I will him to outrage. He is blind to it, or merely reticent; perhaps, again, as a guest of Harriman’s and here, of the cannery’s management, he is reluctant to criticize” (79). But she is never unfair. In choosing this once popular but now relatively neglected writer for her guide, she gives us a fascinating dual perspective on Alaska, a kind of stereopticon or 3-D view, allowing us to see both the big picture and the light that is right there in front of our eyes.
Spontaneous bacterial peritonitis (SBP) is associated with abdominal inflammation and increased m... more Spontaneous bacterial peritonitis (SBP) is associated with abdominal inflammation and increased morbidity in patients with end-stage liver disease. However, SBP has generally not been associated with worse outcomes after liver transplantation (LT) in single center studies. In this analysis, we analyze the impact of a history of SBP on LT outcomes using the SRTR database. METHODS: All patients, 18 years or older, who received an initial LT alone between March 2002 and December 2016 were included leaving 75,576 patients. 2262 were missing SBP data leaving 73,314. Of these 4665 had SBP and 68,649 did not. The two groups were compared for patient and donor characteristics. Groups were compared for length of stay (LOS) and survival using Kaplan-Meier analyses with log rank test. Multivariate analysis was done using Cox regression analysis. RESULTS: Patients with a history of SBP were more ill than those without. They had a higher MELD at LT (26.1 + 10.0 vs 20.7 + 10.3: P\u3c0.001), a higher creatinine (1.69 + 1.23 vs 1.34 + 1.04: P\u3c0.001) and were more likely to be on dialysis (15% vs 7%: P\u3c0.001). They were more likely to be hospitalized (28% vs 16%: P\u3c0.001) or in the ICU (16% vs 11%: P\u3c0.001). They were more likely to have portal vein thrombosis (13% vs 9%: P\u3c0.001) and less likely to have HCC (11% vs 28%). They had an equal incidence of hepatitis C (44% vs 43%). Patients with SBP had longer LOS in days (17.2 + 19.9 vs 14.8 + 18.9: P\u3c0.001). Patients with SBP had worse patient survival at 3 month (94.4% vs 95.1%: P=0.026), 1 year (87.6% vs 90.0%: P\u3c0.001) and 3 year (80.0% vs 81.8%: P=0.001). SBP was not associated with worse graft survival. On univariate analysis, SBP was associated with increased LOS (HR 1.20: 95% CI 1.16-1.23: P\u3c0.001) and diminished patient survival (HR 1.09: 95% CI 1.03-1.016: P=0.002). However on multivariate analysis, SBP was no longer associated with either patient death or increased LOS. CONCLUSIONS: Patients with a history of SBP prior to transplant are considerably more ill than patients without an SBP history and have worse patient survival and longer length of stay. However, when controlling for other medical comorbidities, SBP alone is not associated with worse patient outcomes

This research analyzes Semana Santa's ritual phenomenon as the main indicator of Larantuka society's exclusive appreciation and perception background toward The Virgin Mary and try to seek, Larantuka people as Lamaholot ethnic have a cultural value called lewotana covering all the values they embrace and implement, including the value in their religious system. In order understanding from Larantuka society's point of view as the culprit. This research uses Schultz and Husserl phenomenology theory. Before the arrival of the Catholic culture to understand by finding the background about the phenomenon of change of cultural values of the lewotana developing into the cultural value of Catholic, researcher used the theory of phenomenology. In addition, Geertz's interpretive and ethnoscience approach as a supporting theory. The research method that will be used is qualitative method. This is because qualitative methods are more representative and relevant for observations and detailed interviews.The final outcome of this study answers two problems, the first one revealing how the procession of Semana santa ritual which implicitly contain the value of feminism that appears on activity and ritual attributes. The value is represented through the figure of The Virgin Mary. It also describes the history of Semana santa development associated with the acculturation process of Lamaholot culture with Catholic culture. The second is the cultural dominance of The Virgin Mary figure which motivated by the characteristic and cultural value of Lamaholot (pre-Catholic) which is feminist. The phenomenon of the cultural system religion of Larantuka society alteration from Lera Wulan Tanah Ekan and Lewotana into Catholic culture only occurs at the level of social behavior and artifacts, the idea of the value of the sacred women figure who is considered as a representation of the universe still exist in the cognitive nature of Larantuka society with the figure of The Virgin Mary as a substitution process media of feminist value along with symbol as "place" meaning to the admiration and respect to woman figure, especially mother.

