Lemierre syndrome is characterized by septic thrombophlebitis of the internal jugular vein, after... more Lemierre syndrome is characterized by septic thrombophlebitis of the internal jugular vein, after an oropharyngeal infection, with septic embolization to the lungs or other organs. This case report describes a 37-year-old female patient who presented with edema and pain in the right hemiface with onset 3 days previously and progressive fatigue and dyspnea since the previous day. She had had tooth 48 extracted 3 days previously. Physical examination at admission found tachypnea, with 60% saturation (in room air), edema at the angle of the right mandible, diffuse reduction of vesicular murmur, and calves free from clubbing. Angiotomography of the chest and laboratory tests were compatible with septic emboli, and cervical computed tomography confirmed a diagnosis of septic thrombophlebitis of the internal jugular vein. Low blood pressure control rates in Latin American populations emphasize the need for gathering evidence on effective therapies. Objective: To evaluate the effects of dietary interventions on blood pressure in Latin American populations. Methods: Systematic review. Electronic databases (MEDLINE/PubMed, Embase, Cochrane Library, CINAHL, Web of Science, Scopus, SciELO, LILACS and VHL) were searched and manual search for studies published up to April 2013 was performed. Parallel studies about dietary interventions in Latin American adult populations assessing arterial blood pressure (mm Hg) before and after intervention were included. Gastroesophageal reflux is considered potential factor that may result in damage to the esophageal mucosa and this subject is quite controversial in the literature. Aim: To evaluate patients who underwent to Roux-en-Y gastrojejunal bypass correlating epidemiologic and endoscopic findings in pre and postoperative periods. Method: A retrospective, paired study which evaluated 110 patients. Inclusion criteria were formal indication for bariatric surgery and patients with pre and postoperative endoscopy. Exclusion criteria were previous bariatric surgery, patients subjected to other types of bariatric surgery and those who had no pre or postoperative upper digestive endoscopy. The epidemiological variables were: sex, age, body mass index, type 2 diabetes mellitus or impaired glucose tolerance, and preoperative dyslipidemia. A síndrome de Lemierre é uma doença rara, mais comum em jovens, causada frequentemente pelo Fusob... more A síndrome de Lemierre é uma doença rara, mais comum em jovens, causada frequentemente pelo Fusobacterium necrophorum. Inicia-se com faringite e propaga-se até a veia jugular interna, promovendo uma fonte de bacteremia contínua e êmbolos sépticos pulmonares. Manifestações clínicas incluem febre, alterações respiratórias e massa cervical. O diagnóstico é realizado por tomografia computadorizada e duplex scan, além de hemocultura ou cultura direta. O tratamento é realizado com antibióticos beta-lactâmicos resistentes a beta-lactamases, sendo a cirurgia raramente necessária. Paciente do sexo feminino, 34 anos, com quadro de orofaringite, evoluiu em 48 horas com queda do estado geral, febre, aumento de volume e dor em região cervical esquerda. Radiografia e tomografia de tórax evidenciaram múltiplas lesões pulmonares. The aim of the present investigation was to determine whether the difference in inflammatory tiss... more The aim of the present investigation was to determine whether the difference in inflammatory tissue reaction between the Riccinus communis (castor) polymer with calcium carbonate and the titanium implant is statistically significant. Thirty-two Cavia porcellus were allocated into four groups of eight animals each. We implanted the two types of materials in the retroperitoneal space of all the animals. They were euthanized at 7, 20, 30 and 40 days after surgery, and an histological study of the samples was conducted. All implants showed characteristics of chronic inflammation regardless of the material and timepoint of evaluation. There was no statistically significant difference between Pm+CaCO3 and Ti with regard to the presence of granulation tissue, tissue congestion, histiocytes, lymphocytes, neutrophils, giant cells, and fibrosis (P> 0.05). Baixos índices de controle da pressão arterial em populações latino-americanas reforçam a necessidade de reunir evidências sobre terapias eficazes. Objetivo: Avaliar o efeito das intervenções de modificações de dietas sobre pressão arterial em populações latino-americanas. Métodos: Revisão sistemática. Foram pesquisadas diversas bases de dados (MEDLINE-PubMed, Embase, Cochrane Library, CINAHL, Web of Science, Scopus, SciELO, LILACS e BVS) e realizada busca manual até abril de 2013. Foram incluídos estudos paralelos de intervenções em dieta em populações adultas da América Latina reportando pressão arterial (em mmHg) antes e após a intervenção. Resultados: Dos 405 estudos encontrados, 10 ensaios clínicos randomizados foram incluídos e divididos em 3 subgrupos, de acordo com a dieta proposta como intervenção. Houve redução não significativa da pressão arterial sistólica nos subgrupos de substituição mineral -4,82 (IC 95%: -11,36 a 1,73 mmHg) e padrões complexos -3,17 (IC 95%: -7,62 a 1,28 mmHg). Para a pressão arterial diastólica, com exceção do subgrupo de dietas hiperproteicas, todos os subgrupos apresentaram redução significativa, com -4,66 (IC 95%: -9,21 a -0,12 mmHg) e -4,55 (IC 95%: -7,04 a -2,06 mmHg) para substituição mineral e padrões complexos, respectivamente. Conclusão: A evidência disponível sobre os efeitos de alterações de dieta na pressão arterial em populações latino-americanas indica um efeito homogêneo, porém não significativo, para pressão arterial sistólica. Estudos maiores e com maior rigor metodológico são necessários para construção de evidência robusta.
