The study aimed to evaluate the urinary gene expression of NDRG-2 (N-Myc downstream-regulated gene2) and MCM8 (the mini chromosome maintenance proteins) genes and their importance as novel urinary biomarkers for bladder cancer. In addition, to assess their diagnostic value in comparison with voided urine cytology is the focus of this work. Methods: the study included twenty healthy controls and fifty patients with bladder cancer. Quantitative real-time polymerase chain reaction (qRT-PCR) and voided urine cytology (VUC) were performed to demonstrate the NDRG2 and MCM-8 gene expression levels in the urine of healthy controls and bladder cancer patients. Results: There was a statistically significant decrease in NDRG-2 gene expression in bladder cancer group (4.38±0.66) compared to the control group (8.29±1.67). Gene expression of MCM-8 showed a statistically significant increase in bladder cancer group (5.57±0.79) in comparison to control group (4.55±1.39) with a significant negative correlation (ρ= -0.77) between NDRG-2 expression levels and tumor grade in cancer group (p<0.001), and a positive significant correlation (ρ=0.453) between MCM-8 expression levels and tumor grade in cancer group (p=0.001). NDRG-2 had the highest ability to predict bladder carcinoma (AUC of 1.0). In addition, the most precise differentiation between non–muscle-invasive bladder cancer (NMIBC) and muscle-invasive bladder cancer (MIBC) with AUC of 0.814. Conclusion: Expression of NDRG-2 and MCM-8 may be novel potential noninvasive biomarkers for diagnosis and prognosis of bladder cancer and a good tool for differentiation between NMIBC and MIBC with NDRG-2 is the most precise for diagnosis and differentiation over MCM-8, VUC and combined use of NDRG-2 and MCM-8.

This study determines the prevalence and clinical presentation of detrusor underactivity (DU) and... more This study determines the prevalence and clinical presentation of detrusor underactivity (DU) and its urodynamic characteristics in adult patients with lower urinary tract symptoms (LUTS). Patients and Methods: This retrospective study has reviewed the symptoms and urodynamic study (UDS) findings of 283 patients with LUTS. Chi-square analysis was used to present the prevalence of UDS characteristics in both sexes. Results: Out of records of 206 patients included in this study, fifty-one (24.76%) patients were diagnosed with DU based on bladder contractility index. Storage lower urinary tract symptoms were the most prevalent characteristic presentation in both sexes as compared to the difficulty in voiding, recurrent urine retention, and urinary incontinence. Bladder outlet, sphincter EMG findings, and degree of DU were significantly correlated with gender. Conclusion: DU is a prevalent and sophisticated bladder pathology rather than a simple one. It requires more attention from the urologists, and evaluations, including UDS, to differentiate it from other pathologies.

Object: To determine the prevalence of lower urinary tract symptoms (LUTS) and their severity pop... more Object: To determine the prevalence of lower urinary tract symptoms (LUTS) and their severity population in Jordan. Methods: This cross-sectional survey was conducted using a paper-based survey between August and September in 2019. The study was carried out in the health care centers or hospitals in three different regions of Jordan: North (Irbid and Jarash), Middle (Amman, Madaba, Salt, and Zarqa), and South (Karak and Aqaba). Results: To estimate the prevalence of LUTS, two definitions were used, including the first definition (presence of any LUTS regardless of the degree of its severity) and the second definition (presence of any LUTS that occurs half the time or more). According to the first definition, 1038 (89.9%) reported LUTS (male: 47.3%, female: 52.7%), while 763 (66.1%) reported LUTS according to the second definition (male: 45.6%, female: 54.4%). According to the International Prostate Symptom Score characterization, 73.94% had nocturia and 62.9% reported daytime increased frequency. Conclusion: LUTS is highly prevalent among the Jordanian population, and more than half of them have nocturia or daytime increased frequency as most frequently reported symptoms

The questionnaire also contains a graded response for urgency. The current study is mainly concerned with the development and validation of the OABSS questionnaire for Jordanian patients. The process of translating the English OABSS questionnaire into the Arabic language involved forward and backward translations. Afterward, a prospective study was conducted to validate the Arabic version of the OABSS questionnaire by examining 235 patients from the outpatient clinics of Karak Governorate Teaching Hospital. Methods This retrospective cohort study was conducted from July 2014 till December 2015. Thirty-two boys (aged 4–12 years) with Lower Urinary Tract Symptoms were examined for the presence of urethral meatus stenosis. The collected data were statistically analysed using Univariate and Multivariate analyses. Results A significant improvement of urinary stream, urgency incontinence, urgency, and frequency were found after using an objective assessment tool with p-value ≤ 0.001. The assessment of surgical outcomes, following uncomplicated meatoplasty with objective noninvasive uroflowmetry, has been reported in the pediatric population. Conclusion It highlighted the importance of urethral meatal stenosis diagnosis in boys with lower urinary tract symptoms. The novel contribution was made by assessing surgical outcomes following meatoplasty. However, further research is needed with larger sample cohorts.

