Seventeen patients stung by Tityus serrulatus scorpion were classi®ed as mild (pain at the site o... more Seventeen patients stung by Tityus serrulatus scorpion were classi®ed as mild (pain at the site of the sting, n = 6), moderate (local pain and one of the following manifestations: vomiting, psychomotor agitation, prostration, sweating, tachypnea, tachycardia and mild arterial hypertension, n = 10) and severe cases (equal moderate cases plus cardiac failure, pulmonary edema and shock, n = 1). Venous blood was sampled for biochemical and hematological analysis and for IL-1a, IL-6, IL-10, TNF-a, IFN-g and GM-CSF ELISAs at the time of hospital admission, 6 h (moderate and severe cases), and 12, 18, 36 and 72 h (severe case) later. Ten age-matched healthy volunteers were used as control. Increased serum levels of IL-1a was noticed in all patients, high levels of IL-6, IFN-g and GM-CSF were observed only in a patient with severe envenomation. Our data suggest that a systemic in¯ammatory response-like syndrome is triggered during severe envenomation caused by T. serrulatus sting and that release of cytokines may be involved in this response. #
The scientific and clinical value of a measure of complexity is potentially enormous because comp... more The scientific and clinical value of a measure of complexity is potentially enormous because complexity appears to be lost in the presence of illness. The changes introduced by asthma in respiratory mechanics and control of breathing may result in modifications in the airflow pattern. These changes may be interesting clinically, since they can reduce the ability of the patient to perform daily life activities. In this paper, we examine the effect of elevated airway obstruction on the complexity of the airflow pattern of asthmatic patients using the approximate entropy method (ApEnQ). This study involved 5 healthy and asthmatics with normal spirometric exam (5), mild (5), moderate (6) and severe (5) airway obstruction. A significant (p〈0.002) reduction in ApEnQ was observed in asthmatic patients. This reduction was significantly correlated with spirometric indices of airway obstruction (R=0.60; p〈0.001). These results are in close agreement with pathophysiological fundamentals, and suggest that in asthmatic patients the airflow pattern becomes less complex, which may reduce the adaptability of the respiratory system to perform exercise associated with daily life activities. Furthermore, our findings also suggest that ApEnQ may help the clinical evaluation of asthmatic patients.

Generic active pharmaceutical ingredients (APIs) have been commonly used in Brazil, since 1999, b... more Generic active pharmaceutical ingredients (APIs) have been commonly used in Brazil, since 1999, but most of them are synthetic and small molecules. Recently, a large number of generic enoxaparins were introduced into the market raising concerns related to product-to-product interchangeability, efficiency, and drug counterfeiting. These drugs are produced from biological sources and their production involves complex procedures and purification processes. The present article evaluates several generic enoxaparins, structurally and pharmacologically, and compares them with the branded products. Structural analysis showed that the generic products are, indeed, quite similar to the branded products, however, this similarity cannot be extended to their pharmacological activities. The results showed that generic products must go through extensive structural, pharmacological, and clinical evaluation in order to assess their quality, efficacy and, ultimately, avoid drug counterfeiting before clinical use. Variation was also observed between the branded products, showing that such drugs must be at constant surveillance.

O trabalho teve por objetivo estudar o enraizamento in vitro do porta-enxerto 'Marubakaido' (Malu... more O trabalho teve por objetivo estudar o enraizamento in vitro do porta-enxerto 'Marubakaido' (Malus prunifolia Borkh), visando a diminuir os efeitos prejudiciais causados pelo excesso de auxina. Foram utilizadas brotações do porta-enxerto 'Marubakaido' com três pares foliares (2,5 a 3 cm), oriundas do processo de multiplicação in vitro. O meio utilizado foi o MS/2 acrescido de l00mg/l de mio-inositol, 30 g/l de sacarose e 6g/l de ágar. O pH foi ajustado para 5,9, antes da colocação do ágar. Avaliou-se três períodos (2, 4 e 6 dias) de permanência dos explantes no escuro e três concentrações (0,0; 1,0 e 3,0mM) de ácido indolbutírico (AIB). Após a permanência dos explantes nos devidos períodos de escuro, estes foram transferidos para meio sem reguladores e acondiconados em sala de crescimento, sob condições de 16h de fotoperíodo, 25 ±2° C e 2000 lux. A ausência e a menor concentração de AIB reduziram a percentagem de enraizamento de acordo com os aumentos nos períodos de escuro, mostrando um efeito linear negativo. Para o nível mais elevado da auxina não se observou diferenças no enraizamento para todos os períodos de escuro, apresentando índices superiores a 80%. Na presença da auxina houve um incremento no número de raízes para os aumentos do período de escuro. Houve formação de raízes bem desenvolvidas com formação de calo somente na cicatrização do corte. As plântulas apresentaram um incremento gradual da área foliar com o aumento da concentração de AIB e do período de permanência no escuro. Maiores percentagens de sobrevivência foram obtidas com aumentos na concentração de AIB e períodos de escuro. Melhor qualidade das plântulas foi observada com um período de seis dias de escuro e 3,0mM de AIB.

The stability and half-life time of anthocyanin extracts from mangosteen peel were studied under ... more The stability and half-life time of anthocyanin extracts from mangosteen peel were studied under controlled oxygen supply, undergoing the influence of light source (fluorescent, incandescent, infrared and ultraviolet) and storage temperature (5, 28, 40 and 50 °C). The kinetic parameters for anthocyanin degradation, under different illumination conditions fit the first-order reaction model, and the exposition under fluorescent light resulted in a higher half-life time (597 h), followed by incandescent (306 h), ultraviolet (177 h) and infrared (100 h). The kinetic behaviour for the storage in different temperatures also fit the first order, and at 5 °C the highest half-life time (4006 h) was found, followed by 28 °C (370 h), 40 °C (125 h) and 50 °C (93 h). The activation energy was 14.7 Kcal.mol−1, and Q10 values showed that at 5 °C the anthocyanin extracts were more sensitive to storage temperature changes compared to the other tested temperatures.

Among endocrine disorders, Cushing's syndrome (CS) is certainly one of the most challenging to en... more Among endocrine disorders, Cushing's syndrome (CS) is certainly one of the most challenging to endocrinologists due to the difficulties that often appear during investigation. The diagnosis of CS involves two steps: confirmation of hypercortisolism and determination of its etiology. Biochemical confirmation of the hypercortisolaemic state must be established before any attempt at differential diagnosis. Failure to do so will result in misdiagnosis, inappropriate treatment, and poor management. It should also be kept in mind that hypercortisolism may occur in some patients with depression, alcoholism, anorexia nervosa, generalized resistance to glucocorticoids, and in late pregnancy. Moreover, exogenous or iatrogenic hypercortisolism should always be excluded. The three most useful tests to confirm hypercortisolism are the measurement of 24-h urinary free cortisol levels, low-dose dexamethasone-suppression tests, and determination of midnight serum cortisol or late-night salivary cortisol. However, none of these tests is perfect, each one has different sensitivities and specificities, and several are usually needed to provide a better diagnostic accuracy. The greatest challenge in the investigation of CS involves the differentiation between Cushing's disease and ectopic ACTH syndrome. This task requires the measurement of plasma ACTH levels, non-invasive dynamic tests (high-dose dexamethasone suppression test and stimulation tests with CRH or desmopressin), and imaging studies. None of these tests had 100% specificity and their use in combination is usually necessary. Bilateral inferior petrosal sinus sampling is mainly indicated when non-invasive tests do not allow a diagnostic definition.
