The purpose of this qualitative descriptive study was to examine reasons for participation in cli... more The purpose of this qualitative descriptive study was to examine reasons for participation in clinical research among older adults with mobility limitation. A purposive sample of 20 men and 20 women aged 70 years or older was recruited. Data were collected by audiotaped telephone interviews using a semi-structured interview guide and transcribed verbatim. Participants expect privacy, professionalism by research staff, and respectful treatment. Benefits to protocol adherence include personal education, comparison of their health status with that of others, opportunity to maintain vitality, and altruism. Barriers to protocol adherence are apprehension, in particular a negative impact on their health care, randomization to the control group, and experimental drugs; and inconvenience. Factors promoting study completion are obligation, reciprocity, receipt of test results, health promotion, and socialization. Implications include meeting expectations, providing health education and study results to participants, reducing barriers to participation, and presenting opportunities for interaction with others.

Background: An associate professor in the Program in Nursing Science at UC Irvine, who has taught in a variety of nursing programs across the country, now is embracing the unique challenge of juggling the many roles of nursing faculty in the development of a new nursing program. Description of the Undertaking: In addition to creating the infrastructure of a new discipline on a university campus and forming new research collaborations, faculty must also implement a range of courses, from didactic to clinical experiences for students. Outcomes/ Conclusion: In these difficult economic times and with the shortage of nurse faculty, a founding faculty member will discuss how innovative one can be while trying to meet campus and discipline-specific mandates. The funded CTSCs across the country provide an excellent opportunity for nurse researchers to sho... more The funded CTSCs across the country provide an excellent opportunity for nurse researchers to showcase the strength of nursing science and to develop new partnerships in interdisciplinary research, consistent with the NIH roadmap. The CTSC Nursing Science Special Interest Group (SIG) was formed to promote collaboration across nursing schools with CTSCs, providing an opportunity for leaders in nursing science to network nationally and form coalitions that enhance the work of individual schools. The CTSC program offers new opportunities for nursing involvement and leadership in interdisciplinary research focusing on translation. Currently, several nurse scientists serve on the leadership teams of the CTSC, the Community Engagement Unit, the Biostatistics/Methods Unit, the Education and Training Unit, among others. With a focus on the early stages of developing new assessment tools, we present an example of how... more With a focus on the early stages of developing new assessment tools, we present an example of how researchers can apply qualitative data to the development of conceptual domains and specific items representing those domains for quantitative instruments. Specifically, our previous research examining mothers' perceptions and experiences of engaging in assisted exercise with their infants provided the foundation for the development of the Perceptions of Pediatric Activity Scale (PPAS). We describe the process we used to develop the PPAS as an exemplar for the process of incorporating qualitative data in instrument development. In addition, we address instrument development for diverse populations, and we provide examples illustrating how we extracted concepts using a concept-indicator model to construct the items in the PPAS. We conclude by noting that we are currently in the process of pilot-testing the PPAS to evaluate its utility and reliability.

Background: Each year this home visit program provides access to prenatal care, health screenings, health education, and referral services to more than 3,500 low-income, at-risk pregnant women in underserved communities. This program has established a coordinated model of home visitation services that is characterized by two essential elements: (1) highly trained paraprofessionals serve as home visitors and are closely supervised by RNs; and (2) pregnant women who need higher levels of service are referred promptly to public health nurses. Despite the tremendous contribution to the community over 20 years, the effectiveness of the home visit program on birth outcomes and infant development has lacked rigorous investigation. Sixty parents of children undergoing outpatient surgery were interviewed. Data were analyzed using grounded theory methodology. Parents experienced five subprocesses that comprised the overall process of caring for a child after routine surgery: (a) becoming informed; (b) preparing; (c) seeking reassurance; (d) communicating with one's child; and (e) making pain management decisions. Addressing cultural factors related to pain management in underserved families may instill greater confidence in managing pain.

To explore women's experiences of menopause after infertility. Grounded theory Nine menopausa... more To explore women's experiences of menopause after infertility. Grounded theory Nine menopausal women, ranging in age from 48 to 72, with previous infertility, at various stages of the menopausal transition, including postmenopause, were interviewed in-depth. Three women had conceived a child after infertility treatment, three had adopted children, and three did not have children. The interviews were audiotaped and transcribed verbatim for data analysis. Data were constantly compared against other data, including previous transcripts, to look for evidence of data saturation. A core concept was generated, referred to as "finally feeling normal." After feeling abnormal during infertility, these women viewed menopause as a normal transition. They were able to reconcile some aspects of their infertility and embrace a feeling of normalcy. Two opposing views regarding forensic nursing are found in contemporary nursing literature. Some authors resist the discourse of specialization in nursing, while the discursive practices of others (e.g., leaders in forensic psychiatric nursing) could reflect a desire for power. 3. Using a poststructuralist approach, the controversy that has accompanied the development of forensic nursing through discursive practices is analyzed.

