Em função disso, o objetivo principal do presente trabalho é iniciar uma reflexão acerca do tema "causas não apuradas"(que será considerado sinônimo de "causa indeterminada"), já que parte das conclusões das investigações assim é classificada, sem identificarem efetivamente as prováveis causas do sinistro. Este estudo apresenta um relato sobre os aspectos jurídicos e estatísticos do Departamento responsável pelo serviço investigativo e a visão de seus profissionais peritos sobre sua própria atuação. O leitor encontrará no presente trabalho impressões sobre o método investigativo utilizado no Corpo de Bombeiros Militar do Estado do Espírito Santo e a tentativa de elucidar os fatores envolvidos nas conclusões por "causa não apurada" nos laudos periciais emitidos pela corporação até o fim do ano de 2015. No presente escrito, será visto o enfrentamento do problema a partir de uma visão tripartite, em que se deve levar em consideração a estrutura, requalificação e metodologia como pressupostos principais na reflexão sobre o assunto.

Ela ocorre em pacientes com severa disfunção ventricular esquerda, onde cerca de 40% da massa ventricular é comprometida pelo infarto. O objetivo deste estudo foi relatar um caso de óbito natural mal definido em adulto com 48 anos de idade, sexo masculino, suspeito de Infarto Agudo do Miocárdio fulminante. A necropsia, associada aos informes clínicos e ao exame histopatológico evidenciou que a causa do óbito foi choque cardiogênico por provável doença coronariana aguda aterosclerótica, que culminou no infarto agudo do miocárdio.The cardiogenic shock syndrome has been defined as the inability of the heart to maintain adequate blood flow to tissues, resulting in metabolic deficiency. Observa-se que creches e escolas figuram-se como ambientes favoraveis para o desenvolvimento de estrategias pedagogicas em torno da alimentacao, pois, alem de atenderem aos alunos, podem envolver a familia e a comunidade. Logo, o presente trabalho teve como objetivo relatar a experiencia com a aplicacao de metodos ludico-pedagogicos em Educacao Alimentar e Nutricional (EAN) para pre-escolares. Trata-se de um relato acerca de experiencias educativas realizadas entre setembro de 2011 e junho de 2013 com direcionamento para criancas de 2 a 5 anos de idade, matriculadas em uma organizacao social publica nao-estatal do municipio de Juiz de Fora (MG). This article reports on a scientific work in development. It is being conducted by five undergrad... more This article reports on a scientific work in development. It is being conducted by five undergraduate students and two mentors. The work consists of developing a system to help students and teachers of the physiotherapy college to study clinical cases. A hybrid methodology is being used for the development of the system. Through the system have a high degree of complexity, modern requirements engineering techniques were used to describe them and to model the overall structure of the system. The project is in the implementation phase. This article brings the project results to this point and describes the challenges that have been encountered and the lessons learned.

Síntese dos dados: A descrição desta experiência é relativa à segunda fase de um estudo de coorte, realizada no período de 2015-2016. A primeira fase da coleta de dados ocorreu em 2010-2012, contemplando 778 mulheres, entre 20 e 59 anos, residentes na área de abrangência de duas Unidades Básicas de Saúde (UBSs). As sementes foram submetidas ao desponte do lado oposto ao embrião e parte das sementes foi submetida ao corte do lado oposto ao embrião e sobre a câmara de ar. A semeadura foi realizada em tubetes com substrato comercial ou substrato comercial + fibra de coco (1:1), em viveiro com 50% de luminosidade. O acréscimo de fibra de coco ao substrato comercial na proporção de 50% e a realização de cortes nas sementes não afetam a produção de mudas de noni. Palavras-chave: Morinda citrifolia L., fibra de coco, crescimento de plantas, plantas medicinais. Substrates and excision of seeds for noni seedlings production. This work aimed to study the noni seedlings production in organic substrates after excision seed. The seeds were submitted to the cuts in opposite side of the embryo and part of the seeds have been subjected to this cuts and cuts in the tube region. The seeds were sown in containers with commercial substrate or commercial substrate + coconut fiber (1:1), in nursery with 50% of shading. The addition of coconut fiber in the commercial substrate ratio of 50% and cuts of the seeds does not affect the production of noni seedlings.

