To study the recovery periods of blood flow parameters in muscles after anaerobic exercise, insta... more To study the recovery periods of blood flow parameters in muscles after anaerobic exercise, instantaneous and mean blood flow velocity curves were recorded in the femoral artery in 22 sportsmen at rest and during the first 4 min of recovery after exercise (Ruffier-Dickson test). A flat ultrasonic probe connected to a Doppler system (Flow-Tester) was fixed on the skin at the level of the common femoral artery. From Doppler recordings, we calculated periods of recovery (return to baseline) of femoral blood flow velocity (FBFV RP), heart rate (HR RP) and femoral stroke distance (FSD RP). Also, Ruffier-Dickson index (RDI), VO(2)max in mL/kg(-1)/min(-1) and number of training hours were determined. We observed a high correlation between FBFV RP and VO(2)max (p = 0. 0002), and significant correlation between FSD RP and VO(2)max (p = 0.0238) and RDI (p = 0.0451). In conclusion, there is a excellent correlation between blood flow velocity recovery period in femoral artery after moderate exercise and VO(2)max in high-level sportsmen. The method of testing is simple and based on conventional Doppler technique. It can be used for the follow-up of training levels in sportsmen.

La prévalence et la prise en charge de l'allergie alimentaire en milieu scolaire à Limoges ont ét... more La prévalence et la prise en charge de l'allergie alimentaire en milieu scolaire à Limoges ont été étudiées par un questionnaire. Sept cent quarante-huit réponses sur 1086 ont pu être analysées. Il en ressort une prévalence de 5,35 % soit 40 déclarations. Les principaux aliments en cause appartiennent à la famille des rosacées, à celle des légumineuses (arachide) ou aux aliments du groupe latex. Une polysensibilisation est surtout observée chez les enfants de plus de 10 ans. Quatre chocs anaphylactiques ont été rapportés, mais 1 seul bénéficie d'un projet d'accueil individualisé. Malgré d'évidents progrès ces dernières années, la prise en charge de ces affections reste encore insuffisante. © 2002 É ditions scientifiques et médicales Elsevier SAS. Tous droits réservés.

ABSTRACT Angiotensin II (AII) has a protective effect against acute cerebral ischemia, mediated t... more ABSTRACT Angiotensin II (AII) has a protective effect against acute cerebral ischemia, mediated through non-AT1 receptors and independent of its pressive action (Fernandez L et al, J Cardiovasc Pharmacol 1994; 24: 937-40). The pentapeptide angiotensin IV is no longer regarded as an inactive breakdown product of AII, since its specific receptor (AT4) has been described. AT4 receptors are expressed in the brain and the vessels, are vasodilators and are involved in cerebral blood flow regulation (Wright JW, et al. Brain Res 1996; 717: 1-11). To examine the contribution of AT4 in the adaptation to acute cerebral ischemia, unilateral carotid ligation was performed in penthobarbital anesthetized 80 grams male gerbils pretreated 30 min prior with lisinopril (3mg/kg), candesartan (1mg/kg) or saline intraperitoneously (ip). In each group of animals, post-treatment with AIV or saline as control was administrated ip as a bolus (0.25 mg/kg) followed by a six hours continuous infusion (0.1 mg/kg/h). Mortality was examined at 24, 48 and 72 hours and Kaplan-Meier survival curves were analysed by a logrank test.Survival rate (%) at day 3 were:Survival was markedly reduced in L pretreated animals compared to S and C groups in the absence of AIV rescue, confirming that AII contributes to protective adaptation to cerebral ischemia through non-AT1 receptors. Rescue treatment with AIV significantly improved the survival in the L group, and to a less and non significant extent in the S and C groups. Global analysis of the mortality irrespective to the pretreatment further demonstrated the protective effect of rescue treatment with AIV. In saline pretreated animals, AIV lowered systolic BP measured at the tail (68.9 ±2.9 vs 85.4 ± 2.4 mmHg, in saline post-treated, p = .0002) similarly to C and L in gerbils post-treated with saline (resp 68.6 ± 3.1 and 69.4 ± 3.9 mmHg) but did not further lower blood pressure in C and L pretreated animals (67.1 ± 4 and 70.2 ± 4.6 mmHg). Conclusion: angiotensin IV reverses the deleterious effect of ACE inhibition on acute ischemic stroke mortality in the gerbil. These results suggest that AIV is involved in the BP independent, non AT1-mediated renin-angiotensin system protective effect against cerebral ischemia. (See Table)

