Periprostetik kırıkların tedavisinde plak-vida uygulamalarında termal nekroz, vida başında soğuk kaynak, gevşeme ve kemikte dolaşım bozukluğu görülebilir. Bu çalışmada yeni tasarladığımız U çivili plağın serklaj sistemleri ile karşılaştırılıp, periprostetik kırık tespitinde güvenle kullanılabileceğini test etmeyi amaçladık.GEREÇ VE YÖNTEM: Testler laboratuvar koşullarda numunelerin yorulma sınırlarını belirleyerek gerçekleştirilmiştir. U çivili model, kablo serklaj sistemi ve hibrit sistem olarak 3 farklı model grubu test edilmiştir.BULGULAR: Elde edilen sonuçlara göre 1. gruba yorulma direnci en düşük iken, 2. ve 3. grup arasında anlamlı fark bulunmamıştır. We investigated the relationship between the mechanical-anatomic axes and angles of the knee join... more We investigated the relationship between the mechanical-anatomic axes and angles of the knee joint and functional levels of patients with primary osteoarthritis. A total of 35 patients (27 females, 8 males; mean age 58+/-9 years; range 42 to 77 years) with knee osteoarthritis were included. After physical examination, weight-bearing anteroposterior x-rays of the knees were obtained, on which measurements of the following were made: mechanical axis deviation (MAD), medial proximal tibial angle (MPTA), anatomic tibiofemoral angle (aTFA), mechanical lateral distal femoral angle (mLDFA), and joint line congruence angle (JLCA). Pain was evaluated by both the Western Ontario McMaster Osteoarthritis Index (WOMAC) and a visual analog scale (VAS). Functional capacity was assessed using the physical function and global indices of WOMAC. There was a moderate correlation between body mass index and the WOMAC global index (r=0.47, p<0.05). Radiographic evaluations revealed varus deformity in 28 patients. There was a correlation between aTFA and the physical function (r=0.47) and global (r=0.41) indices of WOMAC (p<0.05). Functional capacity and pain were not correlated with the other mechanical angles of the knee joint (p>0.05). In view of our results, aTFA may be considered one important criterion in designing treatment and planning surgery for patients with primary osteoarthritis.

A 35-year-old man was referred to our clinic by a neurosurgeon for hypersensitivity and severe pa... more A 35-year-old man was referred to our clinic by a neurosurgeon for hypersensitivity and severe pain in his right middle finger. History showed that the symptoms had been present for three years and he had made several attempts to seek medical attention. A sensitive, very painful mass was palpated under the eponychium of his right middle finger. The tumor was surgically removed and histological examination confirmed the clinical diagnosis of a glomus tumor. Inquiry into the family history revealed that the same problem existed in the same localization with similar complaints in the father and two daughters. Both the records of the father's operation 10 years before and clinical-radiological findings of the daughters were consistent with a familial entity. In the presence of hypersensitivity and pain in the finger, family members should be inquired and a diagnosis of familial glomus tumor should be considered.

Lateral dekubit pozisyonda lumbal 3-4 araligindan girilerek 1.3 ml %0.5 hiperbarik bupivakain+ 0.2 ml 10 mg meperidin verildi. Duysal blok seviyesinin torakal 10 oldugu saptandiktan sonra operasyona baslandi. Operasyonun 110. dakikasinda hasta dudaklarinda ve ellerinde uyusmadan sikayet etmeye basladi. Hemen arkasindan hastanin ellerinde ebe eli (Trosseau bulgusu) gelisti. 10 mEq magnezyum, 20 mEq kalsiyum yavas infuzyonla intravenoz olarak verildi. Arteriyel kan gazinda dekompanze solunumsal alkaloz ve iyonize kalsiyum 0.83 mEq/l olarak geldi. Hastamizda organik bir hipokalsemi nedeni bulunamadi. Of a large synpolydactyly kindred the distinctive phenotypes from each other, homozygous phenotyp... more Of a large synpolydactyly kindred the distinctive phenotypes from each other, homozygous phenotypes produced by the marriages of two heterozygote people and a few minor modifications are presented in this study. The kindred was described in 1995 and has been getting larger since then. We have been visiting the kindred since 2002 May. Evaluating the kindred, we found 125 affected people and described the different phenotypic features clinically and radiologically. Six hand and five foot phenotypes were discriminated. A few distinctive phenotypes undescribed in 1995 were found in the last generation mainly formed by the 89 people born after 1995. Development of different phenotypes may have resulted from incomplete penetrance, variable expressivity, consangineous marriages and multiple genetic factors affecting synpolydactyly inheritance. While the kindred's expansion is a problem in view of public health, it is important surgical standpoint because of different defomities and modified malformations, as well.