A program of research on human responses to infertility revealed three salient concepts: sense of... more A program of research on human responses to infertility revealed three salient concepts: sense of self/identity, meaning, and quality of life. These three concepts provided the foundation for a theoretical basis to develop strategies for a counseling intervention with persons experiencing infertility. Advanced practice nurses can assume key roles in carrying out such strategies. The strategies include assisting patients with infertility to make decisions regarding treatment, focus on successful aspects of self, move on with life despite treatment outcome, and cultivate and maintain interpersonal relationships.

The initial transitional year of professional practice is thought to provide the critical foundat... more The initial transitional year of professional practice is thought to provide the critical foundation on which new professionals build their expertise. The purpose of this study was to describe the experiences of new nurse practitioner graduates during their first year of primary care practice. Thirty-five persons were interviewed alone or in focus groups at approximately 1, 6, and 12 months after graduation. Grounded theory methodology guided the data collection and analysis. A theoretical model was constructed that represents the transition to the primary care nurse practitioner role. This model consists of a process called From Limbo to Legitimacy, which encompasses four major categories: Laying the Foundation, Launching, Meeting the Challenge, and Broadening the Perspective. Each category contains a set of subcategories that detail the multiple aspects of the experience. This model highlights both the distress and the accomplishments of the initial year of advanced practice.

The purpose of this 2-phase project was to conduct a formative evaluation and to test the prelimi... more The purpose of this 2-phase project was to conduct a formative evaluation and to test the preliminary efficacy of a newly developed Web-based Tailored Intervention for Preparation of parents and children undergoing Surgery (WebTIPS). Phase 1 enrolled 13 children 2 to 7 years of age undergoing outpatient elective surgery and their parents for formative evaluation of WebTIPS. Parent participation focus groups are common in qualitative research and are a method of asking research participants about their perceptions and attitudes regarding a product or concept. In phase 2, children 2 to 7 years of age in 2 medical centers were assigned randomly to receive the WebTIPS program (n = 38) compared with children receiving the standard of care (n = 44). The primary outcome of phase II was child and parent preoperative anxiety. In phase 2, parents reported WebTIPS to be both helpful (P < 0.001) and easy to use (P < 0.001). In phase 2, children in the WebTIPS group (36.2 ± 14.1) were less anxious than children in the standard of care group (46.0 ± 19.0) at entrance to the operating room (P = 0.02; Cohen d = 0.59) and introduction of the anesthesia mask (43.5 ± 21.7 vs 57.0 ± 21.2, respectively, P = 0.01; Cohen d = 0.63). Parents in the WebTIPS group (32.1 ± 7.4) also experienced less anxiety compared with parents in the control group (36.8 ± 7.1) in the preoperative holding area (P = 0.004; Cohen d = 0.65). WebTIPS was well received by parents and children and led to reductions in preoperative anxiety.

This article presents results of a study using grounded-theory methodology to explore the meaning... more This article presents results of a study using grounded-theory methodology to explore the meaning of infertility to those persons experiencing it. Thirty-two persons were interviewed, including 15 married couples and two married women whose husbands were either unable or unwilling to participate in the study. Forty-five semistructured interviews were conducted, as each person was interviewed separately and 13 couples were interviewed conjointly. Through the ongoing process of data collection and analysis, a substantive grounded theory was generated, with a core concept being that as persons experience unwanted infertility they take on a central identity of themselves as infertile. The process of taking on and managing this identity is described.

Research in women&amp... more Research in women's health has regained momentum with the recent release of several reports from various institutes and organizations. The Office of Research on Women's Health (ORWH) of the National Institutes of Health and the Institute of Medicine (IOM) both reported on women's health research. Within a year, Congress enacted the Affordable Care Act, stipulating support for clinical preventive services for women, prompting further reports focused on clinical care for women. These two research-dominant reports (NIH ORWH and the IOM) are the subject of this manuscript. The purpose is to outline and critically analyze the reports from a grounded nursing perspective and to propose a complementary and expanded agenda for furthering research in women's health. A separate manuscript analyzes and makes recommendations based on additional reports about clinical services and policies that will benefit the health status of women.

These two research-dominant reports (NIH ORWH and the IOM) are the subject of this manuscript. The purpose is to outline and critically analyze the reports from a grounded nursing perspective and to propose a complementary and expanded agenda for furthering research in women's health. A separate manuscript analyzes and makes recommendations based on additional reports about clinical services and policies that will benefit the health status of women.