The aim of this study was to compare the utility of gamma camera using a coincidence detection sy... more The aim of this study was to compare the utility of gamma camera using a coincidence detection system imaging (CDET) with 18-fluorodeoxyglucose to conventional imaging techniques in the detection of recurrence of non-small cell lung cancer. Sixty-nine patients were randomized into two groups for follow-up after surgery from October 2000 to December 2002. Each patient was evaluated every 6 months by conventional technique imaging in group A (n=33) or CDET imaging in group B (n=36) over two years. The direct costs of each procedure were evaluated. The major endpoint was the number of recurrences or new tumours detected. The two groups were similar. A total of 25 recurrences was detected (9 in group A and 16 in group B). Overall survival was similar in the two groups. CDET imaging was more expensive. CDET imaging provides earlier detection of recurrence, but does not modify survival outcome. Further studies are necessary to demonstrate the impact, if any, of 18-FDG imaging.

The objective was to compare cost of home base chemotherapy (HBC) versus outpatient infusions, for non small cell lung cancer (NSCLC). 10 patients were selected in each category. D8 of their cycles was performed at home compared to outpatient infusion. Costs were based on national fees with cytostatic drugs as supplement. The real cost was also assessed through a specific questionnaire. 30 D8 infusions were carried out at hospital and 24 D8 infusions at home. Average cost by cycle was 2,829.51 euros [2 560.74-3 147.02] for hospital infusion, 2,372.50 euros [1 962.75-2 792.88] for HBC (-16.15%). Difference was -457.01 euros by cycle [-919.74; 26.82]. Real costs by injection for BHC was 484.42 euros [424.18; 540.32] versus a fee of 699.89 euros [643.64; 750.23]. There were no difference in terms of adverse events. HBC for NSCLC is feasible. This study aimed to investigate knowledge, attitudes, and behavior among individuals working to a... more This study aimed to investigate knowledge, attitudes, and behavior among individuals working to achieve, or promote, the employment and retention in employment of people with epilepsy (PWE) in Benin. We interviewed 300 subjects using a self-administered questionnaire. Knowledge: 93% had heard of epilepsy; 8.8% had at least one person with epilepsy (PWE) in their family; and 67.3% had witnessed a person having epileptic seizures. Attitudes and behavior: 95.3% and 95%, respectively, expressed their views that PWE should be able to enter mainstream education and that PWE should marry; 76.4% considered PWE equal to themselves; and 28.6% reported they would not hire a PWE. We found some associations between demographic characteristics and knowledge, attitudes, and behavior among respondents. A few unhelpful aspects of knowledge, attitudes, and behavior were identified. Our study showed a fairly high level of accurate knowledge of epilepsy, but some misperceptions and unhelpful attitudes and practices toward PWE in Benin were noted. This study's findings can be used to help plan appropriate strategies based on correcting misperceptions concerning inclusion and retention in employment of PWE in Benin.

Children with epilepsy and their parents face many social and psychological difficulties that rem... more Children with epilepsy and their parents face many social and psychological difficulties that remain insufficiently studied in sub-Saharan Africa. The aim here was to assess the quality of life of children with epilepsy and their parents. A community-based cross-sectional survey was conducted in two urban areas and four rural areas of Gabon. Children were screened through key informants, medical sources, and a door-to-door survey. They were clinically selected based on their medical history and a clinical exam conducted by the investigating physician. Electroencephalography had not been carried out because of a lack of material and financial resources. The quality of life of children and their parents was assessed by a structured interview of parents using a questionnaire. Of 317 suspected cases on screening, 83 children with epilepsy were identified. Their mean age was 11.9±4.4years. Twelve percent of the children had neurosensory abnormalities on clinical exam. Sixty-three percent of them attended school; factors associated with schooling were higher score on the sociability subscale, specialized medical advice, and antiepileptic drug treatment. Sociability difficulties, anxiety, cognitive impairment, and behavioral disorders were suspected in 39.8%, 45.8%, 49.4%, and 42.2% of children, respectively. A total of 48.2% of parents expressed a poor quality of life related to their children's illness. A higher score on the cognition subscale, urban residence, specialized medical advice, and a stable income in the household were predictive of poor parental quality of life. Epilepsy influences many aspects of a child's life and the life of the child's parents. Care should incorporate a cognitive assessment of the child and emphasize information for patients and their relatives.