Bu çalışmanın amacı dikey ofset değerlendirilmesi ile ilgili alternatif yeni bir ölçüt tanımlamaktır.GEREÇ VE YÖNTEM: Çalışmamızda trokanter majör - asetabulum çatı mesafesi ölçümünün (ASTM) ekstremite uzunluk tayininde güvenli bir yöntem olup olmadığını araştırdık. İleri evre gonartrozlarda proksimal tibiafibular eklem (PTFE) ile tibiofemoral eklem (TFE) arasın... more İleri evre gonartrozlarda proksimal tibiafibular eklem (PTFE) ile tibiofemoral eklem (TFE) arasındaki klinik ilişki daha önce gösterilmemiştir. Bu retrospektif çalışmanın amacı son evre gonartrozu olup diz protezi uygulanan genu varum deformiteli hastalarda PTFE'in klinik değerlendirmesidir. GEREÇ VE YÖNTEM: İleri evre osteoartrozu olup diz protezi uygulanan genu varum deformiteli hastalar değerlendirmeye alındı. PTFE'in klinik muayene bulguları ile PTFE tipi, hamstring gerginliği, ve lateral eklem aralığı (LEA) hassasiyeti arasındaki ilişki ki-kare testi kullanılarak araştırıldı. Ayrıca tanımlayıcı istatistikte kullanıldı. BULGULAR: Çalışma grubu (5 erkek ve 49 kadın; ortalama yaş 62.7 yıl; 46-81 arasında) elli dört hastadan oluşmaktadır. 30 hastanın (%55.6) her iki dizi ameliyat edildi. Ortalama takip süresi 21.6 ay (12-49 ay). PTFE hassasiyeti, hamstring gerginliği ve LEA hassasiyeti sırasıyla altı (%7.1), dört (%4.8), ve altı (%7.1) dizde tespit edildi. Altı dizde (%7.1) horizontal tip PTFE varken, yetmiş sekiz dizde (%92.9) oblik tip PTFE vardı. Oblik tip PTFE olan dizlerde PTFE hassasiyeti (ki-kare testi, p=0.000) , LEA hassasiyeti (ki-kare testi, p=0.000), hamstring gerginliği (ki-kare testi, p=0.000) tespit edildi. SONUÇ: Varus dizilim kusuru olan ileri evre dejeneratif eklem hastalığı nedeniyle diz protezi yapılan hastalarda, PTFE'in ameliyat sonrası lateral diz ağrısının kaynağı olmadığı ortaya konmuştur. Aim: In this study, we want to compare clinical and radiological results of 12 patient treated w... more Aim: In this study, we want to compare clinical and radiological results of 12 patient treated with acellular collagen matrix (ACM) and High Tibial osteotomy-cellular collagen matrix(ACM-HTO) combination.Method: In this study, we evaluated 12 patients with chondral lesion prospectively the years of between 2014- 2016 after ethical approval. Cartilage damage was diagnosed with MRI and cartilage degeneration volume was calculated with Mimics medical image processing software and axis deviation was diagnosed with ortho X-ray. Patients were divided into two groups. ACM only (Group 1) and ACM- HTO (Group 2). Results: ACM only group consists of 5 patients. Preoperative mean of chondral lesion volume was 77.11±137.5 (0.27-321.61). Post-operative mean Knee score was 175.20±24.4(150-203) and chondral lesion volume was 0.3±0.03(0-0.8). In such fractures, treatment invariably comprises the extraction of proximal femoral nail and the introduction of a different hip prosthesis. To solve this problem, we aimed to develop a new implant, the modular nail prosthesis, which combines an intramedullary nail with a hip prosthesis. The aim of this study was to determine the effect of stress load distribution on the stem using finite element analysis under laboratory conditions. For this, the shortest stem of 175 mm was chosen. In addition, six proximal femoral nail-hip prosthesis combinations were produced and tested with a biomechanical test device and passed 2300-N load bearing. According to the test results, our newly developed modular nail prosthesis can be converted to hip prosthesis securely.

