he composition of peripheral nerves primarily consists of axons, which are elongated structures r... more he composition of peripheral nerves primarily consists of axons, which are elongated structures responsible for transmitting nerve impulses. These axons exhibit two distinct characteristics in terms of their myelination status. Some axons possess a myelin sheath, a protective covering that enhances the speed and efficiency of nerve signal conduction. Conversely, other axons lack this myelin sheath and are considered non-myelinated. Surrounding both myelinated and non-myelinated axons are specialized cells known as Schwann cells, which provide support and insulation. Additionally, the axons and Schwann cells are enveloped by a connective tissue layer called the endoneurium, which further contributes to the structural integrity of peripheral nerves. The organization of nerve fibers involves their arrangement in fascicles, which are encased by the perineurium. Additionally, the peripheral nerve sheath is formed by the epineurium encompassing the nerve trunk. Partial and complete tears can potentially lead to the formation of posttraumatic neuromas, which are characterized by a hyperplastic and disorganized proliferation of cells. These neuromas are considered as an adaptive response by the body in an attempt to regenerate the damaged nerves. These neoplasms are infrequent in occurrence and arise from Schwann cells. In the present scenarios, the preferred reconstructive modalities involve the utilization of regional pedicled flaps or free tissue transfers. The utilization of flaps originating from the temporalis muscle, fascia, or a combination of them, was initially documented by Yolovine in the year 1898. The options for reconstruction are limited. Reverse sural flap is relatively easy to perform and considered a good option for reconstruction. The complication rates are variable in studies. These defects commonly occur as a result of trauma, infection and tumor excision. Sural artery neurovascular island flap is a relatively thin, pliable and insensate flap with minimum donor-site morbidity and acceptable aesthetic outcome.

Deep endometriosis (DE) is an uncommon cause of bowel obstruction; preoperative diagnosis is a ch... more Deep endometriosis (DE) is an uncommon cause of bowel obstruction; preoperative diagnosis is a challenging task due to its rarity and pathological confirmation. Surgery is the appropriate treatment and complications are common. A 26-year-old Latin female was admitted to emergency department with 72 hours history of abdominal pain associated with inability to pass stool or gas, vomiting and nausea. Abdominal distention and pain without acute abdomen signs. Laboratory tests reported normal. Abdominal contrast-enhanced computed tomography showed distal small bowel obstruction. Patient underwent exploratory laparotomy with segmental resection bearing ileal strictures and Brook´s ileostomy was performed. Postoperative course of patient was uneventful and after pathology report treatment with dienogest was established. DE remains challenging entity to treat, medical treatment can reduce symptoms, but surgical resection is required. Bowel resection is reserved for mayor stenosis lesions. A...

The Graph Partitioning Problem (GPP) is a well-known NP-hard combinatorial problem that involves ... more The Graph Partitioning Problem (GPP) is a well-known NP-hard combinatorial problem that involves the finding of a partition of vertexes that minimizes the number of cut edges while fulfilling a set of constraints. This paper presents a newly designed optimizer for the GPP: the Memetic Algorithm with Hungarian Matching Based Crossover and Diversity Preservation (MAHMBCDP). MAHMBCDP is a population-based scheme that incorporates an explicit mechanism to control the diversity with the aim of making a proper use of resources when dealing with long-term executions. Among the novelties of our proposal, the design of a crossover operator that is based on the Hungarian Algorithm to calculate a maximum matching is particularly important. Experimental validation with a set of well-known instances of the graph partitioning archive shows the proper performance of our proposal. In fact, new best-known solutions could be attained in ten test cases.