This survey aimed to describe the current use of laryngeal mask (LM) and to define the difference... more This survey aimed to describe the current use of laryngeal mask (LM) and to define the difference of use according to a group defined like experts. A self-questionnaire available on Internet was sent by e-mail to French anesthesiologists. Results were compared by a Chi(2) test between anesthesiologists according to their expertise defined by a >750 LM use. Nine hundred and fifty-three anesthesiologists answered to the survey. Only 19 reported never using LM and 37% of responders were defined as expert. More than half of the MAR used the ideal weight to set the size of the MLA to use. Pressure leak was sought by less than one-third of the MAR. In case of leakage, a mobilization of the ML or the patient's head was performed first. In second intention, nearly a quarter of MAR changed technique for the benefit of oral intubation. Only 50% of responders used LM as a rescue technique in case of difficult intubation and ventilation. Experts had less insertion failure and laryngospasm. Complications and medico-legal suites are few. Variability of practice may be explained by low-grade recommendations in the literature. LM insertion in case of difficult intubation/ventilation to provide adequate oxygenation and should be formally reminded to clinicians.

This study aimed to investigate knowledge, attitudes, and behavior among individuals working to a... more This study aimed to investigate knowledge, attitudes, and behavior among individuals working to achieve, or promote, the employment and retention in employment of people with epilepsy (PWE) in Benin. We interviewed 300 subjects using a self-administered questionnaire. Knowledge: 93% had heard of epilepsy; 8.8% had at least one person with epilepsy (PWE) in their family; and 67.3% had witnessed a person having epileptic seizures. Attitudes and behavior: 95.3% and 95%, respectively, expressed their views that PWE should be able to enter mainstream education and that PWE should marry; 76.4% considered PWE equal to themselves; and 28.6% reported they would not hire a PWE. We found some associations between demographic characteristics and knowledge, attitudes, and behavior among respondents. A few unhelpful aspects of knowledge, attitudes, and behavior were identified. Our study showed a fairly high level of accurate knowledge of epilepsy, but some misperceptions and unhelpful attitudes and practices toward PWE in Benin were noted. This study's findings can be used to help plan appropriate strategies based on correcting misperceptions concerning inclusion and retention in employment of PWE in Benin.

Children with epilepsy and their parents face many social and psychological difficulties that rem... more Children with epilepsy and their parents face many social and psychological difficulties that remain insufficiently studied in sub-Saharan Africa. The aim here was to assess the quality of life of children with epilepsy and their parents. A community-based cross-sectional survey was conducted in two urban areas and four rural areas of Gabon. Children were screened through key informants, medical sources, and a door-to-door survey. They were clinically selected based on their medical history and a clinical exam conducted by the investigating physician. Electroencephalography had not been carried out because of a lack of material and financial resources. The quality of life of children and their parents was assessed by a structured interview of parents using a questionnaire. Of 317 suspected cases on screening, 83 children with epilepsy were identified. Their mean age was 11.9±4.4years. Twelve percent of the children had neurosensory abnormalities on clinical exam. Sixty-three percent of them attended school; factors associated with schooling were higher score on the sociability subscale, specialized medical advice, and antiepileptic drug treatment. Sociability difficulties, anxiety, cognitive impairment, and behavioral disorders were suspected in 39.8%, 45.8%, 49.4%, and 42.2% of children, respectively. A total of 48.2% of parents expressed a poor quality of life related to their children's illness. A higher score on the cognition subscale, urban residence, specialized medical advice, and a stable income in the household were predictive of poor parental quality of life. Epilepsy influences many aspects of a child's life and the life of the child's parents. Care should incorporate a cognitive assessment of the child and emphasize information for patients and their relatives.

This survey aimed to describe the current use of laryngeal mask (LM) and to define the difference... more This survey aimed to describe the current use of laryngeal mask (LM) and to define the difference of use according to a group defined like experts. A self-questionnaire available on Internet was sent by e-mail to French anesthesiologists. Results were compared by a Chi(2) test between anesthesiologists according to their expertise defined by a >750 LM use. Nine hundred and fifty-three anesthesiologists answered to the survey. Only 19 reported never using LM and 37% of responders were defined as expert. More than half of the MAR used the ideal weight to set the size of the MLA to use. Pressure leak was sought by less than one-third of the MAR. In case of leakage, a mobilization of the ML or the patient's head was performed first. In second intention, nearly a quarter of MAR changed technique for the benefit of oral intubation. Only 50% of responders used LM as a rescue technique in case of difficult intubation and ventilation. Experts had less insertion failure and laryngospasm. Complications and medico-legal suites are few. Variability of practice may be explained by low-grade recommendations in the literature. LM insertion in case of difficult intubation/ventilation to provide adequate oxygenation and should be formally reminded to clinicians.