This study was designed to investigate the effect of a quadriceps exercise program along with a patellar brace in reducing pain and increasing function in patello-femoral pain syndrome (PFPS) patients. M Ma at te er ri ia al ls s a an nd d M Me et th ho od ds s: : A total of 86 patients with PFPS were included in this study. Patients were randomized into two groups: Group 1 (n=45) received a home based exercise program only; group 2 (n=41) received the same home based exercise program along with a patellar brace. The duration of the intervention was six weeks. At the end of the therapy patients were evaluated for pain and functional capacity. Pain was assessed using the visual analogue scale (VAS) and the Western Ontario McMaster Osteoarthritis Index (WOMAC) Pain Scale. Functional capacity was evaluated by using the WOMAC Functional Capacity Index and the Fulkerson-Shea Patellofemoral Evaluation Score (FSPES). This study was carried out to determine the effects of organic and inorganic manganese (Mn) at di... more This study was carried out to determine the effects of organic and inorganic manganese (Mn) at different levels supplemented to diets of the broilers under stocking density stress on bone characteristics, immune response to vaccine and oxidative stress. A total of 1000, day-old broiler chicks were divided into one control group and nine treatment groups, each consisting of 100 chicks. They were randomly located at a density of 0.064 kg/m 2 until reaching seven days of age and in the compartments with 2 m 2 space at a density of 60 kg/m 2 until they are 49 days old. The control group was fed the basal diet only, whereas experimental groups were fed with basal diet supplemented with organic Mn (6.25, 12.5 and 25 mg/kg of Mn-methionine), inorganic Mn (12.5, 25 and 50 mg/kg Mn-oxide) and organic + inorganic Mn (3.125+6.25, 6.25+12.5 and 12.5+25 mg/kg Mn-methionine + Mn-oxide). There were no changes on days 25 and 49 in terms of physical properties such as humerus, femur and tibia as well as tibia fracture resistance and tibial metaphyseal chondral tissue lenght in all experimental groups (P>0.05). Tibia bone elasticity was higher in the 50 mg/kg Mn-oxide group compared to the control and other groups on day 49 (P<0.05). The serum calcium level on day 25 increased in the 6.25 and 12.5 mg/kg Mn-methionine and 6.25+12.5 mg/kg Mn-methionine + Mn-oxide groups (P<0.01). Antibody titre against Newcastle Disease Virus in Mn-methionine group on day 25 and titer against Infectious Bursal Disease in Mn-methionine and Mn-oxide groups on day 49 were higher compared to the control group (P<0.01). Serum levels of malondialdehyde and nitric oxide on day 49 decreased in all exprimental groups, whereas serum glutathione level increased in the same groups (P<0.01). As a result, dietary Mn supplementation to broilers hosted at 60 kg/m 2 of stocking density increased immune response and antioxidant activity by reducing the impact of density stress; yet, did not affect the physical properties of the bone. The effects of organic and inorganic forms of Mn on the examined properties were concluded to be similar.

Thirty-six knees [Bilateral knees were operated simultaneously in 12 patients (50%)] in 24 patients [3 men (12.5%), 21 women (87.5%); average age 67 (59-80 age)] were included in the study. During surgery, PCA+3° ER line and cTEA line were drawn on the distal femoral cutting surface by electrocautery pencil following distal femoral cut. The both lines on distal femur were recorded by digital camera and relationship between lines was ascertained in reference to PCA+3° ER line [parallel, internal rotation (IR), ER]. Statistical analysis was performed by the McNamara chi square test and Kappa (κ) value. Data were collected on disabilities from a sample population in order to estimate the prevalence ... more Data were collected on disabilities from a sample population in order to estimate the prevalence of orthopedic disabilities in the general population. A two-stage field study was conducted in Cay, a district of Afyon, Turkey. In the first stage, a list of all individuals with orthopedic disabilities was derived. Then, medical histories were elicited and examinations were made by a team of specialists of orthopedics and traumatology. Radiographic studies were made when necessary. Orthopedic disabilities were defined as any muscle or skeletal abnormality that was associated with a permanent functional loss and were classified as congenital, traumatic, cerebral palsy, poliomyelitis, and other causes. The overall population was 35,571, of whom 189 had orthopedic disabilities (53/10,000). The male-to-female ratio was 1.8. A sensitive, very painful mass was palpated under the eponychium of his right middle finger. The tumor was surgically removed and histological examination confirmed the clinical diagnosis of a glomus tumor. Inquiry into the family history revealed that the same problem existed in the same localization with similar complaints in the father and two daughters. Both the records of the father&amp;amp;amp;amp;amp;amp;amp;amp;amp;#39;s operation 10 years before and clinical-radiological findings of the daughters were consistent with a familial entity. In the presence of hypersensitivity and pain in the finger, family members should be inquired and a diagnosis of familial glomus tumor should be considered.