San Diego County has seen a rise in the number of syphilis cases over the past decade. Syphilis i... more San Diego County has seen a rise in the number of syphilis cases over the past decade. Syphilis is relatively simple to treat, but requires close serological monitoring to ensure the patient received adequate treatment. Repeat serological testing should be completed at 3 months for patients diagnosed with primary and secondary syphilis. The patient population at a public health clinic in San Diego County has a poor rate of compliance with follow-up appointments (7%). Patient reminders are known to improve patient adherence with follow-up visits. The goal of this project was to incorporate patient reminder cards and patient education to improve follow-up rates among those diagnosed with primary and secondary syphilis.

Clostridium chauvoei causes blackleg in cattle. The disease has been reported worldwide, and alth... more Clostridium chauvoei causes blackleg in cattle. The disease has been reported worldwide, and although it can be prevented by vaccination, sporadic cases and occasional outbreaks still occur. We describe a case of blackleg in a 2-y-old, pregnant Gyr cow with in utero transmission to the fetus. The cow had characteristic gross and microscopic lesions of blackleg including widespread necrohemorrhagic and emphysematous skeletal and myocardial myositis, and fibrinous pericarditis. Her uterus contained a near-term, markedly emphysematous fetus with skeletal muscle and myocardial lesions similar to those seen in the dam. Histopathology of dam and fetal tissues revealed numerous gram-positive bacilli, many of them with sub-terminal spores, in multiple tissues. These bacilli were identified as C. chauvoei by immunohistochemistry. Anaerobic culture and fluorescent antibody tests performed on skeletal muscle from both the dam and fetus were positive for C. chauvoei, confirming a diagnosis of b...

Identification of fungal organisms often poses a problem for pathologists because the histomorpho... more Identification of fungal organisms often poses a problem for pathologists because the histomorphology of some fungal organisms is not specific, fresh tissues may not be available, and isolation and identification in culture may take a long time. The purpose of this study was to validate the use of panfungal polymerase chain reaction (PCR) to identify fungal organisms from formalin-fixed paraffin-embedded (FFPE) tissues. Formalin-fixed paraffin-embedded curls were tested from 128 blocks containing canine, feline, equine, and bovine tissues with cutaneous, nasal, pulmonary, and systemic fungal infections, identified by the presence of fungi in histologic sections. Quantitative scoring of histologic sections identified rare (11.9%), occasional (17.5%), moderate (17.5%), or abundant (53.1%) fungal organisms. To delineate the influence of hemodynamic force on cell adhesion processes, model in vitro fluidi... more To delineate the influence of hemodynamic force on cell adhesion processes, model in vitro fluidic assays that mimic physiological conditions are commonly employed. Herein, we offer a framework for solution of the three-dimensional Navier-Stokes equations using computational fluid dynamics (CFD) to estimate the forces resulting from fluid flow near a plane acting on a sphere that is either stationary or in free flow, and we compare these results to a widely used theoretical model that assumes Stokes flow with a constant shear rate. We find that while the full three-dimensional solutions using a parabolic velocity profile in CFD simulations yield similar translational velocities to those predicted by the theoretical method, the CFD approach results in approximately 50% larger rotational velocities over the wall shear stress range of 0.1-5.0 dynes cm(-2). Most small ruminant farms in tropical climates are plagued by Haemonchus contortus, a hematophago... more Most small ruminant farms in tropical climates are plagued by Haemonchus contortus, a hematophagous, abomasal parasite. Heavy burdens of this parasite can cause anemia, hypoproteinemia, weight loss, and mortality in susceptible animals. Haemonchus contortus is becoming a major health concern in New World camelids as well, namely llamas (Llama glama) and alpacas (Vicugna pacos), yet little research has been conducted regarding its prevalence or pathology in these species. Herein, we present a retrospective review of llamas and alpacas that were admitted to The University of Georgia Veterinary Teaching Hospital and Athens Diagnostic Laboratory between the years 2002 and 2013. Antemortem fecal egg count (FEC) estimates performed on 30 alpacas were negatively correlated with hematocrit, hemoglobin, and red blood cell count. Selectins facilitate the recruitment of circulating cells from the bloodstream by mediating rolli... more Selectins facilitate the recruitment of circulating cells from the bloodstream by mediating rolling adhesion, which initiates the cell-cell signaling that directs extravasation into surrounding tissues. To measure the relative efficiency of cell adhesion in shear flow for in vitro drug screening, we designed and implemented a microfluidic-based analytical cell adhesion chromatography system. The juxtaposition of instantaneous rolling velocities with elution times revealed that human metastatic cancer cells but not human leukocytes have a reduced capacity to sustain rolling adhesion with P-selectin. We define a new parameter, termed adhesion persistence, which is conceptually similar to migration persistence in the context of chemotaxis but instead describes the capacity of cells to resist the influence of shear flow and sustain rolling interactions with an adhesive substrate that may modulate the probability of extravasation.
