDA serum levels, as compared with healthy control subjects. Our study confirmed the presence of high oxidative stress in active IBD. More importantly, we have demonstrated a lower antioxidant capacity of patients in remission versus control group. This may represent a risk factor for the disease and can be an additional argument for the direct implication of oxidative stress in the pathogenesis of IBD.

The epidemiological characteristics, clinical, biological and endoscopic profiles are very different worldwide, with little data existing from Moldova region. This study aims to characterize IBD in North-East Romania from the mentioned points of view. All patients with Crohn's disease or ulcerative colitis (UC) admitted to a tertiary hospital during a three year interval were retrospectively analyzed in terms of epidemiological characteristics. All IBD patients hospitalized during 2010 were complexly analyzed by noting the type and form of disease, the biological picture, the clinical and endoscopic scores. UC represents 77% of all cases (one of the highest values of the recent studies). Age at onset is higher in the studied region, with the average over 40 years.