Lateral dekubit pozisyonda lumbal 3-4 araligindan girilerek 1.3 ml %0.5 hiperbarik bupivakain+ 0.2 ml 10 mg meperidin verildi. Duysal blok seviyesinin torakal 10 oldugu saptandiktan sonra operasyona baslandi. Operasyonun 110. dakikasinda hasta dudaklarinda ve ellerinde uyusmadan sikayet etmeye basladi. Hemen arkasindan hastanin ellerinde ebe eli (Trosseau bulgusu) gelisti. 10 mEq magnezyum, 20 mEq kalsiyum yavas infuzyonla intravenoz olarak verildi. Arteriyel kan gazinda dekompanze solunumsal alkaloz ve iyonize kalsiyum 0.83 mEq/l olarak geldi. Hastamizda organik bir hipokalsemi nedeni bulunamadi. Of a large synpolydactyly kindred the distinctive phenotypes from each other, homozygous phenotyp... more Of a large synpolydactyly kindred the distinctive phenotypes from each other, homozygous phenotypes produced by the marriages of two heterozygote people and a few minor modifications are presented in this study. The kindred was described in 1995 and has been getting larger since then. We have been visiting the kindred since 2002 May. Evaluating the kindred, we found 125 affected people and described the different phenotypic features clinically and radiologically. Six hand and five foot phenotypes were discriminated. A few distinctive phenotypes undescribed in 1995 were found in the last generation mainly formed by the 89 people born after 1995. Development of different phenotypes may have resulted from incomplete penetrance, variable expressivity, consangineous marriages and multiple genetic factors affecting synpolydactyly inheritance. While the kindred's expansion is a problem in view of public health, it is important surgical standpoint because of different defomities and modified malformations, as well.

İleri evre gonartrozlarda proksimal tibiafibular eklem (PTFE) ile tibiofemoral eklem (TFE) arasın... more İleri evre gonartrozlarda proksimal tibiafibular eklem (PTFE) ile tibiofemoral eklem (TFE) arasındaki klinik ilişki daha önce gösterilmemiştir. Bu retrospektif çalışmanın amacı son evre gonartrozu olup diz protezi uygulanan genu varum deformiteli hastalarda PTFE'in klinik değerlendirmesidir. GEREÇ VE YÖNTEM: İleri evre osteoartrozu olup diz protezi uygulanan genu varum deformiteli hastalar değerlendirmeye alındı. PTFE'in klinik muayene bulguları ile PTFE tipi, hamstring gerginliği, ve lateral eklem aralığı (LEA) hassasiyeti arasındaki ilişki ki-kare testi kullanılarak araştırıldı. Ayrıca tanımlayıcı istatistikte kullanıldı. BULGULAR: Çalışma grubu (5 erkek ve 49 kadın; ortalama yaş 62.7 yıl; 46-81 arasında) elli dört hastadan oluşmaktadır. 30 hastanın (%55.6) her iki dizi ameliyat edildi. Ortalama takip süresi 21.6 ay (12-49 ay). PTFE hassasiyeti, hamstring gerginliği ve LEA hassasiyeti sırasıyla altı (%7.1), dört (%4.8), ve altı (%7.1) dizde tespit edildi. Altı dizde (%7.1) horizontal tip PTFE varken, yetmiş sekiz dizde (%92.9) oblik tip PTFE vardı. Oblik tip PTFE olan dizlerde PTFE hassasiyeti (ki-kare testi, p=0.000) , LEA hassasiyeti (ki-kare testi, p=0.000), hamstring gerginliği (ki-kare testi, p=0.000) tespit edildi. SONUÇ: Varus dizilim kusuru olan ileri evre dejeneratif eklem hastalığı nedeniyle diz protezi yapılan hastalarda, PTFE'in ameliyat sonrası lateral diz ağrısının kaynağı olmadığı ortaya konmuştur. Aim: In this study, we want to compare clinical and radiological results of 12 patient treated w... more Aim: In this study, we want to compare clinical and radiological results of 12 patient treated with acellular collagen matrix (ACM) and High Tibial osteotomy-cellular collagen matrix(ACM-HTO) combination.Method: In this study, we evaluated 12 patients with chondral lesion prospectively the years of between 2014- 2016 after ethical approval. Cartilage damage was diagnosed with MRI and cartilage degeneration volume was calculated with Mimics medical image processing software and axis deviation was diagnosed with ortho X-ray. Patients were divided into two groups. ACM only (Group 1) and ACM- HTO (Group 2). Results: ACM only group consists of 5 patients. In such fractures, treatment invariably comprises the extraction of proximal femoral nail and the introduction of a different hip prosthesis. To solve this problem, we aimed to develop a new implant, the modular nail prosthesis, which combines an intramedullary nail with a hip prosthesis. The aim of this study was to determine the effect of stress load distribution on the stem using finite element analysis under laboratory conditions. For this, the shortest stem of 175 mm was chosen. In addition, six proximal femoral nail-hip prosthesis combinations were produced and tested with a biomechanical test device and passed 2300-N load bearing. According to the test results, our newly developed modular nail prosthesis can be converted to hip prosthesis securely.

