Coronaviruses (CoV) are RNA respiratory viruses that present with a wide range of symptomatology ... more Coronaviruses (CoV) are RNA respiratory viruses that present with a wide range of symptomatology which range from common cold to severe diseases such as Middle East Respiratory Syndrome (MERS-CoV) and Severe Acute Respiratory Syndrome (SARS-CoV). In December 2019 a new strain was discovered in China named Severe Acute Respiratory Syndrome Corona Virus 2 (SARS-CoV-2), named “COVID-19” by World Health Organization (WHO). Objective: To study the prevalence and clinical presentation of COVID-19 among children attended pediatrics emergency department. Result: from 1st March to 30th of June 2020. During the study period, a total of 223 patients with age between 16 days and 12 years were diagnosed as positive COVID-19. Male were 52%. Fever was found in 39.9%, cough in 14.3%, and diarrhea in 3.6%. Conclusion: our study gives a clue to the clinical presentation of COVID-19 in pediatric populations.

Background: Discharged against medical advice (DAMA) is defined as any instance when a patient wa... more Background: Discharged against medical advice (DAMA) is defined as any instance when a patient wants to leave the hospital against the managing physician's decision. This study aimed to identify factors that influence patients to decide to be DAMA. Methods: A descriptive, cross-sectional study. The study was conducted in the emergency department (ED) of King Fahad Medical City (KFMC)-Saudi Arabia-Riyadh city. A questionnaire in both Arabic and English was distributed to all participants to fill in either English or Arabic. Results: Between 1 March and 30 April 2021, 510 responses were collected. Most of the study participants (31.4%) were over the age of 54. Our findings showed that 12.5% of our participants had taken discharge against medical advice in the past. Results Regarding Factors That Influence Patients to Decide on DAMA Showed: Regarding Inappropriate behavior and disrespect of the physician or staff to the patient and his relatives, 262 (51.4%) participants, 85 (16.7%) participants, and 163 (32%) participants agreed, neutral, and disagreed, respectively. Regarding the Lack of physicians' and nurses' attention to the patient and his relatives (emotionally), our result showed that 278 (54.5%) participants, 95 (18.6%) participants, and 137 (26.9%) participants agreed, neutral, and disagree, respectively. Regarding failure to inform the patient or his relatives of his condition, it showed that 257 (50.4%) participants, 95 (18.6%) participants, and 158 (31%) participants agreed, neutral, and disagreed, respectively. Regarding feeling better from DAMA, our result showed 226 (44.3%) participants, 119 (23.3%) participants, and 165 (32.4%) participants agreed, neutral, and disagreed, respectively. Vigilance must always be practiced when examining these children to avoid missing any hidden serious injuries that may result in devastating complications. We describe the acute and definitive management of a child treated 17 h after transorbital craniocerebral penetration by a sharp object. Despite the rarity of these types of injury, a good outcome can be achieved if they are promptly recognized and managed.

I N 1901, Osler proclaimed that the "most widespread and fatal of all acute diseases, pneumonia, ... more I N 1901, Osler proclaimed that the "most widespread and fatal of all acute diseases, pneumonia, is now the Captain of the Men of Death." 1 Today, pneumonia, along with influenza, continues to be a leading cause of death in the United States, 2 despite advances in antimicrobial therapy, vaccines, and critical care. Pneumonia also has a significant effect on morbidity, resulting in more than 10 million outpatient visits, 600,000 hospitalizations, and 64 million days of restricted activity each year. 3 Consequently, nurse practitioners (NPs) in a variety of settings encounter pneumonia on a regular basis, and in order to provide the most optimal care they must review and evaluate the available evidence. Given the growing amount of research on community-acquired pneumonia (CAP), this can be a daunting task for any clinician. Consequently, the purpose of this article is to summarize and evaluate the latest guideline developed by the Infectious Diseases Society of America and the American Thoracic Society (IDSA/ATS) for diagnosis and management of CAP in adults. 4 OVERVIEW OF CAP Pneumonia is an infection of the lower respiratory tract caused by bacteria, viruses, fungi, protozoa, or parasites. 5 Respiratory pathogens reach the lungs through inhalation of microorganisms, aspiration of oropharyngeal secretions, and hematogenous spread from other bodily sites of infection. 5 CAP refers to pneumonia that is acquired outside health care organizations, including hospitals, nursing homes, and other long-term care facilities.Although antibiotic resistance has increasingly become a recognized problem in pneumonia, older age and underlying disease, not antibiotic resistance, are more important factors in mortality. 1 Despite advances in diagnostic testing, the causative agent in as many as 50% of clients with CAP is not identified even when extensive testing has been performed. 6 Unfortunately, no diagnostic test exists that can identify all potential pathogens. Streptococcus pneumoniae, Mycoplasma pneumoniae, and Chlamydia pneumoniae have been the most commonly identified organisms, although their incidence depends on the type of diagnostic testing and criteria used (Table 1). Legionella spp and variable infections are also common. Infections by gram-negative bacilli may be increasing in outpatient settings because of the complexity of the clients treated outside of health care settings.
