Due to torrential haemorrhage, placenta percreta is one of the main reasons for emergency peripartum hysterectomies and mostly results in subsequent maternal mortalities and morbidities. In such cases, caesarean hysterectomy leaving the placenta in situ without any separation of placenta is preferred. We observed the outcome of such patients managed with planned caesarean hysterectomy. This cross-sectional observational study was conducted at the Department of Obstetrics, Faridpur Medical College Hospital, Bangladesh. A total of 14 patients who underwent planned caesarean hysterectomy due to placenta percreta were studied. We performed delivery of the baby through upper segment transverse incision without placental separation. A uterine malformation is the result of an abnormal development of the Mullerian duct(s) during e... more A uterine malformation is the result of an abnormal development of the Mullerian duct(s) during embryogenesis. The arcuate uterus is a type of congenital uterine malformation where the uterine fundus displays a small midline indentation towards the uterine cavity. A 25 years old woman with history of two abortions was diagnosed as a case of arcuate uterus during her first caesarean section. With proper antenatal care and counseling she has been able to give birth to 2nd baby successfully. No correctable surgery was essential for her. The literature regarding the diagnosis, management, and reproductive outcomes for arcuate uterus is limited and conflicting. A woman with an arcuate uterus can carry a baby to full term pregnancy. However, this condition is associated with a higher risk for miscarriage and premature births. Arcuate uterus is usually managed similarly to septate uterus, and only selected patients who fulfill poor reproductive performance criteria are recommended for surgical correction.

Over the last two decades many biochemical factors have been selected to detect their predictive value in PE. More recent studies have claimed that increased urinary levels of collagen IV can predict early diabetic nephropathy. This study was done to explore whether elevated urinary concentration of collagen IV in early pregnancy is potentially a more sensitive predictor of PE than microalbumin (MA). Among 119 study subjects, the sensitivity of ACR for PE was 80%, specificity 49.54%, PPV 12.69% and NPV 96.42%. In case of collagen IV, the sensitivity was found to be 70%, specificity 52.5%, PPV 11.5% and NPV 94.8%. Since detection of urinary collagen IV is a relatively costly test compared to MA rather it does not give any added advantage, its use may not be advocated to predict PE in cases of pregnancy. Visual inspection of cervix with acetic acid (VIA) is adopted as national cervical cancer screening programme in Bangladesh. All positive VIA cases are evaluated by colposcopy. Objective: This population-based study was conducted in Faridpur Medical College Hospital for 6 years (2013 2018) to evaluate the prevalence of CIN among the VIA positive cases and performance of the clinic for colposcopic management of CIN. Materials and Methods: A total of 2102 VIA positive cases were evaluated by colposcopy during the study period. A hospital based cross sectional study was carried out to analyze prevalence of risk factors for ... more A hospital based cross sectional study was carried out to analyze prevalence of risk factors for stroke in hospitalized patient in a medical college hospital. 100 patients were chosen using purposive sampling technique. Highest incidence of stroke was between the 6th and 7th decade. Patients came from both urban (54%) and rural (46%) areas and most of them belong to the low-income group (47%). In occupational category; service holder (28%) and retired person (21%) were the highest groups. Most of the study subjects were literate (63%). CT scan study revealed that the incidence of ischaemic stroke was 61% and haemorrhagic stroke 39%. Analysis indicated hypertension as major risk factor for stroke (63%) and major portion of the patients (42.85%) were on irregular or no treatment. The purpose of the study was to measure the urinary collagens IV (U-coll IV) and microalbumin (MA) level in early pregnancy and to explore the role of excretion U-coll IV in prediction of preeclampsia (PE). A total number of 119 pregnant women at 10-14 weeks of pregnancy were selected on the basis of availability. MA by immunoturbidimetry assay and U-coll IV by enzyme immunoassay method were measured in these subjects and they were followed up to the term for the possible development of PE. MA was defined by albumincreatinine ratio (ACR) above 32mg/g and high U-coll IV was defined by values above the cutoff point 2.74 ng/ml determined by the median value of the (controls). The data were analyzed by grouping the subjects who developed PE in later stages of pregnancy (the PE group) and those who did not develop PE in later stages (Control group). From the total subjects, 10 developed PE which shows a prevalence of about 8.4%. The PE group had higher value of ACR as compared to Control [ACR, mg/g, median (range), 42.3]. The sensitivity of ACR in predicting PE was 80%, specificity 49.54%, PPV 12.69% and NPV 96.42% respectively. The sensitivity of high U-coll IV in predicting PE was 70%, specificity 50.5%, PPV 11.5% and NPV 94.8%. It may be concluded that early pregnancy levels of MA and U-coll IV can be used as predictors of PE with high negative predictive value; and U-coll IV has no added advantage over MA in this respect.