This study was designed to investigate the effect of a quadriceps exercise program along with a patellar brace in reducing pain and increasing function in patello-femoral pain syndrome (PFPS) patients. M Ma at te er ri ia al ls s a an nd d M Me et th ho od ds s: : A total of 86 patients with PFPS were included in this study. Patients were randomized into two groups: Group 1 (n=45) received a home based exercise program only; group 2 (n=41) received the same home based exercise program along with a patellar brace. The duration of the intervention was six weeks. At the end of the therapy patients were evaluated for pain and functional capacity. Pain was assessed using the visual analogue scale (VAS) and the Western Ontario McMaster Osteoarthritis Index (WOMAC) Pain Scale. Functional capacity was evaluated by using the WOMAC Functional Capacity Index and the Fulkerson-Shea Patellofemoral Evaluation Score (FSPES). R Re es su ul lt ts s: : After the program, there were statistically significant decreases in the VAS and WOMAC pain scale for both group 1 and 2 (p<0.001). Also, statistically significant increases were observed in the WOMAC functional capacity index and the FSPES for both groups (p<0.001). This study was carried out to determine the effects of organic and inorganic manganese (Mn) at di... more This study was carried out to determine the effects of organic and inorganic manganese (Mn) at different levels supplemented to diets of the broilers under stocking density stress on bone characteristics, immune response to vaccine and oxidative stress. A total of 1000, day-old broiler chicks were divided into one control group and nine treatment groups, each consisting of 100 chicks. They were randomly located at a density of 0.064 kg/m 2 until reaching seven days of age and in the compartments with 2 m 2 space at a density of 60 kg/m 2 until they are 49 days old. The control group was fed the basal diet only, whereas experimental groups were fed with basal diet supplemented with organic Mn (6.25, 12.5 and 25 mg/kg of Mn-methionine), inorganic Mn (12.5, 25 and 50 mg/kg Mn-oxide) and organic + inorganic Mn (3.125+6.25, 6.25+12.5 and 12.5+25 mg/kg Mn-methionine + Mn-oxide). There were no changes on days 25 and 49 in terms of physical properties such as humerus, femur and tibia as well as tibia fracture resistance and tibial metaphyseal chondral tissue lenght in all experimental groups (P>0.05). Tibia bone elasticity was higher in the 50 mg/kg Mn-oxide group compared to the control and other groups on day 49 (P<0.05). The serum calcium level on day 25 increased in the 6.25 and 12.5 mg/kg Mn-methionine and 6.25+12.5 mg/kg Mn-methionine + Mn-oxide groups (P<0.01). Antibody titre against Newcastle Disease Virus in Mn-methionine group on day 25 and titer against Infectious Bursal Disease in Mn-methionine and Mn-oxide groups on day 49 were higher compared to the control group (P<0.01). Serum levels of malondialdehyde and nitric oxide on day 49 decreased in all exprimental groups, whereas serum glutathione level increased in the same groups (P<0.01). As a result, dietary Mn supplementation to broilers hosted at 60 kg/m 2 of stocking density increased immune response and antioxidant activity by reducing the impact of density stress; yet, did not affect the physical properties of the bone. The effects of organic and inorganic forms of Mn on the examined properties were concluded to be similar.

Thirty-six knees [Bilateral knees were operated simultaneously in 12 patients (50%)] in 24 patients [3 men (12.5%), 21 women (87.5%); average age 67 (59-80 age)] were included in the study. During surgery, PCA+3° ER line and cTEA line were drawn on the distal femoral cutting surface by electrocautery pencil following distal femoral cut. The both lines on distal femur were recorded by digital camera and relationship between lines was ascertained in reference to PCA+3° ER line [parallel, internal rotation (IR), ER]. A two-stage field study was conducted in Cay, a district of Afyon, Turkey. In the first stage, a list of all individuals with orthopedic disabilities was derived. Then, medical histories were elicited and examinations were made by a team of specialists of orthopedics and traumatology. Radiographic studies were made when necessary. Orthopedic disabilities were defined as any muscle or skeletal abnormality that was associated with a permanent functional loss and were classified as congenital, traumatic, cerebral palsy, poliomyelitis, and other causes. The overall population was 35,571, of whom 189 had orthopedic disabilities (53/10,000). The male-to-female ratio was 1.8.