Objective: To investigate whether tri-ponderal mass index and body mass index Z scores are equiva... more Objective: To investigate whether tri-ponderal mass index and body mass index Z scores are equivalent for screening children and adolescents with insulin resistance. Methods: Cross-sectional study with 296 children and adolescents enrolled at public schools of Vitória, Espírito Santo, Brazil, aged eight to 14 years. The tri-ponderal mass index was calculated as the ratio between weight and height cubed. The body mass index was calculated as the ratio between weight and height squared. Insulin resistance was defined with the homeostatic model assessment (HOMA-IR). Results: The HOMA-IR was higher in the 4th quartile of body mass index Z scores and tri-ponderal mass index compared to 1st and 2nd quartiles for both girls and boys. Background and aims: Sorafenib is the first proved target therapy that shows significant survival... more Background and aims: Sorafenib is the first proved target therapy that shows significant survival benefit in advanced hepatocellular carcinoma. This study was aimed to investigate whether add-on sorafenib be beneficial for those experienced transarterial chemoembolization refractoriness. Methods: From 2005 to 2016, a total of 656 treatment-naive hepatocellular carcinoma patients receiving transarterial chemoembolization treatment were recruited. Transarterial chemoembolization refractoriness was defined as progressive disease after two consecutive of transarterial chemoembolization treatment within 6 months. Patient's baseline characteristics, tumor burden, and parameters for liver function evaluation during treatment were analyzed. All the variables were compared between patients with and without transarterial chemoembolization refractoriness, as well as with and without add-on sorafenib. Results: Among the 656 patients, the median age was 62.5 (range 27.3-91.5) years old, and 74.5% were male. Transarterial chemoembolization refractoriness events were documented in 202 patients (30.8%). After multivariate logistic regression analysis, tumor size ≧5 cm, baseline alpha-fetoprotein level ≧200 mg/dl, elevation of alpha-fetoprotein ≧20%, and elevation of Child-Turcotte-Pugh score ≧2 points after first transarterial chemoembolization were the independent predictive factors for transarterial chemoembolization refractoriness. Twenty-two patients (10.9%) received add-on sorafenib treatment and 146 (72.3%) patients continued transarterial chemoembolization treatment alone. After 1:2 propensity score matching, patients with add-on sorafenib therapy had significantly longer median overall survival than transarterial chemoembolization treatment alone (23.1 vs. 11.0 months, log-rank P = 0.001). Conclusion: The tumor size, baseline alpha-fetoprotein, and elevation of alpha-fetoprotein and Child-Turcotte-Pugh score after first transarterial chemoembolization were the predictors for transarterial chemoembolization refractoriness. For patients with transarterial chemoembolization refractoriness, add-on sorafenib achieved better survival benefit than transarterial chemoembolization treatment alone. This cross-sectional study involved students from nine public schools. In total, 296 children and adolescents of both sexes, aged 8-14 years, composed the sample. Waist-to-height ratio, triglycerides/glucose index, and triglycerides-to-HDL-C ratio were determined according to standard protocols. A systematic review was performed, according to the standards proposed by the PRISMA guideline, on PubMed, Science Direct, and Lilacs databases. The descriptor "chemerin" was used in combination with "children" and "adolescent", no time limit applied. The research encompassed only original articles written in English, conducted with human subjects - the adult and elderly populations excluded -, as well as literature reviews, brief communications, letters, and editorials. Fast forward to late 2015: another epidemic of another virus-Zika-together with its major neurological complication-microcephaly-explodes in Brazil. Again, as with the EVD epidemic, the world's attention is focused on issues such as where the virus originated, the need to control vectors, how quickly we can develop a vaccine, and how long the epidemic will continue. While these are important discussions to have, they are irrelevant if the world does not recognize and address a crucial reason why these explosive epidemics continue to occur in the first place: the world must talk about urban slums. Zika is, and will continue to be, a disease of the urban poor. Slum-defining characteristicspoor water and sanitation infrastructure, crowding, and poor structural quality of housingoffer ample opportunities for mosquitoes to breed and spread the Zika virus. It was recently estimated that 1.6 million childbearing women and 93 million people will be infected in the Americas' first epidemic wave [2]. From this reservoir of infections, the world is witnessing the largest epidemic ever of a congenital complication-microcephaly. Except for sporadic reports, largely absent from discussion is the fact that the greatest proportion of Zika infections and its complications have occurred, and will continue to occur, among residents of the large, densely packed informal human settlements of Latin America and the Caribbean. The Aedes aegypti mosquito-the species that most commonly transmits Zika-will, on average, travel a mere 100 meters in its lifetime [3]. In the densely populated favelas, or slums of Brazil and elsewhere in the region, a single 100 square meter space could contain more than 100 housing structures, 2-3 stories high, with a resident population upwards of 1,000. These residents can be infected multiple times by mosquitoes circulating in such neighborhoods. Wealthy