Objetivo: Construir e validar uma cartilha educativa sobre o uso seguro de medicamentos para gest... more Objetivo: Construir e validar uma cartilha educativa sobre o uso seguro de medicamentos para gestantes. Métodos: Estudo de abordagem metodológica, descritivo, transversal e quali-quantitativo, realizado duas etapas: construção da cartilha e validação por especialistas especialista na área. Resultados: A cartilha foi construída apontando respostas para algumas dúvidas sobre o uso de medicamentos, de forma dinâmica e interativa. O processo de validação foi realizado por 7 juízes especialistas, dentre eles 3 (43%) enfermeiros, 2 (28,5%) médicos e, 2 (28,5%) farmacêuticos, sendo o perfil profissional: 2 (28,5%) doutores, um doutor em obstetrícia e um em cuidados clínicos; 1 (14,2%) mestre e 4 (57,3%) especialistas. Obteve-se 147 respostas distribuídas em totalmente adequado (67,34%, n=99), adequado (30%, n=44) e (2,72%, n=4) em parcialmente adequado. To contribute to the formation of knowledge in Brazil about the impact of the clinical pharmacist... more To contribute to the formation of knowledge in Brazil about the impact of the clinical pharmacist's role in the care of inpatient post-transplant by analyzing the results of the pharmaceutical interventions performed. A descriptive study was conducted from January to July/2014. Data were collected from the records of the Clinical Pharmacy Service (medication reconciliation, prescription analysis and pharmacotherapeutic follow-up), including all patients with drug-related problems and documented pharmaceutical interventions. Epidemiological variables and related to the clinical activities of the pharmacist were described. A total of 131 patients had drug-related problems, especially, during “Pharmacoterapeutic follow-up”. The role of clinical pharmacists in the patient care in hospitals has evolved over time, with emp... more The role of clinical pharmacists in the patient care in hospitals has evolved over time, with emphasis on pharmaceutical care. In this scenario, the resident pharmacist has substantially contributed to liver and kidney transplant patients and to the benefits from pharmaceutical interventions that act as an effective tool to improve the quality of the hospital care. Purpose: To report pharmaceutical interventions (PI) performed by the resident pharmacist in a kidney and liver transplantation unit at a University Hospital, identifying drug related problems (DRP) along the admission period and discharge of transplanted patients. Methods: A retrospective study analyzing pharmaceutical interventions made by the resident pharmacist in kidney and liver transplanted patients from May to September, 2010. RESULTS: During the studied period, 219 patients were followed, and among them, it was detected 47 DPR (21.50%). From those patients, 51.06% (n= 24) were female. Drugs involved in the identified DPR were classified according to the Anatomical Therapeutic Chemical (ATC), with a higher prevalence of the cardiovascular and nervous systems, 43.47% and 17.39% respectively. It was identified 81 DPR, 98.75% of them were prevented through the PI, with 98.76% acceptance. Doctors were the most requested professional (97.50%), followed by nurses (2.50%). When ranked by the II Granada Consensus, needing DPR occurred in 68.80% (n= 55), safety: 28.80% (n=23), and effectiveness, 2.50% (n= 2). It was found no statistical significance in the intervention acceptance among the different DPR (p=0.873). Conclusions: Results suggest that the participation of a resident pharmacist in the medical care is useful to prevent DRP, and it is well accepted by other team members, contributing for a safe and necessary treatment for liver and kidney transplanted patients.

