Objetivos: Presentación de caso clínico de paciente con trastorno bipolar e infección por VIH, en... more Objetivos: Presentación de caso clínico de paciente con trastorno bipolar e infección por VIH, en el que se instaura tratamiento con palmitato de paliperidona de administración trimestral (PDP3M). Revisión bibliográfica del uso de palmitato de paliperidona en pacientes VIH+ Material y métodos: Caso clínico: Varón de 58 años que hace seguimiento en consultas de psiquiatría desde 2009. Antecedentes: - Trastorno bipolar en seguimiento en consultas de psiquiatría, con cinco ingresos previos desde 2014 en fase maniaca o depresiva, con presencia de síntomas psicóticos. - VIH en estadio A3 diagnosticado en 1994. Buen estado viro-inmunológico. En tratamiento con Triumeq 1cp/día. - Cirrosis compensada VHC, G1a. - Uso de sustancias: Consumo activo de tabaco. Hazardous alcohol consumption is a common diagnosis among people living with HIV infection. The r... more Hazardous alcohol consumption is a common diagnosis among people living with HIV infection. The relationship between alcohol consumption and poor adherence to antiretroviral therapy has been highlighted in different studies, yet few of them performed a parallel analysis of other substance use. In Spain, alcohol consumption is frequently associated with other substance use, mainly cannabis and cocaine. The aim of this study is to assess the influence of hazardous alcohol consumption both combined with other substances (cocaine, heroin, methadone and/or cannabis) or alone on antiretroviral therapy adherence in our social environment. We performed an observational case-control study including 119 HIV+ individuals. We recruited 40 non-adherent patients, defined by less than 90% compliance according to hospital pharmacy refill data, and corroborated by the Simplified Medication Adherence Questionnaire (SMAQ) and referring professional's opinion. Control cases (n=79) were defined as those patients with similar characteristics but considered adherent according to the same parameters. Data collection took place between May 2013 and September 2015. Statistical analysis was performed using a binary logistic regression model. Our results indicate that alcohol consumption decreases adherence to antiretroviral therapy. The use of methadone represents a statistically significant increased risk of poor adherence. No significant differences were found between adherent and non-adherent groups regarding cocaine, heroin or cannabis use in this study. In summary, the detection of substance use and especially alcohol consumption in HIV+ patients can improve the effectiveness of antiretroviral therapy by identifying and treating at-risk individuals for a poor therapeutic adherence.

La repercusion del consumo de estas sustancias en el medio laboral es muy importante (enfermedades, accidentes laborales, absentismo, incapacidades laborales, disminucion de la productividad…). Se valora la necesidad de una politica en el medio laboral encaminada a prevenir o minimizar los riesgos laborales derivados del consumo de estas sustancias, con programas de prevencion y apoyo que aporten informacion basica de referencia y orienten al abordaje asistencial de los trabajadores afectados EnglishThe consumption of alcohol and other drugs has high prevalencia in the society in general, and in the population of workers especially, affecting on the occupational area. The repercussion of the consumption of these substances in the working enviroment is very important (diseases, occupational accidents, absenteeism, occupational disabilities, decrease of the productivity…). There should be included programs of prevention and support, which offer basic information and orientation to the medical aproach of the affected workers.

Psychiatric comorbidity between anxiety disorders and substance-related disorders is studied. Thi... more Psychiatric comorbidity between anxiety disorders and substance-related disorders is studied. This relationship is complex due to the overlapping of symptoms (during the abstinence of alcohol and opiates as well as in the intoxication of cocaine) and also for the inaccurate definition of several disorders. Epidemiologic studies show an increase of prevalence of anxiety disorders, mainly agoraphobia and social phobia, in patients who are alcohol dependents and in heroine and cocaine users. Such studies also point out an augmentation of consume of substances (drugs and alcohol), with a prevalence of 24% in patients with several anxiety disorders. The relationship between these disorders is analysed. It is also studied the influence of such a comorbidity in the evolution and outcome of treatment.