The aim of the study was to compare the efficacy of Letrozole over Clomiphene citrate (CC) for ovulation induction in patients with PCOS. It was a prospective randomized trial in a private practice setting. The study period was 3 years, which includes 240 sub fertile patients with PCOS. Patients were divided into two groups. Group-A: 120 patients got Letrozole (2.5 mg) tab, 2 tabs once daily from D2-D6 for 3 cycles. Group-B: 120 patients took tab. Clomiphene citrate (50mg), 2 tabs once daily from D2-D6 for 3 cycles. Trans-vaginal ultrasound was done on D12-D13 to document number of follicles, measurement of dominant follicle and endometrial thickness. Ovulation and pregnancy rate was measured. There is significant association between placenta praevia and previous c-section pregnancy. It is a leading cause of APH and it affects approximately 0.5% of all labour. This cross sectional type of observational descriptive study based on non- probability technique was done in Faridpur Medical College Hospital from July 2015 - June 2016. A total of 150 pregnant women were studied. Pregnant women with H/O previous c-section once or more beyond 28 weeks gestation were included. Those who were primigravidae or pregnant women without previous c-section were excluded. The data were subjected to chi-square test with SPSS software version 20.0. Nonsyndromic aplasia cutis congenita is a condition in which babies are born with localized areas... more Nonsyndromic aplasia cutis congenita is a condition in which babies are born with localized areas of missing skin. These areas resemble ulcers or open wounds, although they are sometimes already healed at birth. Lesions most commonly occur on the top of the head (skull vertex). In some cases, the bone and other tissues under the skin defect are also underdeveloped. If the baby is born by cesarean section, then the patient's attendants recognize the wound as an injury which might be performed by the surgeon at the time of cesarean section. We are reporting this type of a case whose father attempted to take legal procedure against the surgeon, but finally he understood that this was a congenital disease of his baby.

Thousands of pregnant women are exposed to arsenic (As), which has been shown to lead to a higher... more Thousands of pregnant women are exposed to arsenic (As), which has been shown to lead to a higher risk of maternal and infant morbidity. We hypothesized that As-induced modifications to the humoral immune system may be partly responsible, and examined the relationship between As and immunoglobulin G (IgG). Pregnant women were recruited in As-contaminated rural areas in Bangladesh. Blood and urine samples, and questionnaire data were collected. We analyzed data from 202 pregnant women and a subset of 121 mother-infant pairs. Urinary As was measured on inductively coupled plasma-mass spectrometry and adjusted with specific gravity (U-AsSG ). Maternal (IgGmat ) and cord (IgGcord ) serum total IgG were measured using immunoturbidity assay. The geometric mean U-AsSG (n = 202) was 69 μg/L (range, 3.1-1356 μg/L). Urinary-AsSG was significantly associated with IgGmat (n = 202; (r = 0.24; 95% confidence interval [CI]: 0.10-0.36; P < 0.001) and remained so after the inclusion of maternal-associated variables in a multiple-regression model (β = 1.26; 95%CI: 0.47-2.05; P < 0.01). U-AsSG , however, was not significantly associated with IgGcord (n = 121), while IgGmat and IgGcord were also not associated with each other. Maternal As exposure was positively associated with IgGmat but not IgGcord . Elevated IgGmat may have implications as regards maternal morbidity and the placental transfer of specific IgGs. Further studies are required to better understand how As may affect maternal and child health by modifying the humoral immune system.

Usually it has a dramatic and catastrophic consequence both for the fetus & the mother; rather subsequent delivery of a viable full term fetus is exceptional. It is difficult to diagnose preoperatively. Though relatively rare, we received an AAP at term with live fetus in Faridpur Medical College & hospital. Illiteracy, poverty and lack of antenatal care had resulted in her late presentation. After laparotomy the diagnosis was confirmed, a healthy male baby was delivered. Urinary-AsSG was significantly associated with IgGmat (n = 202; (r = 0.24; 95% confidence interval [CI]: 0.10-0.36; P < 0.001) and remained so after the inclusion of maternal-associated variables in a multiple-regression model (β = 1.26; 95%CI: 0.47-2.05; P < 0.01). U-AsSG , however, was not significantly associated with IgGcord (n = 121), while IgGmat and IgGcord were also not associated with each other. Maternal As exposure was positively associated with IgGmat but not IgGcord . Elevated IgGmat may have implications as regards maternal morbidity and the placental transfer of specific IgGs. Further studies are required to better understand how As may affect maternal and child health by modifying the humoral immune system.

Usually it has a dramatic and catastrophic consequence both for the fetus & the mother; rather subsequent delivery of a viable full term fetus is exceptional. It is difficult to diagnose preoperatively. Though relatively rare, we received an AAP at term with live fetus in Faridpur Medical College & hospital. Illiteracy, poverty and lack of antenatal care had resulted in her late presentation. After laparotomy the diagnosis was confirmed, a healthy male baby was delivered.