Objetivo: Avaliar a cultura de segurança dos pacientes na perspectiva dos médicos que atuam em um... more Objetivo: Avaliar a cultura de segurança dos pacientes na perspectiva dos médicos que atuam em uma maternidade. Método: Trata-se de um estudo transversal e quantitativo, com abordagem descritiva, realizado entre os meses de março e maio de 2016 com 124 médicos atuantes em uma maternidade pública de ensino do município de Fortaleza, Ceará, Brasil. As informações foram obtidas por meio do survey Hospital Survey on Patient Safety Culture (HSOPSC), Criado pela Agency for Healthcare Research and Quality (AHRQ), que avalia as percepções de profissionais em relação à segurança do paciente em 12 dimensões. Resultados: entre as doze dimensões avaliadas, uma apresentou taxa de resposta positiva acima de 75% (área fortalecida): aprendizado organizacional e melhoria contínua (80,4%), e abaixo de 50% (áreas com potencial de melhoria): as dimensões passagens de plantão/turno, transferências internas (45,96%) e frequência de eventos notificados (39,51%). This study aimed to determine the prevalence of off-label or unlicensed drugs prescribed for use in a pediatric unit of a Brazilian public hospital, analyzing the therapeutic indication. This was a descriptive, retrospective study, which analyzed the medical records of patients admitted in the 1st trimester of 2012 in the pediatric unit. It was determined the profile of patients, the pharmacotherapeutic profile according ATC (anatomical-therapeutic-chemical classification index) and evaluated the use of off-label or unlicensed drugs using the information available on the National Agency for Sanitary Surveillance (ANVISA) database and the MICROMEDEX 2.0 database. Objetivo: identificar as interacoes medicamentosas induzidas pelo aprazamento e os erros no prepa... more Objetivo: identificar as interacoes medicamentosas induzidas pelo aprazamento e os erros no preparo de antibacterianos administrados. Metodos: estudo observacional e transversal, realizado em clinicas medicas de hospital da rede sentinela. A coleta de dados ocorreu por meio da observacao das prescricoes e do preparo de 265 doses de antibacterianos com a utilizacao de um check-list. Resultados: prevaleceu a administracao da Piperaciclina e Tazobactan, com 51 doses. O aprazamento de horario dos antibacterianos com outro medicamento ocorreu prevalentemente no periodo matutino, destacando-se interacoes com anticoagulantes injetaveis e entre antimicrobianos de classes diferentes, e se associou com a nao disponibilizacao no setor de orientacoes impressas acerca dos cuidados com a administracao do medicamento (p=0,003). Aim: To analyze prescriptions in brazilian public maternity as information related about patient,... more Aim: To analyze prescriptions in brazilian public maternity as information related about patient, prescriber and drugs. Methods: Prescriptions of April 2013 were analyzed to obtain a number of completed prescriptions containing a minimum of 1000 items containing medications. The observance of prescribers about legal and institutional procedures was checked. An individual analysis of all prescription drugs for the occurrence of errors has also accomplished. Results: 194 prescriptions were analyzed with 1002 items containing medications. The predominant form of writing was handwritten (n=142; 73,19%). Absence or illegibility of the patients name was identified in 31.96% (n=62) of the prescriptions and lack of signature and stamp of the prescribing physician in 13,40% (n=26). Errors related to medications were observed in 108 prescriptions (55,67%), highlighting the error in dose. An exploratory research using UpToDate® and Micromedex® databases of drug interactions, incompatibilities and adverse reactions related to the chemotherapeutic agents. The drugs analyzed were included in the induction protocol for acute myeloid leukemia and in the Pharmacotherapeutic Guide from a reference University Hospital in onco-hematology. Assim, os servicos de saude e os profissionais devem estar adeptos a assumir a postura de educadores que compartilham saberes e praticas assegurando qualidade de vida e que as mulheres possam viver sua autoconfianca no processo de sua saude. Pouco se sabe sobre o acesso, o conhecimento, a habilidade e a atitude dos farmaceuticos no uso d... more Pouco se sabe sobre o acesso, o conhecimento, a habilidade e a atitude dos farmaceuticos no uso das tecnologias da informacao e comunicacao no Brasil. Baseado em survey, com aplicacao de questionario eletronico validado, elaborou-se diagnostico, fundamentando a proposicao de modelo teorico-esquematico para software de suporte a pratica clinica (PC) dos farmaceuticos. Foram obtidas 348 respostas validas, as quais identificaram que os farmaceuticos clinicos possuem acesso a computador no trabalho (89,4%; n=311); com internet (83,9%; n=292) e sentem-se competentes para realizar buscas de artigos em base de dados (93,4%; n=325), e para usar termos MeSH (97,4%; n=347); porem, revelaram a inexistencia de educacao formal para tal. Apesar do acesso aos buscadores e as bases de dados cientificos, a maioria utiliza o Google® como principal buscador de informacoes (78,7%; n=274) e reconhece a necessidade de expandir seus conhecimentos na busca de informacoes de suporte para a pratica clinica. ...

Background: This study aimed to describe the steps to construct and validate a questionnaire to i... more Background: This study aimed to describe the steps to construct and validate a questionnaire to identify the knowledge, skills and attitudes of hospital pharmacists in the use of information technology and electronic tools to support clinical practice. Methods: The electronic questionnaire with 26 items, 8 dimensions and 74 variables was constructed based on a bibliographical review and validated by a experts panel (n=10), who evaluated in two cycles the relevance and clarity of each question by using scale (relevance: 1-irrelevant, 2-somewhat relevant, 3-relevant and 4-very relevant and Clarity: 1-not clear, 2-somewhat clear, 3-clear and 4-very clear). The Content Validity Index and the degree of agreement were calculated. The questionnaire was tested with hospital pharmacists, adjusted and used in definitive data collection in Brazil. Results: The 1 st cycle Content Validity Index-CVI was 0.9635±0.0566 and 1 in the 2 nd cycle. The questions with CVI<0.8 were adjusted. The degree of agreement was 82.5%(1 st cycle) and 99.61%(2 nd cycle). The validated questionnaire was submitted to the receptivity test with a rate of 4.67 responses per day. After the changes suggested the questionnaire was applied to Brazilian hospital pharmacists (n=1373) obtaining a response rate of 25.3% (n=348). Conclusion: The validated questionnaire presents acceptable content validity psychometric measures and may reveal the hospital pharmacists' gaps in knowledge, skills and attitudes in the use of information technology and electronic tools.