Objective: To assess the impact of double layer dartos flap on urethro-cutaneous fistula rate red... more Objective: To assess the impact of double layer dartos flap on urethro-cutaneous fistula rate reduction and to compare it with single layer dartos flap in Tubularized Incised Plate Urethroplasty (TIPU) for hypospadias in children. Methods: Between January 2008 and month 2011 retrospective charts review for 120 patients who had hypospadias repair by the author. 26 patients were excluded from the study because they were not suitable for TIPU procedure. Therefore, our study sample involved 94 children. Results: Mean age at time of surgery was 16 months (10-96 months), with a mean follow up of 18.2 months (10-36 months). All patients had hypospadias repair by utilizing TIPU. Patients were divided into two groups: Group one involved 50 patients had double layer dartos flap. Group two involved 44 patients had single-layer dartos flap. All patients had stent insertion at the time of repair. 6/44 patients [13.6%] developed fistula in group two and 2/50 patients [4%] in group one had fistula. Conclusions: Dartos coverage appears to have significant effect in reducing urethro-cutaneous fistula in TIPU repair. With double layer dartos coverage further reduction can be achieved.

Objective: To review our initial experience with prescrotal orchipexy, to illustrate this surgica... more Objective: To review our initial experience with prescrotal orchipexy, to illustrate this surgical approach, and to compare it with the standard two incision inguinal approach. Material and Methods: This is a single institution retrospective comparative review of 135 patients (pts) who underwent orchiopexy for UDT over 4 years. Pts undergoing the standard inguinal orchiopexy were compared to those undergoing the emerging single-incision prescrotal approach in regards to operative time, complications and cosmesis. Results: The study was completed on 96 pts who met the inclusion criteria. Group A (72 pts, 75%) underwent the standard two-incision inguinal orchiopexy while Group B (24 pts, 25%) underwent the single-incision prescrotal orchiopexy. Average age at surgery was 3.2 years, with a mean follow up of 13 months. Average operative time was 56 minutes for Group A and 32 minutes for Group B, by using SPSS significant P value difference (p < 0.05) was calculated. All patients from the two groups had uneventful postoperative period with one pt in Group A who had a scrotal wound infection. None of the pts in Group B need conversion to the inguinal approach. At last follow-up, all patients in both groups had testicles in the scrotum with no atrophy. While the cosmetic appearance in Group A was acceptable, cosmesis in Group B was superior. Conclusion: Prescrotal orchipexy is a safe alternative to the standard inguinal orchiopexy for the palpable UDT. Our study demonstrates similar outcome, shorter operative time and superior cosmesis.

Objective: To assess the impact of double layer dartos flap on urethro-cutaneous fistula rate red... more Objective: To assess the impact of double layer dartos flap on urethro-cutaneous fistula rate reduction and to compare it with single layer dartos flap in Tubularized Incised Plate Urethroplasty (TIPU) for hypospadias in children. Methods: Between January 2008 and month 2011 retrospective charts review for 120 patients who had hypospadias repair by the author. 26 patients were excluded from the study because they were not suitable for TIPU procedure. Therefore, our study sample involved 94 children. Results: Mean age at time of surgery was 16 months (10-96 months), with a mean follow up of 18.2 months (10-36 months). All patients had hypospadias repair by utilizing TIPU. Patients were divided into two groups: Group one involved 50 patients had double layer dartos flap. Group two involved 44 patients had single-layer dartos flap. All patients had stent insertion at the time of repair. 6/44 patients [13.6%] developed fistula in group two and 2/50 patients [4%] in group one had fistula. Conclusions: Dartos coverage appears to have significant effect in reducing urethro-cutaneous fistula in TIPU repair. With double layer dartos coverage further reduction can be achieved.

Objective: To review our initial experience with prescrotal orchipexy, to illustrate this surgica... more Objective: To review our initial experience with prescrotal orchipexy, to illustrate this surgical approach, and to compare it with the standard two incision inguinal approach. Material and Methods: This is a single institution retrospective comparative review of 135 patients (pts) who underwent orchiopexy for UDT over 4 years. Pts undergoing the standard inguinal orchiopexy were compared to those undergoing the emerging single-incision prescrotal approach in regards to operative time, complications and cosmesis. Results: The study was completed on 96 pts who met the inclusion criteria. Group A (72 pts, 75%) underwent the standard two-incision inguinal orchiopexy while Group B (24 pts, 25%) underwent the single-incision prescrotal orchiopexy. Average age at surgery was 3.2 years, with a mean follow up of 13 months. Average operative time was 56 minutes for Group A and 32 minutes for Group B, by using SPSS significant P value difference (p < 0.05) was calculated. All patients from the two groups had uneventful postoperative period with one pt in Group A who had a scrotal wound infection. None of the pts in Group B need conversion to the inguinal approach. At last follow-up, all patients in both groups had testicles in the scrotum with no atrophy. While the cosmetic appearance in Group A was acceptable, cosmesis in Group B was superior. Conclusion: Prescrotal orchipexy is a safe alternative to the standard inguinal orchiopexy for the palpable UDT. Our study demonstrates similar outcome, shorter operative time and superior cosmesis.
