This study examined specific family factors associated with alcohol use among racial minority adolescent daughters of HIV-positive mothers living in selected inner cities in the United States. A nonprobability sample of 176 mothers (37% HIV-positive and 63% HIV-negative) and their 14- to 18-year-old daughters completed self-administered surveys. Hierarchical multivariate logistic regressions were used to examine these mother-daughter data. Within the full sample, maternal report of importance of religion was negatively associated with alcohol use among daughters. Further analysis of samples separated by maternal HIV status revealed that daughter's age and depression rate and mother's alcohol use were only associated with higher odds of alcohol use among daughters in households with HIV-positive mothers. Background-Although adoption and utilization of sexually transmitted infection (STI) testing is a... more Background-Although adoption and utilization of sexually transmitted infection (STI) testing is a cost effective public health intervention, it is inconsistently offered or referred out for by outpatient substance abuse treatment (OSAT) programs where at-risk racial/ethnic and sexual minorities receive services. Methods-We explored the organizational adoption and client utilization of STI testing using a nationally representative sample of OSAT facilities in the U.S. in 2005 (N=566). Data missing at random was imputed and the resulting database was analyzed using multivariate Tobit and logistic regressions. Results-The analyses suggest that private non-profit facilities, which are the largest providers of OSAT treatment are less likely than public facilities to offer STI testing or to report adequate client utilization rates. Higher utilization was instead associated with professionally accredited facilities, and with facilities whose majority of clients were Latino/a, reported a history of treatment, stayed in treatment longer, or received case management. Conclusion-While OSAT facilities are poised to be primary intervention points for diagnosis and treatment of STIs, only a segment of these facilities provide this preventive practice or manage to refer clients out. As such, U.S. health care policy should ensure the adoption and comprehensive utilization, particularly among high risk clients, of this cost-effective prevention strategy in OSAT admission protocols.

Our study examined maternal factors associated with daughters' alcohol use among inner-city racial minority adolescents of HIV-infected and HIV-uninfected mothers. A nonprobability sample of 176 mothers (37% with HIV) and their adolescent daughters completed self-administered surveys. Between-and within-group analyses were conducted using hierarchical multivariate logistic regressions. Findings showed that in the full sample, difficulty talking with daughters about alcohol was positively associated with alcohol use among daughters, whereas maternal report of importance of religion was negatively associated with alcohol use among daughters. Within-group analysis of participants by maternal HIV status revealed that maternal beliefs that drinking alcohol in front of their daughters was normative were associated with higher odds of adolescent alcohol use in households with HIV-infected mothers. These preliminary findings highlight the potential increased vulnerability of racial minority adolescent girls living in households with HIV-infected mothers.

Background: To examine risk factors for use of hospital services among racial and ethnic minority... more Background: To examine risk factors for use of hospital services among racial and ethnic minority clients in publicly funded substance abuse treatment in Los Angeles County, California. We explored cross-sectional annual data (2006 to 2009) from the Los Angeles County Participant Reporting System for adult participants (n = 73,251) who received services from treatment programs (n = 231). Methods: This retrospective analysis of county admission data relied on hierarchical linear negative binomial regression models to explore number of hospital visits, accounting for clients nested in programs. Client data were collected during personal interviews at admission. Findings: Our findings support previous work that noted increased use of emergency rooms among individuals suffering from mental health-and substance use-related issues and extend the knowledge base by highlighting other important features such as treatment need, i.e., residential compared to outpatient treatment. Conclusions: These findings have implications for health care policy in terms of the need to increase prevention services and reduce costly hospitalization for a population at significant risk of co-occurring mental and physical disorders.

This work examined qualities associated with communication about alcohol among 176 mothers and the influence of this communication on daughters’ alcohol use. Path analyses by maternal HIV status indicated significant differences. Relationship satisfaction was associated with self-efficacy for both HIV-positive (β = 0.545, p < .001) and HIV-negative (β = 0.557, p < .001) mothers. Maternal self-efficacy was associated with communication for both HIV-positive (β = 0.364, p < .01) and HIV-negative (β = 0.310, p < .05) mothers; maternal attitudes toward alcohol use were associated with communication among HIV-negative mothers (β = 0.20, p < .05).