An exploratory study was conducted to present the approach of Brazilian hospital pharmacists to r... more An exploratory study was conducted to present the approach of Brazilian hospital pharmacists to registering, documenting, archiving and disseminating clinical practice. The data were collected using an electronic questionnaire (n=348). In fact, 97.41% of pharmacists record their clinical practice, out of which 64.01% (n=217) do in electronic form, mainly in private hospitals (p<0.000), in the central, southern and southeastern regions (p=0.040), and by professionals with 1-5 years of experience (p=0.001). The main software used is non-specific to clinical practice: an electronic spreadsheet (47.93%; n=104) and text editor (13.37%; n=29). The archiving of records is performed by 87.61% (n=297) of professionals, where 80.13% (n=238) do so in physical form; however, 77.31% (n=184) of these files are kept for less than the standard recommended time. Documentation in medical records is carried out by 55.17% (n=192) of pharmacists, increasing among those with 1-5 years of clinical practice (p=0.001), and dissemination is performed by 74.71% (n=260) of hospital pharmacists, with a lower frequency in public hospitals (p=0.012) and among professionals with fewer hours dedicated exclusively to clinical pharmacy (p=0.012). These results can undergird the revision of competency-based training programs of Brazilian clinical pharmacists to remain pharmacists as a valuable health team member.

Methods: Prescriptions of April 2013 were analyzed to obtain a number of completed prescriptions containing a minimum of 1000 items containing medications. The observance of prescribers about legal and institutional procedures was checked. An individual analysis of all prescription drugs for the occurrence of errors has also accomplished. Results: 194 prescriptions were analyzed with 1002 items containing medications. The predominant form of writing was handwritten (n=142; 73,19%). Absence or illegibility of the patients name was identified in 31.96% (n=62) of the prescriptions and lack of signature and stamp of the prescribing physician in 13,40% (n=26). Errors related to medications were observed in 108 prescriptions (55,67%), highlighting the error in dose. Baseado em survey, com aplicacao de questionario eletronico validado, elaborou-se diagnostico, fundamentando a proposicao de modelo teorico-esquematico para software de suporte a pratica clinica (PC) dos farmaceuticos. Foram obtidas 348 respostas validas, as quais identificaram que os farmaceuticos clinicos possuem acesso a computador no trabalho (89,4%; n=311); com internet (83,9%; n=292) e sentem-se competentes para realizar buscas de artigos em base de dados (93,4%; n=325), e para usar termos MeSH (97,4%; n=347); porem, revelaram a inexistencia de educacao formal para tal. Background: This study aimed to describe the steps to construct and validate a questionnaire to i... more Background: This study aimed to describe the steps to construct and validate a questionnaire to identify the knowledge, skills and attitudes of hospital pharmacists in the use of information technology and electronic tools to support clinical practice. Methods: The electronic questionnaire with 26 items, 8 dimensions and 74 variables was constructed based on a bibliographical review and validated by a experts panel (n=10), who evaluated in two cycles the relevance and clarity of each question by using scale (relevance: 1-irrelevant, 2-somewhat relevant, 3-relevant and 4-very relevant and Clarity: 1-not clear, 2-somewhat clear, 3-clear and 4-very clear). The Content Validity Index and the degree of agreement were calculated. The questionnaire was tested with hospital pharmacists, adjusted and used in definitive data collection in Brazil. Results: The 1 st cycle Content Validity Index-CVI was 0.9635±0.0566 and 1 in the 2 nd cycle. The questions with CVI<0.8 were adjusted. The degree of agreement was 82.5%(1 st cycle) and 99.61%(2 nd cycle). The validated questionnaire was submitted to the receptivity test with a rate of 4.67 responses per day. After the changes suggested the questionnaire was applied to Brazilian hospital pharmacists (n=1373) obtaining a response rate of 25.3% (n=348). Conclusion: The validated questionnaire presents acceptable content validity psychometric measures and may reveal the hospital pharmacists' gaps in knowledge, skills and attitudes in the use of information technology and electronic tools.

An exploratory study was conducted to present the approach of Brazilian hospital pharmacists to r... more An exploratory study was conducted to present the approach of Brazilian hospital pharmacists to registering, documenting, archiving and disseminating clinical practice. The data were collected using an electronic questionnaire (n=348). In fact, 97.41% of pharmacists record their clinical practice, out of which 64.01% (n=217) do in electronic form, mainly in private hospitals (p<0.000), in the central, southern and southeastern regions (p=0.040), and by professionals with 1-5 years of experience (p=0.001). The main software used is non-specific to clinical practice: an electronic spreadsheet (47.93%; n=104) and text editor (13.37%; n=29). The archiving of records is performed by 87.61% (n=297) of professionals, where 80.13% (n=238) do so in physical form; however, 77.31% (n=184) of these files are kept for less than the standard recommended time. Documentation in medical records is carried out by 55.17% (n=192) of pharmacists, increasing among those with 1-5 years of clinical practice (p=0.001), and dissemination is performed by 74.71% (n=260) of hospital pharmacists, with a lower frequency in public hospitals (p=0.012) and among professionals with fewer hours dedicated exclusively to clinical pharmacy (p=0.012). These results can undergird the revision of competency-based training programs of Brazilian clinical pharmacists to remain pharmacists as a valuable health team member.