residents of high-rise apartment buildings with screened windows, air conditioning, and regular spraying of insecticides, even if located adjacent to these favelas, are less likely to be exposed to such mosquitoes. The burden of symptomatic dengue, another mosquito-borne disease, has been shown to be greater among the neighborhoods with the lowest socioeconomic status within a slum [4]. Such methods, however, can be expensive, laborious or unable to accurately discriminate among certain serotypes. Therefore, determination of capsular types, although extremely important for epidemiological purposes and for estimating the impact of pneumococcal conjugate vaccines, is mainly restricted to research laboratories, being rarely performed in the clinical setting. In the present study, MALDI-TOF MS was evaluated as an alternative tool to characterize 416 pneumococcal isolates belonging to serotypes 6A, 6B, 6C, 9N, 9V or 14. This study was aimed to investigate whether add-on sorafenib be beneficial for those experienced transarterial chemoembolization refractoriness. Methods: From 2005 to 2016, a total of 656 treatment-naive hepatocellular carcinoma patients receiving transarterial chemoembolization treatment were recruited. Transarterial chemoembolization refractoriness was defined as progressive disease after two consecutive of transarterial chemoembolization treatment within 6 months. Patient's baseline characteristics, tumor burden, and parameters for liver function evaluation during treatment were analyzed. All the variables were compared between patients with and without transarterial chemoembolization refractoriness, as well as with and without add-on sorafenib. Results: Among the 656 patients, the median age was 62.5 (range 27.3-91.5) years old, and 74.5% were male. Transarterial chemoembolization refractoriness events were documented in 202 patients (30.8%). After multivariate logistic regression analysis, tumor size ≧5 cm, baseline alpha-fetoprotein level ≧200 mg/dl, elevation of alpha-fetoprotein ≧20%, and elevation of Child-Turcotte-Pugh score ≧2 points after first transarterial chemoembolization were the independent predictive factors for transarterial chemoembolization refractoriness. Twenty-two patients (10.9%) received add-on sorafenib treatment and 146 (72.3%) patients continued transarterial chemoembolization treatment alone. After 1:2 propensity score matching, patients with add-on sorafenib therapy had significantly longer median overall survival than transarterial chemoembolization treatment alone (23.1 vs. 11.0 months, log-rank P = 0.001). Conclusion: The tumor size, baseline alpha-fetoprotein, and elevation of alpha-fetoprotein and Child-Turcotte-Pugh score after first transarterial chemoembolization were the predictors for transarterial chemoembolization refractoriness. For patients with transarterial chemoembolization refractoriness, add-on sorafenib achieved better survival benefit than transarterial chemoembolization treatment alone. Fast forward to late 2015: another epidemic of another virus-Zika-together with its major neurological complication-microcephaly-explodes in Brazil. Again, as with the EVD epidemic, the world's attention is focused on issues such as where the virus originated, the need to control vectors, how quickly we can develop a vaccine, and how long the epidemic will continue. While these are important discussions to have, they are irrelevant if the world does not recognize and address a crucial reason why these explosive epidemics continue to occur in the first place: the world must talk about urban slums. Zika is, and will continue to be, a disease of the urban poor. Slum-defining characteristicspoor water and sanitation infrastructure, crowding, and poor structural quality of housingoffer ample opportunities for mosquitoes to breed and spread the Zika virus. It was recently estimated that 1.6 million childbearing women and 93 million people will be infected in the Americas' first epidemic wave [2]. From this reservoir of infections, the world is witnessing the largest epidemic ever of a congenital complication-microcephaly. Except for sporadic reports, largely absent from discussion is the fact that the greatest proportion of Zika infections and its complications have occurred, and will continue to occur, among residents of the large, densely packed informal human settlements of Latin America and the Caribbean. The Aedes aegypti mosquito-the species that most commonly transmits Zika-will, on average, travel a mere 100 meters in its lifetime [3]. In the densely populated favelas, or slums of Brazil and elsewhere in the region, a single 100 square meter space could contain more than 100 housing structures, 2-3 stories high, with a resident population upwards of 1,000. These residents can be infected multiple times by mosquitoes circulating in such neighborhoods. Wealthy residents of high-rise apartment buildings with screened windows, air conditioning, and regular spraying of insecticides, even if located adjacent to these favelas, are less likely to be exposed to such mosquitoes. The burden of symptomatic dengue, another mosquito-borne disease, has been shown to be greater among the neighborhoods with the lowest socioeconomic status within a slum [4]. Such methods, however, can be expensive, laborious or unable to accurately discriminate among certain serotypes. Therefore, determination of capsular types, although extremely important for epidemiological purposes and for estimating the impact of pneumococcal conjugate vaccines, is mainly restricted to research laboratories, being rarely performed in the clinical setting. In the present study, MALDI-TOF MS was evaluated as an alternative tool to characterize 416 pneumococcal isolates belonging to serotypes 6A, 6B, 6C, 9N, 9V or 14.
